Local Cutaneous Effects Associated with Chlorhexidine-Impregnated Gel Dressing in Hematopoietic Stem Cell Transplantation Patients  [PDF]

Abstract:

Introduction: Hematopoietic stem cell transplantation (HSCT) often requires a central venous catheter (CVC) for quick and safe vascular access. Currently, new technologies are available to protect the catheter insertion site, such as chlorhexidine-impregnated gel dressings (CIGD). Objectives: To evaluate local cutaneous effects associated with CIGD in patients undergoing HSCT. Methods: In this cross-sectional, prospective study, we evaluated 25 HSCT patients who had a CVC inserted. Patients were visited daily to monitor the CIGD changing procedures and evaluate abnormalities of the underlying skin after dressing removal. Findings: Local erythema was the most frequently detected abnormality, although usually transient and considered secondary to the mechanical trauma of dressing removal. The most severe lesions, consisting of areas of skin loss, erythematous plaques and/or vesicles were classified as skin irritation and presented in 11 (44%) of the 25 patients. An association test showed that skin irritation was more frequent in patients who underwent allogeneic HSCT (p = 0.03). Skin irritation was most frequently observed in areas of contact with the non-woven polyester adhesive tape (n = 22; 88%), which made up the adhesive margins of the dressing. The CIGD was discontinued in 6 (54%) of the 11 patients who presented severe skin injuries. Conclusion: In this study, we detected that skin irritation was more frequent in patients who underwent allogeneic HSCT. The most common skin manifestation was skin loss.

Schistosoma mansoni Sambon, 1907: morphometric differences between adult worms from sympatric rodent and human isolates

Abstract: A computer software for image analysis (IMAGE PRO PLUS, MEDIA CYBERNETICS) was utilized in male and females adult worms, aiming the morphological characterization of Schistosoma mansoni samples isolated from a slyvatic rodent, Nectomys squamipes, and humans in Sumidouro, Rio de Janeiro, Brazil and recovered from Mus musculus C3H/He. The following characters for males's testicular lobes were analyzed: number, area, density, larger and smaller diameter, longer and shorter axis and perimeter and extension; for females: area, longer and shorter axis, larger and smaller diameter and perimeter of the eggs and spine; oral and ventral suckers area and distance between them in both sex were determined. By the analysis of variance (one way ANOVA) significant differences (p<0.05) were observed in all studied characters, except for the density of testicular lobes. Significant differences (p<0.05) were detected for all characters in the female worms. Data ratify that sympatric isolates present phenotypic differences and the adult female characters are useful for the proper identification of S. mansoni isolates.
Synchaeta jollyae (Shiel & Koste) (Rotifera, Synchaetidae): an overlooked species or a newcomer in Neotropical freshwaters?

Abstract: populations of synchaeta jollyae (shiel & koste, 1993) (rotifera), a species recently recorded for the first time in brazil and south america, were analyzed in reservoirs in southeast brazil. sampling was carried out monthly from august 2006 to july 2007 at furnas reservoir in the rio grande basin, state of minas gerais, and in four cascade reservoirs in the tietê river basin (barra bonita, bariri, ibitinga and nova avanhandava) state of s?o paulo, in june and september 2008 and in january and may 2009. synchaeta jollyae occurred in most samples and periods. from the results obtained it is evident that s. jollyae occurs in water bodies of varied trophic status but reaches larger populations in eutrophic water bodies and during lower temperature periods. the greatest densities of s. jollyae were found in the eutrophic bariri reservoir, on the tietê river, during the winter. mann-whitney test confirmed the significant difference between the population densities in periods of high and low temperatures, with populations reaching higher densities at lower temperatures. it is not yet possible to tell whether s. jollyae is a widely distributed species that has been overlooked in previous plankton studies in south america. wherever these populations of s. jollyae might have originated, it appears to be a species well established and adapted to a wide range of conditions in the neotropics.
Collagen V-induced nasal tolerance downregulates pulmonary collagen mRNA gene and TGF-beta expression in experimental systemic sclerosis

Abstract: Female New Zealand rabbits (N = 12) were immunized with 1 mg/ml of collagen V in Freund's adjuvant (IM). After 150 days, six immunized animals were tolerated by nasal administration of collagen V (25 μg/day) (IM-TOL) daily for 60 days. The collagen content was determined by morphometry, and mRNA expressions of types I, III and V collagen were determined by Real-time PCR. The TGF-beta expression was evaluated by immunostaining and quantified by point counting methods. To statistic analysis ANOVA with Bonferroni test were employed for multiple comparison when appropriate and the level of significance was determined to be p < 0.05.IM-TOL, when compared to IM, showed significant reduction in total collagen content around the vessels (0.371 ± 0.118 vs. 0.874 ± 0.282, p < 0.001), bronchioles (0.294 ± 0.139 vs. 0.646 ± 0.172, p < 0.001) and in the septal interstitium (0.027 ± 0.014 vs. 0.067 ± 0.039, p = 0.026). The lung tissue of IM-TOL, when compared to IM, showed decreased immunostaining of types I, III and V collagen, reduced mRNA expression of types I (0.10 ± 0.07 vs. 1.0 ± 0.528, p = 0.002) and V (1.12 ± 0.42 vs. 4.74 ± 2.25, p = 0.009) collagen, in addition to decreased TGF-beta expression (p < 0.0001).Collagen V-induced nasal tolerance in the experimental model of SSc regulated the pulmonary remodeling process, inhibiting collagen deposition and collagen I and V mRNA synthesis. Abstract: the effect of recipients and substrate compositions was evaluated on seedling production, the height of cutting and harvest interval on the production of lemon balm (melissa officinalis l.) cultivated in greenhouse. for the transplants trial a randomized experimental design in factorial scheme 5 x 4 with three replications was used. five compositions of vermicompost (v) and bovine manure (b) (1v:0b; 2v:1b; 1v:1b; 1v:2b; 0v:1b) and four concentrations of the compositions mixed with coconut dust (0; 20; 40; 60%). for the harvest interval essay a completely randomized block experimental design was used, in a 4 x 2 factorial scheme with three replications using the split plot model. in the plots four harvest intervals (every 8; 9; 10 and 11 weeks) and in the split-plots two cutting heights (5 and 10 cm) were evaluated. in the seedling essay the presence of vermicompost on the substrate composition resulted in vigorous transplants. better root system was observed in treatments using 60% of vermicompost and 40% of coconut dust followed by the substrate composed of 26.7% of vermicompost, 13.3% of bovine manure and 60% of coconut dust, which also proportioned highest plant height. in the regrowth essay the cutting at 5 cm of height resulted in a reduction of plant survival, and this fact did not affect the dry matter of leaves per plant. harvesting of regrowth plants can be carried out eleven weeks after the first cutting.

