Effectiveness of participatory-action-research to put in practice evidence at a nursing onco-hematology unit

Abstract: the aim was to evaluate the effectiveness of evidence implementation through participatory-action research (par). a prospective quasi-experimental design with two non-equivalent and non-concurrent groups (2006 and 2008) was adopted. the research was conducted at the bone marrow transplant unit of a tertiary-level spanish hospital. to put the evidence in practice, par was adopted as an "intervention studied". the dependent variables were: professional performance and patient outcomes (psycho-emotional area and adverse effects). in total, 125 patients were recruited (1st period=56; 2nd period=69). the results in the second period show significant improvements in professional performance in terms of the quality of the registers of signs and symptoms. in the psycho-emotional area, the psycho-social adjustment improved significantly; without caregiver burden or satisfaction showing any clear tendencies. among the adverse effects, catheter-related thrombosis and catheter-related infection improved significantly; there were no significant differences in the level of pain or mucositis. through the par, evidence could be put in practice and the outcomes under analysis could be improved.
Reduction in inappropriate hospital use based on analysis of the causes

Abstract: Background To reduce inappropriate admissions and stays with the application of an improvement cycle in patients admitted to a University Hospital. The secondary objective is to analyze the hospital cost saved by reducing inadequacy after the implementation of measures proposed by the group for improvement. Methods Pre- and post-analysis of a sample of clinical histories studied retrospectively, in which the Appropriateness Evaluation Protocol (AEP) was applied to a representative hospital sample of 1350 clinical histories in two phases. In the first phase the AEP was applied retrospectively to 725 admissions and 1350 stays. The factors associated with inappropriateness were analysed together with the causes, and specific measures were implemented in a bid to reduce inappropriateness. In the second phase the AEP was reapplied to a similar group of clinical histories and the results of the two groups were compared. The cost of inappropriate stays was calculated by cost accounting. Setting: General University Hospital with 426 beds serving a population of 320,000 inhabitants in the centre of Murcia, a city in south-eastern Spain. Results Inappropriate admissions were reduced significantly: 7.4% in the control group and 3.2% in the intervention group. Likewise, inappropriate stays decreased significantly from 24.6% to 10.4%. The cost of inappropriateness in the study sample fell from 147,044 euros to 66,642 euros. The causes of inappropriateness for which corrective measures were adopted were those that showed the most significant decrease. Conclusions It is possible to reduce inadequacy by applying measures based on prior analysis of the situation in each hospital.
Utilización de la ventilación no invasiva en la insuficiencia respiratoria aguda: Estudio multicéntrico en unidades de cuidados intensivos

Abstract: objectives. study the use of non-invasive ventilation (niv) in patients with acute respiratory failure in intensive care units (icus) in spain. methods. a questionnaire was sent to 254 icus, after which, they were invited to participate in a multicenter, retrospective study, providing detailed information on ventilated patients. results. answers were received from 123 hospitals. of these, 119 used niv, although its use varied greatly. niv is the treatment of choice in 89% of the units for chronic obstructive pulmonary disease (copd), in 79% for acute pulmonary edema (ape), in 53% for postextubation failure, in 53% for pneumonia 53%, and in 17% for acute respiratory distress syndrome (ards). it was used occasionally in copd in 11% of the units, and in 21% of the units for ape. eighteen hospitals provided additional information on 432 ventilated patients, 232 (54%) of whom received niv as first line therapy. presence of pneumonia or acute respiratory distress syndrome (ards) was an independent predictive factor of niv failure (ora = 5.71; ci 95%, 1.83-17.8; p = 0.003). admission in a unit with experience in niv in > 50 patients/year (ora = 0.22; ci 95%, 0.07-0.63; p = 0.005) and a higher pao2/fio2 ratio after one hour of ventilation (ora = 0.98 per point; ci 95%, 0.97-0.99; p < 0.001) were protector factors. conclusions. in spain, niv is widely used but it may continue to be underused in copd and ape. the diagnosis of pneumonia or ards was an independent predictive risk factor. admission in an icu with niv in more than 50 patients/year also have higher pao2/fio2 ratio after one hour of ventilation were predictive factors of success.

Abstract: the implementation of the ects system (european credit transfer system) has been initially tested in the course ?fundamentals of chemistry? with the aim of improving the teaching methodology and student assessments used in previous years. with this aim in mind, the number of lectures has been reduced and students? participation encouraged using both individual tasks (problem solving and class discussions) and group work (cooperative actions). these changes have increased the workload for both student and faculty, but the achieved results indicate learning and attitudinal gains as the number of students approved increased in a 10% respect to previous years.

