Impact of supervised physiotherapeutic pelvic floor exercises for treating female stress urinary incontinence

Abstract: context and objective: urinary incontinence is a public health problem that affects more than 200 million people worldwide. stress incontinence is the most prevalent type. pelvic floor muscle exercises have been used for treating it, although there is no consensus regarding their application. the aim of this study was to compare the results from treating female stress urinary incontinence with pelvic floor muscle exercises with or without physiotherapist supervision. design and setting: this was a randomized, prospective, controlled trial in the urogynecology and vaginal surgery sector, universidade federal de s?o paulo. methods: forty-four women were randomized to be treated for stress urinary incontinence with pelvic floor exercises for three consecutive months, into two groups: one with and the other without physiotherapist supervision. they were evaluated before and after treatment using a quality-of-life questionnaire, pad test, micturition diary and subjective evaluation. descriptive analysis was used to evaluate the population. the homogeneity of the two groups was evaluated using the kruskal-wallis and chi-squared tests. the success of the two groups after treatment was evaluated using the wilcoxon test. results: the supervised group showed statistically greater improvement in the pad test, micturition diary and quality of life than did the control group. in the subjective evaluation, only 23.8% of the control group patients were satised with their treatment. in the supervised group, 66.8% of patients did not want any other treatment. conclusion: supervised pelvic floor muscle exercises presented better results in objective and subjective evaluations than did unsupervised exercises.

Abstract: taking advantage of the empirical opportunity supplied by the education service, this work proposes investigating two themes related to the analysis of service sequences: the variation in expectations and perceptions over time and their predictive capacity relative to future service quality. two research objectives were defined: (a) to test for possible alterations in perception and expectations as to graduation, by accompanying the evolution of experience with the service, and (b) to test the capacity of expectations and perceptions for forecasting service quality in a future post-graduate situation. the convenience sample comprised 71 students, divided into two groups in accordance with the term they attended to. the research instrument used was a simplified version of the servqual questionnaire. both perceptions and expectations showed a significant decrease between the groups. the variations in the two groups for the three variables were practically the same. although the individuals are not the same and have different demographic characteristics, the result suggests that the differences must be associated with their experience of the service. finally, current expectations demonstrated positive and significant impact on future expectations.
Soil macrofauna in cover crops of figs grown under organic management

Abstract: soil fauna plays an important role in organic management through their effects on soil organic decomposition, nutrient mineralization, and amelioration of the soil's physical properties. this work evaluates the density and diversity of the soil macrofauna under types of cover plants in areas cultivated with ficus carica l. under organic management. the soil macrofauna was collected in 0.25 × 0.25 m areas, down to a soil depth of 0.3 m, and at the surface layer. the treatments consisted of bahiagrass living mulch (paspalum notatum), siratro living mulch (macroptilium atropurpureum), and bahiagrass mulch. the highest macrofauna density and the lowest diversity were observed in bahiagrass, of which 80% were represented by ants, thus characterizing the soil under this cover crop as showing the lowest functional diversity and quality.

Abstract: Capital structure and financial performance of Brazilian, Argentinean and Chilean companies were compared to identify differences using a multiple cross section study made of 1996 to 2005. Samples were taken from the Economatica for examination of the variables of debt over PL (ENDV), operational profitability over total assets (ROAOP) and profitability over PL (ROE). Variance analysis was used to detect significant differences between companies in these countries and the Scheffé test identified countries differing from the averages. Significant differences were found in average ENDV and ROAOP in five of the ten years and in ROE for seven years of these ten years. Brazilian companies, in general, differed from the Chilean and Argentinean ones; however, no significant differences were noted between the Argentinean and Chilean companies. Descriptive analysis showed that Brazilian companies had a higher ENDV for all the years, lower ROAOP during the first four years and a lower ROE for the period studied. Este trabalho tem como objetivo verificar se a estrutura de capital e o desempenho financeiro diferem entre as empresas brasileiras, argentinas e chilenas. O estudo consiste em um cross section múltiplo que compreendeu o período de 1996 a 2005. A amostra foi extraída do Economática , tendo como variáveis o endividamento sobre o PL (ENDV), a rentabilidade operacional sobre o ativo total (ROAOP) e a rentabilidade sobre o PL (ROE). A fim de apurar se havia diferen as significativas entre as firmas dos três países, utilizou-se a Análise da Variancia, e, para identificar em quais países diferem tais médias, utilizou-se o teste Scheffé. Os resultados apontaram diferen as significativas nas médias do ENDV e da ROAOP em cinco dos dez anos pesquisados, e na ROE houve diferen as significativas em sete dos dez anos analisados. Em geral, as empresas brasileiras diferem das chilenas e argentinas; no entanto, os resultados n o apontaram diferen a significativa entre as empresas argentinas e as chilenas. Quanto à análise descritiva, as empresas brasileiras demonstraram maior ENDV em todos os períodos, menor ROAOP nos primeiros quatro períodos e menor ROE em todos os anos pesquisados.