Abstract: Trata-se de um estudo histórico, que enfoca a produ o científica acerca da temática “Saúde da Crian a” dos Congressos Brasileiros de Enfermagem nos anos 70, destacando os temas oficiais e livres dos referidos eventos. Os objetivos s o: verificar o número de temas oficiais e livres apresentados nos CBEn e identificar as temáticas relacionadas à saúde da crian a. Constata-se que a referida temática foi pouco abordada, sendo que o XXVII CBEn, em 1975, e o XXX CBEn, em 1978, apresentaram os maiores percentuais, ou seja, 1,53% de temas oficiais e 3,08% de temas livres relacionados à temática respectivamente.

Abstract: the study attempts to describe the nursing care given to newborns and to analyze the modifications in the nursing care for newborns in the period from 1937 to 1979. it is a historical study. the primary sources were fourteen articles published in the brazilian magazine of nursing during the period in question, and the secondary sources were national and international books and periodicals which refer to the history of pediatrics and of neonatology. the detailed description of the direct nursing cares (basic and specific) and indirect nursing care given to the newborn from the nursing staff during this period became evident through such analysis. among the basic direct nursing cares are detached: corporal hygiene and care for the umbilical stump. the specific direct cares are: respiratory infections and syndromes. the indirect cares are related to material and equipment. it was concluded that the official speech of the nurses was directed towards the systematization of nursing care for newborns, as pioneers in neonatal intensive cares.

Abstract: the objectives of the present study were to assess the reliability of the critical care family needs inventory on a sample of 72 relatives of newborns staying in a neonate intensive care unit and to identify the importance of these needs. this quantitative, descriptive, cross-sectional study was carried out from july to september 2008. to assess the importance of the needs, the mean scores of each subscale were calculated together with the total of the items in the instrument. to assess reliability, cronbach's alpha coefficient was calculated. the instrument presented adequate values of internal consistency for the total of items and for the subscales. the most important needs were those related to safety, information and closeness. to be aware of family members needs enables nurses to introduce actions to ensure a better quality of care provided to newborns and their families.
Assistência ao recém-nascido: perspectivas para o saber de enfermagem em neonatologia (1937-1979)

Abstract: El estudio tiene como objetivo describir los cuidados de enfermería al recién nacido y analizar las modificaciones en la atención de enfermería al recién nacido durante el período de 1937 hasta 1979. Se trata de un estudio histórico. Las fuentes primarias son 14 artículos publicados en la Revista Brasile a de Enfermería en el período en cuestión, y las fuentes secundarias son los libros y periódicos nacionales e internacionales con relación a la historia de la pediatría y de la neonatología. Es evidente, en este período, la descripción detallada de los cuidados directos (básicos y específicos) y los cuidados indirectos oferecidos por el equipo de enfermería al recién nacido. Entre los cuidados directos básicos, se destacan: higiene corporal y cuidados con el cordón umbilical. Los cuidados directos específicos son: infecciones y afecciones respiratórias. Los cuidados indirectos se relacionan con el material y el equipamiento. Se concluye que el discurso oficial de las enfermeras estuvo volcado para a la sistematización de los cuidados de enfermería al recién nacido, como pionera de los cuidados intensivos neonatales.
NECESSIDADES DE FAMILIARES DE PACIENTES INTERNADOS EM UNIDADE DE TERAPIA INTENSIVA NEONATAL

Abstract: Los objetivos del presente estudio son: evaluar la confiabilidad del Inventario de Necesidades y Estresores de Familiares de Pacientes en Cuidados Intensivos en 72 familiares de recién nacidos internados en una unidad de cuidados intensivos neonatales e identificar la importancia de dichas necesidades. El estudio cuantitativo, descriptivo y transversal se realizó durante el período de julio a septiembre de 2008. Para evaluar la importancia de las necesidades se calculó el promedio de cada subescala y del total de los ítems del instrumento, y para la confiabilidad se calculó el coeficiente alfa de Cronbach. El instrumento presentó satisfactorios valores de coherencia interna para todos los ítems y las subescalas del instrumento. Las necesidades consideradas como las de mayor importancia fueron las relacionadas con seguridad, información y proximidad. El conocer las necesidades de los familiares le permite al enfermero implementar acciones que garanticen una asistencia de mejor calidad a los recién nacidos y sus familias.
