Predictive Factors of Multiple Hospitalizations for Acute Exacerbations of COPD  [PDF]

Abstract: Background: Multiple hospitalizations for acute exacerbations of chronic obstructive pulmonary disease (AECOPD) are associated with considerable morbidity and mortality. Objectives: To identify predictive factors of multiples hospitalizations for AECOPD. Methods: This is a retrospective single center study of consecutive patients with COPD hospitalized at the Department of Respiratory Medicine between January 1990 and December 2015. We calculated for each patient the mean number of hospitalizations for AECOPD/year (H/y). We distinguished 2 groups (G) of patients. G1: <2 H/y and G2: ≥2 H/y. Predictors of multiple admissions identified by univariate analysis were included in the multivariate analysis. Results: The study included 1167 COPD patients (mean age 67 ± 10 years, 97% males). Three hundred six (26%) COPD patients had a mean number of hospitalizations per year ≥ 2. Multivariate logistic regression analysis demonstrated that an mMRC ≥ 2 (Odd ratio [OR] 1.8, 95% confidence interval [CI] 1.08 - 2.99, p = 0.022), a low PaO2 (PaO2 OR 0.97, 95% CI 0.95 - 0.99, p = 0.007) and frequent exacerbations (OR 2.95, 95% CI 2.56 - 3.39, p < 0.001) are independent factors associated with multiple admissions for AECOPD. Conclusions: An mMRC ≥ 2, a low PaO2 and frequent exacerbations are independently associated with multiple hospitalizations for AECOPD. The identification of these high risk COPD patients will be helpful in the decision of intervention strategies.

This study aims to highlight evidences of tectonic and geodynamic features within Lakhssas high plateau karst area (Moroccan Western Anti-Atlas), using an original approach based on geospeleothems analysis. This work is organized by the speleogenesis process, controlled by tectonics and interference of karst intrinsic parameters. Regarding the first issue, a multi-layered study compiling inventoried speleological spaces with tectonic field data analysis then with lithological, hydrogeological and geomorphological features, has demonstrated that speleogenesis processes are deeply controlled by tectonic framework so described as “speleotects”. Subsequent studies in terms of sedimentology, geomorphology, hydrogeology and dating deposits, are planned to ground those conclusions.

Abstract: gamma-rays following the B and B-n decay of the very neutron rich 84Ga produced by photo-fission of 238U have been studied at the newly built ISOL facility of IPN Orsay: ALTO. Two activities were observed and assigned to two B-decaying states: 84gGa, I = (0\^-) and 84mGa, I = (3\^-, 4\^-). Excitation energies of the 2+1 and 4+1 excited states of 84Ge were measured at E(2+1) = 624.3 keV and E(4+1) = 1670.1 keV. Comparison with HFB+GCM calculations allows to establish the collective character of this nucleus indicating a substantial N=50 core polarization. The excitation energy of the 1/2+1 state in 83Ga known to carry a large part of the neutron 3s1/2 strength was measured at 247.8keV. Altogether these data allow to confirm the new single particle state ordering which appears immediately after the double Z=28 and N=50 shell closure and to designate 78Ni as a fragile and easily polarized doubly-magic core.

Abstract: A new and innovative method for solving the 1D Poisson Equation is presented, using the finite differences method, with Robin Boundary conditions. The exact formula of the inverse of the discretization matrix is determined. This is the first time that this famous matrix is inverted explicitly, without using the right hand side. Thus, the solution is determined in a direct, very accurate (O(h2)), and very fast (O(N)) manner. This new approach treats all cases of boundary conditions: Dirichlet, Neumann, and mixed. Therefore, it can serve as a reference for solving the Poisson equation in one dimension.

Abstract: An innovative, extremely fast and accurate method is presented for Neumann-Dirichlet and Dirichlet-Neumann boundary problems for the Poisson equation, and the diffusion and wave equation in quasi-stationary regime; using the finite difference method, in one dimensional case. Two novels matrices are determined allowing a direct and exact formulation of the solution of the Poisson equation. Verification is also done considering an interesting potential problem and the sensibility is determined. This new method has an algorithm complexity of O(N), its truncation error goes like O(h2), and it is more precise and faster than the Thomas algorithm.

Abstract: In this article, we give a new proof of the Itô formula for some integral processes related to the space-time Lévy noise introduced in [1] [2] as an alternative for the Gaussian white noise perturbing an SPDE. We discuss two applications of this result, which are useful in the study of SPDEs driven by a space-time Lévy noise with finite variance: a maximal inequality for the p-th moment of the stochastic integral, and the Itô representation theorem leading to a chaos expansion similar to the Gaussian case.
On Concordance Measures for Discrete Data and Dependence Properties of Poisson Model

Abstract: We study Kendall's tau and Spearman's rho concordance measures for discrete variables. We mainly provide their best bounds using positive dependence properties. These bounds are difficult to write down explicitly in general. Here, we give the explicit formula of the best bounds in a particular Fréchet space in order to understand the behavior of the ranges of these measures. Also, based on the empirical copula which is viewed as a discrete distribution, we propose a new estimator of the copula function. Finally, we give useful dependence properties of the bivariate Poisson distribution and show the relationship between parameters of the Poisson distribution and both tau and rho.

Abstract: The subject of this article is to optimize the design of synchronous reluctance machines with massive rotor and multi-flux barrier rotor. The optimization procedure, which aims to improve simultaneously the machines' torque and the power factor, uses the cyclic coordinate method coupled with the magnetostatic finite-element (FE) field solutions. The optimization results regarding these two types of machines, which provide the optimized rotor geometrical dimensions and the influence of the current angle, are discussed.

Abstract: Personalization and adaptation to the user profile capability are the hottest issues to ensure ambientassisted living and context awareness in nowadays environments. With the growing healthcare andwellbeing context aware applications, modeling security policies becomes an important issue in thedesign of future access control models. This requires rich semantics using ontology modeling for themanagement of services provided to dependant people. However, current access control models remainunsuitable due to lack of personalization, adaptability and smartness to the handicap situation.In this paper, we propose a novel adaptable access control model and its related architecture in whichthe security policy is based on the handicap situation analyzed from the monitoring of user’s behavior inorder to grant a service using any assistive device within intelligent environment. The design of ourmodel is an ontology-learning and evolving security policy for predicting the future actions of dependentpeople. This is reached by reasoning about historical data, contextual data and user behavior accordingto the access rules that are used in the inference engine to provide the right service according to theuser’s needs.
