Paracoccidioidomicose do sistema nervoso central: relato de caso

Abstract: O envolvimento do sistema nervoso central na paracoccidioidomicose vem ganhando importancia dentro do espectro de manifesta es clínicas da doen a. O primeiro registro na literatura de que o Paracoccidioides brasiliensis seria também capaz de comprometer órg os do sistema nervoso central foi descrito em 1919 por Pereira & Jacobs; desde ent o muitos outros estudos têm demonstrado esta forma de apresenta o clínica, sendo que em alguns sua frequência atinge os 27,27%. Descrevemos o caso de uma paciente de 34 anos, residente em Juiz de Fora-MG, admitida com quadro clínico compatível com pneumonia e que durante a interna o iniciou sintomatologia cerebelar (náuseas, v mitos, ataxia de marcha e dismetria). A tomografia computadorizada de cranio revelou les o cerebelar sugestiva de abscesso. O tratamento proposto baseou-se na exerese cirúrgica da les o e, como a anátomo-patologia mostrasse presen a de leveduras em gemula o múltipla ("roda de leme de navio"), diagnóstico de neuroparacoccidioidomicose p de ser firmado, e o tratamento complementado pela associa o de sulfametoxazol-trimetoprim. A paciente evoluiu bem, persistindo discreta paresia e hipoestesia em membro superior direito.

Abstract: this study had the aim of ascertaining the situation regarding dentistry courses in brazil that are involved in the national reorientation of vocational healthcare training (pro-saúde), in relation to different learning spaces. document analysis was performed on the reports on the courses included in pro-saúde i. the item "teaching activity within the service network" and the timetabled workload of the course were the inclusion criteria. out of 20 reports, 18 were included. there was diversity among the timetabled workloads of the higher education institutions for the primary care course, although all the institutions had students within this scenario. five courses also included hospitals and specialized dentistry centers, and 11 included community projects. the incorporation of healthcare services as diversified learning scenarios varied, reflecting the different interpretations given regarding its importance, with consensus regarding the expectations generated by the pro-health stimulus.

Abstract: this is a research with adolescents from a public municipal school located in a district of the north area of minas gerais brazil, with the objective of understanding how they live and they exercise their sexuality. the study was developed through of playful-pedagogical workshops in the qualitative approach. the results show that the sexuality concept is limited to the sexual relationship between two people of opposite sex. the students emphasized the risk of an undesired pregnancy and they recognized the importance of the use of contraceptive methods. the workshops have propitiated a favorable environment to discuss of attitude changes on the adolescents through the information, reflection and expression of ideas and feelings, representing a process to be complemented by the family, school and local social politics.

Abstract: the involvement of the central nervous system in paracoccidioidomycosis is more frequent than previously thought. the first reference to the possibility that paracoccidioides brasiliensis could affect the central nervous system was by pereira & jacobs in 1919. since then, a great number of other studies has showed this form of clinical behavior and, in some of them, the frequency has ranged 27.27%. we report a clinical case of a 34-year-old white brazilian woman admitted because of bacterial pneumonia. in the sixth day of admission, the patient developed cerebellar symptomatology with nausea, vomiting, dysmetria and gait disturbance. central nervous system computer tomographic scanning disclosed a hypodense lesion in the right cerebellar hemisphere. the patient was submitted to surgery with total excision of the lesion. histopathological examination confirmed the diagnosis of neuroparacoccidioidomycosis. coadjuvant treatment with sulfamethoxazole-trimetoprim was introduced. the patient had a good outcome and was discharge 30 days after surgery.

Abstract: the lack of compliance with treatment and the consequent lack of control of hypertension represent a major challenge to primary care in brazil. understanding the subjective relationship of patients with hypertension can expand the possibilities of more effective control measures. the aim was to understand and analyze the social representations of women with hypertension about the disease and their family environment. study participants were 26 women diagnosed with hypertension and registered in a unit of primary health care in the city of porto firme, brazil. the study was based on qualitative research and elected social representations as theoretical and methodological principle. the data, collected through focus groups, interviews and notes in a diary, were examined through content analysis. in the analysis emerged semantic attractions depicting the representations of women on hypertension - psychological effects and feelings related to the diagnosis of hypertension; daily life of patients with hypertension and changes in their daily lives; and family life with hypertension - through which have identified different dimensions and spaces connected to membership, both individually and collectively, emphasizing access to information associated with social support as a factor in favor of compliance with the treatment.
Oviposition and Embryotoxicity of Indigofera suffruticosa on Early Development of Aedes aegypti (Diptera: Culicidae)

Abstract: Aqueous extract of Indigofera suffruticosa leaves obtained by infusion was used to evaluate the oviposition, its effect on development of eggs and larvae, and morphological changes in larvae of Aedes aegypti. The bioassays were carried out with aqueous extract in different concentrations on eggs, larvae, and female mosquitoes, and the morphological changes were observed in midgut of larvae. The extract showed repellent activity on A. aegypti mosquitoes, reducing significantly the egg laying by females with control substrate (343 (185–406)) compared with the treated substrate (88 (13–210)). No eclosion of A. aegypti eggs at different concentrations studied was observed. The controleclodedin 35%. At concentration of 250 μg/mL, 93.3% of larvae remained in the second instar of development and at concentrations of 500, 750, and 1000 μg/mL the inhibitory effect was lower with percentages of 20%, 53.3%, and 46.6%, respectively. Morphological changes like disruption on the peritrophic envelope (PE), discontinued underlying epithelium, increased gut lumen, and segments with hypertrophic aspects were observed in anterior region of medium midgut of larvae of A. aegypti. The results showed repellent activity, specific embryotoxicity, and general growth retardation in A. aegypti by medium containing aqueous extract of I. suffruticosa leaves.

