Abstract: barogenic rupture of the stomach is a rare complication following cardiopulmonary resuscitation, administration of nasal oxygen by catheter and diving accidents. we report a case of gastric barotrauma following oroesophageal intubation. in most cases, the tears occur along the lesser curvature, what have been already attributed to laplace's formula and, more recently, to morphological features of the stomach.

Abstract: background: the aim of this study is to determine the feasibility, reability and safety of the subtotal splenectomy performed by laparoscopic means, comparing these data with those obtained by mean of laparotomic splenectomy. methods: an experimental study was conduced in accordance with the principles of the international guiding for biomedical research involving animals. fourteen adult mongrel dogs of both sexes were divided into two groups (n = 7): group 1 - laparoscopic subtotal splenectomy, preserving the upper pole supplied by splenogastric vessels; the splenic section, haemostasis and closure was performed with a 45 mm linear laparoscopic surgical stapler. group 2 - total splenectomy was performed through a median laparotomy. after three months, we accomplished haematological, immunological and scintigraphic exams in all animals. sections of the remnant spleens, livers and lynph nodes were histologically processed. results: blood loss was minimal and all animals survived to operation and during the three months follow-up. reduction (p < 0.005) of the platelets was verified in the group 1. the rest of the haematological mean values remained normal. the remnant spleen, liver and lymph nodes showed normal histological aspects, with increasing amounts of polymorphonuclear cells observed in both groups. conclusion: size it is feasible and safe to perform subtotal splenectomy by laparoscopic means in dog.

Abstract: Cutaneous mast cell tumor (MCT) shows a variable biological behavior in dogs and may present either as solitary masse that can be treated and cured with surgical removal or as a systemic metastatic and fatal disease. Histological grade, KIT pattern and proliferative index are typically prognostic factors in MCTs. In the present study, we have investigated correlation between clinical data (breed, age, gender, tumour location, tumor size, time before surgery, number of tumours, occurrence of metastasis and recurrence), cellular proliferation (Ki-67, mitotic index), intratumoural microvessel density (IMVD) and apoptotic index with the histological grade and KIT pattern. 28 tumors, from 20 dogs with cutaneous MCT were evaluated. There was association between histological grade, IMVD, tumor ulceration and number of tumors. A significant increase of Ki-67 expression and mitotic index was observed in MCTs with cytoplasmic KIT staining pattern. Patnaik histological grade system was related to mitotic index. Histological grade in canine cutaneous MCT should not be assessed as the only prognostic factor, but associated with KIT pattern, IMDV, cellular proliferation, presence of tumour ulceration, number of tumours, recurrences and metastases.

Abstract: background: since 1979 this group of authors is doing conservative splenic surgical procedures by mean of subtotal splenectomy and splenic autotransplantation. these procedures were used in over 300 patients to treat different pathological conditions. aim: to present for the first time in the world subtotal splenectomy and splenic autotransplantation by laparoscopic means, as a new treatment for severe pain due to ischemic spleen. methods: three patients presented left abdominal severe pain due to diffuse ischemia of the spleen. this symptom was resistant to all conservative treatment. laparoscopic subtotal splenectomy, with preservation of the upper splenic pole was indicated in two cases because this was the only part of the spleen without signs of ischemia. in a third patient ischemia was diffuse and total splenectomy with 20 implants of the splenic tissue on the greater was carried out. results: these procedures were safely conducted with minor bleeding and no technical difficulties or complications. the postoperative follow-up has been uneventful and hematological, immunological, tomographic and scintigraphic exams confirmed the vitality and functionality of the splenic remnants. the pain disappeared since the first post-operative day in the three patients. conclusion: it is feasible and safe to perform subtotal splenectomy and splenic autotransplants by laparoscopy and these procedures seem to be a good treatment for pain due to ischemic spleen.

Abstract: cytomegalovirus infection frequently develops in immunocompromised patients, either in post-transplanted patients or in patients with diseases that affects the immune system, such as aids; we present a case of a 49 year old female patient in late post surgery (14 years) of a kidney transplant, using immunosuppressing medication. the patient presented abdominal pain, weight loss and initial diarrhea, she was hospitalized and developed severe hematochezia and hypotension. the patient was submitted to a colonoscopy and presented friable aspect mucosa, with aphthoid ulcers, fibrin and pseudopolyp formation; biopsy revealed cytomegalovirus. her condition did not improve after the use of foscarnet, and surgical treatment was necessary. the patient died 5 days later.

