Redesign of Clinical Practicum for End of Life Care with Integrated Perspectives  [PDF]

Abstract: With the cycling of nursing research, theory, practice, education, and policy making, nursing knowledge has been developed and disseminated. A clinical practicum is a good way to deliver nursing knowledge. Also an area of end-of-life (EOL) care is a good field to integrate nursing knowledge with different perspectives and multiple ways of knowing. The purposes of this paper were 1) to describe nursing knowledge and EOL with an integrated view, 2) to critique the current phenomenon of the field of EOL, and 3) to propose a redesign of the clinical practicum in EOL care. An integrative review was performed as the method of the study. The results showed that the reflection of nursing knowledge from different perspectives regarding EOL may help the nursing discipline develop various nursing knowledge, practice, education, and research. However, this paper shows that recent education is more focused on empirical knowledge, problem-solving approaches with a totality paradigm, and didactic teaching-learning methods. For redesigning the clinical practicum, it is necessary to reestablish a caring relationship between nursing instructors and nursing students. In terms of the teaching-learning process, the human-becoming teaching-learning process is recommended over the nurse-becoming process.
Development and Evaluation of Korean Nurses’ Core Competency Scale (KNCCS)  [PDF]

Abstract: Purpose: The purpose of this study was to develop and evaluate a Korean Nurses’ Core Competency Scale (KNCCS). Methods: This methodological research was conducted over two major steps. The first step involved developing a preliminary 70-item KNCCS based on an extensive literature review and the standards and criteria proposed by the Korean Accreditation of Nursing. The second step involved evaluating validity and reliability of the KNCCS. 528 newly graduated nurses recently employed in nine major hospitals were participated in this research for validation. Results: The final KNCCS consisted of 70 items. The exploratory factor analysis identified five subscales under which each item could be categorized: 1) human understanding and communication skills; 2) professional attitudes; 3) critical thinking and evaluation; 4) general clinical performance; and 5) specific clinical performance. Conclusion: The KNCCS shows good reliability and validity. However, the test of both criterion and construct validity were recommended further.
Stroke Management Awareness and Behavior among Nursing Students in Bangladesh  [PDF]

Abstract: Background: Stroke is a significant cause of morbidity and mortality throughout the world. In Bangladesh, stroke accounts for 27% of deaths, making it the third leading cause of death and disability [1]. Nurses have a critical role to play in reducing death and disability among stroke victims, but many nursing students are not being educated to meet the challenges of this complex condition. Objectives: This study examined undergraduate nursing students’ awareness of strokes and their stroke patient management behaviors. Method: This was a descriptive study that surveyed undergraduate nursing students about their awareness of strokes and stroke patient management behaviors. One hundred and forty-four students were recruited from Dhaka University’s and Rajshahi University’s nursing colleges in Bangladesh. The students completed and returned the Awareness of Stroke Risk Factors, Awareness of Stroke Warning Signs, and Behavior of First Responses to Strokes and Behaviors of Special Managements of Stroke questionnaires between December, 2014 and February, 2015. Data were analyzed using descriptive statistics and the Pearson’s Product-Moment Correlation-Coefficient. Results: The results showed that nursing students had a moderate awareness level of stroke risk factors (M = 74.24, SD = 12.30) and a very low awareness of stroke warning signs (M = 55, SD = 10.72). In addition, all of the students had a low to very low level of behaviors of special stroke patient management (M = 62.11, SD = 9.75) and a very low level of behaviors of first responses to strokes (M = 0.24, SD = 0.43). There was a significant positive correlation between nursing students’ awareness of stroke risk factors and stroke warning signs (r = 0.247**, p < 0.001). However, no significant correlation was found between students’ awareness of stroke risk factors with their behaviors of first responses to strokes (r = 0.043, p > 0.05). In addition, their awareness of stroke warning signs was not correlated with their behaviors of special stroke patient management (r = 0.031, p > 0.05). These findings suggested that nursing students need to increase their awareness level to develop stroke management behaviors in order to improve practicum preparedness. Conclusion: Work-based education must be a part of nursing students’ curricula to increase their awareness of strokes and improve their stroke management behaviors to improve practicum preparedness.
Trend Analysis of Water Pollutant at Summer Rainfall Season  [PDF]

