Evaluation of laboratory markers of progression of HIV disease to death

Abstract: introduction: one of the important current problems in hiv/aids infection is the establishment of epidemiological and laboratorial prognostic parameters during patient follow-up. this study aimed at analyzing the evolution of laboratory tests: cd4 lymphocyte count, viral load, hemoglobin (hb), aspartate aminotransferase (ast), alanine aminotransferase (alt), and the epidemiological variables sex and age as prognostic factors for survival in progression to death among aids patients. methods: a retrospective study was conducted using analysis of medical records, and prospective 24-month follow-up of patients with hiv/ aids attended at the president vargas hospital outpatient clinic, a reference center in hiv/ aids attendance in the state of maranh?o, brazil. the study analyzed patients aged 10 to 60 years old, who manifested aids and who were not using antiretroviral therapy or had used it for less than 5 years. the chi-square test was used for statistical analysis. results: the sample included 100 patients - 57 were current outpatients, and 43 had died. the variables viral load (p=0.726), alt (p=0.314), sex (p=0.687), and age (p=0.742) were analyzed, and no evidence of association between them and worst prognosis was observed. conclusions: a significant relation was verified between low hb levels (p=0.000) and cd4 (p=0.000) and shorter survival.

Abstract: introduction: visceral leishmaniasis is a serious public health problem that requires global control strategies, especially with respect to factors that may intervene in reducing the incidence of endemicity. in this work, rainfall density and temperature were correlated with the incidence of human cases in an area endemic for leishmaniasis in s?o luis do maranh?o, northeastern brazil. methods: notification of human cases by the national health foundation/regional coordination of maranh?o (funasa/corema) from 2002 to 2010 was used. ecological data (mean temperature and rainfall density) were provided by the meteorological office of state. results: a significant association was verified between the number of vl cases and rainfall rate but not in the analysis concerning mean temperatures. conclusions: these data suggest that the control actions in visceral leishmaniasis should be performed during rainy season in the state of maranh?o, which is in the first half of the year.

Abstract: o objetivo dessa revis?o é o acesso crítico à literatura a respeito dos efeitos tóxicos dos aminoglicosídeos nas estruturas histológicas do rim. foram pesquisados os artigos cientí?cos nas bases de dados da sciencedirect, cujas palavras-chaves foram “nephrotoxicity”, “aminoglycosides”, “gentamicin” e “renal injury”. os artigos que apresentavam apenas o abstract disponível, bem como as revis?es sistemáticas foram excluídos. foram considerados somente artigos relacionados com a morfologia do sistema renal publicados em língua inglesa e espanhola, e aqueles acessados através da base de dados da capes. dois revisores independentes avaliaram a elegibilidade e a qualidade metodológica de cada ensaio experimental e extraíram os dados. sete foram selecionados. as variáveis estudadas foram as seguintes: autor(es), ano, objetivo, metodologia e conclus?es. os artigos selecionados utilizaram desde técnicas histoquímica e imunoistoquímica para identi?ca??o morfológica das altera??es renais até espectroscopia. foi possível também, veri?car métodos de preven??o à nefrotoxicidade induzida pelos aminoglicosídeos, minimizando efeitos tóxicos. os achados histopatológicos mostraram que a administra??o de aminoglicosídeos causaram apoptose e edema celular, interrup??o da membrana basal, redu??o do espa?o de bowman e necrose tubular aguda.
Climatic Projections of Lightning in Southeastern Brazil Using CMIP5 Models in RCP’s Scenarios 4.5 and 8.5  [PDF]

Abstract: Given the high and increasing lightning incidence over the Southeast of Brazil and the various impacts that this phenomenon generates to society, there is a growing need in predicting its occurrence, in order to minimize its consequences. In this context, this work presents the development of a methodology for the projection of lightning in the State of S?o Paulo (Southeastern Brazil), using the HadGEM2-ES and CSIRO-Mk3.6 models in two IPCC climate change scenarios: RCP4.5 and RCP8.5. Since lightning is not an output variable of climate models, tests were carried out to evaluate the relationship between the observed data of oceanic and atmospheric fields, which are known as outputs of the models, and the lightning from the RINDAT and BrasilDAT detection networks. As result, a correlation of 0.84 was obtained. In the projections, it was verified that, while during a large portion of the current climate we observed events of lightning below the average, the future climate reveals the preponderance of anomalously above average events, both in the scenario of intermediate-low emissions (RCP4.5) and in the scenario of high emissions (RCP8.5), suggesting a change in the pattern of the lightning incidence in the State of S?o Paulo.

Abstract: with the objective to evaluate the spatial variability of the soil properties and coffee production in different geomorphic surfaces (gs) in the region of the high paranaíba, mg, two regular grids of 200 x 850 m with intervals of 50 m were installed with 68 sampling points under coffee crop. the soil samples were collected at 0-0.20 m depth at the intersection points of these grids. the coffee production was evaluated in plants closer to the intersection of the grid point. descriptive statistics and geostatistics analysis of the soil properties and production of coffee crop were performed. the chemical and textural properties of soil intensely managed possess dependence of the relief, even for small expression. the coffee production presented spatial dependence in both surfaces and largest variability in gs ii. the limits between the geomorphic surfaces can represent limits of specific places of management.
Partial Nephrectomy as Treatment of an Atypical Metastasis from Prostate Cancer—A Case Report and Review of Literature  [PDF]

