Oseltamivir-resistant pandemic (H1N1)2009 in Yemen - case report

Abstract: History, physical examination and laboratory investigations including Complete Blood Count, chest x-ray, blood cultures, CSF examination, LFTs, RFTs, blood for sugar, H1N1 test and oseltamivir resistance test.Nasal swabs indicated positivity on both H1N1 test and the RNP gene (Human R Nase P gene that serves as internal positive control for Human RNA. Both clinical specimens presented the mutation S31N in the M2 gene associated with resistance to adamantanes and H274Y in NA gene associated with resistance to oseltamivir. This was the first diagnosed case of resistance to oseltamivir in Yemen and also it is the first reported case of oseltamivir resistance virus in the Eastern Mediterranean Region.The pattern of resistance found in the oseltamivir resistant isolate collected from Yemen is the same as has been reported elsewhere in other WHO regions. Clinical description and outcomes are not different from what is described elsewhere.During the influenza season of 2007-08, oseltamivir-resistant influenza A (H1N1) viruses emerged in several countries in Europe, North America, and Asia. Despite substantial prevalence of oseltamivir-resistant viruses, few data are available on the clinical profile of subjects infected with these viruses [1]. In this paper we describe the first oseltamivir-resistant pandemic (H1N1) 2009 influenza virus from the Eastern Mediterranean Region including Yemen and determine the evidence of clinical presentation of children infected with these viruses regarding the occurrence of oseltamivir-resistance.3 years old female patient, known case of congenital heart disease with pulmonary hypertension and cerebral palsy was admitted to the hospital on 7th October 2009 with high grade of fever, cough, sore throat, difficulty in breathing and body aches. Clinical examination: patient was semiconscious, disoriented and severe pale. Heart: Pan-systolic-murmur. Chest: bronchial breathing and crepitation on the right lung. Abdomen: soft, mild hepatomegaly.
Clinical and Histopathological Characteristics of Gastric Adenocarcinoma in Yemeni Patients: A 2 Years Prospective Study  [PDF]

Objectives: To study the clinical and the histopathological characteristics of gastric cancer concentrating more on the gastric adenocarcinoma. Design: Prospective study of one hundred thirty Yemeni patients confirmed with gastric malignancies during the period from July 2016 to July 2017. Setting: Specialized center of gastrointestinal and liver diseases in Ibb city, Yemen. Subjects: All patients with histopathologically proved gastric cancer. Results: We collect 130 patients with gastric cancer during the study period, they were 93 males and 37 females with age ranged between 25 years and 100 years and the mean age was 64.98 ± 15.15 years. The most frequent clinical presentations of our patients were epigastric pain (81.5%), weight loss (74.6%), dysphagia (46.9%), vomiting (49.2%), palpable epigastric mass (19.2%) and hematemesis/melena (20%). Proximally located gastric cancer represented 46.1%; 48.9% of males patients and 38.9% of females patients. Adenocarcinoma constituted 92.4%, of which the intestinal type was 82.5% and the diffuse type was 17.5%. Squamous cancer accounted in 1.5%, and the non-epithelial tumor was in 6.1%, of which 4.6% were lymphoma and 1.5% were GIST. Conclusion: Adenocarcinomas are the major histological type of gastric cancer and represent 92.4% of all gastric malignancies in which intestinal type represented 82.5% and diffuse type represented 17.5%, and proximally located gastric cancer within stomach was the commonest in both sexes.

Dual identities in fractional difference calculus within Riemann

Abstract: We investigate two types of dual identities for Riemann fractional sums and differences. The first type relates nabla- and delta-type fractional sums and differences. The second type represented by the Q-operator relates left and right fractional sums and differences. These dual identities insist that in the definition of right fractional differences, we have to use both nabla and delta operators. The solution representation for a higher-order Riemann fractional difference equation is obtained as well.

Abstract: The tasks of semantic web service (discovery, selection, composition, and execution) are supposed to enable seamless interoperation between systems, whereby human intervention is kept at a minimum. In the field of Web service description research, the exploitation of descriptions of services through semantics is a better support for the life-cycle of Web services. The large number of developed ontologies, languages of representations, and integrated frameworks supporting the discovery, composition and invocation of services is a good indicator that research in the field of Semantic Web Services (SWS) has been considerably active. We provide in this paper a detailed classification of the approaches and solutions, indicating their core characteristics and objectives required and provide indicators for the interested reader to follow up further insights and details about these solutions and related software.

Abstract: In the second volume of the monumental biography R. Skidelsky established on J. M. Keynes, we can find an enigmatic, if not strange, passage. According to R. Skidelsky, J. R. Commons was “an important, if unacknowledged influence on Keynes”. This feeling of strangeness is even stressed by the remarks expressed by J. M. Keynes himself in a letter that remained to J. R. Commons.In that letter indeed, dated from the 26th April 1927, Keynes was writing, “there seems to me to be no other economist with whose general way of thinking I feel myself in such a genuine accord”. R. Skidelsky’s passage, as well as J. M. Keynes’ epistolary sentiments, raise a lot of questions to which this thesis is attempting to answer: First of all, when, and how did the relationship between J. M. Keynes and J. R. Commons begin? Then, if there actually was an influence by J. R. Commons on J. M. Keynes, at which level must it be situated? Eventually, insomuch as J. R. Commons represents one of the central figures of the American Historical Institutionalism and beyond the relationship between J. R. Commons and J. M. Keynes, can one consider that not only the institutions play a fundamental role in J. M. Keynes’ economic thought, but also that his conception of institutions is itself peculiar?In order to answer these questions, one must now seriously reconsider the evolution of J. M. Keynes’s economic thought in the light of J. R. Commons influence. We suggest to call this influence “the Commons Hypothesis”. Accordingly, J.M. Keynes’ economic thought is revisited through the concept of collective action as one among the incontrovertible ways of the resolution of modern capitalism “Economic Problem” that leads to a Reasonable capitalism. That way, which acknowledges the dynamics of conflicts and compromises, necessarily goes through the institutions as entities of mediation both between the micro and macroeconomic levels and between the groups of economic agents and the State.
On the existence of finite critical trajectories in a family of quadratic differentials

Abstract: In this note, we discuss the possible existence of finite critical trajectories connecting two zeros a(t) and b(t) of a family of quadratic differentials satisfying some properties. We treat the cases of holomorphic and meromorphic quadratic differentials, and we give new proofs concerning the supports of limit measures of the root-counting measures of the generalized Laguerre and Jacobi polynomials with varying parameters.
On Multiplicative Fractional Calculus

Abstract: We set the main concepts for multiplicative fractional calculus. We define Caputo, Riemann and Letnikov multiplicative fractional derivatives and multiplicative fractional integrals and study some of their properties. Finally, the multiplicative analogue of the local conformable fractional derivative and integral is studied.

On conformable fractional calulus

Abstract: Recently, the authors Khalil, R., Al Horani, M., Yousef. A. and Sababheh, M., in " A new Denition Of Fractional Derivative, J. Comput. Appl. Math. 264. pp. 6570, 2014. " introduced a new simple well-behaved definition of the fractional derivative called conformable fractional derivative. In this article we proceed on to develop the definitions there and set the basic concepts in this new simple interesting fractional calculus. The fractional versions of chain rule, exponential functions, Gronwall's inequality, integration by parts, Taylor power series expansions, Laplace transforms and linear di?erential systems are proposed and discussed.
