Tendências de mortalidade por cancer de boca e orofaringe no Município de S?o Paulo, Brasil, 1980/2002

Abstract: the current study assessed trends in oral cancer mortality in the city of s?o paulo, brazil, from 1980 to 2002. the official mortality information system supplied data on deaths whose underlying cause was classified as oral cancer, stratified by sex, age, and anatomic site. death rates were estimated and adjusted by the direct method, using population data supplied by national censuses from 1980, 1991, and 2000 and a population count performed in 1996. there was an upward trend in overall cancer mortality, at a yearly rate of 0.72%. accounting for more than one third of these deaths, tongue cancer was the main mortality category. labial, gengival, and retromolar cancer showed a downward trend, while oropharyngeal cancer and cancer in unspecified parts of the mouth and oropharynx showed increasing mortality. monitoring the magnitude and trends in cancer mortality can assist the planning of health initiatives aimed at reducing the disease burden from oral cancer in brazil.
Sonographic features of portal hypertension in Schistosomiasis Mansoni

Abstract: the diagnostic value of real-time sonography in the study of portal hypertension was assessed in 66 patients with hepatosplenic schistosomiasis mansoni, all with symmers's fibrosis and esophageal varices. seventy-one individuals without schistosomiasis were selected as controls. the inner diameters of the portal vessels were measured by sonography in all patients and controls: splenoportography was also performed in the schistosomal group. intra-splenic pressure was over 30 cm of water in 44 of 60 patients with schistosomiasis. the upper limit of normality for portal vessel diameters was set through receiver operating characteristic curve at 12 mm for portal vein, 9 mm for splenic vein at splenic hilus, and 9 mm for superior mesenteric vein. the best discriminative vein for the diagnosis of portal hypertension was the splenic vein followed by the portal vein. a direct correlation was observed between the diameter of the splenic vein, measured by sonography, and the intra-splenic pressure. except for the paraumbilical and mesenteric veins, more frequently identified by sonography, there was no statistical difference in the frequency of visualization of splanchnic vessels by sonography or splenoportography.

Abstract: Objetivou-se descrever e explorar analiticamente tendências de mortalidade por cancer de boca e orofaringe na cidade de S o Paulo, Brasil, de 1980 a 2002. Dados sobre os óbitos com esta causa básica foram levantados junto à Funda o Sistema Estadual de Análise de Dados, discriminados por sexo, idade e localiza o anat mica. A estima o e ajuste pelo método direto dos coeficientes usaram dados de popula o fornecidos pelos censos de 1980, 1991 e 2000, e pela contagem populacional de 1996, realizados pelo Instituto Brasileiro de Geografia e Estatística. Foi observada tendência crescente da mortalidade devida ao agravo, a uma taxa anual de 0,72%. Responsável por mais de um ter o desses óbitos, o cancer de língua foi a categoria com mortalidade mais elevada. Cancer de lábio, gengiva e área retromolar apresentaram tendência decrescente, enquanto orofaringe e partes n o especificadas da boca e orofaringe sofreram incremento de mortalidade. O monitoramento da magnitude e tendências da mortalidade por cancer pode configurar importante implemento para o planejamento de iniciativas voltadas à redu o da carga de doen a em nosso meio.
Simpatectomia torácica ao nível de 4a e 5a costelas para o tratamento de hiper-hidrose axilar

Abstract: objective: to describe the clinical results and the degree of satisfaction of patients submitted to thoracic sympathectomy at the level of the fourth and fifth ribs (r4-r5) for the treatment of axillary hyperhidrosis. methods: we included 118 patients diagnosed with axillary hyperhidrosis and having undergone axillary sympathectomy at the r4-r5 level between march of 2003 and december of 2007 at the paraná federal university hospital de clínicas, located in the city of curitiba, brazil. all procedures were carried out by the same surgeon. data regarding the resolution of axillary hyperhidrosis and the degree of patient satisfaction with the surgical outcome, as well as compensatory hyperhidrosis in the early and late postoperative periods (after 7 days and after 12 months, respectively), were collected. results: of the 118 patients evaluated, 99 (83.9%) and 81 (68.6%) showed complete resolution of the symptoms in the early and late postoperative periods, respectively. compensatory hyperhidrosis occurred in 49 patients (41.5%) in the early postoperative period and in 77 (65.2%) in the late postoperative period. of those 77, 55 (71.4%) categorized the compensatory hyperhidrosis as mild. in the early postoperative period, 110 patients (93.2%) were satisfied with the surgical results, and 104 (88.1%) remained so in the late postoperative period. conclusions: sympathectomy at the r4-r5 level is efficient in the resolution of primary axillary hyperhidrosis. the degree of patient satisfaction with the long-term surgical results is high. mild compensatory hyperhidrosis is the main side effect associated with this technique.

