Biomarcadores cardíacos na avalia o da síndrome coronariana aguda =Cardiac biomarkers for assessment of acute coronary syndrome

Abstract: Objetivos: abordar o uso dos biomarcadores cardíacos associados à necrose, inflama o, desestabiliza o da placa, isquemia e disfun o cardíaca na avalia o da síndrome coronariana aguda a fim de propiciar a melhor compreens o sobre as vantagens e desvantagens do uso clínico desses biomarcadores laboratoriais. Fonte de dados: artigos científicos originais e de revis o indexados nas bases de dados PubMed e SciELO. Síntese dos dados: as doen as cardiovasculares, especialmente a síndrome coronariana aguda, representam a principal causa de mortalidade no Brasil e no mundo, sendo que seu contínuo crescimento representa uma das quest es de saúde pública mais relevantes da atualidade. Nesse sentido, os biomarcadores cardíacos constituem elementos fundamentais para o diagnóstico da síndrome coronariana aguda, sendo que as troponinas cardíacas continuam sendo consideradas o padr o ouro para o diagnóstico. Com o desenvolvimento dos recentes métodos ultrassensíveis para mensura o das troponinas cardíacas, tem sido possível detectar a les o cardíaca dentro de duas horas, melhorando substancialmente o diagnóstico precoce do infarto agudo do miocárdio. Outros biomarcadores vêm apresentando relevancia, especialmente em rela o ao prognóstico e estratifica o de risco dos pacientes com cardiopatia isquêmica. Conclus es: vários marcadores têm apresentado eficácia na detec o da necrose, isquemia, inflama o, desestabiliza o da placa e disfun o cardíaca. No entanto, as troponinas cardíacas continuam sendo consideradas o padr o ouro para o diagnóstico do infarto agudo do miocárdio. Aims: To address the use of cardiac biomarkers associated with necrosis, inflammation, plaque destabilization, and cardiac ischemia in the evaluation of acute coronary syndrome in order to provide a better understanding of the advantages and disadvantages of the clinical use of these biomarkers. Source of data: Scientific original and review articles from the PubMed and SciELO databases. Summary of findings: Cardiovascular diseases, especially acute coronary syndrome, are the leading cause of mortality in Brazil and worldwide, and its continued growth is one of public health issues most relevant today. In this sense, the cardiac biomarkers are key to the diagnosis of acute coronary syndrome, and the cardiac troponins are still considered the gold standard for its diagnosis. With the recent development of ultra-sensitive methods for measuring cardiac troponins, it has been possible to detect heart damage within two hours, substantially improving the early diagnosis of acute myocardial infarct
Interactive Reference Point-Based Guided Local Search for the Bi-objective Inventory Routing Problem

Abstract: Eliciting preferences of a decision maker is a key factor to successfully combine search and decision making in an interactive method. Therefore, the progressively integration and simulation of the decision maker is a main concern in an application. We contribute in this direction by proposing an interactive method based on a reference point-based guided local search to the bi-objective Inventory Routing Problem. A local search metaheuristic, working on the delivery intervals, and the Clarke & Wright savings heuristic is employed for the subsequently obtained Vehicle Routing Problem. To elicit preferences, the decision maker selects a reference point to guide the search in interesting subregions. Additionally, the reference point is used as a reservation point to discard solutions outside the cone, introduced as a convergence criterion. Computational results of the reference point-based guided local search are reported and analyzed on benchmark data in order to show the applicability of the approach.

Abstract: Das Besondere an Franz BREUERs Lehrbuch, das unter Mitarbeit von Barbara DIERIS und Antje LETTAU entstanden ist, ist weniger die Einführung in die Grounded-Theory-Methodik (GTM), als vielmehr die Einführung in eine reflexive Forschungspraxis. Das bedeutet, die Subjektivit t des Forschers, der Forscherin wird als bedeutsame Erkenntnisquelle in den Forschungsprozess miteinbezogen. Drei methodologische Bestandteile charakterisieren nach BREUER eine reflexive GTM und strukturieren auch den Aufbau des Lehrbuches. In meiner Besprechung befasse ich mich mit zweien davon ausführlicher: mit dem ethnografischen Zugang zum Forschungsfeld, insbesondere seinen erkenntnistheoretischen Voraussetzungen, und mit dem Einbezug der (Selbst-) Reflexivit t. Letztere wirft Fragen auf, denen am Schluss der Besprechung nachgegangen wird. In the present review, I focus on two of these in detail: the approach to the research field based on ethnography, particular its epistemological prerequisites, and the inclusion of (self-) reflexivity. Brazilian Sugar in Domestic Retail: The Quality of Sugar Consumed in the State of Sao Paulo  [PDF]

Abstract: Brazil is the largest producer of sugarcane and its derivatives in the world, and one of the largest consumers of sugarcane derivatives. Sugar places a position of great prominence in Brazilian feed, either by direct consumption or as a component of industrialized products, from increasing consumption in the country. Sugar samples taken from local market were analyzed as the quality of table sugar consumed in the state of Sao Paulo (Brazil). The analysis of the parameters and basic attributes of characterization of sugar was according to the methodology developed by ICUMSA (2005; 2011) and ICUMSA modified by Copersucar (2002). Samples of sugar from this region showed that medium-low quality. It can be observed in fundamental aspects such as purity and insoluble residues, which were below the standard. In addition, it should be noted the difficulty of obtaining information about the limit values for each attribute, by virtue of Legislation not be assertive to the product and, also, the scientific studies have not been unified to present the specifics for each type of sugar.
Attention Changes and Victimization in a Sample of Adolescents Victims of Domestic Violence  [PDF]

Abstract: Objective: To report attention changes in a sample of adolescents victims of domestic violence and discuss how this change may be related to an addictive and complex cycle of victimization and abuse. Methods: 78 adolescents went through psychiatric and neuropsychological evaluation. The Kiddie-Sads-Present and Lifetime Version and the Brazilian version of the Childhood Trauma Questionaire (CTQ) were applied. The neuropsychological assessment was conducted by trained professionals using the Wechsler Children’s Global Assessment Scale, 3rd edition-WlSC III. The sample was divided into 3 groups according to the intensity of the reported maltreatment-obtained by the CTQ score (comparison group, moderate degree of abuse and severe maltreatment). Results: The group that showed moderate maltreatment showed better performance in an attention task compared to the comparison group. At the severe maltreatment group, this difference was not observed. Conclusions: Children and adolescents who are victims of maltreatment are more alert to environmental details that surround them as a form of adaptation to defend themselves against potential threats. However, they could react precipitously with inappropriate behavior in some contexts. This can lead to a favorable environment for new and repeated situations of violence, abuse and a perpetuation of this victimization.

Abstract: This research aims to investigate school bullying, specifically in the setting of a Brazilian medical school. It presents part of the results of a doctoral research carried out in the Graduate Program in Bioethics, Applied Ethics and Collective Health (PPGBIOS), in Rio de Janeiro city. We confirm that few studies have investigated this phenomenon in the university, especially using the qualitative research methodology in education and health. It is possible to observe that the phenomenon of school bullying occurs in this setting and that the fieldwork is fundamental to give rise to important reflections on the subject among the students.
