Abstract: this is the report of a five-month-old child presenting clinical evidence of pompe's disease: severe hypotonicity, hyporeflexia and congestive heart failure. the ecg showed a short pr interval, the chest radiography disclosed marked cardiomegaly, and the echocardiogram revealed marked left ventricular hypertrophy - the most typical finding of this disease. a skeletal muscle biopsy led to final diagnosis, because in the histopathologic study marked increased glycogen accumulation was evident. death occurred two months after symptom onset.

Private Incentives for Specialization in a Changing and Unpredictable Labor Market  [PDF]

Abstract: Technological change is a distinctive characteristic of modern labor markets. New technologies change the demand for different skills in the labor market and thus introduce uncertainty in the wage structure that agents will face in the future. In this way, technological change affects agents decisions about which skills to invest in. In this paper, I study how labor market uncertainty arising from technological change influences the private incentives for specialization. I show that in a world populated by risk-averse agents, technologies that generate a positive covariance of wages across sectors or tasks within sectors will strengthen the incentives for specialization, whereas technological progress that generates a negative covariance of wages will generate strong private incentives for agents to become generalists. Therefore, there is no unique relationship between technological progress and specialization. The nature of the new technologies introduced in the labor market is what matters.
Trombose de prótese biológica mitral: importancia do ecocardiograma transesofágico no diagnóstico e acompanhamento pós-tratamento

Abstract: OBJETIVO: Descrever os achados clínicos e ecocardiográficos de trombose em prótese biológica mitral e o valor do ecocardiograma transesofágico (ETE) no diagnóstico e monitora o da lise do trombo. MéTODOS: Foram estudados ao ETE 111 pacientes com prótese biológica mitral, e incluídos 4 de 7 com suspeita de trombo nestas próteses (idade média = 60,2±10,2 anos; 2 homens). O diagnóstico p de ser confirmado pela realiza o seriada do ETE e pela evolu o clínica. Investigaram-se os aspectos morfológicos dos folhetos da prótese, e presen a e característica das massas ecogênicas anexas. Foram obtidos o gradiente médio pela prótese e a área valvar. RESULTADOS: O diagnóstico de trombo em prótese biológica mitral foi realizado 48,7±55,2 meses após a cirurgia. Dois pacientes apresentaram acidente vascular encefálico isquêmico no pós-operatório imediato. O gradiente médio global estava elevado (11,4±3 mmHg) e a área valvar reduzida (1,24±0,3cm2). Ao ETE, em todos foram evidenciadas massas ecogênicas em face ventricular esquerda da prótese biológica mitral, sugestivas de trombo. No ETE seriado (136±233 dias), houve desaparecimento do trombo em dois casos e redu o do trombo nos outros dois, após tratamento. O gradiente médio reduziu para 6,2±3 mmHg (p=0,004; IC 95%), e a área valvar aumentou para 2,07±0,4 (p=ns). CONCLUS O: O ETE mostrou-se útil na detec o de trombo em prótese biológica mitral e eficaz na monitora o do tratamento, em todos casos.

Abstract: objective: to report the clinical and echocardiographic findings of bioprosthetic mitral valve thrombosis and the value of transesophageal echocardiography (tee) in its diagnosis and monitoring of thrombolysis. methods: one hundred and eleven patients with mitral bioprostheses underwent tee, and 4 out of 7 suspected of having a thrombus on these prostheses were included in the study (mean age = 60.2±10.2 years; 2 men). the diagnosis was confirmed with serial tee and clinical evolution. the morphologic features of the prosthetic leaflets, as well as the presence and characteristics of attached echogenic masses were investigated. the mean gradient through the prosthesis and the valvular area were obtained. results: the diagnosis of bioprosthetic mitral valve thrombosis was established 48.7±55.2 months after surgery. two patients had ischemic stroke in the early postoperative period. the mean overall gradient was high (11.4±3 mmhg) and the valvular area reduced (1.24±0.3 cm2). on tee, echogenic masses on the left ventricular face of the mitral bioprosthesis suggestive of thrombus were evidenced in all patients. on serial tee (136±233 days), in 2 patients the thrombus had disappeared and in 2 others it was smaller after treatment, the mean gradient dropped to 6.2±3 mmhg (p = 0.004; 95% ci), and the valvular area increased to 2.07±0.4 (p = ns). conclusion: tee proved to be useful for detecting bioprosthetic mitral valve thrombosis and was effective in monitoring the treatment in all patients.