Abstract: human t-cell lymphotropic vírus type i (htlv-1) was the first human retrovírus described. some time after its discovery a group of diseases were related to this vírus, such as, adult t-cell leukemia lymphoma (atll), htlv-1 associated myelopathy/tropical spastic paraparesis (ham/tsp) and htlv-1 associated uveitis (hau). in the nineties, htlv-1 was associated to a severe eczema of children, called infective dermatitis (id). since then, several other skin manifestations have been observed in htlv-1-infected individuals, particularly in patients with atll or ham/tsp. however, according to some reports, dermatologic lesions are also common in asymptomatic htlv-1 carriers. besides id, all other skin lesions reported are nonspecific. the aim of this review is to outline the dermatologic manifestations reported in htlv-1 infected patients, emphasizing the clinical and epidemiological value of these findings.
Hepatosplenic schistosomiasis in field-based studies: a combined clinical and sonographic definition

Abstract: a combined clinical and sonographic classification of hepatosplenic schistosomiasis mansoni to be used in field-based studies is proposed herein. seven hundred forty one individuals out of 892 (83%), living in an area endemic for schistosomiasis in brazil, have been ubmitted to clinical and ultrasound examinations. based on two stool examinations the overall prevalence for schistosomiasis in this area was 73%. abdominal palpation was performed with patients in dorsal decubit, during deep breath, by two experienced physicians and a portable ultrasound was used for the evaluation of liver fibrosis, portal collaterals and spleen size. four groups of individuals were identified using data obtained by abdominal palpation and ultrasound examination: (1) palpable spleen and intense periportal thickening in 9 individuals (1.2%); (2) spleen not palpable and intense periportal thickening in 15 (2%); (3) palpable spleen with light to moderate periportal thickening in 32 (4.3%), and (4) palpable spleen with a normal liver on ultrasound in 30 (4%). the definition of hepatosplenic schistosomiasis in field-based studies as the finding of schistosoma mansoni eggs in the stools in an individual with splenomegaly is not acceptable anymore. abdominal ultrasound should be combined with clinical examination to accurately identify hepatosplenics in endemic areas for schistosomiasis.

Abstract: the uni-axial tensile strength test was used for loading austenitic stainless steel of type 304 at different temperatures below room temperature (from 77 k to 300 k). the relation between austenite stability and work hardening, as affected by testing temperature, is discussed in terms of the relationship between the strain-induced martensitic transformation, which occurs during plastic deformation, and the dislocation slip in austenite. in stress-strain curves that assume the ludwik equation s = so + ken, where s is the true stress and e the true plastic strain, a meaningful way to analyze work hardening is by plotting log ds / de against log e. a significant aspect is the variation of the work hardening rate ds/de with the true plastic strain at different temperatures. the changes in work hardening behavior, which provoked up to three deformation stages, may be associated with different micro-structural processes. the martensitic transformation may be regarded as a deformation process that competes with the usual slip process. the investigation of these stages within the plastic range gives a qualitative picture of how different factors, such as the degree of deformation, temperature and chemical composition of austenite affect the austenite-martensite transformation.

Abstract: this article is a critical analysis from bamboozled film, launched in 2000 by spike lee. in his work, particularly in this film, there are conceptual influences from two of the most important authors: bertold brecht and his alienation effect or "v-effekt" (from the german verfremdungseffekt ), and henri bergson with his conception of laughter and its specific function and social meaning. the spike lee's work, opened in the modernist sense, brings a didactic-pedagogic intention about the using and the deconstruction of eurocentric image - which has not been sufficiently examined and understood by the criticism yet.

Abstract: this paper deals with a study made with the purpose of observing dental health, education of patients coming for the first time to a health center. the study was carried out during two weeks and 25 patients were observed through "participant observation". data for the "educational diagnosis" was collected in a card specially designed for it. results showed that dental health orientation was not given by the staff to patients. an educational programme about dental health is suggested to be integrated with the activities of the various services, being the dentist responsible for the staff programme coordination and evaluation. finally the importance of the health center for the development of preventive, curative and educational integrated programmes is emphasized.

Abstract: a comparative study of the anticariogenic effect of sodium fluoride solution, with a concentration of 0.5%, applied weekly and under supervision, in mouthwashing and toothbrushing, used independently and in combination, was carried out on 424 children of 9 and 10 years of age from five public schools in s. paulo, brazil, over a period of 10 school months. the children were equally divided into three experimental groups and one control group according to the dental age (number of erupted permanent teeth surfaces) and history of caries (dmf-s). three hundred and seven children were examined at the end of the study and the results show a statistically significant decrease at the 0.05 level, between the experimental groups and control group, in the incidence of caries - 35.19% for the mouthwashing group (group i); 37.91% for the toothbrushing group (group ii); and 49.77% for the mouthwashing and toothbrushing group (group iv) - on the surfaces present at the beginning and the surfaces that erupted during the study. when the experimental groups were compared there was no signicant difference in proportions at the 0.05 level.