Abstract: Background: Prostate cancer is the second most common type of cancer in man and the second in cancer-specific deaths in this population in the world. Most of the causes of death related to prostate cancer are due to its distant metastases, with the most common sites being: skeleton, distant lymph nodes, liver and lung. Renal metastasis is rare, and studies suggest infiltration due to arterial microembolization of the tumor. A key point in this scenario is the clinical suspicion of differential diagnoses, to offer the patient an effective therapy in such a specific case. Aim: To report a case of a patient with prostate cancer undergoing partial nephrectomy whose histopathological report revealed a metastatic lesion of that primary site. Case Presentation: 74 years old man, referred in May 2015 due to high PSA level and lumbago. PSA 323.11 ng/dl, rectal examination cT3a; biopsy was performed and histopathological study reported bilateral prostate adenocarcinoma, Gleason’s score 8 (4 + 4). Patient’s staging showed multiple secondary implants on skeletal scintigraphy. Tomography revealed solid exophytic lesion in the lower pole of the right kidney (4.7 × 3.6 cm); prostate without cleavage planes with seminal vesicles and pelvic node enlargement. Hormone therapy was initiated, PSA levels dropped to 9.51 ng/dl and total testosterone < 50 ng/dl. Partial nephrectomy was planned, initially by laparoscopy, but converted to laparotomy in December 2015. Procedure lasting 3 hours, minimal blood loss, no perioperative complications, discharged on the 3rd postoperative day. Histopathological report described undifferentiated malignant neoplasm, requiring immunohistochemistry that confirmed prostate adenocarcinoma. Patient remains hormone therapy, with no progression of the disease so far. Conclusion: Given the rarity of these cases, it is not possible to presume that nephrectomy enhances the survival rates. However, we observed that partial nephrectomy was a good choice for our patient, being the first case described in the literature. More reports should be available and studies with higher levels of evidence should be conducted to assist us in patient orientation and decision making.

Abstract: oxygen free radicals are molecules that present unpaired electrons in their outer orbit and can transform other molecules such as proteins, carbohydrates, lipids and deoxyribonucleic acid. oxygen free radicals are produced in various clinical conditions in which hypoxic microenvironments are generated and reoxygenation follows. such situations include clinical shock, septicemia, systemic inflammatory response, fulminant hepatitis, organ transplant and respiratory failure. in this review, we discuss the main concepts related to oxygen free radicals: the principal types and their formation, as well as the way in which they affect cellular structures and cause significant tissue damage. we present also the main antioxidants that guard against oxidative stress, including glutathione, glutathione peroxidase, superoxide dismutase, catalase, and n-acetylcysteine. the influence of oxygen free radicals on the principal pulmonary diseases are also discussed, with special emphasis given to oxygen free radicals in cigarette smoke, chronic obstructive pulmonary disease, asthma, sleep apnea syndrome and acute respiratory distress syndrome.
Evaluation of Milk Compositional Quality and Mammary Gland Health of Dairy Herds in the Southwestern Brazilian Amazon  [PDF]

Abstract: Samples of raw milk from bulk cooling tanks were collected in five municipalities of the Southwestern Brazilian Amazon to establish the prevalence of Staphylococcus aueus and Streptococcus agalactiae, as well as to evaluate the milk composition and its correlation with the bulk tank somatic cell count (BTSCC). A total of 250 samples were collected to investigate the causal agents of bovine mastitis in whole milk, from 50 bulk milk tanks in each municipality. Under laboratory conditions, the samples were diluted to 1/10 and 1/100, and samples of 0.1 ml from each dilution were plated in triplicate on selective media for Staphylococcus aureus and Streptococcus agalactiae. To evaluate the correlation between the major milk components (fat, protein and lactose) and the BTSCC, samples were collected for 18 months from 73 dairy herds. The presence of the above-mentioned contagious mastitis pathogens was detected in 97.2% (243/250) of refrigerated raw milk samples evaluated. Analysis of the major milk components and BTSCC demonstrated that during the study period, fat compo-nent showed the largest variance, followed by protein and lactose, which also showed significant variances.

Abstract: background: despite refinements in the field of gastrointestinal surgery, anastomotic leakage still remains a serious problem associated with substantial morbidity and mortality and is the fearest life threatening complication in digestive surgery. to prevent or reduce these complications, a diversity of line reinforcement techniques have been investigated and several adjunctive measures have been developed. aim: to review the data associated with the application of adhesives in digestive surgery procedures. method: review of papers available at medline/pubmed database using the follow headings: adhesive, gastrointestinal tract, surgical anastomosis, fistula. it was chosen recent articles on the subject which showed adhesives as strategy to reduce morbidity and mortality related to digestive surgery. conclusion: tissue adhesives appears to be safe in the clinical setting and are been used with increasing frequency in a variety of surgical field for its unique hemostatic and adhesive abilities, gaining increasing acceptance among surgeons. studies suggest that surgical adhesive can be useful to reinforce and improve intestinal anastomotic healing, increase anastomotic bursting pressures and tensile strength. the potentially harmful tissue effects of the preparations might compromise the use of these substances. because of the heterogeneity and lack of high level evidence, new studies have to be performed.

Abstract: the mathematical model associated with the knowledge of the soil variability properties and mapping the relief forms can assist in the management of soil fertility in sugarcane companies. this study aimed to evaluate the use of geostatistics and mathematical modeling in the fertilization expense estimative in different relief forms. in an area of 200 ha has been identified two relief forms , one concave and one convex, and the soils collected was at the crossing points of a grid with intervals of 50 m, a total of 623 points. the samples were subjected to chemical analysis and subsequently the data were analyzed using descriptive statistics, geostatistics and mathematical modeling. the results showed that when the relief forms are incorporated into the geostatistical analysis and mathematical modeling, there is an increase in the efficiency of limestone, phosphorus and potassium application in the soil.
