Unique type of isolated cardiac valvular amyloidosis

Abstract: A 72 years old gentleman underwent urgent aortic valve replacement. Intraoperatively, a lesion was found attached to the inferior surface of his bicuspid aortic valve.Histopathology examination of the valve revealed that the lesion contained amyloid deposits, identified as AL amyloidosis. The serum amyloid A protein (SAP) scan was normal and showed no evidence of systemic amyloidosis. The ECG and echocardiogram were not consistent with cardiac amyloidosis.Two major types of cardiac amyloidosis have been described in literature: primary-myelomatous type (occurs with systemic amyolidosis), and senile type(s). Recently, a localised cardiac dystrophic valvular amyloidosis has been described. In all previously reported cases, there was a strong association of localised valvular amyloidosis with calcific deposits.Ours is a unique case which differs from the previously reported cases of localised valvular amyloidosis. In this case, the lesion was not associated with any scar tissue. Also there was no calcific deposit found. This may well be a yet unknown type of isolated valvular amyloidosis.Amyloid deposition in heart is a common occurrence in systemic amyloidosis. But localised valvular amyloid deposits are very uncommon. It was only in 1922 that the cases of valvular amyloidosis were reported [1]. Then in 1980, Goffin et al reported another type of valvular amyloidosis, which he called the 'dystrophic valvular amyloidosis'. We report a case of aortic valve amyloidosis which is different from the previously described isolated valvular amyloidosis.A 72 year old gentleman was admitted with shortness of breath and palpitations via accident & emergency department. He was found to be in atrial fibrillation and in left ventricular failure. He was a hypertensive diabetic with poorly controlled blood sugar. Examination revealed an ejection systolic murmur. Abstract:

OBJECTIVES: To evaluate outcome and risk factors, particularly the (APATCHE II) score in elderly patients after admission to a geriatrics intensive care unit (ICU). Methods: A cross sectional study of patients ≥ 60 years admitted to the intensive care unit (ICU) of the Geriatrics department at Ain Shams University Hospital over 2 years period. We recorded age, sex, previous medical history, primary diagnosis, date of admission and discharge or death and APACHE II score on admission. Results: 202 patients admitted to the ICU were studied. The mean ICU mortality rates for these patients were (32, 5%), the mean APATCHE II score was (19.07). 27.3% of patients who died had hypokalemia and 43.2% had hyponatremia. Conclusion: ICU mortality rate are higher in elderly patients particularly with long ICU stay and hyponatremia.

Abstract: Sons and Lovers, the third novel of D.H. Lawrence is a masterpiece in English literature. Though it has received severe criticism, it is one of the most highly acclaimed novels of Lawrence. Lawrence wrote I have patiently and laboriously constructed the novel . It has been widely thought of as a simply and directly [1] autobiographical novel by an author who invariably included in his fiction his own experiences and portraits of people he knew. In Sons and Lovers Lawrence analyzes the growth, development of three man-woman relationship. Among the three major women characters Mrs Gertrude Morel is the most important. This paper aims to discuss her effort to realize herself.

New Word Vector Representation for Semantic Clustering Une nouvelle représentation vectorielle pour la classi cation sémantique

Abstract: The idea we defend in this paper is the possibility to obtain signi cant semantic concepts using clustering methods. We start by de ning some semantic measures to quantify the semantic relations between words. Then, we use some clustering methods to build up concepts in an automatic way. We test two well known methods: the K-means algorithm and the Ko- honen maps. Then, we propose the use of a Bayesian network conceived for clustering and called AutoClass. To group the words of the vocabulary in various classes, we test three vector representations of words. The rst is a simple contextual representation. The second associates to each word a vector which represents its similarity with each word of the vocabulary. The third representation is a combination of the rst and the second one.
HYPOGLYCEMIA IN LIVER CIRRHOSIS

Abstract: Objectives: To determine the frequency of hypoglycemia inpatients with advanced cirrhosis and its possible precipitating factors. Design: It was a simple descriptive study.Setting: Ist Medical Unit Nishtar Hospital Multan. Period: July 1997 to July 1998. Material and Methods:Fifty patients with advanced cirrhosis (child B and C) were studied. Diabetic patients, those who had fastingplasma glucose above normal and those on I/V dextrose infusion were excluded from study. Results: 34 (68%)patients had child C disease and 16 (32%) patients had child B disease. On the basis of plasma glucose level,patients were divided into 3 groups. Group I (fasting plasma glucose <50 mg%) included 3 (6%) patients. Allhad symptoms of hypoglycemia and evidence of sepsis (spontaneous bacterial peritonitis 2, Urinary tractinfection 1). 18 (36%) patients in the second group (blood glucose 50-60 mg%) and 79 (58%) patients in thethird group (blood glucose >60 mg%) had no compounding factors. Their ages were between 35 and 63 years.Conclusions: Fasting hypoglycemia was seen only in decompensated cirrhosis. It was more likely to occur inthe presence of complicating factors response to hypoglycemia was intact.
The role of rational immunotherapy for renal cell carcinoma

Abstract: Metastases to distant organs are the principal cause of death from renal cell carcinoma (RCC). No commonly accepted therapy is available for disseminated RCC at present. The rationale for immunotherapy of RCC is based on the fact that there is no other therapy for advanced cases. Biologic therapies are the only current treatment modalities that have produced promising therapeutic results in metastatic RCC (mRCC). Therapy with cytokines usually has typical and sometimes severe side effects. Response rates and toxicity were higher with combined therapy. The administration of cytokines that augment the function of the immune system can be accomplished safely and without toxicity, provided a rational approach is used. The toxic effects that are frequently observed with combined therapy emphasize the need for careful selection of patients.
