Multidirectional instability of the shoulder – current concept

Abstract: The multidirectional instability of the shoulder is a complex problem in terms of diagnosis and treatment. Distinct from multidirectional hyperlaxity, multidirectional instability has symptoms related with increased translations in more than one direction. Increased translation by the increased capsular ligamentous laxity is underlying pathology of the posterior and multidirectional instability. This increased capsular laxity can be in-borne or developmental and asymptomatic or minimally symptomatic initially. In this stage, attempted translation does not produce symptoms. Jerk and Kim tests reveal posterior clunk without shoulder pain [1,2].However, repetitive subluxation overloads the posteroinferior glenoid labrum by the excessive rim-loading of the humeral head. This excessive rim-loading eventually develops posteroinferior labral lesion varying from simple retroversion to incomplete detachment (Figure 1). In this stage, patient's symptom which is shoulder pain, originates from the labral lesion when the humeral head glides over the pathologic labrum. The compressive force on the torn labrum in the jerk and Kim tests generates shoulder pain. Therefore, intact labrum does not produce shoulder pain no matter how lax the glenohumeral joint is. Increased translation alone produces asymptomatic posterior clunk until the repetitive rim-loading eventually develops posteroinferior labral lesion [3].Four type of the labral lesion have been reported. The Kim lesion is a concealed and incomplete tear of the posteroinferior labrum which is characterized by loss of labral height and retroversion, marginal crack, and loose inside. The lesion is similar to the intratendinous tear of the rotator cuff in that it is not evident in the initial observation. Abstract: In this article, we have attempted to analyze current situation and the problem of domestic and overseas harbor container delivery & cargo work automation centered on major harbors and to suggest effective way to deal with the issue in order to improve the productivity of container cargo work per crane, the major index of productivity of high value-added shipbuilding industry. In particular, we have suggested the way to realize effective automation system that can improve the efficiency of harbor container delivery & cargo work through the development of high-tech measuring automation technology using microwave radar and applied design that have broken away from traditional automation system and traditional problems such as dependence on manual work and the problem of laser method in which workers cannot identify laser beam under sunlight and workers’ eyesight can be weakened by being exposed to laser beam.

Abstract: We report here a case of an inflammatory myofibroblastic tumor in the retroperitoneum, which mimicked a germ cell tumor of the undescended testis. A 75-year-old healthy man presented with a palpable abdominal mass. On the computed tomography image, there was large, well-defined soft tissue mass in the left side of the retroperitoneum, and there was no visible left testis or seminal vesicle. After contrast enhancement, the mass appeared to be relatively homogeneous, considering its large size. With ultrasonography, it appeared as a well-defined, hypoechoic mass with intratumoral vascularity. This solid mass was surgically diagnosed as an inflammatory myofibroblastic tumor.
Outcome after gastrectomy in gastric cancer patients with type 2 diabetes

Abstract: AIM: To evaluate the prognosis of type II diabetes mellitus (T2DM) after gastrectomy and related factors in gastric cancer patients. METHODS: 403 gastric cancer patients with T2DM were studied, who underwent gastrectomy between May 2003 and September 2009. A review of medical records and telephone interviews was performed in this cross-sectional study. The factors included in the statistical analysis were as follows: gender, age, type of surgery, preoperative body mass index (BMI), current BMI, BMI reduction ratio, preoperative insulin or oral diabetic medicine requirement, follow-up duration, and current state of diabetes. Assessment of diabetes status after surgery was classified into four categories according to the change in hypoglycemic agents after surgery and present status of T2DM: resolution, improvement, same, and worse. RESULTS: The mean follow-up duration was 33.7 mo (± 20.6 mo), preoperative BMI was 24.7 kg/m2 (± 3.0 kg/m2), and BMI reduction ratio was 9.8% (± 8.6%). After surgery, T2DM was cured in 58 patients (15.1%) and was improved in 117 patients (30.4%). According to the type of surgery, the BMI reduction ratio was significantly higher in the total gastrectomy and Roux-en-Y reconstruction group [14.2% ± 9.2% vs 9.2% ± 7.7% (Billroth II group), P < 0.001] and significantly lower in the subtotal gastrectomy and Billroth I reconstruction group [7.6% ± 8.0%, 9.2% ± 7.7% (Billroth II group), P < 0.001]. The BMI reduction ratio, follow-up duration after surgery, type of surgery, extent of gastrectomy, and performance of duodenal bypass were significantly correlated to the course of T2DM (P < 0.05). The BMI reduction ratio was the most influential factor on T2DM status. In a subgroup analysis of patients with a BMI reduction ratio of 10% or less (n = 206), T2DM was cured in 15 (7.6%) patients and was improved in 57 (28.8%) patients after surgery, and only the duration of surgery was significantly correlated to T2DM status (P = 0.022). CONCLUSION: The course of T2DM was significantly correlated to the BMI reduction ratio but not to the type of surgery without a significant change in BMI.
Cellulosic ethanol production using a yeast consortium displaying a minicellulosome and β-glucosidase