Abstract: Most researchers agree that the essence of humour lies in its social nature. Thus it is only reasonable to assume that there is a link between laughter and success in relationships with other people and circumstances. Indeed, such conclusions have been drawn. Humour is said to develop the capacity to foster a multi-perspective comprehension of life, which can in extreme situations determine the difference between surviving and perishing. The people who fled Estonia during World War II and did not want to return for fear of being repressed by the Soviet regime, were generally allowed to apply for immigration in Western countries as refugees. This article pertains to some aspects of humour used in the written press by the New York Estonian refugee community in 1949. The character of a young lady called Salme is narrating her family’s first clumsy steps in their new homeland. The body of the text is in the form of letters addressed to former schoolmates who have not yet immigrated to their new permanent countries of residence, but are presumably still living in a displaced persons’ camp in Germany. It is argued that Salme’s letters were designed to provide comic relief from tensions common to new immigrants and old refugees. By showcasing the problems, concerns and actions that most (if not all) New York refugee Estonians could relate to, they promoted a light-hearted view of the difficulties of this particular type of existence. This set of texts is also used to make observations regarding the collective identity of the writers and their perceived audience, the intra-group relationships and connections with the outer world, Americans, the City and the array of cultural differences that they entail.

Abstract: despite reform efforts, the economic performance of latin american countries during the 1990s was disappointing with the exception of chile, which grew at almost 7% per year. this paper tries to explain this difference. following recent literature that highlights the role played by institutions and policies on economic growth, we estimate a cross-section econometric model over the 1960-2005 period and find that chile's better performance can largely be explained by a combination of better institutions and reforms that have been deeper and broader in scope than those in the rest of latin america. in addition, we estimate that improving institutions in other latin american countries to the chilean standard would have increased per-capita gdp growth rates by about one and a half percentage points.

Following recent literature that highlights the role played by institutions and policies on economic growth, we estimate a cross-section econometric model over the 1960-2005 period and find that Chile's better performance can largely be explained by a combination of better institutions and reforms that have been deeper and broader in scope than those in the rest of Latin America. In addition, we estimate that improving institutions in other Latin American countries to the Chilean standard would have increased per-capita GDP growth rates by about one and a half percentage points. A pesar de las importantes reformas de la última década, el desempe o económico de América Latina durante los a os 90 fue pobre, a excepción de Chile, que creció a una tasa cercana al 7% anual. En este trabajo intentamos explicar esta diferencia de desempe o entre Chile y el resto de la región. Siguiendo la literatura reciente, que destaca el rol que tienen las instituciones y políticas sobre el crecimiento argumentamos que el mejor desempe o de Chile se debe a que el país llevó adelante reformas más profundas y comprehensivas que otros países, en particular fortaleciendo su marco institucional. Basados en un modelo econometrico de corte transversal estimado para el periodo 1960- 2005, en este trabajo argumentamos que el mejor desempe o de Chile se explica principalmente porque el país cuenta con mejores instituciones y que América Latina podría incrementar el crecimiento de su ingreso per capita promedio en un 1,5% si tuviese instituciones de calidad similar a las chilenas.

Abstract: En el presente artículo se pretende abordar diferentes cuestiones con el fin de ubicar, o familiarizar al lector sobre la problemática de la Vigorexia, patología psicológica que ha ido cobrando protagonismo en los últimos a os debido a la cada vez más creciente importancia del aspecto físico. Se comenzará con conceptos básicos sobre apariencia física e imagen corporal, derivando ésta a alteraciones de la misma en el que se presta especial atención en la aparición de la Dismorfia Muscular, también conocida como Vigorexia. Presentada la vigorexia, se abordarán varios aspectos sobre el origen da la misma y su concepto, describiendo posteriormente sus principales manifestaciones adversas y consecuencias. Es por ello, que el presente artículo tiene una finalidad informativa, ya que la actualidad de este trastorno psicológico provoca un desconocimiento general que puede originar efectos nocivos sobre la salud del deportista.