Abstract: otorhinolaryngologics manifestations of rheumatologic disorders represent a diagnostic challenge for the rheumatologist, the otorhinolaryngologist, and the general practicioner. not uncommonly otorhinolaryngologics symptoms represent the initial sign of an otherwise asymptomatic or even undiagnosed autoimmune disorder which often calls for prompt and aggressive immunosuppressive treatment. hearing disturbances may be seen in patients with systemic lupus erythematosus, wegener's granulomatosis, relapsing polychondritis, polyarteritis nodosa, cogan's syndrome, sj?gren's syndrome, and less frequently in churg-strauss syndrome and beh?et's disease. nose and paranasal sinuses are variably affected during the course of wegener's granulomatosis, churg-strauss syndrome, relapsing polychondritis and sarcoidosis. recurrent mucosal ulcerations are common in systemic lupus erythematosus and beh?et's disease. xerostomia is a common feature of primary and secondary sj?gren's syndrome; salivary gland enlargement may be also seen in these patients, as well as in patients with sarcoidosis. the cricoarytenoid joint can be involved during the course of rheumatoid arthritis, ankylosing spondylitis and gout; osteoarthritic changes have also been described. trigeminal nerve dysfunction may occur in patients with sj?gren's syndrome, systemic sclerosis, systemic lupus erythematosus and mixed connective tissue disease. peripheral facial nerve palsy has been described to complicate the course of sj?gren's syndrome and sarcoidosis.
Aging increases Oxidative Stress and Renal Expression of Oxidant and Antioxidant Enzymes that Are Associated with an Increased Trend in Systolic Blood Pressure

Abstract: The aim of this study was to investigate whether the effects of aging on oxidative stress markers and expression of major oxidant and antioxidant enzymes associate with impairment of renal function and increases in blood pressure. To explore this, we determined age-associated changes in lipid peroxidation (urinary malondialdehyde), plasma and urinary hydrogen peroxide (H2O2) levels, as well as renal H2O2 production, and the expression of oxidant and antioxidant enzymes in young (13 weeks) and old (52 weeks) male Wistar Kyoto (WKY) rats. Urinary lipid peroxidation levels and H2O2 production by the renal cortex and medulla of old rats were higher than their young counterparts. This was accompanied by overexpression of NADPH oxidase components Nox4 and p22phox in the renal cortex of old rats. Similarly, expression of superoxide dismutase (SOD) isoforms 2 and 3 and catalase were increased in the renal cortex from old rats. Renal function parameters (creatinine clearance and fractional excretion of sodium), diastolic blood pressure and heart rate were not affected by aging, although slight increases in systolic blood pressure were observed during this 52-week period. It is concluded that overexpression of renal Nox4 and p22phox and the increases in renal H2O2 levels in aged WKY does not associate with renal functional impairment or marked increases in blood pressure. It is hypothesized that lack of oxidative stress-associated effects in aged WKY rats may result from increases in antioxidant defenses that counteract the damaging effects of H2O2.

Abstract: a recent survey of the species of indigofera l. occuring in the rio grande do sul state, brazil, revealed four species in the area: indigofera sabulicola benth., i. asperifolia bong., i. suffruticosa mill, and i. campestris bong. it is given a short historical review on the systematic position of the genus. identification key to the species studied, descriptions, illustrations, and data such as distribution, habitat, flowering and frutification are also- presented.

Abstract: Este artigo trata da rela o professor-aluno na educa o superior e da influência que esta recebe da gest o educacional. Foi realizada uma pesquisa de abordagem qualitativa do tipo etnográfico, englobando entrevistas e observa o de campo, com alunos e professores de um curso de Pedagogia, no segundo semestre de 2002. Como resultado, verificou-se que a gest o educacional interfere na rela o professor-aluno, em três níveis, concomitantemente: através das tomadas de decis o do gestor no processo pedagógico em geral; especificamente, em seu relacionamento com o professor e com o aluno e na orienta o deste nas suas dificuldades e vicissitudes. Conclui-se que há necessidade de coerência entre teoria e prática na organiza o do trabalho pedagógico do professor e maior integra o entre gestor e professor para melhor compatibilizar interesses e objetivos educacionais. This is a study on the teacher-student relationship in undergraduation and the educational management influence on it. A qualitative ethnographicac aproach was applied, with enfolding interviews and observations with the students and the professors of the Pedagogy course in the second semester of the last year. We noticed that the educational management interferes in the teacher-student relationship at three levels concomitantly: through the manager’s decision-making in connection with the pedagogical process in general; specifically, in the manager’s relationship with the teacher and with the student and in the orientation given to the students in relation to their difficulties and vicissitudes. It was concluded that in the organization of the professor’s pedagogic work it stands clear the need of more coherence between theory and practice and also more integration between the educational management and the professor in order to make the interests and the educational objectives more compatible.
