Usefulness of Fractional Exhaled Nitric Oxide-Guided Treatment in Patients with Asthma-Chronic Obstructive Pulmonary Disease Overlap  [PDF]

Abstract:
Background: Some patients present clinical features of both asthma and chronic obstructive pulmonary disease (COPD), which has led to the recent proposal of asthma-COPD overlap (ACO) as a diagnosis. Fractional exhaled nitric oxide (FeNO) is a candidate biomarker to diagnose ACO. We assessed the effect of an add-on treatment with budesonide/formoterol (BUD/FM) combination in patients with ACO, which was diagnosed by FeNO. Methods: This was a prospective, single-arm, open-label, before and after comparison study. Subjects included 83 patients with COPD who attended outpatient clinics for routine checkups at Shizuoka General Hospital between June and November 2016. All patients fulfilled the GOLD definition of COPD and were receiving long-acting muscarinic antagonist (LAMA) or LAMA/long-acting β2 agonist (LABA) combinations. After an 8-week run-in period, BUD/FM was added to the patients with FeNO levels of ≥35 ppb, defined as having ACO. For patients receiving LAMA/LABA, BUD/FM was added after the discontinuation of LABA. The modified British Medical Research Council (mMRC) score, COPD assessment test (CAT) score, spirometric indices, forced oscillation parameters, and FeNO were assessed before and after 8 weeks of BUD/ FM add-on treatment. Results: Twenty-four patients (28.9%) had FeNO levels ≥ 35 ppb, and 17 patients completed the study (mean age: 73 years and GOLD I/II/III/IV, 5/10/1/1). The mean CAT scores significantly improved (9.2 to 5.4, p = 0.015) and 10 patients (58.8%) showed ≥2 points improvement, a minimal clinically important difference. The mean FeNO levels significantly decreased from 63.0 to 34.3 ppb (p < 0.006). However, there were no changes in mMRC scores, spirometric indices, or forced oscillation parameters. Conclusions: FeNO-guided treatment with BUD/FM improves symptoms in patients with ACO.
Engineering Management Framework to Achieve Safety of a Service of an Organization Based on Social Acceptance  [PDF]

Abstract: We aim at a management framework of an organization in order to achieve its safety in a limited case for a system in-volving the organization for the service, its users and the surrounding society. The proposed framework is possible to allow safety managers to specify tasks involving safety measures based on the concept of “a study of safety”. In the beginning, we discuss the definitions of safety and acceptance, which will be the target of safety management. Next, problems in which the hierarchy of components in the safety systems involves are noted. We then propose the following details of the management framework to achieve the safety of the service of the organization based on acceptance by the surrounding society. Applications of the components in the proposed framework are effectively demonstrated. The shown application can aid in visualizing a way of specification of the proposed concept in a target system.
Relationship between Abdominal Strength Measured by a Newly Developed Device and Abdominal Muscle Thickness  [PDF]

Abstract:

Although measurement of abdominal strength is important for evaluating trunk myofunction, adequate devices for these measurements have not been developed. As a consequence, the relationships between abdominal strength and abdominal muscle thickness have also not been determined. This study aimed to develop a new device for measuring abdominal strength and to examine the relationship between abdominal muscle strength and thickness. The subjects in the study included 50 young males (age, 19.1 ± 0.32 years; height, 171.0 ± 6.55 cm; weight, 69.5 ± 12.15 kg). The measurement of abdominal flexion strength had high reliability with an intra-class correlation coefficient (ICC) >0.9. The thickness of the rectus abdominis muscle was measured under two conditions using an ultrasound imaging method; relaxation and strength exertion. Abdominal muscle thickness was significantly thicker during exertion than during relaxation. Abdominal flexion strength showed a significant correlation with muscle thickness during both relaxation (r = 0.69) and strength exertion (r = 0.71). No significant difference was found between these correlation coefficients. In conclusion, the values of abdominal strength determined by our newly-developed measuring device had high reliability, with abdominal flexion strength showing a relatively close relationship with the thickness of the rectus abdominis during strength exertion and relaxation.

Motivations Underlying the Snack Choices of Japanese Junior High School Students: A Study Based on Cluster Analysis  [PDF]

Abstract: The aims of the present study were to group junior high school students according to factors influencing their snack choice and identify differences between the characteristics and snack choice behaviors of the groups. An anonymous self-report questionnaire was administered to 1936 students (13 - 15 years old) in eight junior high schools in Tokyo, Japan. The students responded to the Snack Choice Questionnaire (SCQ) as well as questions concerning snacking behaviors, lifestyles, and demographics. Cluster analysis was applied to sort the sample according to SCQ scores. Each cluster’s characteristics were examined by gender using the χ2 test or the Kruskal-Wallis test with a Bonferroni adjustment. Participants were divided into four groups: high in all factors, low in all factors, low value in health, and value in health. The “value in health” group was composed of more girls than boys and the “low in all factors” group had fewer girls than boys. Those in the “high in all factors” and “low value in health” groups indicated that they consumed more snacks. Moreover, gender differences were revealed. By assessing the SCQ prior to providing nutrition education, we can obtain valuable information to make nutrition education planning.
Pancreas Procurement and Preservation for Islet Transplantation: Personal Considerations

