Trends in Tuberculosis Epidemiology among Children in the Democratic Republic of Congo  [PDF]

Abstract: Setting: The epidemiology of tuberculosis (TB) among children in the Democratic Republic of Congo (DRC) is not well known. Objective: This study aimed to describe the trends in TB epidemiology among children in the DRC and to compare these trends in children and adults. Design: Data from the National TB program, the WHO Global TB Report, and a demographic survey of health in the DRC were retrospectively analyzed. The study period was from 1995 to 2014. The notification rate, absolute incidence and incidence rate of TB per 100,000 population were reported. Results: In 2014, 12,785 (12.6% of adult cases) TB cases were reported in children and 101,303 in adults. Among children, 3438 (26.89%) had PTB+; 2828 (22.11%) had PTB; and 6519 (50.98%) had extrapulmonary TB (EPTB). Children under 5 years had a lower reported prevalence of TB (184 cases). The incidence rate per 100,000 population was 10 in children and 181 in adults. The TB incidence decreased between 2010 (11.47) and 2014 (10.46). The proportion of children in overall cases of PTB+ was 4% to 5% in all districts. Conclusion: Caring for childhood TB remains a challenge in the DRC. Improved diagnostic procedures and effective training of providers who care for childhood TB are needed.

Abstract: Introduction: To participate effectively in the fight against tuberculosis (TB), mothers need to have a good knowledge of TB and its prevention. The objective of this study was to evaluate the knowledge of mothers about TB and Bacillus Calmette Guérin (BCG). Methods: This was a cross-sectional survey performed from September to December 2015 in 4 health care facilities of Kinshasa. It concerned mothers of children who received BCG vaccine. The frequencies and chi-square of Pearson were used to report results. Results: A total of 380 couples of the children and their mothers were recruited. The median age of children was 16 months (ranges: 6 days to 59 months); 224 (58.9%) of them received BCG during the period recommended; 62 (16.3%) experienced a side effect. There was a significant association (p = 0.00) between sides effects and the delayed vaccination. The average age of the mothers was 29.3 ± 6.4 years; 352 (92.6%) had heard about TB; 28 (7.4%) never heard about it; 168 (44.2%) knew that TB is a contagious disease; only 111 (29.2%) knew the mode of transmission; 87 (22.9%) did not know any signs of TB, and 54 (14.2%) knew about prevention with the BCG vaccine. Factors significantly associated with the mothers’ lack of knowledge were a low level of education (p = 0.01), young age (p = 0.02), and place of residence (p = 0.04). Conclusion: There is an urgent need to improve the education of the population, particularly those who lives in poor conditions and who are uneducated.
Cancer immunotherapy; integration of T cell biology with nanogel- and vector-technology in translational research

Abstract: We intended to integrate immunobiological strategy of T cells with two technologies, nanogel technology and retroviral vector technology for translational research of cancer immunotherapy. Cholesterol-bearing hydrophobizedpullulan (CHP), physically cross-linked nanogels by self-assembly, form nanoparticle complex with protein in water.We found that antigen protein with multiple T cell epitopes, when complexed with CHP, was efficiently transported to lymph nodes and well captured by antigen presenting cells such as dendritic cells and macrophages leading to cross presentation. Hence, CHP-antigen protein complex may become excellent cancer vaccine to induce both CD8+ killer T cells and CD4+ helper T cells of high quality.Intrinsic weakness of insufficiency in number of cancer specific T cells in hosts, prompted us to develop adoptive T cell therapy with lymphocytes engineered to possess cancer specificity. For this purpose, we developed novel retroviral vectors to highly express exogenously transduced cancer specific T cell receptor (TCR), yet suppressing expression of endogenous polyclonal TCR. This approach allowed us to prepare T cells with finer specificity of expressed TCR. In addition, use of RetroNectin?, a recombinant fragment of fibronectin opened a way to ex vivo prepare T cells of sufficient quantity and good quality for clinical use.Translational clinical trials of these cancer vaccine and adoptive T cell therapy are now on-going.An open innovation to promote fusion of different fields of science and technology played an essential role in our development of cancer immunotherapy.

Abstract: The string background AdS7XS4 is adopted and the early universe is modeled in the eleven dimensional SUGRA theory that is dual to this background. Specifically the ground state of the vacuum is associated with an arbitrary distribution of closed, spin-2 strings, and excited states are modeled as geometric combinations of individual strings. Combinations or combining iterations are, by hypothesis, admissible or geometric if each iteration intrinsically incorporates the metrical scale that is assigned to the individual spin-2 string. It is demonstrated that a generalization of this process, if appropriately calibrated, establishes theoretical fermionic masses that correspond approximately to observed values. The proposed model also predicts a new quark of mass .

