Diabetes does not affect outcome in patients with Enterobacteriaceae bacteremia

Abstract: We reviewed the medical charts of 1112 patients who were treated at a community teaching hospital for Enterobacteriaceae bacteremia from January 1997 through June 2007. Factors associated with in-hospital mortality were analyzed by logistic regression analysis.Among the 1112 patients with Enterobacteriaceae bacteremia, 181 (16.3%) were diabetic patients; 90 patients (8.1%) died while in the hospital. Compared to non-diabetic patients, diabetic patients were older (75.4 ± 11.9 years vs. 70 ± 16.6 years, p < 0.001) and had more comorbidities. However, mortality among diabetic and non-diabetic patients was not different [7.2% vs. 8.2%, RR 1.13; 95% CI (0.67–1.9); p = 0.39]. In a multivariate analysis, the variables associated with in-hospital mortality were age, the origin of the bacteremia, and the presence of immunosuppression. Diabetes was not associated with outcome.In this cohort of patients with Enterobacteriaceae bacteremia, diabetes was not associated with a poorer prognosis.Enterobacteriaceae are the dominant causal agents of Gram-negative bacteremia and are a major cause of morbidity and mortality [1]. The prevalence of diabetes is increasing worldwide [2]; considering the predisposition of diabetes patients to Enterobacteriaceae bacteremia [3], the number of patients with both diabetes and bacteremia caused by Enterobacteriaceae is likely to increase as well. However, information about the effect of diabetes on the prognosis of patients with infections in general, and with Enterobacteriaceae bacteremia in particular, is limited. In addition, some data are contradictory [3-6]. It is significant to determine whether diabetes has a negative impact on the prognosis of Enterobacteriaceae bacteremia so this would support the convenience of a more aggressive approach for these patients. Abstract: objective: to study the prevalence of delayed diagnosis of hiv infection and associated factors. methods: a cross sectional study of patients included in the spanish vach cohort who had been diagnosed with hiv infection between 1997 and 2002 was performed. delayed diagnosis was defined as patients diagnosed with hiv infection and aids simultaneously or within the first month after the first positive serologic test, or those with a first cd4+ cell count below 200/ml. the epidemiological characteristics of these patients were compared with those of the remaining patients. results: of 2,820 new cases of hiv infection, delayed diagnosis was found in 506 (18%). these patients differed from the remaining patients in their lower mean age and higher hiv viral load, as well as in their distribution by sex (higher proportion of males), occupational status, history of incarceration in prison, and hiv-risk transmission group. the median survival during follow-up was significantly lower among aids patients with a delayed diagnosis. conclusions: delayed diagnosis remains a cause for concern in our environment, due to its magnitude and its association with mortality. some epidemiological characteristics provide clues to guide future programs directed at increasing information and improving prevention.
Dyslipidaemia in HIV-infected women on antiretroviral therapy. Analysis of 922 patients from the Spanish VACH cohort

