Complications of high grade liver injuries: management and outcomewith focus on bile leaks

Abstract: This is a retrospective study of patients with liver injury admitted to Hadassah-Hebrew University Medical Centre over a 10-year period. Grade 3-5 injuries were considered to be high grade. Collected data included the number and types of liver-related complications. Interventions which were required for these complications in patients who survived longer than 24 hours were analysed.Of 398 patients with liver trauma, 64 (16%) were found to have high-grade liver injuries. Mechanism of injury was blunt trauma in 43 cases, and penetrating in 21. Forty patients (62%) required operative treatment. Among survivors 22 patients (47.8%) developed liver-related complications which required additional interventional treatment. Bilomas and bile leaks were diagnosed in 16 cases post-injury. The diagnosis of bile leaks was suspected with abdominal CT scan, which revealed intraabdominal collections (n = 6), and ascites (n = 2). Three patients had continuous biliary leak from intraabdominal drains left after laparotomy. Nine patients required ERCP with biliary stent placement, and 2 required percutaneous transhepatic biliary drainage. ERCP failed in one case. Four angioembolizations (AE) were performed in 3 patients for rebleeding. Surgical treatment was found to be associated with higher complication rate. AE at admission was associated with a significantly higher rate of biliary complications. There were 24 deaths (37%), the majority from uncontrolled haemorrhage (18 patients). There were only 2 hepatic-related mortalities due to liver failure.A high complication rate following high-grade liver injuries should be anticipated. In patients with clinical evidence of biliary complications, CT scan is a useful diagnostic and therapeutic tool. Abstract: In this paper, we propose a new infeasible interior-point algorithm with full NesterovTodd (NT) steps for semidefinite programming (SDP). The main iteration consists of a feasibility step and several centrality steps. We used a specific kernel function to induce the feasibility step. The analysis is more simplified. The iteration bound coincides with the currently best known bound for infeasible interior-point methods.
A Solution for Fighting Spammer's Resources and Minimizing the Impact of Spam  [PDF]

Abstract: Spam or unsolicited emails constitute a major threat to the Internet, the corporations, and the end-users. Statistics show that about 70% - 80% of the emails are spam. There are several techniques that have been implemented to react to the spam on its arrival. These techniques consist in filtering the emails and placing them in the Junk or Spam folders of the users. Regardless of the accuracy of these techniques, they are all passive. In other words, they are like someone is hitting you and you are trying by all the means to protect yourself from these hits without fighting your opponent. As we know the proverbs \"The best defense is a good offense\" or \"Attack is the best form of defense\". Thus, we believe that attacking the spammers is the best way to minimize their impact. Spammers send millions of emails to the users for several reasons and usually they include some links or images that direct the user to some web pages or simply to track the users. The proposed idea of attacking the spammers is by building some software to collect these links from the Spam and Junk folders of the users. Then, the software periodically and actively visit these links and the subsequent redirect links as if a user clicks on these links or as if the user open the email containing the tracking link. If this software is used by millions of users (included in the major email providers), then this will act as a storm of Distributed Denial of Service attack on the spammers servers and there bandwidth will be completely consumed by this act. In this case, no human can visit their sites because they will be unavailable. In this paper, we describe this approach and show its effectiveness. In addition, we present an application we have developed that can be used for this reason.

Abstract: In Egypt irrigation water is becoming more scarce with the continuously increasing demand for agriculture, domestic and industrial purposes. To face this increasing irrigation demand, the available water supply in Egypt is supplemented by the reuse of agricultural drainage water as in El-Salam Canal that do not satisfy water quality standards defined for the canal. This paper introduces an automation system for El-Salam Canal to control the flow of the fresh water and drainage water supplied to the canal. This automatic control system (ACS) is able to process data of various flows and water quality data along the canal. This control system is represented by a canal computer model. This system computes the required control actions at the Damietta branch and the feeding drains. It is also able to generate optimum solutions for the canal to satisfy the pre-defined canal conditions and standards.

Abstract: The traditional production planning model based upon the famous linear programming formulation has been well known in the literature. The consideration of uncertainty in manufacturing systems supposes a great advance. Models for production planning which do not recognize the uncertainty can be expected to generate inferior planning decisions as compared to models that explicitly account the uncertainty. This paper deals with production planning problem with fuzzy parameters in both of the objective function and constraints. We have a planning problem to maximize revenues net of the production inventory and lost sales cost. The existing results concerning the qualitative and quantitative analysis of basic notions in parametric production planning problem with fuzzy parameters. These notions are the set of feasible parameters, the solvability set and the stability set of the first kind.
Efficient Spam Filtering System Based on Smart Cooperative Subjective and Objective Methods  [PDF]