Abstract: Background Cellulosic biomass is considered as a promising alternative to fossil fuels, but its recalcitrant nature and high cost of cellulase are the major obstacles to utilize this material. Consolidated bioprocessing (CBP), combining cellulase production, saccharification, and fermentation into one step, has been proposed as the most efficient way to reduce the production cost of cellulosic bioethanol. In this study, we developed a cellulolytic yeast consortium for CBP, based on the surface display of cellulosome structure, mimicking the cellulolytic bacterium, Clostridium thermocellum. Results We designed a cellulolytic yeast consortium composed of four different yeast strains capable of either displaying a scaffoldin (mini CipA) containing three cohesin domains derived from C. thermocellum, or secreting one of the three types of cellulases, C. thermocellum CelA (endoglucanase) containing its own dockerin, Trichoderma reesei CBHII (exoglucanase) fused with an exogenous dockerin from C. thermocellum, or Aspergillus aculeatus BGLI (β-glucosidase). The secreted dockerin-containing enzymes, CelA and CBHI, were randomly assembled to the surface-displayed mini CipA via cohesin-dockerin interactions. On the other hand, BGLI was independently assembled to the cell surface since we newly found that it already has a cell adhesion characteristic. We optimized the cellulosome activity and ethanol production by controlling the combination ratio among the four yeast strains. A mixture of cells with the optimized mini CipA:CelA:CBHII:BGLI ratio of 2:3:3:0.53 produced 1.80 g/l ethanol after 94 h, indicating about 20% increase compared with a consortium composed of an equal amount of each cell type (1.48 g/l). Conclusions We produced cellulosic ethanol using a cellulolytic yeast consortium, which is composed of cells displaying mini cellulosomes generated via random assembly of CelA and CBHII to a mini CipA, and cells displaying BGLI independently. One of the advantages of this system is that ethanol production can be easily optimized by simply changing the combination ratio of the different populations. In addition, there is no limitation on the number of enzymes to be incorporated into this cellulosome structure. Not only cellulases used in this study, but also any other enzymes, including cellulases and hemicellulases, could be applied just by fusing dockerin domains to the enzymes.
The Biological Safety of Stainless Steel Needles Used in Warm-Needling

Abstract: Warm-needling (also called thermo-acupuncture) is a combination of acupuncture and moxibustion. Due to the intense heat involved, there have been concerns over the biological safety of the acuneedles used in the treatment. This paper reports two phases of a safety test. For a preliminary test, we compared the temperature change patterns of stainless steel (SS304) needles and traditional gold alloy needles, which have been increasingly replaced by the former. To verify the effects of the presence of coating materials, the main test involved three different kinds of SS304: silicone-coated, salicylic acid-coated and non-coated needles. Each group of needles was tested for pH level, heavy metals and UV absorbance spectrum along with biological tests on the cytotoxicity and hemolysis of the needle. All the tests on the extractants from the needles were negative. In the biological tests, each test result showed a significant difference from the positive control samples, while no significant difference was observed compared with the negative control samples. In the hemolysis tests, all samples satisfied the Korean Government Standards. All the results suggest that SS304 needles are biologically safe to be used in warm-needling, though they can be improved to perform as well as the gold alloy needles in terms of temperature fluctuations.
Promoter hypermethylation and loss of CD133 gene expression in colorectal cancers