Abstract: A quantifica o da evapotranspira o na regi o semiárida é de grande importancia, visto que essa regi o apresenta grande ocorrência de períodos de déficit hídrico que limitam a produ o agrícola.Objetivou-se com este trabalho avaliar a correla o entre a evapotranspira o de referência (ET0),determinada pelo método de Penman-Monteith (ET0 – PM), com a estimada pelos métodos do Tanque Classe A (ET0 – TA), de Linacre (ET0 – L), de Hargreaves-Samani (ET0 – H&S), e a evapora o de um tanque alternativo denominado de Minitanque (EV – MT), na regi o Norte de Minas Gerais. Os dados foram coletados durante 12 meses de uma esta o automática e de um minitanque alocados na Unidade de Pesquisa do Instituto Federal do Norte Minas Gerais, Campus Januária, sob coordenadas geográficas de latitude 15o28’55’’ S e longitude 44o22’41’’ W, altitude 474 m. Foram ajustadas equa es de regress o linear, tendo como variável independente, a ET0 –PM, e, como variáveis dependentes, a ET0 – H&S, a ET0 – L, a ET0 – TA e a EV – MT. Os valores de ET0 – H&S, ET0 – L, ET0 – TA e EV – MT apresentaram boa correla o e superestimaram a ET0 – PM. As regress es entre a ET0 – PM (método padr o) e a ET0, calculadas pelos métodos estudados e, entre ET0 – PM e a evapora o do minitanque, apresentaram maiores coeficientes de determina o no período sem chuvas.
Electrochemical degradation of reactive dyes at different DSA? compositions

Abstract: this paper investigates the electrochemical oxidation of the reactive dyes reactive blue 4 (rb-4) and reactive orange 16 (ro-16) on ruo2 dimensionally stable anode (dsa?) electrodes. electrolysis was achieved under galvanostatic control as a function of supporting electrolyte and electrode composition. the electrolyses, performed in either the presence or absence of nacl, were able to promote efficient color removal; moreover, at low chloride concentration (0.01 mol l-1), total color removal was obtained after just 10 min of electrolysis, and a significant increase in total dye combustion was achieved for all the studied anodes in chloride medium (reaching ca. 80% - chemical oxygen demand - cod removal). no significant enhancement in dye color removal or mineralization was observed upon increasing chloride concentration. the influence of oxide composition on dye elimination seems to be significant in both media (with or without chloride), being ti/ru0.30ti0.70o2, the most active material for organic compound oxidation. the oxygen evolution reaction (oer) was shown to be a limiting reaction in both supporting electrolytes; i.e., nacl and na2so4, and its competition with organic compound oxidation remained an obstacle. the adsorbable organo halogens (aox) formation study revealed that there is slight consumption of the undesirable species formed within the first minutes of the electrolysis, being ti/(ruo2)0.70(ta2o5) 0.30 the most environmentally friendly composition. both anode composition and chloride concentration affect the formation of these undesirable compounds.
Implante orbital misto para reconstru??o de cavidade anoftálmica: relato de caso

Abstract: a new model of orbital implant, matching the low cost of acrylic implants (pmma) with the advantages of medpor (porex?) is presented herein. the new implant model consists of a pmma implant with a dura mater scrap adhered on its anterior face of the same diameter of the acrylic implant and a medpor snip adhered on its posterior face by sterile cyanoacrylate. dura mater was fixed to medpor with 6.0 mononylon. one patient who presented previous extrusion of the implant was submited to surgery and was evaluated on the 1st, 7th, 15th, 30th postoperative days, and 3 and 6 months after surgery when a computed tomography was performed. a good cosmetic, mobility and volume result was noted after an escleral prosthesis was adapted, with no infection, migration or extrusion of the implant until this moment. this case showed us that it is possible to use a more economic implant model with the same results of medpor (porex?).