Abstract:

This study, with Hongdong Reservoir, is intended to evaluate the changes in water quality in the lake before and after rainfall in summer. Various non-point source pollutions are scattered around the reservoir, and to determine the pollution level by pollution source, samples were taken at the same point before rainfall (1st), during rainfall (2nd) and after rainfall (3rd) and concentration was measured. Pollutant concentration curve at the measuring points (HDS1, HDS2, HDS3) appeared to be similar with the hydrological curve. When comparing the concentration immediately before rainfall with event mean concentration (EMC), SS and COD were 4 - 59 times and 1 - 4 times, respectively. However, when it comes to total nitrogen (T-N), concentration arrived at the reservoir stayed the range of 1.3 - 12.0 mg/L in all 3 cases without significant variation, which indicated that total nitrogen load is critical when arriving at the reservoir, irrespective of rainfall, and thus it’s necessary to consider non-point source pollution runoff also in addition to point source pollution when developing the water quality improvement measures in reservoir.

The functional and structural characterization of a novel oncogene GIG47 involved in the breast tumorigenesis

Abstract: As a first step to understand the molecular role of GIG47, we analyzed the expression profile of GIG47 in multiple human cancers including the breast cancer and characterized its function related to human carcinogenesis. Based on this oncogenic role of GIG47, we then embarked on determining the high-resolution structure of GIG47. We have applied multidimensional heteronuclear NMR methods to GIG47.GIG47 was over-expressed in primary breast tumors as well as other human tumors including carcinomas of the uterine cervix, malignant lymphoma, colon, lung, skin, and leukemia. To establish its role in the pathogenesis of breast cancer in humans, we generated stable transfectants of MCF7 cells. The ectopic expression of GIG47 in MCF7 cells promoted the invasiveness in the presence of 50% serum. In addition, it also resulted in the increased tumorigenicity in in vivo tumor formation assay. The tumorigenesis mechanism involving GIG47 might be mediated by the activation of MAPK and PI3K pathways. These results indicate that GIG47 plays a role in the breast tumorigenesis, thus representing a novel target for the treatment of breast cancer. To facilitate the development of GIG47-targeted therapeutics, we determined the structural configuration of GIG47. The high-resolution structure of GIG47 was obtained by combination of NMR and homology modeling. The overall structure of GIG47 has four α-helices and 6 β-strands, arranged in a β1-α1-β2-β3-α2-β4-α3-α4-β5-β6 topology. There is a potential heme/steroid binding pocket formed between two helices α2 and α3.The determined three-dimensional structure of GIG47 may facilitate the development of potential anti-cancer agents.
Solid-Phase Immunoassay of Polystyrene-Encapsulated Semiconductor Coreshells for Cardiac Marker Detection

Abstract: A solid-phase immunoassay of polystyrene-encapsulated semiconductor nanoparticles was demonstrated for cardiac troponin I (cTnI) detection. CdSe/ZnS coreshells were encapsulated with a carboxyl-functionalized polystyrene nanoparticle to capture the target antibody through a covalent bonding and to eliminate the photoblinking and toxicity of semiconductor luminescent immunosensor. The polystyrene-encapsulated CdSe/ZnS fluorophores on surface-modified glass chip identified cTnI antigens at the level of ~ng/mL. It was an initial demonstration of diagnostic chip for monitoring a cardiovascular disease.
A State-of-the-Art Review on Debonding Failures of FRP Laminates Externally Adhered to Concrete

Abstract: There is significant concern in the engineering community regarding the safety and effectiveness of fiber-reinforced polymer (FRP) strengthening of RC structures because of the potential for brittle debonding failures. In this paper, previous research programs conducted by other researchers were reviewed in terms of the debonding failure of FRP laminates externally attached to concrete. This review article also discusses the influences on bond strength and failure modes as well as the existing experimental research and developed equations. Based on the review, several important conclusions were re-emphasized, including the finding that the bond transfer strength is proportional to the concrete compressive strength; that there is a certain bond development length that has to be exceeded; and that thinner adhesive layers in fact lower the chances of a concrete-adhesive interface failure. It is also found that there exist uncertainty and inaccuracy in the available models when compared with the experimental data and inconsistency among the models. This demonstrates the need for continuing research and compilation of data on the topic of FRP’s bond strength.
Seebeck effect at the atomic scale

