A New Laparoscopic Surgical Procedure to Achieve Sufficient Mesorectal Excision in Upper Rectal Cancer

Abstract: Objective. Mesorectal excision corresponding to the location of a tumor, termed tumor-specific mesorectal excision (TSME), is commonly performed for resection of upper rectal cancer. We devised a new laparoscopic procedure for sufficient TSME with rectal transection followed by mesorectal excision. Operative Technique. After mobilization of the sigmoid colon and ligation of inferior mesenteric vessels, we dissected the mesorectum along the layer of the planned total mesorectal excision. The rectal wall was carefully separated from the mesorectum at the appropriate anal side from the tumor. After the rectum was isolated and transected using an endoscopic linear stapler, the rectal stump drew immediately toward the anal side, enabling the mesorectum to be identified clearly. In this way, sufficient TSME can be performed easily and accurately. This technique has been successfully conducted on 19 patients. Conclusion. This laparoscopic technique is a feasible and reliable procedure for achieving sufficient TSME. 1. Introduction Total mesorectal excision (TME) is recognized as an extremely important surgical technique for the prevention of local recurrence of rectal cancer [1–3]. On the other hand, TME is not necessarily applicable in every case of rectal cancer: for upper rectal cancer, mesorectal excision for limited lengths of 5?cm from a tumor toward the anal side is widely conducted, and this method is reportedly associated with adequate rates of cure [4, 5]. This technique is referred to as partial mesorectal excision (PME), but rather should be called tumor-specific mesorectal excision (TSME) reflecting its correspondence to the localization or T-stage of the tumor [5]. In a narrow pelvic cavity, performing sufficient TSME is difficult, and there is a risk of local recurrence when TSME is inadequate [6–8]. Whether surgery is performed laparoscopically or via a conventional open route, TSME is usually conducted obliquely to the anal side, introducing unnecessary rectal resection which may lead to postoperative bowel malfunction [9]. In addition, there is of the potential for slippage of the TSME between the right and left sides of the rectal wall. Particularly in the case of laparoscopic surgery, straight and sharp dissection of the mesorectum is difficult to perform and the dissection line is likely to be in zigzags. Of course, TSME shifting toward the oral side from the starting line is inappropriate and should be strictly avoided in order to prevent local recurrence (Figure 1). Abstract: A 77-year-old Japanese woman presented to our emergency department with sudden hematemesis, occurring at least four to five times over a 12-hour period. No ulcer or gastrointestinal bleeding was detected on gastroendoscopy. A 40mm mass in the inferior lobe of the right lung was found on a chest X-ray, and our patient’s symptoms were therefore initially ascribed to aspirated blood from lung tumor-associated hemoptysis. However, our patient continued to show signs of severe abdominal pain and decreased urine output despite aggressive hydration, leading her examining physicians to search for a possibly severe, occult abdominal pathology. On emergent computed tomography imaging, we found an acute strangulated internal hernia within the falciform ligament. Diagnosis was made by helical computed tomography, permitting rapid surgical intervention.Our findings on computed tomography imaging assisted with the pre-operative diagnosis and enabled us to make a rapid surgical intervention. Early diagnosis may help preclude significant strangulation with unnecessary resection.

Abstract: Patent pools are established by the patent holders in order to promote R & D and technological standards, etc. This paper investigates the patent holders’ incentive to form a patent pool, the patent pool’s licensing behavior, and the anticompetitive effect of a patent pool. Our model is characterized by the following two features. First, we consider the different two types of patent: basic and optional. Second, we consider a patent pool that offers two types of a package license: single and multiple. Our results yield some implications for a patent pool that is characterized by the complementarity between basic and optional patents.

Abstract: Permanent waving is very popular in Japan. Polypeptide chains (main chains) form the principal components of hair, and they are lined up longitudinally. Hair relaxation is also called straight permanent waving, and there are methods that change curly or wavy hair into straight hair. Hair damage as a result of winding, combing, and using high-temperature hairdressing irons is also often seen. By using scanning electron micrographs (SEM) we showed broken hairs and hair damage caused by permanent wave solutions. The hair damage is obvious when comparisons are made with the condition of the hair surface, condition of the cuticle, etc. Hair swelling by permanent wave solutions, manipulations such as winding, etc., inadequate rinsing with water, procedures on injured hair at the outset, etc., are considered possible reasons for any of these types of injury.

Identification of the Transgenic Integration Site in Immunodeficient tgε26 Human CD3ε Transgenic Mice

Abstract: A strain of human CD3ε transgenic mice, tgε26, exhibits severe immunodeficiency associated with early arrest of T cell development. Complete loss of T cells is observed in homozygous tgε26 mice, but not in heterozygotes, suggesting that genomic disruption due to transgenic integration may contribute to the arrest of T cell development. Here we report the identification of the transgenic integration site in tgε26 mice. We found that multiple copies of the human CD3ε transgene are inserted between the Sstr5 and Metrn loci on chromosome 17, and that this is accompanied by duplication of the neighboring genomic region spanning 323 kb. However, none of the genes in this region were abrogated. These results suggest that the severe immunodeficiency seen in tgε26 mice is not due to gene disruption resulting from transgenic integration.
Cloud-resolving simulation of heavy snowfalls in Japan for late December 2005: application of ocean data assimilation to a snow disaster

Abstract: We applied eddy-resolving ocean data assimilation to a cloud-resolving atmospheric simulation of a snow disaster and investigated the effects of mesoscale eddies on a heavy snowfall event in late December 2005. Ocean circulation model (OCM) data assimilation reproduces mesoscale sea surface temperature (SST) structures, which are smoothed out by optimum interpolation. This difference between OCM-assimilation and optimum-interpolation SSTs affects the atmospheric boundary layers over oceanic mesoscale eddies. The atmospheric response to the SST difference is complex at the cold tongue in the central Sea of Japan. Although the horizontal wind and turbulent mixing are quickly and locally affected by the low SST, the atmospheric temperature and water amounts are greatly affected by the upstream high SST via the northwesterly advection. In the heavy snowfall areas, the OCM assimilation greatly affects 10-day accumulated precipitation, though it does not largely influence 10-day mean vertical structures of wind, temperature and water vapor. Thus, we should recognize the significance of oceanic mesoscale eddies for heavy snowfall.

Position Uncertainty in the Heisenberg Uncertainty Relation

Abstract: Position measurements are examined under the assumption that object position x_t and probe position X_t just after the measurement are expressed by a linear combination of positions x_0 and X_0 just before the measurement. The Heisenberg uncertainty relation between the position uncertainty and momentum disturbance holds when the measurement error \epsilon(x_t) for the object position x_t is adopted as the position uncertainty. However, the uncertainty in the measurement result obtained for x_0 is the standard deviation of the measurement result, and not the measurement error \epsilon(x_0). This difference is due to the reduction of a wave packet. The validity of the linearity assumption is examined in detail.
