Simple and safe heparin regimen for acute ischaemia

Abstract: the risk/benefit ratio of acute anticoagulation in ischaemic cerebro-vascular disease is not clearly established. a simple and safe intermittent intravenous heparin regimen (20000 iu daily) was used prospectively in 50 patients of 57 ± 14 (m ± sd) years of age whose blood pressures ranged from normal to severe hypertension. twenty-two patients had cardiogenic embolism and the remaining had recurrent severe transient ischaemic attacks of recent onset or progressive cerebral infarcts. time of exposure to heparin was 6.4 ± 4 (m±sd) days. two patients had recurrences of cerebral thromboembolism and none had bleeding complications. this is a safe and efficient method of anticoagulation for patients with cerebral ischaemia when continuous infusion of heparin or close monitoring of clotting times are not used routinely.

Abstract: O presente estudo tem como objetivo discutir se é necessária a forma o em Educa o Especial, como aponta a legisla o, para a atua o do pedagogo na Classe Hospitalar. Para tanto, foi realizada uma pesquisa qualitativa de fundamenta o fenomenológica, que buscou, por meio de entrevistas com duas professoras que atuam em Classe Hospitalar, analisar e discutir a pertinência ou n o da forma o em Educa o Especial na atua o profissional do pedagogo no hospital. Formar-se professor de Classe Hospitalar parece ser um aprendizado que vai muito além dos ensinamentos adquiridos na gradua o em Pedagogia e nas antigas habilita es em Educa o Especial, hoje extintas, sendo que a forma o continuada deve encontrar-se presente para e na atua o do professor da Classe Hospitalar. O estudo mostrou que há a necessidade de mais pesquisas referentes à forma o de professores para atua o em Classe Hospitalar. Surround suppression and sparse coding in visual and barrel cortices

Abstract: During natural vision the entire retina is stimulated. Likewise, during natural tactile behaviors, spatially extensive regions of the somatosensory surface are co-activated. The large spatial extent of naturalistic stimulation means that surround suppression, a phenomenon whose neural mechanisms remain a matter of debate, must arise during natural behavior. To identify common neural motifs that might instantiate surround suppression across modalities, we review models of surround suppression and compare the evidence supporting the competing ideas that surround suppression has either cortical or sub-cortical origins in visual and barrel cortex. In the visual system there is general agreement lateral inhibitory mechanisms contribute to surround suppression, but little direct experimental evidence that intracortical inhibition plays a major role. Two intracellular recording studies of V1, one using naturalistic stimuli (Haider et al., 2010), the other sinusoidal gratings (Ozeki et al., 2009), sought to identify the causes of reduced activity in V1 with increasing stimulus size, a hallmark of surround suppression. The former attributed this effect to increased inhibition, the latter to largely balanced withdrawal of excitation and inhibition. In rodent primary somatosensory barrel cortex, multi-whisker responses are generally weaker than single whisker responses, suggesting multi-whisker stimulation engages similar surround suppressive mechanisms. The origins of suppression in S1 remain elusive: studies have implicated brainstem lateral/internuclear interactions and both thalamic and cortical inhibition. Although the anatomical organization and instantiation of surround suppression in the visual and somatosensory systems differ, we consider the idea that one common function of surround suppression, in both modalities, is to remove the statistical redundancies associated with natural stimuli by increasing the sparseness or selectivity of sensory responses.
Análises visual e volumétrica por ressonancia magnética das forma??es hipocampais em um grupo de pacientes com diagnóstico clínico de epilepsia do lobo temporal

Abstract: the purpose of this study is to test the sensitivity of the volumetric analysis compared to the visual analysis of the hippocampal formations of a group of 153 patients with clinical diagnosis of temporal lobe epilepsy and candidates to temporal lobectomy, evaluated by magnetic resonance (mr), using a 0.5 tesla machine and inversion-recovery t1-weighted images and 5 mm coronal slices. there was agreement between the prospective visual analysis and another retrospective visual analysis carried out by two independent observers (c=0.748 and c=0.720). there was also agreement between the retrospective analysis of the two investigators (c=0.733). there was genuine agreement (c=0.788) between the results of the qualitative and quantitative analyses carried out prospectively. volume measurements showed a nonsignificant tendency to lateralize more cases of clinically presumed hippocampal atrophy. our results confirm the reliability of a qualitative visual analysis and indicate the utility of hippocampal volumetry as a supplementar, objective and quantitative measure of hippocampal sclerosis.

