Busulfan and melphalan as conditioning regimen for allogeneic hematopoietic stem cell transplantation in acute myeloid leukemia in first complete remission

Abstract: background: allogeneic hematopoietic stem cell transplantation with hla-identical donors has been established for the treatment of acute myeloid leukemia patients for over 30 years with a cure rate of 50% to 60%. objectives: to analyze the overall survival of patients and identify factors that influence the outcomes of this type of transplant in patients in 1st complete remission who received a busulfan and melphalan combination as conditioning regimen. methods: twenty-five consecutive patients with acute myeloid leukemia were enrolled between 2003 and 2008. the median age was 34 years old (range: 16 - 57 years). all patients received cyclosporine and methotrexate for prophylaxis against graft-versus-host disease. median neutrophil engraftment time was 16 days (range: 7 - 22 days) and 17 days (range: 7 - 46 days) for platelets. sinusoidal obstructive syndrome was observed in three patients, seven had grade ii acute graft-versus-host disease and one extensive chronic graft-versus-host disease. results: the overall survival by the kaplan-meier method was 48% after 36 months with a plateau at 36 months after transplantation. intensive consolidation with high-dose arabinoside resulted in an improved survival (p-value = 0.0001), as did grade ii acute graft-versus-host disease (p-value = 0.0377) and mild chronic graft-versus-host disease (p-value < 0.0001). thirteen patients died, five due to infection within 100 days of transplant, two due to hemorrhages, one to infection and graftversus-host disease and three relapses followed by renal failure (one) and infection (two). the cause of death could not be determined for two patients. conclusion: the busulfan and melphalan conditioning regimen is as good as other conditioning regimens providing an excellent survival rate.

Abstract: twenty-two consecutive patients with acute myeloid leukemia in first complete remission submitted to autologous hematopoietic stem cells transplantation conditioned with busulfan and melphalan were evaluated between 1993 and 2006. the overall survival, according to the kaplan-meier curve, was 57.5% at 36 months, with a "plateau" at 20 months after transplant. factors such as gender, french-american-british (fab) classification of acute myeloid leukemia, induction therapy, intensive consolidation, remission after the first cycle of induction and source of cells had no impact on survival. one patient with poor prognosis before the procedure died a year after transplantation. nine patients died, eight by relapse and one because of bleeding. death before 100 days occurred for two patients, one due to relapse and the other bleeding caused by refractory thrombocytopenia related to the procedure. in conclusion, the conditioning regiment with busulfan and melphalan is a valid option compared to the other conditioning regimens, with an excellent overall survival.
Autologous hematopoietic stem cell transplantation in classical Hodgkin's lymphoma

Abstract: background: hodgkin's lymphoma has high rates of cure, but in 15% to 20% of general patients and between 35% and 40% of those in advanced stages, the disease will progress or will relapse after initial treatment. for this group, hematopoietic stem cell transplantation is considered one option of salvage therapy. objectives: to evaluate a group of 106 patients with hodgkin's lymphoma, who suffered relapse or who were refractory to treatment, submitted to autologous hematopoietic stem cell transplantation in a single transplant center. methods: a retrospective study was performed with data collected from patient charts. the analysis involved 106 classical hodgkin's lymphoma patients who were consecutively submitted to high-dose chemotherapy followed by autologous transplants in a single institution from april 1993 to december 2006. results: the overall survival rates of this population at five and ten years were 86% and 70%, respectively. the disease-free survival was approximately 60% at five years. four patients died of procedure-related causes but relapse of classical hodgkin's lymphoma after transplant was the most frequent cause of death. univariate analysis shows that sensitivity to pre-transplant treatment and hemoglobin < 10 g/dl at diagnosis had an impact on patient survival. unlike other studies, b-type symptoms did not seem to affect overall survival. lactic dehydrogenase and serum albumin concentrations analyzed at diagnosis did not influence patient survival either. conclusion: autologous hematopoietic stem cell transplantation is an effective treatment strategy for early and late relapse in classical hodgkin's lymphoma for cases that were responsive to pre-transplant chemotherapy. refractory to treatment is a sign of worse prognosis. additionally, a hemoglobin concentration below 10 g/dl at diagnosis of hodgkin's lymphoma has a negative impact on the survival of patients after transplant. as far as we know this relationship has not been previously report

Abstract: El presente artículo es producto de más de una década de trabajo en escuelaspúblicas, implementando programas de alumnos mediadores. Estos intervienenen confl ictos entre sus pares. A largo de todo el trabajo desarrollado se han formados más de 2.600 alumnos mediadores y capacitados más de5.000 docentes. Las conclusiones que se presentan son el resultado de larecolección de testimonios e informes de los involucrados (alumnos, familias,docentes y directivos), y como consecuencia de esta experiencia es quepodemos postular el carácter formativo y preventivo de la inclusión de lamediación en la escuela.

Abstract: obesity has emerged as an alarming public health problem, having increased rapidly in both adults and children. considering 12 countries with reported levels of overweight and obesity, one third have a 20% prevalence of one or both conditions. in 17 countries reporting data on obesity, the mean rate was 4.6%; four countries reported rates over 6%, while three reported less than 2%. the majority of these countries showed increases of 70% in overweight and 60% in obesity during the last decade. only two countries reported decreasing prevalence rates. countries may be classified in four groups: (1) low or even downward prevalence of overweight and obesity; (2) underweight reported simultaneously with an increase in obesity; (3) a large territory and population with differences in the prevalence of overweight and obesity between regions and socioeconomic groups; and (4) little underweight and high prevalence of obesity. in conclusion, there is no doubt about the increasing trend in overweight and obesity among latin american children. however, since there is no consensus as to diagnostic criteria for overweight and obesity, the real prevalence is not known.
Obesidad en el ni o en América Latina: situación, criterios de diagnóstico y desafíos

Abstract: En la actualidad, la obesidad constituye un problema de Salud Pública y ha aumentado en adultos y ni os. De 12 países latinoamericanos que tenían información completa de sobrepeso y obesidad algo más de un tercio supera el 20% en ambas situaciones. Para obesidad 17 países contaban con información; el promedio de ésta fue 4,6%, en el que 4 de ellos presentaron valores sobre 6% y 3 inferiores al 2%. En la mayoría se observó incremento en los últimos a os, el 70% presentó aumento de sobrepeso y el 60% de obesidad, sólo en dos de ellos hubo descenso. Esta situación permite clasificar a los países en 4 grupos: el primero los que presentan bajos valores de exceso de peso y tendencia al descenso; el segundo los que poseen altos valores de déficit de peso y aumento de obesidad; el tercero son los que tienen gran extensión territorial, en que se observa diferenciación geográfica y poblacional en el exceso de peso; y finalmente los que presentaron disminución del déficit y un importante aumento del exceso de peso. En conclusión, no hay dudas sobre la tendencia al aumento del sobrepeso y la obesidad en el ni o, sin embargo, debido a la falta de consenso sobre los criterios diagnósticos los reales niveles de prevalencia están en discusión.

Abstract: this research is a descriptive study with the intention to determine the type of impact produced in the communicative competence in english when drama was implemented by the participants of the english theatre club of the modern languages department at the instituto pedagógico de barquisimeto. in order to contrast mean variations between the expected impact and the perceive impact of this activity students answered a 40 items questionnaire. in addition, the coordinators were consulted regarding the communicative competence of the club members in the target language. after applying the paired or correlated student t test, no significant difference was found between the expected and perceived mean. in terms of coordinator opinions, they pointed to students? good development of their communicative competence in english as well as a personal and professional improvement.