Abstract: L’article presenta una investigació en l’àmbit d’estudi de l’educació i el gènere que adopta la metodologia de l’estudi de cas per visibilitzar i analitzar els models de gènere de les i els adolescents. En les conclusions s’apunten canvis en els gèneres que s’adopten, moviments que constaten que els models de gènere femení i masculí considerats més tradicionals estan en evolució, perduren trets però altres es reestructuren amb característiques diverses o evolucionen canviant el seu significat. Els resultats de la recerca són una contribució al coneixement sobre els models de gènere i constitueixen un document base per al debat i la reflexió pedagògica sobre les relacions entre educació, diferència sexual i ciutadania inclusiva ______________________________________________ L’article présente une recherche dans le domaine d’étude de l’éducation et du genre qui adopte la méthodologie de l’étude de cas pour découvrir et analyser les modèles de genre chez les adolescentes et les adolescents. Dans les conclusions, il note l’adoption de changements de perspective quant aux genres, de mouvements qui montrent que les modèles des genres féminin et masculin considérés comme les plus traditionnels sont en évolution, que perdurent cependant certains traits mais que d’autres se restructurent avec des caractéristiques différentes ou évoluent en changeant de signification. Les résultats de la recherche constituent une contribution à la connaissance sur les modèles de genre en même temps qu’un document de base pour le débat et la réflexion pédagogiques sur les relations entre éducation, différence sexuelle et citoyenneté inclusive The article describes research in the field of education and gender, using case studies as a methodology to examine and analyse the gender models of adolescents. The conclusions point to changes in the genders adopted, movements which reveal that the models of female and male gender considered traditional are evolving; some features remain but others are being restructured with a range of features or developing, changing their meaning. Abstract: En este estudio se muestra el efecto del desarrollo de una intervención educativa en alumnado con trastornos del espectro autista (TEA) sobre la comprensión de emociones y creencias como medio para la mejora de sus habilidades sociales. Para ello, doce alumnos con TEA de Educación Primaria y Secundaria participaron en un dise o de investigación colaborativa con evaluación inicial y final donde, para la obtención de datos, se elaboraron materiales didácticos para la ense anza y evaluación de la comprensión de emociones y creencias y una escala de valoración de habilidades emocionales y sociales para personas con TEA (EVHES), previamente validada. Los resultados muestran que el desarrollo de métodos colaborativos entre profesorado de alumnos con TEA y especialistas universitarios incide de forma positiva sobre los procesos de ense anza de la comprensión de emociones y creencias y mejora de las habilidades sociales.
Finding evidences on oncohematological patients (2nd part): Catheter-related infection and pressure ulcers

Abstract: There is an enormous knowledge base in the field of health which has no application. Its growth is not limited by its discovery but by the lack of its implementation. The necessity of implementing conclusions is evident in evaluating the real repercussion on quality daily cares; as “strategies” and “outcomes”. In talking about oncohematological nursing, some health outcomes would be: the reduction of adverse events such as nosocomial infections and pressure ulcers. Consequently, our objective has been: to find and summarize the evidences about Venous Catheterization and Pressure Ulcers; in order to encourage the spread of knowledge and promote changes in practice.Method: Databases as CINAHL, MEDLINE, Cochrane Library, EMBASE, Pascal Biomed, LILACS, CUIDEN, CUIDEN qualitative y CUIDATGE were revised in Spanish, French and English. No time restrictions were applied.Findings: The main findings and recommendations were synthesised on a poster, next to suggestions for practical changes to implement, evidence levels used, and the clinic problem significance. The suggested changes arose from nursing staff based on evidences found that includes behaviours and attitudes changes, which should facilitate more rapid innovations diffusion.
El uso de los indicadores cualitativos y cuantitativos en la evaluación de las publicaciones periódicas: el caso del Centro de Investigación y de Estudios Avanzados del Instituto Politécnico Nacional de México

Abstract: A method for evaluation of the Advanced Studies and Research Center (Cinvestav) at the National Technology Institute of Mexico, Library’s periodical collection is described. An evaluation matrix is used to calculate the cost-benefit of periodicals from quantitative and qualitative indicators. Citing analysis of periodicals and dissertations are made and aspects like the impact factor, statistical analysis of use, and suscription costs are also considered. The results obtained allow the evaluation of the general collection status, according to user’s necessities. This study offers elements which make the taking of decisions easier during budget restrictions, reorientation of financial resources, improvement of collections and to increase the efficiency of library services. This method could be useful for any libraries with similar characteristics to the Cinvestav. En este trabajo se aplica un método para evaluar la colección de publicaciones periódicas vigentes del Centro de Investigación y de Estudios Avanzados (Cinvestav) del Instituto Politécnico Nacional (IPN), de México. Se utiliza una matriz de evaluación para la obtención del índice costobeneficio, a partir de la aplicación tanto de indicadores cuantitativos como cualitativos. Se realizan estudios de citación, tanto en revistas como en tesis, y se toman en consideración aspectos tales como el factor de impacto de las publicaciones, datos estadísticos de uso y costo de las publicaciones. Con los resultados obtenidos se pudo evaluar el estado de este acervo en función de las necesidades de los usuarios potenciales y reales. Se ofrecen elementos que facilitarán la toma de decisiones por ajuste presupuestal, la reorientación de recursos, la mejora de la colección y de algunos servicios bibliotecarios. El método aplicado puede ser de utilidad para cualquier biblioteca con características similares a las de este centro de investigación.
Informal and Formal Labour Flexibility in Mexico

Abstract: we address a vital topic about labour markets in developing countries: the flexibility of the formal and informal labour sectors. for the mexican economy, we use a panel from the national survey of urban employment (eneu) from 1995 to 2001. we control for workers? observable characteristics, sample selection and non observable regional heterogeneity. we also take into account the possible endogeneity of the unemployment rate. the results show evidence that in the formal sector, unemployment does not affect wages. on the contrary, we found a clear negative effect of unemployment on wages in the informal sector. we also found evidence of a positive relation between formal-informal wage differential and unemployment. these results suggest that the informal sector is more flexible than the formal sector.