Abstract: Pancreatic islet transplantation is a promising option for the treatment of type 1 diabetic patients. After the successful demonstration of the Edmonton protocol, islet transplantation has advanced significantly on several fronts, including improved pancreas procurement and preservation systems. Since we frequently use pancreata from donors after cardiac death in Japan,we have applied the in situ regional organ cooling system for pancreas procurement to reduce the warm ischemic time. To reduce the apoptosis of pancreatic tissue during cold preservation, we have applied the ductal injection of preservation solution. For pancreas preservation, we use modified Kyoto solution, which is advantageous at trypsin inhibition and less collagenase inhibition. In this paper, we show pancreas procurement and preservation in our group for islet transplantation.
Mechanism of Reflex Regulation of the Gastroduodenal Function by Acupuncture

Abstract: Many clinical studies focus on the effects of acupuncture on digestive disorders. However, few studies describe the mechanism by which these effects are produced. We present some recent experimental work on the mechanism of acupuncture for reflex regulation of gastroduodenal function in anesthetized rats. In anesthetized rats, it has been proven that acupuncture to the abdomen excites sympathetic nerves via spinal reflexes causing inhibition of motilities while acupuncture of limbs excites vagus nerves via supraspinal reflexes causing an increase in the motilities. It has also been shown that in order to inhibit gastric motilities, acupuncture stimulation of the abdomen must be strong enough to excite group VI fibers of the afferent intercostal nerves. To increase gastric motilities, acupuncture stimulation to hind limbs must be strong enough to excite the high-threshold group III fibers of tibial nerves. It has also been shown that the neural mechanism of duodenal motility stimulation by acupuncture involves the same body regions and intensity of stimulation as that of gastric motilities. Theories regarding the underlying mechanism have proposed somato-autonomic reflexes and responses via endogenous opioids, etc., but without definitive conclusions.
Choice of Domestic Air-Sourced Solar Photovoltaic Thermal Systems through the Operational Energy Cost Implications in Scotland

Abstract: In Scotland, homebuilders are requested to take valiant efforts to meet the government’s ambition that all newly built homes should be carbon-neutral by 2016/17. In delivering net zero carbon homes, the application of renewable energy technologies, such as solar photovoltaic (PV) power generating systems, is almost inevitable. Cost-effectiveness of emerging green technologies is a major factor that affects stakeholders’ housing design decision-making on whether or not the innovations can be applied in practical terms. Based on the United Kingdom (UK) government’s Standard Assessment Procedure (SAP) for energy rating of dwellings, this study conducted a comparative value assessment of 19 design alternatives set. The options also included ones that encompassed both electricity and heat generation potentials of PV applications— i.e., air-sourced PV thermal (PV/T) systems. Based on the SAP simulation results, it concluded that operational energy use and cost, as well as carbon dioxide (CO 2) emission levels, can drastically be reduced particularly when a PV/T system is combined with a low-energy and high-performance mechanical ventilation with heat recovery (MVHR) system that can extract fresh air heated by PV. This study led to visualizing the cost-effectiveness of PV/T MVHR systems and identifying the economic value over 10 years at the interest rate of 10%, based on an assumption that the innovations are applied to Scottish homes today.

Recent advances in stem cell research for the treatment of diabetes

Abstract: The success achieved over the last decade with islet transplantation has intensified interest in treating diabetes, not only by cell transplantation, but also by stem cells. The formation of insulin-producing cells from pancreatic duct, acinar, and liver cells is an active area of investigation. Protocols for the in vitro differentiation of embryonic stem (ES) cells based on normal developmental processes, have generated insulin-producing cells, though at low efficiency and without full responsiveness to extracellular levels of glucose. Induced pluripotent stem cells, which have been generated from somatic cells by introducing Oct3/4, Sox2, Klf4, and c-Myc, and which are similar to ES cells in morphology, gene expression, epigenetic status and differentiation, can also differentiate into insulin-producing cells. Overexpression of embryonic transcription factors in stem cells could efficiently induce their differentiation into insulin-expressing cells. The purpose of this review is to demonstrate recent progress in the research for new sources of β -cells, and to discuss strategies for the treatment of diabetes.

Introduction Diabetes mellitus is a devastating disease, and over 200 million people are affected worldwide, thus representing about 6% of the world population. Type 1 diabetes results from the autoimmune-mediated destruction of insulin-secreting β cells in the islets of Langerhans of the pancreas. Pancreatic islet transplantation represents a viable option for the treatment of patients with unstable type 1 diabetes mellitus with frequent severe hypoglycemia and hypoglycemia unawareness [1, 2]. Recent advances in islet transplantation, including the utilization of donors after cardiac death (DCD) [3–6], single-donor islet transplantation [7–10], and living-donor islet transplantation [11], were based on advanced pancreas transport systems [9, 12, 13], improved islet isolation methods [14–17], enhanced islet engraftment [18–21], and revised immunosuppressant protocols [6, 14, 22]. One of the most important issues affecting islet transplantation is concerned with donor quality [23]. Several critical donor factors have been identified, including donor age, body mass index (BMI), cause of death, usage of vasopressor, hypotensive episode, length of hospitalization, blood glucose levels, transaminases level, creatinine levels, cold preservation time, and procurement team [23–26]. Therefore, effective pancreas procurement and preservation are important for successful islet isolation and transplantation. In this paper, the current advances in pancreas procurement and preservation for islet transplantation in our group are described. 2. Pancreas Procurement Pancreata from donors with brain death (DBD) are procured using a standardized technique to minimize warm ischemia. A preservation solution, such as the