Abstract: AIM: To understand CD133 promoter hypermethylation and expression in 32 colorectal cancer cell lines.METHODS: Nucleic acid was isolated from 32 colorectal cancer cell lines and CD133 expression levels were measured by reverse transcription-polymerase chain reaction (RT-PCR) and real-time PCR. Promoter methylation status of the CD133 gene was analyzed with a methylation-specific PCR after sodium-bisulfite modification and by clonal sequencing analysis. The correlation between expression and promoter methylation of CD133 gene was confirmed with treatment of 5-aza-2’-deoxycytidine.RESULTS: We measured CD133 expression levels in 32 colorectal cancer cell lines. RT-PCR analysis showed undetectable or low levels of CD133 expression in 34.4% of cell lines. To verify the relation between CD133 expression and methylation status of the CD133 gene promoter in colorectal carcinogenesis, CD133 gene promoter hypermethylation was analyzed in 32 cancer cell lines. Promoter hypermethylation was detected in 13 (40.6%) of the cell lines using methylation specific-PCR and confirmed by bisulfite sequencing analysis. Treatment of 11 of the cell lines with the demethylation agent 5-aza-2’-deoxycytidine recovered CD133 expression in most of them.CONCLUSION: Transcriptional repression of CD133 is caused by promoter hypermethylation of the CD133 CpG islands in some of colorectal cancer cell lines. The study may contribute to the understanding of the role of CD133 inactivation in the progression of colorectal cancers.
Geodesic Motions in 2+1 Dimensional Charged Black Holes

Abstract: We study the geodesic motions of a test particle around 2+1 dimensional charged black holes. We obtain a class of exact geodesic motions for the massless test particle when the ratio of its energy and angular momentum is given by square root of cosmological constant. The other geodesic motions for both massless and massive test particles are analyzed by use of numerical method.
Antibacterial Activity of Phytochemicals Isolated from Atractylodes japonica against Methicillin-Resistant Staphylococcus aureus

Abstract: Methicillin-resistant Staphylococcus aureus (MRSA) has been emerging worldwide as one of the most important problems in communities and hospitals. Therefore, new agents are needed to treat acute oral infections from MRSA. In this study, antibacterial compounds from the roots of Atractylodes japonica (A. japonica) were isolated and characterized. The compounds were isolated from the root extracts using HPLC-piloted activity-guided fractionations. Four A. japonica compounds were isolated and identified as atractylenolide III (1), atractylenolide I (2), diacetylatractylodiol [(6E,12E)-tetradeca-6,12-diene-8,10-diyne-1,3-diol diacetate, TDEYA, 3). and (6E,12E)-tetradecadiene-8,10-diyne-1,3-diol (TDEA, 4), which was obtained by hydrolysis of TDEYA. The minimum inhibitory concentrations (MICs) was determined in the setting of clinical MRSA isolates. Compound 4 showed anti-MRSA activity with a MIC value of 4-32 μg/mL. The overall results provide promising baseline information for the potential use of the extract of A. japonica as well as some of the isolated compounds in the treatment of bacterial infections.
Cyclosporine A and bromocriptine attenuate cell death mediated by intracellular calcium mobilization

Abstract: To identify the novel inhibitors of endoplasmic reticulumstress-induced cell death, we performed a high throughput assaywith a chemical library containing a total of 3280 bioactive smallmolecules. Cyclosporine A and bromocriptine were identified aspotent inhibitors of thapsigargiin-induced cell death (cut-off at4σ standard score) . However, U74389G, the potent inhibitor oflipid peroxidation had lower activity in inhibiting cell death. Theinhibition effect of cyclosporine A and bromocriptine wasspecific for only thapsigargin-induced cell death. The mechanismof inhibition by these compounds was identified as modificationof the expression of glucose regulated protein-78 (GRP-78/Bip)and inhibition of phosphorylation of p38 mitogen activatedprotein kinase (MAPK). However, these compounds did notinhibit the same events triggered by tunicamycin, which was inagreement with the cell survival data. We suggest that theinduction of protective unfolded protein response by thesecompounds confers resistance to cell death. In summary, weidentified compounds that may provide insights on cell deathmechanisms stimulated by ER stress.