Abstract: Observational, multicentre, cross-sectional study from the Spanish VACH Cohort. 922 women on stable ART without lipid-lowering treatment were included.Median age was 42 years, median CD4 lymphocyte count was 544 cells/mm3, and 85.6% presented undetectable HIV-1 viral load. Median total cholesterol (TC) was 189 mg/dL (interquartile range, IQR, 165-221), HDL cholesterol 53 mg/dL (IQR, 44-64), LDL cholesterol 108 mg/dL (IQR, 86-134), and triglycerides 116 mg/dL (IQR, 85-163). Mean accumulated time on ART was 116 months; 47.4% were on NNRTI-based regimes, 44.7% on PI, and 6.7% on only-NRTI therapy. 43.8% were also hepatitis C (HCV) coinfected. Patients on PI treatment presented higher TC/HDL ratio than those on NNRTI (p < 0.001). Significantly higher HDL values were observed in NNRTI-treated patients. HCV-coinfected patients presented lower TC/HDL ratio than the non HCV-coinfected. In multivariate analysis, factors independently associated with TC/HDL ratio were age, triglyceride levels and HCV co-infection. PI treatment presented a non-significant association with higher TC/HDL ratio.In HIV-infected women, the NNRTI-based ART is associated with a better lipid profile than the PI-based. Factors unrelated to ART selection may also exert an independent, significant influence on lipids; in particular, age, and triglyceride levels are associated with an increased TC/HDL ratio while HCV co-infection is associated with a reduced TC/HDL ratio.The increase in cardiovascular risk (CVR) observed in HIV-infected patients is a cause for concern. Most clinical studies have detected a relationship between cardiovascular disease and traditional risk factors, among which age, male gender, smoking, hypertension and diabetes are the most important. Abstract: Como, para el consumidor, la mayor o menor aceptabilidad de un corte de carne bovina depende principalmente de su terneza, los cortes minoristas son comercializados básicamente con un precio acorde a la terneza esperada y a su facilidad de preparación. No obstante, éste es un parámetro sumamente variable y las causas tienen relación con factores genéticos, condiciones medio-ambientales y tratamiento post-mortem aplicado. A esta particularidad debe adicionársele el hecho de que ningún sistema de tipificación actual es capaz de clasificar las reses garantizando un nivel mínimo de terneza aceptable. Los intentos por medir directamente la terneza u obtener una estimación de ella mediante la utilización de otros parámetros simples no han arrojado hasta el momento resultados expresivos o de fácil aplicación industrial. No obstante, el conocimiento actual ofrece alternativas para no sólo disminuir la alta variabilidad intrínseca de este parámetro, sino también para mejorarlo en determinadas ocasiones. Aún quedan muchas incógnitas por resolver, pero la integración de la cadena productiva de carnes bovinas, incluidos aquí los consumidores, junto al control de los puntos críticos de elaboración y la aplicación de modernas tecnologías significará, seguramente un buen punto de partida para el mejoramiento de la terneza de las carnes para consumo.

Abstract: This paper analyses the normative justification of the adoption of randomized clinical trials as a methodological standard for pharmaceutical policy by the British health authorities in 1946. Through a discussion of the interests of the different parties involved in the process (doctors, patients, pharmaceutical companies and the State) I argue that randomization was adopted namely as an impartial mechanism to allocate treatments, even if its statistical foundations were not properly understood. I contend that such justification is still valid. Este artículo estudia la justificación normativa de la adopción de los ensayos clínicos aleatorizados por parte de las autoridades sanitarias británicas como estándar metodológico en 1946. A partir de un análisis de los distintos intereses de los participantes en el proceso (pacientes, médicos, farmacéuticas y el propio Estado) argumentamos que la aleatorización se adoptó como mecanismo de asignación imparcial de tratamientos, aunque sus propiedades estadísticas no fueran bien comprendidas. Tal justificación permanece aún vigente.

Abstract: Philippe van Parijs' case for a basic income attempts at convincing the moral philosopher and the concerned citizen, both at once. In case it happened, the theory of justice in the rawlsian tradition, as applied to basic income implementation, would have proven its political performance. In this paper, 1 contend that, in order to make his case, van Parijs brings down political debate to reflective equilibrium, excluding prudential considerations, on the one hand, and leaving sentimental education as the only available alternative, on the other, for those who resist the cogency of his arguments. La renta básica se nos presenta en la obra de Philippe van Parijs como una propuesta política filosóficamente argumentada, de modo tal que convencerá tanto al teórico de la justicia como al ciudadano que votará su implantación. En este artículo analizamos la argumentación de Van Parijs mostrando cómo la efectividad política de sus tesis sólo se sostiene a costa de reducir el debate sobre la renta básica a los términos de su propia concepción de la ética. Ponemos en duda, por nuestra parte, el alcance de esta reducción sobre un doble plano: diluye por completo la dimensión prudencial de toda argumentación política, y no deja más alternativa que la educación sentimental para quienes no se dejen convencer por el equilibrio reflexivo.

Abstract: In this paper, we present the virtual desktop that uses a novel methodology and related metrics to benchmark thin client based on Data Delivery Networks (DDN) in terms of scalability and reliability. Most studies of the wireless networks mainly focus on system performance and power consumption circuit system; the main target has been separated in terms of Data operation and GUI operation by DDN. The communication protocol for wireless communication may play a major role in energy consumption and other important factors. The portable devices like Personal Digital Assistance (PDA) and others are mainly focusing on the efficient energy consumption (power control) in wireless networks. Here we focus on energy efficiency, algorithmic efficiency, virtualization and resource allocation; these are the main aims of the authors. The foremost research in the direction of wireless computing in saving energy and reducing carbon foot prints is also the challenging part. This is the study proof of brief account of wireless networks.
