Abstract: this study aimed to compare clinical characteristics, evolution and severity of adult patients admitted in public and private intensive care units. it is a retrospective, longitudinal and quantitative analysis of 600 patients admitted in four intensive care units of s?o paulo, brazil. differences were found between patients admitted in private and public hospitals regarding the following variables: age, origin, length of stay and mortality in the critical unit, cardiac, hematological, neurological and kidney failures and some comorbidities. the results reveal the importance of analyzing in detail clinical characteristics and healthcare of patients admitted in public institutions, because of the high mortality found. the intensive care nurse can contribute to change this scenario, because she/he has a leadership role in planning and provision of resources for intensive care.

Abstract: La investigación tuvo como objetivo comparar las características clínicas, evolución y gravedad de pacientes adultos ingresados en Unidades de Cuidados Intensivos públicas y privadas. Este es un estudio retrospectivo, longitudinal y cuantitativo de 600 pacientes ingresados en cuatro Unidades de Cuidados Intensivos en S o Paulo, Brasil. Fueron encontradas diferencias entre los pacientes de los hospitales privados y públicos para las variables: edad, procedencia, tiempo de internación y mortalidad en la unidad crítica, insuficiencia cardiológica, hematológica, neurológica y renal, además de algunas comorbilidades. Los resultados revelan la importancia de se analizar las características clínicas y la asistencia prestada en las instituciones públicas, en comparación a la tasa de mortalidad. La Enfermera de Cuidados Intensivos puede ayudar a cambiar esta situación, ya que tiene un papel de liderazgo en la planificación de la atención y la provisión de recursos para cuidados intensivos.
Patient Destination after Discharge from Intensive Care Units: Wards or Intermediate Care Units?

Abstract: this study characterizes patients hospitalized in intensive care units (icus) of hospitals that have intermediate units (iu) regarding their demographic and clinical data and identifies factors related to discharge from these units. this prospective longitudinal study involved 600 adult patients hospitalized in general icus in four hospitals in s?o paulo, sp, brazil. demographic and clinical characteristics were similar to those found in other studies addressing patients hospitalized in icus. factors associated with discharge from icu to iu were: age ≥60 years, diseases related to the nervous, circulatory or respiratory systems, originated from the iu, and simplified acute physiologic score ii (saps ii), logistic organ dysfunction (lods) and nursing activities scores (nas) at admission and discharge from the icu. age and risk of death at admission in the icu, according to saps ii, stood out as indicators of discharge to ius in the multiple logistic regression analysis.
Factors associated with death and readmission into the Intensive Care Unit

Abstract: this study identifies the factors associated with death and readmission into the intensive care unit. this is a longitudinal prospective study of 600 adult patients admitted in general intensive care units of four hospitals in s?o paulo, brazil. the multiple regression analysis showed that patients with a longer length of stay and higher nursing activities score, simplified acute physiology score ii and logistic organ dysfunction score on the last day in the unit had a higher chance of death. nevertheless, higher nursing activities score and logistic organ dysfunction score on the first day in the intensive care unit were protective factors against death. viral hepatitis as an antecedent increased the chance of readmission. on the other hand, higher nursing activities score at discharge decreased this risk. it was concluded that the nursing workload was the main variable related to the outcomes analyzed.
Clinical evolution of adult, elderly and very elderly patients admitted in Intensive Care Units

Abstract: this study compared clinical outcomes among adult, elderly and very elderly patients admitted to intensive care units (icus) located in s?o paulo, brazil. this retrospective, longitudinal and comparative study included 279 adult (≥18 and <60 years), 216 elderly (≥60 and <80 years) and 105 very elderly (≥80 years) patients. adult patients differed from other groups regarding the unit to which they were referred and severity, according to the simplified acute physiology score ii. adults were most frequently sent to hospitalization wards; elderly and very elderly patients who survived hospitalization in critical units showed sharper improvement before discharge. there were differences in relation to mortality between adult and elderly patients, with a higher rate in the elderly group; however, the mortality rate of very elderly and adult patients was similar. in general, the results indicated that older age was not associated with undesirable outcomes in icus.

Abstract: the study's objectives were to compare nursing workload among adults, elderly and very elderly patients, including in the analyses the interventions and evolution of the workload between admission and discharge. this prospective longitudinal study involved 600 adult patients in general icu in four city hospitals in sao paulo. the results showed that, independent of the age, it had the similarity of the nursing workload in the admission, as well as in the evolution of the patients' care. differences between the groups were observed in the nas on icu's discharge and the following instrument's items: monitoring and titration and hygiene procedures in admission, respiratory support and intravenous hyperalimentation in discharge and mobilization and positioning in discharge and admission. in conclusion, the results evidenced that the age interfered only in specific aspects of the nursing workload required by patients in icus.

Abstract: the study had the purpose to investigate aspects of the icus from the municipal district of s?o paulo, regarding the composition of the clinical staff; characteristics of the nursing personnel and nurses' characterization. data were collected through 47 questionnaires distributed in 32 hospitals. results show that: the morning and the afternoon shifts have a larger number of people from all clinical categories, the majority of the nursing personnel work 36 hours per week, nurses' wages varied from 700 to 1,500 "reais" and 66.1% of them were enrolled in master programs. authors concluded that 79,2% of the clinical staff was formed by the nursing personnel.

Abstract: this study is part of a project about icu's characteristics in s?o paulo city. this article describes the continuing education programs for nursing staff. 43 icus were analysed and the questionnaire answered by the icu nurse cordinator was used to collect data. results showed that 34 (79,1%) of the icus have initial program for training each nursing staff category and 18 (41,9%) had regular continuing education program focusing primarily on nursing procedures and routines and the update in pathologies. continuing education programs are developed primarily by icu's nurses. 50,2% of the nurses answered that they attended specialisation/ extension courses too, in medical surgical nursing or in other areas or in both areas. due to this result and the development of the icu's nurses as specialists, some suggestions are presented to improve the continuing education program in these units.

Abstract: therapeutic intervention scoring system-28 (tiss-28) is a tool that enables the measurement of the nursing work load in intensive care units and the estimate of how grave the disease is. in this study are presented the operational definitions for its application, proposed by a group of specialists in the area, with the aim of rendering uniform the meaning of each of the items and preventing interpretation biases.

Abstract: objective: to characterize motorcyclists who have been victims of traffic accidents in maringá, paraná regarding their occupation, rehabilitation, or disability. methods: a sample of 61 subjects participated in the study. data were collected in february and march, 1999 though interviews and review of medical records. results: prior to the accident, the majority of subjects (45,9%) were employed. a great number of them (45.9%) were performing work-related activities in the moment of the accident. among the remainder of victims, 29.5% were performing leisure activities. nine to twelve months post accident, 20.4% of the victims had altered productive activities. conclusion: there was a significant statistical association between productive activities and injury severity score, between productive activities and new injury severity score, and between productive activities and perception of health.
