Abstract: background and objectives: perioperative myocardial ischemia is commonly observed, and it can increase significantly postoperative morbidity and mortality. the cardioprotective properties of volatile anesthetics and opioids have been studied during several decades and currently constitute powerful tools in the management of patients with ischemic coronariopathy. the objective of this review was to provide the fundaments of myocardial protection by preconditioning. contents: the concepts of cellular damage secondary to ischemia and reperfusion, ischemic preconditioning (ipc), and anesthetic preconditioning (apc), as well as the mechanisms of myocardial protection, are discussed. recent studies in cardiac surgery demonstrated that the use of short periods of ischemia during reperfusion can reduce the area of myocardial infarction. volatile anesthetic can also have a protective effect in myocardial reperfusion. independently of the signaling pathway that leads to preconditioning, both anesthetic and ischemic, mitochondrial dependent katp channels are considered the final mediators of cardioprotection by controlling the mitochondrial influx of calcium and, therefore, preventing the induction of necrosis and apoptosis. although ipc and apc effectively reduce the area of myocardial infarction and improve postoperative ventricular function, it is important to stress that those treatments should be instituted before ischemic events to justify their clinical applicability. conclusions: phenomena known as myocardial ischemic preconditioning and anesthetic preconditioning are well known, and the mechanism of protection is similar in both situations; however, not every step that leads to this protection has been fully explained. further studies are necessary to increase the clinical applicability of the cardioprotective properties of anesthetics.
Different Molecular Weight Chitosan-Based Membranes for Tissue Regeneration

Abstract: Natural polymers, such as chitosan, obtained from chitin, are been widely studied for use in the tissue regeneration field. This study established a protocol to attain membranes made from this biopolymer, consisting of high or low molecular weight chitosan. The biocompatibility of these membranes was histologically evaluated, comparing them to collagen membrane surgically implanted in rat subcutaneous tissue. Fifteen Holtzmann rats were divided in three experimental groups: High and Low Molecular Weight Chitosan membranes (HMWC and LMWC) and Collagen membranes (C—control group); each of them with three experimental periods: 7, 15 and 30 days. As a result, after the seven days evaluation, the membranes were present and associated with a variable degree of inflammation, and after the 15 and 30 days evaluations, the membranes were absent in all groups. It is concluded that the chitosan-based membranes were successfully attained and presented comparable resorption times to collagen membranes.

Abstract: the objective of this study was to generate information relative to the allele distribution and dynamics within and among populations of araucaria angustifolia, a naturally-occurring conifer in the south of brazil, being known popularly as "pinheiro-do-paraná", "araucaria" or pine tree. in order to elucidate the levels and the distribution of the genetic variability, the population's genetic structure and the genetic distance among natural populations of this species with different levels of disturbance in different geographical areas were studied in detail. for this, samples of leaf tissue were collected from 328 adult individuals in nine natural populations in santa catarina state. to analyze the samples, the allozyme technique was applied in starch gel electrophoresis (penetrose 13%), with citrate/morfholine buffer. nine enzymatic systems (pgm, pgi mdh, prx, skdh, 6pgdh, acp, idh and g6pdh) revealed 15 loci. the analysis provided values for he and ho of 0.084 and 0.072, respectively. the general average of polymorphic loci was 73% in the species and 26.6% in the studied populations and the allele number per locus was 1.6. wright's f-statistical estimates indicated the existence of inbreeding in populations (fis= 0.148) and a low divergence among populations (fst = 0.044). however, the inbreeding values were variable in different populations. taken together, the results indicated that the greater part of the genetic variability is contained within populations. the working hypothesis that originally there was greater genetic diversity can be supported by these results which indicate that in the degraded populations the diversity indexes are lower in the degraded populations than those found in better-conserved populations. thus the fragmentation of the forest followed by "araucaria" exploitation could have contributed to the genetic differentiation expressed through the allele frequency of the studied population.

Abstract: purpose: to evaluate the benefits of an alternative technique of anterior capsulotomy created to guarantee the complete implantation of the iol in the capsular bag, during extracapsular cataract extraction. methods: one hundred and nine eyes were operated on through this technique and followed during a period of 1 year. the possibilities of the technique were evaluated regarding two aspects: the guarantee of a perfect placement of the iol in the capsular bag and ability of providing an effective fixation of the implant through the characteristics of the anterior capsule remains. the first aspect was analyzed considering the number of cases where the two flaps could be properly observed during the implantation. the second aspect was evaluated through the positioning of the lens after 1 year. specific difficulties and complications of this technique were also investigated. results: the two flaps could be properly observed during the implantation in 96 (90.6%) cases. one year after the surgery, the lens was centered in 81.9% of the cases, slightly off the center (less than 1 mm) in 13.3% and off the center (more than 1 mm) in 4.8%. conclusion: these results, if compared to those presented in relation to other types of capsulotomy, indicate that this technique is a good alternative to provide the appropriate implantation of the lens in the bag, in extracapsular cataract extraction.
Qualidade de vida e custos diretos em pacientes com blefaroespasmo essencial e espasmo hemifacial, tratados com toxina botulínica-A

