Utilidad de la integración y convergencia de los métodos cualitativos y cuantitativos en las investigaciones en salud

Abstract: the objective of this paper is to comment on the possibility of integration and convergence of the quantitative and qualitative methods in health research. it also intends to characterize both methods and to illustrate them with concrete researches in the health field. the information offered is based on the bibliographic review, the theoretical exposition and the presentation of results obtained in social and educational research in the diabetes field. both methods have different research aims and different ways of approaching the phenomenon to be studied, its advantages and limitations. the quantitative method is useful to measure the magnitude of a phenomenon, whereas the qualitative method serves to interpret how and why this phenomenon occurs. it also allows to get closer to the phenomenon from the perspective of the meanings that the actors give to the process in which they are immersed. the health researches were not foreign to the controversial debate caused by the introduction of the qualitative method into scientific research, but starting from the basis that the set of qualitative and quatitative data do not oppose or exclude each other, the integration of both methods may potentiate scientific research instead of making it difficult. the integration and convergence of both methods may contribute to a better understanding of the phenomenon under study. the application of social sciences to the study and understanding of the health/disease processes facilitate the transfer of the bio-medical to the bio-social approach and leads to the use of qualitative methodology for research.

Abstract: El propósito del presente trabajo es comentar la posibilidad de la integración y convergencia de los métodos cuantitativo y cualitativo en las investigaciones en salud. Pretende, además, caracterizar ambos métodos y ejemplificarlo con investigaciones concretas en el campo de la salud. La información ofrecida se basa en la revisión bibliográfica, la exposición teórica y la presentación de resultados obtenidos en investigaciones sociales y educacionales en el campo de la diabetes. Los dos métodos tienen diferentes objetivos de investigación y forma de abordar el fenómeno a estudiar, sus ventajas y limitaciones. El método cuantitativo es útil cuando se quiere medir la magnitud de un fenómeno, el método cualitativo lo es cuando se quiere interpretar cómo y por qué sucede dicho fenómeno; permite, además, un mayor acercamiento al fenómeno desde la perspectiva de los significados que los propios actores dan al proceso en que están inmersos. Las investigaciones en salud no estuvieron ajenas al debate controversial que provocó la introducción del método cualitativo en la investigación científica pero partiendo de la base de que el conjunto de datos cuantitativos y cualitativos no se oponen ni se yuxtaponen o excluyen, la integración de ambos métodos, lejos de dificultarla puede potenciarla. La integración y convergencia de ambos métodos puede contribuir a una mejor comprensión del fenómeno en estudio. La aplicación de las ciencias sociales al estudio y comprensión de los procesos de salud/enfermedad facilitan el tránsito del enfoque biomédico al enfoque biosocial y obliga al uso de la metodología cualitativa de investigación. Abstract:

Light quality response is a vital environmental cue regulating plant development. Conifers, like angiosperms, respond to the changes in light quality including the level of red (R) and far-red (FR) light, which follows a latitudinal cline. R and FR wavelengths form a significant component of the entire plant life cycle, including the initial developmental stages such as seed germination, cotyledon expansion and hypocotyl elongation. With an aim to identify differentially expressed candidate genes, which would provide a clue regarding genes involved in the local adaptive response in Scots pine (Pinus sylvestris) with reference to red/far-red light; we performed a global expression analysis of Scots pine hypocotyls grown under two light treatments, continuous R (cR) and continuous FR (cFR) light; using Pinus taeda cDNA microarrays on bulked hypocotyl tissues from different individuals, which represented different genotypes. This experiment was performed with the seeds collected from northern part of Sweden (Ylinen, 68?N). Interestingly, gene expression pattern with reference to cryptochrome1, a blue light photoreceptor, was relatively high under cFR as compared to cR light treatment. Additionally, the microarray data analysis also revealed expression of 405 genes which was enhanced under cR light treatment; while the expression of 239 genes was enhanced under the cFR light treatment. Differentially expressed genes were re-annotated using Blast2GO tool. These results indicated that cR light acts as promoting factor whereas cFR antagonises the effect in most of the processes like C/N metabolism, photosynthesis and cell wall metabolism which is in accordance with former findings in Arabidopsis. We propose cryptochrome1 as a strong candidate gene to study the adaptive cline response under R and FR light in Scots pine as it shows a differential expression under the two light conditions.