Most of the spam filtering techniques are based on objective methods such as the content filtering and DNS/reverse DNS checks. Recently, some cooperative subjective spam filtering techniques are proposed. Objective methods suffer from the false positive and false negative classification. Objective methods based on the content filtering are time consuming and resource demanding. They are inaccurate and require continuous update to cope with newly invented spammer’s tricks. On the other side, the existing subjective proposals have some drawbacks like the attacks from malicious users that make them unreliable and the privacy. In this paper, we propose an efficient spam filtering system that is based on a smart cooperative subjective technique for content filtering in addition to the fastest and the most reliable non-content-based objective methods. The system combines several applications. The first is a web-based system that we have developed based on the proposed technique. A server application having extra features suitable for the enterprises and closed work groups is a second part of the system. Another part is a set of standard web services that allow any existing email server or email client to interact with the system. It allows the email servers to query the system for email filtering. They can also allow the users via the mail user agents to participate in the subjective spam filtering problem.

Comparative prevalence of pathogenic and spoilage microbes in chicken sausages from Egypt and Greece  [PDF]

This study investigated the spread of foodborne pathogens: Listeria monocytogenes, Es-cherichia coli O157:H7, Staphylococcus aureus and Salmonella in chicken sausage samples collected from retail markets in Greece and Egypt during 2006 and from Egypt through 2010. Other microbiological parameters; total viable count (TVC), lactic acid bacteria (LAB), pseudomonads (PS), staphylococci (STAPH), Brochothrix thermosphacta (BT), Enterobacteriaceae (EN), Escherichia coli (EC), yeasts and moulds (Y&M) were also counted. Egyptian chicken sausage samples were found to harbor L. mono- cytogenes, Staph. aureus and E. coli O157:H7; with frequencies equivalent to 24%, 60% and 26% of the total samples during 2006 and 37.87%, 64.44% and 41.11% of the total samples during 2010, respectively, while Greek samples were entirely free of theses pathogens. Enrichment techniques indicated the absence of Salmonella from both Greek and Egyptian samples. The obtained results may mobilize food producers and handlers in developing countries to take the due measures reducing food-borne pathogen risks and spoilage flora alongside the poultry chain.

Smart Street Lighting Control and Monitoring System for Electrical Power Saving by Using VANET  [PDF]

The huge amount of electrical power of many countries is consumed in lighting the streets. However, vehicles pass with very low rate in specific periods of time and parts of the streets are not occupied by vehicles over time. In this paper, we propose a system that automatically switches off the light for the parts of the streets having no vehicles and turns on the light for these parts once there are some vehicles that are going to come. Logically, this system may save a large amount of the electrical power. In addition, it may increase the lifetime of the lamps and reduce the pollutions. This system automatically controls and monitors the light of the streets. It can light only the parts that have vehicles and help on the maintenance of the lighting equipments. Vehicular Ad-Hoc Networks (VANET) make it possible to propose such system. VANET enables the possibility to know the presence of vehicles, their locations, their directions and their speeds in real time. These quantities are what are needed to develop this system. An advantage of using VANET is that there is no need to use specific network and equipments to design the system, but VANET infrastructure will be used. This decreases the cost and speed up the deployment of such system. This paper focuses on the proposal of different possible architectures of this system. Results show that the saved energy may reach up to 65% and an increase of the lifetime of the lamps of 53%.

The Wave Properties of Zuara Coast and Their Effects on the Marine Navigation  [PDF]

Waves are the most important phenomena affecting marine navigation, either in the field of fishing, military or transporting of goods. This paper tries to answer the following important questions: What are the causes and types of waves in the coast of Zuaracity? What are their characteristics? And how do those waves affect the marine navigation and human activities on this coast? The research finds significant results devoted in that: the coast is exposed to on type of waves; wind waves. Zuara coast has never been exposed to waves of Tsunami or landslides. The largest size of the wave forms in winter season due to the wind of north-west which is the fastest wind type that the Libyan coast is exposed to. However, the highest speed is up to 65 knots accompanied with waves reach a height of more than 7 meters. The research also classifies the wind speeds that lead to cancelling ships and boats trips that depend on this work is studying the waves in Zuara coast and the relationship with the waves of Libyan coast and Mediterranean sea. Also, it focuses on the effect of waves on boat speed, design, fuel consumption, and other effects.

Income Disparities: The Case of Unskilled Workers in Canada (1996-2010)  [PDF]

Abstract: In this paper we analyse the gaps in economic welfare that exist between skilled and unskilled labor in Canada. Following the work of Chardon [1] [2]?and Amossé and Chardon [3], we use compe- tency levels as defined in the National Classification of Occupations to distinguish these two groups and then analyse the income disparities that exist between them. Our main findings show that unskilled workers are worse off economically than their skilled counterparts and that the Canadian workforce seems to be more bipolarized than the Canadian population as a whole. We also find strong intra-categorical inequalities within unskilled labor, workers from the sales and services occupational domain being at a disadvantage relative to their peers in other occupational groups. Finally, we show that state intervention, through taxation and social transfers, plays an important role in tightening the inter-categorical and intra-categorical income gaps.