Abstract: The atomic variations of electronic wavefunctions at the surface and electron scattering near a defect have been detected unprecedentedly by tracing thermoelectric voltages given a temperature bias [Cho et al., Nature Mater. 12, 913 (2013)]. Because thermoelectricity, or Seebeck effect, is associated with heat-induced electron diffusion, how the thermoelectric signal is related to the atomic-scale wavefunctions and what the role of the temperature is at such a length scale remain very unclear. Here we show that coherent electron and heat transport through a point-like contact produces an atomic Seebeck effect, which is described by mesoscopic Seebeck coefficient multiplied with an effective temperature drop at the interface. The mesoscopic Seebeck coefficient is approximately proportional to the logarithmic energy derivative of local density of states at the Fermi energy. We deduced that the effective temperature drop at the tip-sample junction could vary at a sub-angstrom scale depending on atom-to-atom interaction at the interface. A computer-based simulation method of thermoelectric images is proposed, and a point defect in graphene was identified by comparing experiment and the simulation of thermoelectric imaging.
TCF/β-catenin plays an important role in HCCR-1 oncogene expression

Abstract: In this study, we showed how the expression of HCCR-1 is modulated. The luciferase activity assay indicated that the HCCR-1 5'-flanking region at positions -166 to +30 plays an important role in HCCR-1 promoter activity. Computational analysis of this region identified two consensus sequences for the T-cell factor (TCF) located at -26 to -4 (Tcf1) and -136 to -114 (Tcf2). Mutation at the Tcf1 site led to a dramatic decrease in promoter activity. Mobility shift assays (EMSA) revealed that nuclear proteins bind to the Tcf1 site, but not to the Tcf2 site. LiCl, Wnt signal activator by GSK-3β inhibition, significantly increased reporter activities in wild-type Tcf1-containing constructs, but were without effect in mutant Tcf1-containing constructs in HEK/293 cells. In addition, endogenous HCCR-1 expression was also increased by treatment with GSK-3β inhibitor, LiCl or AR-A014418 in HEK/293 and K562 cells. Finally, we also observed that the transcription factor, TCF, and its cofactor, β-catenin, bound to the Tcf1 site.These findings suggest that the Tcf1 site on the HCCR-1 promoter is a major element regulating HCCR-1 expression and abnormal stimulation of this site may induce various human cancers.Proto-oncogenes normally help regulate cell growth and differentiation under well-controlled conditions, including mitogenic signal transductions in cells [1,2]. Abstract: Oncogene HCCR-1 expression levels were determined in normal breast tissues, breast cancer tissues and cancer cell lines. We examined whether HCCR-1 protein expression in breast cancer is related to different biological characteristics, including ER, PR, p53 genotype, and HER2 status in 104 primary breast cancer tissues using immunohistochemical analyses.HCCR-1 was upregulated in breast cancer cells and tissues compared with normal breast tissues. In this study, overexpression of HCCR-1 was well correlated with known breast cancer prognostic markers including the presence of steroid receptors (ER and PR), p53 mutation and high HER2 overexpression. HCCR-1 was not detected in the ER-negative, PR-negative, p53 negative and low HER2 breast cancer tissues. These data indicate that the level of HCCR-1 in breast cancer tissues is relatively well correlated with known breast cancer factors, including the HER2 overexpression, p53 mutation, and ER/PR status.Determination of HCCR-1 levels as options for HER2 testing is promising although it needs further evaluation.Cells expressing HCCR-1 are tumorigenic in nude mice [1]. The functional role of this oncogene in tumorigenesis is manifested as a negative regulator of the p53 tumor suppressor [1]. In the previous study, we investigated HCCR-1 protein expression in breast cancer and the possibility of using HCCR-1 as a useful biomarker for human breast cancer [2]. We also examined whether HCCR-1 protein expression in breast cancer is related to different biological characteristics including ER, PR, p53 genotype, and the HER2 status. Northern and Western blot analyses and immunohistochemical studies indicate that the HCCR-1 mRNA and protein are overexpressed in breast cancer tissues compared to the normal breast tissues [1,2].