Abstract: a 33-year-old female with neurofibromatosis and intracranial vascular lesions of the moya-moya type is reported. clinical and angiographic aspects of both syndromes are discussed with respect to earlier reports of this rare association, with the conclusion that it produces slow intelectual and motor deterioration in adolescents or young adults. there are no specific guidelines to therapy, and the prognosis is mixed, some patients ceasing to progress at least for a few years.
Lateraliza??o da área epileptogênica através de ressonancia magnética na epilepsia do lobo temporal

Abstract: we studied 5 patients with medically refractory temporal lobe epilepsy using special methods of mri, in order to localize the epileptogenic area. coronal sequences were obtained with 5 mm sections from the posterior margin of the brainstem to the base of the frontal lobe using: "inversion recovery t1" (tr=2500 msec, te=26 msec, t1=600 msec) to analyse loss of internal structure and atrophy of the hippocampus, and morphology of the temporal lobe; and spin-echo t2w (tr=250 msec, te=120 msec) looking for abnormal high intensity signal. in two cases with left focal eeg the mri showed atrophy, loss of internal structure and abnormal high intensity signal in the hippocampus, and atrophy of the anterior temporal lobe, ipsilateraly. one case with independent bilateral foci at the eeg showed atrophy, loss of internal structure and abnormal high intensity signal in the right hippocampus. other case with right focal eeg showed bilateral atrophy of the anterior temporal lobes. another case was normal. we conclude that this methodology can facilitate the surgical indication in temporal lobe epilepsy.

Os autores destacam a raridade da ocorrência de pseudo-aneurisma da artéria carótida comum pós-traumático e a associa o de pseudo-aneurisma com osteocondroma femoral. Enfatizam a importancia dos métodos por imagem, em especial a angiotomografia, no diagnóstico de afec es vasculares, como na ocorrência de pseudo-aneurismas. Realizam, ainda, revis o bibliográfica pertinente ao assunto.

Abstract: Visando comparar a sensibilidade da avalia o volumétrica com a análise visual na avalia o por ressonancia magnética (RM) das forma es hipocampais de pacientes com epilepsia do lobo temporal refratária e candidatos à lobectomia temporal, estudamos 153 casos com diagnóstico clínico de epilepsia do lobo temporal, utilizando um equipamento de 0,5 Tesla, com técnica de inversion-recovery ponderada em T1, com cortes de 5 mm no plano coronal. Houve boa concordancia entre a análise visual prospectiva e outra retrospectiva, realizada por dois observadores independentes (C=0,748 e 0,720). Houve também concordancia entre a análise retrospectiva dos dois observadores (C=0,733). Houve ainda concordancia genuína (C=0,788) entre os resultados das análises quantitativa e qualitativa realizadas prospectivamente. A análise quantitativa demonstrou uma tendência n o-significativa a lateralizar mais casos de atrofia hipocampal presumida clinicamente. Nossos resultados confirmam a confiabilidade da análise visual qualitativa e indicam a utilidade da volumetria hipocampal como uma medida suplementar, objetiva e quantitativa, de esclerose hipocampal.

Abstract: Antisense oligodeoxynucleotides (oligos) are the tools that bind to complemen tary sequence of targeted mRNA and block specifically protein translation. In the present study, a novel 20 mer oligo as an antisense for human IL-13 is introduced. This oligo is designed according to the IL-13 mRNA coding region and synthesized in two HPLC purified and FITC conjugated forms. Fluorescence oligo cell uptake is confirmed using flowcytometry and confocal microscopy, and cytotoxicity evaluation is performed us ing BrdU proliferation assay. Human tonsilar B-lymphocytes are purified by positive selection using magnetic cell sorting method and cultured with anti CD40 monoclonal antibody plus rIL-4 to induce IL-13 production. IL-13 antisense is added to medium and Real Time PCR for mRNA, and ELISA for protein assays. Data indicate that antisense application leads to down regulation and complete suppression of IL-13 pro tein with no significant effects on mRNA, suggesting in vitro protein translation arrest. Since 11-13 is a crucial cytokine in allergic conditions, we conclude that interference with the protein synthesis by a nontoxic and efficient antisense oligo can provide an available tool for the investigators on allergic diseases.