Abstract: purpose: to evaluate both the costs and the impact on the quality of life of essential blepharospasm and hemifacial spasm patients treated with botulinum toxin-a. methods: 28 patients (16 essential blepharospasm and 12 hemifacial spasm), treated with btx-a, were evaluated by means of two research tools, namely medical outcomes study short form healthy survey, and fahn disability rating scale. the costs were mainly calculated on the basis of the toxin units used by each patient during a one-year period. results: the total estimated direct cost was estimated at 1,081.62 reais per year (±89.39) for essential blepharospasm and 618.06 reais per year (±60.06) for hemifacial spasm, at an outpatient level (p<0.001). before treatment all domains of sf-36 were significantly worse in both diseases. under effective treatment with botulinum toxin-a the increases in the indices of sf-36 show changes in the general health rates of the population under study, with a statistically significant difference between the pre and post answers (p<0.05). in relating sf-36 with the fahn disability rating scale, the essential blepharospasm cases showed significant positive relationships (functional capacity, general health, emotional aspect and social aspects) while in the hemifacial spasm patients no correlation was found. conclusion: the present study demonstrates a remarkable negative impact of both diseases on the quality of life of essential blepharospasm and hemifacial spasm patients, as well as a significant improvement of both diseases after botulinum toxin-a treatment. furthermore, the research data seem to justify the high costs of botulinum toxin-a treatment in view of its considerable benefits regarding the patients' quality of life.

Abstract: OBJETIVO: Avaliar os custos e a qualidade de vida dos pacientes com blefaroespasmo essencial e espasmo hemifacial, submetidos à terapia com a toxina botulínica-A. MéTODOS: Vinte e oito pacientes, sendo 16 com blefaroespasmo essencial e 12 com espasmo hemifacial, tratados com toxina botulínica-A, foram avaliados por meio de dois instrumentos: o SF-36 e a Escala de Disfun o de Blefaroespasmo. Os custos foram calculados segundo o número de unidades da toxina utilizadas por cada paciente no período de um ano. RESULTADOS: O custo total direto foi estimado em 1081,62 reais por ano (±89,39) para blefaroespasmo essencial e 618,06 reais por ano (±60,06) para espasmo hemifacial, para tratamento ambulatorial (p<0,001). Antes do tratamento, todos os domínios do SF-36 foram significantemente piores em ambas as doen as. Sob tratamento efetivo com toxina botulínica-A, os aumentos nos índices do SF-36 evidenciaram mudan as nas taxas de saúde geral da popula o em estudo, com diferen a estatisticamente significante entre as respostas pré e pós-tratamento (p<0,05). Relacionando-se o SF-36 com Escala de Disfun o de Blefaroespasmo, foram observadas nos casos de blefaroespasmo essencial, rela es significantes positivas (capacidade funcional, estado geral, aspecto emocional e aspectos sociais), ao passo que nos pacientes com espasmo hemifacial n o foi encontrada nenhuma correla o. CONCLUS O: Este estudo demonstra notável impacto negativo das doen as na qualidade de vida desses pacientes, e uma significante melhora em ambas as doen as após tratamento com toxina botulínica-A. Além disso, os dados fornecem subsídios para justificar o alto custo da aquisi o da toxina botulínica-A, devido a seus consideráveis benefícios para a qualidade de vida.

Abstract: Este trabalho surge da necessidade de discuss es sobre o papel educacional do profissional de educa o física na aprendizagem esportiva e Pedagogia do Esporte. Relacionamos correntes teóricas da Pedagogia do Esporte com o ensino dos jogos desportivos coletivos. Para tanto, utilizamos como instrumento o jogo, ferramenta fundamental de ensino e oportunidade social, através de suas características lúdicas. O jogo pode contribuir para o desenvolvimento de valores éticos, sociais e morais, assim como para a constru o de metáforas que estabele am rela es entre o esporte e a vida, fazendo uso da ludicidade e do ensino reflexivo. Ambos fundamentam nosso embasamento teórico para a atua o em Pedagogia do Esporte, trazendo mais oportunidades, op es de escolha e reflex es sobre nossa atua o profissional. This research work derives from the need to discuss the educational role of the physical education professional in sport learning and sport pedagogy. We have related sport pedagogy theories with the teaching of team sports. We have used the game as an instrument which is a fundamental teaching tool as well as a social opportunity, through its playful characteristics. Games can contribute to the development of ethical, social, and moral values, and to the building of metaphors that can establish relations between sport and life, making use of playfulness and reflexive teaching. Both concepts are fundamental to our theoretical foundation and bring more opportunities, choice, and reflections to our professional work. Keywords: sport pedagogy – sport initiation – game – reflexive teaching Este trabajo aparece de la necesidad de discusiones a respecto del papel educativo del profesional de educación física en la ense anza deportiva y Pedagogía del deporte. Relacionamos las corrientes teóricas de la pedagogía del deporte con la ense anza de los juegos colectivos. Para eso, utilizamos como instrumento el juego, la herramienta básica de la ense anza y la oportunidad social, por medio de sus características lúdicas. El juego puede contribuir en el desarrollo de valores éticos, sociales y morales; como también en la construcción de metáforas que establecen relaciones entre el deporte y la vida, haciendo uso de lo lúdico y la educación reflexiva. Ambos fundamentan nuestro basamiento teórico para el funcionamiento en Pedagogía del deporte, trayendo más oportunidades, opciones de elección y reflexiones en nuestra actuación profesional.