Abstract: peritoneal pseudomyxoma is a pathological condition that compromises the peritoneum, characterized by the production of large quantities of mucinous liquid, which progressively fills the peritoneal cavity, originating in general from mucinous appendicular or ovarian tumors. we report a peritonial pseudomyxoma associated with mucinous adenocarcinoma of the appendix synchronic with adenocarcinoma of the rectum in 44 years old patient, where the initial diagnosis was rectal adenocarcinoma. tumour of the appendix and peritonial pseudomyxoma were incidental and found intraoperativelly. we focus the histological patterns of the lesions, diagnosis and the treatment, with revision of the literature.
Cirurgia ambulatorial em proctologia: análise retrospectiva de 437 casos

Abstract: background: the number of outpatient surgical procedures performed in hospitals, as well as in private clinics, increases daily. in some countries, such as france, outpatient operations outnumber inpatient operations. objective: to evaluate results obtained at the surgical outpatient clinic of the clinics hospital of the medical school, botucatu, sp, brazil. patients and methods: a retrospective study of 437 clinical cases of anorectal anomalies, analyzing the variables age bracket, gender, pathology, and postoperative complications. results: we observed predominance of patients younger than 45 (62.8%) and of females (56%). the most common ailment was hemorrhoids (45.1%) and the most frequent postoperative complications were pain (9.8%) and bleeding (7.3%). conclusions: our results demonstrate that it is possible to perform various simple procedures related to the treatment of anorectal abnormalities at the outpatient level and therefore at a lower cost. the number of postoperative complications was no higher than that resulting from inpatient surgery.

Abstract: malignant tumors of the anal canal and anus are rare pathologic events, representing less than 2% of all tumors of the colon, rectum and anus; according to reports, the incidence of melanoma does not achieve 0,1 to 1,2% of malignant tumors. authors describe two cases of melanoma, discussing the data described in literature, focusing on the diagnosis aspects, treatment, evolution and prognosis. cure rates are low and the disease presents high rates of mortality in short term.

Abstract: two-hundred fifteen patients were followed for a 20 year period, being chagas disease was the main cause of the disease. all patients were submitted to a transversal extramucosal esophagogastroplasty and parcial esophagogastrofundoplicature, which we call "girard-toupet-lind" procedure. this procedure was used for the 2nd, 3rd and 4th degree megaesophagus. the mean age of the patients ranged from 30 to 40 years, being 55,6% for males and 44,4% for females. the systematization of the surgical technique is described by the authors. attention is called for the simplicity of the procedure through abdominal approach, as well as the need of large passage in the esophagogastric region through a transverse extramucosal esophago- cardiomioplasty and parcial antireflux valve. the authors report early and late postoperative complications, pointing out the very low morbidity. long term results of the surgery were carried out through clinic, radiological, endoscopic and histopathologic examinations. the authors realized that this surgical procedure is safe and brought real benefits to the patients with megaesophagus, even to those 4th degree patients.

Abstract: Canine visceral leishmaniasis is an endemic infectious disease in some S o Paulo state regions and even though it is a systemic disease, in the dog, the main clinical sign is dermatological. Thirty dogs with positive results in serology and parasitological exams for leishmaniasis from Ara atuba city were evaluated. They just showed dermatological signs and were divided in two groups, one with exfoliative (n=1 ) and other with ulcerative (n=1 ) lesions. Histopathological patterns in the group of exfoliative lesions were: periadnexial dermatitis (5/15, 33,3%), superficial perivascular dermatitis (1/15, 6,6%), nodular dermatitis (1/1 , , %) and mixed dermatitis (8/1 , , %), including intersticial/periadnexial dermatitis (1/8, 12,5%), lichenoid/perivascular superficial and deep dermatitis (1/8, 12,5%), perivascular superficial and deep/periadnexial dermatitis (1/8, 12,5%) and superficial perivascular/perianexial dermatitis ( /8, , %). In the group of ulcerative lesions, the histopathological patterns were: perivascular superficial and deep dermatitis (5/15, 33,3%), diffuse dermatitis (3/15, 20%), periadnexial dermatitis ( /1 , 1 , %), nodular dermatitis (1/1 , , %) and mixed dermatitis ( /1 , , %), including intersticial/ perivascular superficial and deep dermatitis (1/4, 25%), nodular/periadnexial dermatitis (1/4, 25%), fibrosing/perianexial dermatitis (1/4, 25%) and perivascular superficial and deep/ periadnexial dermatitis(1/ , %). Parasites were found in eight dogs (8/1 , , %) with exfoliative dematitis and seven ( /1 , , %) with ulcerative dermatitis. A leishmaniose visceral canina é uma doen a infecciosa cr nica considerada endêmica em algumas regi es do estado de S o Paulo e, apesar de ser uma enfermidade sistêmica, no c o, a maioria dos sinais clínicos s o dermatológicos. Foram avaliados trinta c es com diagnóstico de leishmaniose, da regi o de Ara atuba, pelos exames parasitológico e sorológico. Os c es apresentando apenas sinais dermatológicos foram divididos em dois grupos: um grupo com descama o e outro com ulcera o. Os padr es histopatológicos da pele encontrados nos c es com les es descamativas (n=15) foram de dermatite perianexial (5/15, 33,3%), dermatite perivascular superficial (1/15, 6,6%), dermatite nodular (1/15, 6,6%), e dentre as dermatites mistas (8/15, 53,3%), dermatite intersticial/perianexial (1/8, 12,5%), dermatite liquenóide/perivascular superficial e profunda (1/8, 12,5%), dermatite perivascular superficial e profunda/perianexial (1/8, 12,5%) e dermatite perivascular superficial/perianexial (5/8, 62,5%).