Abstract: This article reports design and development efforts associated with one school-university collaborative partnership project focused on assisting K-12 school community stakeholders struggling with difficult school improvement challenges learn how to reinvent themselves as organizational teams to engage effectively in positive school change and renewal. The project employs an alternative, immersive organizational learning design in conjunction with available multimedia technology to leverage stakeholders’ own lived experiences and leadership challenges as opportunities to engage school community participants in a unique organizational case development and team learning adventure. Multimedia case development and production activities involving school community stakeholders in one elementary school are highlighted. Insights derived from the use of multimedia case development as an organizational leading and learning tool for educators and other stakeholders in school communities are discussed.

Abstract: The properties of a wave equation for a six-component wave function of a photon are re-analyzed. It is shown that the wave equation presents all the properties required by quantum mechanics, except for the ones that are linked with the definition of the position operator. The situation is contrasted with the three-component formulation based on the Riemann-Silberstein wave function. The inconsistency of the latter with the principles of quantum mechanics is shown to arise from the usual interpretation of the wave function. Finally, the Lorentz invariance of the six-component wave equation is demonstrated explicitly for Lorentz boosts and space inversion.

Abstract: This article reports on activities associated with one school-university partnership project focused on providing multimedia-integrated organizational learning and development opportunities to stakeholders in K-12 school communities struggling with real-world school change and improvement challenges. A central feature of the project’s organizational case learning design is the use of multimedia case production technology in conjunction with context-specific school situational data to immerse school stakeholders in the theatrical portrayal and analysis of their own school dilemma challenges as a means to jumpstart collaborative teaming and data-driven problem solving. Multimedia case development and analysis efforts involving stakeholders in one middle school community who are confronting a difficult, politically charged set of school improvement challenges are highlighted. Project findings associated with the use of multimedia technology and data-driven organizational case learning as tools to inform an alternative staff development approach for promoting positive school leadership teaming and real-world problem solving among education stakeholders in K-12 school communities are discussed.
On Approximating Fermion Masses in Terms of Stationary Super-String States  [PDF]

AdS/CFT correspondence is adopted and fermion masses are modeled as analogues of Weyl curvature states, which occur by hypothesis when closed spin-2 strings sweep out closed world tubes. Admissible curvature states are established by gauge invariance and fundamental mass is attributed to admissible curvature. A consequent spectrum of masses forms an SU(3) symmetry that is invariant under appropriate realizations of the SUGRA GUT interaction. Finally the spin-h/2 nature of the masses that are attributed to curvature emerges as a necessary condition for the relevant SUGRA GUT realizations. Calibration of the proposed model reveals a spectrum of fermion masses that corresponds approximately to observation. Moreover, the proposed model predicts a new quark that is characterized by I3 = -1/2 and by a mass of about 30 GeV/c2.

Abstract: This article highlights case production and analysis efforts associated with a multi-year, multimedia case development project focused on providing alternative organizational learning opportunities to regional K-12 schools and school districts. The School Leadership Case Simulation (SLCS) Project utilizes a unique multi-entity partnership and school-based collaborative learning design in which teams of university-based education researchers, multimedia production specialists, and regional education service center school improvement consultants work in tandem with K-12 school educators and community members to help these school stakeholders develop and produce multimedia organizational learning cases about their own real-world, context-specific school dilemma challenges. Collaborative team learning efforts of one group of school educators and community stakeholders grappling with an organizationally entrenched set of instructional leadership and school improvement challenges in one regional high school community are presented. Project findings relating to the viability and usefulness of the project’s multi-entity, immersive collaborative teaming design and multimedia-integrated case learning tools for helping groups of K-12 school community stakeholders learn how to adopt a team-centered organizational learning approach to reframing their real-world school leadership dilemma challenges and engage directly in data-informed, collaborative decision-making are presented and discussed.

Anecdotal evidence suggests that there is a stark contrast between the project management tools and techniques taught in schools and those actually used in industrial settings. This exploratory study provides evidence of those differences in the form of qualitative data collected from project managers in the information technology areas of major firms from the banking, cosmetics, and electronics manufacturing industries. Evidence suggests that there is a significant variation in which the formal project management processes taught in traditional project management curricula are used, if any. The study explores the reasons behind these differences and suggests possible approaches to ease the strain for new hires and current employees.