Abstract: O teor de água do solo tem grande influência sobre a absor o de fósforo e produtividade do algodoeiro. Objetivou-se avaliar a influência de diferentes métodos de irriga o e doses de aduba o fosfatada sobre o crescimento e produtividade do algodoeiro na regi o norte do estado de Minas Gerais. O experimento foi conduzido no Instituto Federal do Norte de Minas (IFNMG), Campus Januária. Os tratamentos consistiram de três doses de P (0, 50 e 120 kg ha-1 de P2O5) e de três sistemas de cultivo (Aspers o, Gotejamento e Sequeiro). Adotou-se o delineamento em blocos casualizados, em esquema fatorial, com três repeti es. Aos 50 e 80 dias após a emergência (DAE) coletaram-se folhas, caules e estruturas reprodutivas. Cada órg o foi seco, e após triturado foi determinado o teor de P. Aos 80 DAE foi avaliado a altura e o número de estruturas reprodutivas por planta. Na colheita avaliou-se a massa e o número de capulhos por planta, o rendimento de fibra e a produtividade de algod o em caro o. A irriga o aumentou o número de estruturas reprodutivas, a altura de plantas, o número de capulhos por planta e a produtividade do algodoeiro. A aplica o do P aumentou o número de capulhos por planta e a produtividade de algod o em caro o. N o houve diferen a na altura de plantas, número de capulhos por planta e na produtividade de algod o em caro o entre os cultivos irrigados por aspers o convencional ou por gotejamento. O maior rendimento de fibra ocorreu nos cultivos irrigados, especialmente no irrigado por gotejamento. O maior conteúdo de P foi observado nos cultivos irrigados, independente do método de irriga o.
Gene Expression Analysis of Extracellular Matrix and Cytokines after Uterine Artery Embolization  [PDF]

Abstract: Arterial embolization of myomas (AEM) is an established option for the conservative treatment of uterine leio-myomas; it treats all present uterine nodules at once, is less invasive than other procedures and effective in controlling symptoms, and does not require long term hospitalizations. Nevertheless, the potential impact on endometrial morphological and functional outcomes after the procedure is still controversial based on reports of endometritis or eventual transient ischemia. This study evaluated endometrial reorganization in uterine leiomyoma patients, before and after AEM, through gene expression analyses of extracellular matrix and cytokines genes in theendometrial tissue. Eight patients with leiomyomas were evaluated before AEM and 6 months after. The examinations included transvaginal pelvic ultrasonography, dosing of the follicle-stimulating hormone, and endometrial biopsy during the second phase of the menstrual cycle. RNA was extracted from endometrial samples, cDNA was synthesized, and applied on PCR arrayTM plates to evaluate the expression of extracellular matrix (ECM) genes and cytokines and their receptors’ genes (CYT). The ECM overexpressed genes were MMP (1, 3, 10, 11, and 14), CTGF1, ICAM1, TBHS1, ITGA2, ITGA3, ITGB3, COL7A1, COL12A, SPP1, and TNC; ADAMTS8 was underexpressed. The CYT overexpressed genes were SPP1, BCL6, CXCL12, IL-8, and CEBPB; CXCL13 and CCL21 were underexpressed. The ECM results showed overexpression of proteases that are responsible for dysfunctions in the ECM, and of genes responsible for adhesion and membrane components. The CYT results showed overexpression of chemokines responsible for endometrial repair, and underexpression of cytokines involved in inflammatory processes in the endometrial tissue. AEM treatment did not negatively affect the endometrial function at 6 months after embolization. This study broadens the knowledge about using a procedure that is relevant to the treatment of leiomyomas and contributes to the establishment of future guidelines for the decision making process for physicians and patients.

Abstract: Objetivo: analisar a rela o entre a press o de perda com manobra de Valsalva e a press o máxima de fechamento uretral com a queixa clínica em mulheres com incontinência urinária de esfor o. Métodos: estudo retrospectivo no qual foram incluídas 164 pacientes com diagnóstico de incontinência urinária de esfor o ou mista atendidas no setor de Uroginecologia e Cirurgia Vaginal do Departamento de Ginecologia da UNIFESP/EPM. As pacientes submeteram-se à anamnese padronizada, exame físico e estudo urodinamico. A press o de perda foi mensurada sob manobra de Valsalva (Valsalva leak point pressure - VLPP), com volume vesical de 200 mL. O perfil uretral foi realizado utilizando-se cateter de fluxo número 8, sendo medida a press o máxima de fechamento uretral (PMFU). As pacientes foram agrupadas conforme a queixa clínica de perda urinária aos esfor os e realizou-se análise estatística por meio do teste de chi2 para verificar a propor o entre as variáveis. Utilizou-se, a seguir, a análise de variancia (ANOVA) para verificar diferen as entre VLPP e PMFU com rela o à gravidade subjetiva da incontinência. Resultados: a média de idade foi de 51,2 anos (19-82), sendo que 79 encontravam-se no menacme (48,2%) e 85 (51,8%) na pós-menopausa. A paridade média foi de 4,0 filhos (0-18). Houve correla o entre o número de pacientes com VLPP inferior a 60 cmH2O e a queixa clínica (p<0,0001), sendo que o grupo com perda urinária aos mínimos esfor os teve média de 69,1 cmH2O na press o de perda, o grupo com perda urinária aos moderados esfor os teve média de 84,6 cmH2O e o grupo com perda urinária aos grandes esfor os teve média de 90,6 cmH2O. Conclus es: VLPP correlacionou-se com a queixa clínica, sendo menor no grupo com perda aos mínimos esfor os. N o houve correla o entre a PMFU e a queixa clínica.