Eficacia de un seguimiento a largo plazo con educación interactiva en diabéticos tipo 1

Abstract: the present paper is aimed at evaluating the effect of 10 years of follow-up under interactive consultation on the comprehension and adhesion to the treatment, as well as on the clinical, biochemical and therapeutic changes in a group of type 1 diabetics. 40 persons with type 1 diabetes mellitus and no more than 5 years of diagnosis were included in the study. they were selected by the consecutive order in which they were received at the center during the month of recruitment and appointed to the interactive consultation (ic). the results were compared to those obtained in a control group with the same general and clinical variables at the beginning of the study that continued the educative process by the traditional consultation (tc). the knowledge and skills to follow the treatment were evaluated through quantitative questionnaires. the feelings about the disease were quantitative and qualitatively assessed. the knowledge, skills, responsibility for the treatment, self-steem, and autonomy were significantly higher (p=0,000) in the group of the ic. the acceptance of the interactive treatment was faster and the levels of glycosylated hemoglobin also showed marked differences (p=0,001) in favor of this group, which influenced on the lower frequency of acute complications and hospitalizations due to metabolic decompensation . this group presented lesser frequency (p=0,04) in the appearance of long-term complications. those who participated in the ic attained better results in all the analyzed paremeters on comparing them with those that followed the traditional form of consultation.

Abstract: diabetes mellitus is an example of the need for a therapeutical education process that together with clinical care assures the treatment efficacy in persons suffering from this disease. it was shown how this action has been undertaken within the national program of diabetes in cuba. the first working phase (in the 70' s) centered on declaring education as a primary function, setting basic information courses for diabetic persons and publishing educational leaflets. knowledge about this disease increased significantly and average glycemia concentrations temporarily lowered. in the second phase (in the 80' s), the design of an educational program for diabetes with well-defined objectives was a quality leap where better adherence to treatment, better metabolic control and a reduction of complications were found. in the third phase (1990 to date) the program was extended to all services of the national healthcare system and support was given to other countries of the regions in preparing their own programs. a better quality of services in primary health care and the international acknowledgement of the cuban educational strategy in diabetes were achieved. the theoretical model outlined in the national program proved to be effective after 25 years of experience. its development from practice to theory and then back to practice to make changes allowed progresses from the initial product to the generalization extended to the whole national healthcare system.
La educación a personas con diabetes mellitus en la atención primaria de salud

Abstract: diabetes science of end of this century had the clear idea on non-existence of efficaceous treatment for diabetes without education and training of its carrier. but this educational process is achieved in a systematic way, and frequently, lacks of ability of that another knowledge demanding care and teach on how to care. national progam of diabetes of national health system of cuba, has a teaching program developing in a continued way, training of health staff to perform this educating function, and emphasizes on primary level of health, where more than 85 % of persons presenting diabetes mellitus is seen. although there is a lack of papers publishing with a scientific strictness, achievements and difficulties that related with that tasks, have challenged health areas, it is known that there is a high percentage of personas presenting with diabetes mellitus non-involved in educational process, with a poor control, and are complicated. en this context, goal of present paper is to spread principles, structure and methodology of this educational program as well as all directions more general for its application in primary care of health, to contribute to its effective and uniform implementation in such servives, with a deep consciousness of that in each care action is implicit need of teaching to take care one self, and that only a person learning to care herself, will achieves a good cuality of life.
Resultados de un seguimiento educativo a personas con diabetes mellitus tipo 2 y sobrepeso u obesidad

Abstract: the present paper showed the results of educational follow-up of persons with type 2 diabetes mellitus and overweight/obesity carried out as part of the latin american study pednid-la, with the objective of prospectively evaluating the clinical, biological and therapeutic changes after their participation in a group educational program with interactive methodology. the study comprised 40 non-insulin dependent, obese or overweighed patients chronologically selected according to their date of attendance to ambulatory service and preferably with recent onset of disease. interactive medical appointment was set every three months where care and education combined and educational contents was emerging from the real perceived needs of the group, which centered on the promotion of active participation of the patients and increased compliance with the treatment, giving priority to meal planning, exercising, reduction of body weight and self-monitoring of urine glucose as it has been applied to other groups of diabetic persons. knowledge on diabetes was measured in similar questionnaires designed in the general multicenter protocol of the pednid-la study and they were applied at the beginning and at the end of the study and at 4 years of the interactive follow-up. the rest of variables were assessed before, after a year of the pednid-la study and after 4 years of the interactive follow-up. software epi-info processed data; chi-square test determined statistical differences and accepted significance criteria was set at p< 0,05. the main results included a significant rise of knowledge on the disease and the reduction of body weight, of classical symptoms of the disease and of daily dosage of oral hypoglycemic drugs. the average levels of glycosylated hemoglobin at the end of the study had substantially improved. on balance, it was confirmed once again that educational intervention brings about better understanding and greater compliance with the treatment as well as normalizat
