Predicting risk of atrial fibrillation after heart valve surgery: evaluation of a Brazilian risk score

Abstract: objective: the aim of this study is to evaluate the applicability of a brazilian score for predicting atrial fibrillation (af) in patients undergoing heart valve surgery in the division of cardiovascular surgery of pronto socorro cardiológico de pernambuco - procape (recife, pe, brazil). methods: retrospective study involving 491 consecutive patients operated between may/2007 and december/2010. the registers contained all the information used to calculate the score. the outcome of interest was af. we calculated association of model factors with af (univariate analysis and multivariate logistic regression analysis), and association of risk score classes with af. results: the incidence of af was 31.2%. in multivariate analysis, the four variables of the score were predictors of postoperative af: age >70 years (or 6.82; 95%ci 3.34-14.10; p<0.001), mitral valve disease (or 3.18; 95%ci 1.83-5.20; p<0.001), no use of beta-blocker or discontinuation of its use in the postoperative period (or 1.63; 95%ci 1.05-2.51; p=0.028), total fluid balance > 1500 ml at first 24 hours (or 1.92; 95%ci 1.28-2.88; p=0.002). we observed that the higher the risk class of the patient (low, medium, high, very high), the greater is the incidence of postoperative af (4.2%; 18.1%; 30.8%; 49.2%), showing that the model seems to be a good predictor of risk of postoperative af, in a statistically significant association (p<0.001). conclusions: the brazilian score proved to be a simple and objective index, revealing a satisfactory predictor of development of postoperative af in patients undergoing heart valve surgery at our institution.

Abstract: objective: the aim of this study is to evaluate the applicability of guaragnascore for predicting mortality in patients undergoing heart valve surgery in the division of cardiovascular surgery of pronto socorro cardiológico de pernambuco - procape, recife, pe, brazil. methods: retrospective study involving 491 consecutive patients operated between may/2007 and december/2010. the registers contained all the information used to calculate the score. the outcome of interest was death. association of model factors with death (univariate analysis and multivariate logistic regression analysis), association of risk score classes with death and accuracy of the model by the area under the roc (receiver operating characteristic) curve were calculated. results: the incidence of death was 15.1%. the nine variables of the score were predictive of perioperative death in both univariate and multivariate analysis. we observed that the higher the risk class of the patient (low, medium, high, very high, extremely high), the greater is the incidence of postoperative af (0%; 7.2%; 25.5%; 38.5%; 52.4%), showing that the model seems to be a good predictor of risk of postoperative death, in a statistically significant association (p <0.001). the score presented a good accuracy, since the discrimination power of the model in this study according to the roc curve was 78.1%. conclusions: the brazilian score proved to be a simple and objective index, revealing a satisfactory predictor of perioperative mortality in patients undergoing heart valve surgery at our institution.
Efeito de épocas de semeadura sobre cultivares de girassol, no Sul do Estado do Tocantins

Abstract: this study had as its objective, evaluating the agronomic performance of sunflower (helianthus annuus l.) cultivars when sown at different times, under the environmental conditions in the south of the state of tocantins. the experiment was carried out during the 2008/2009 agricultural year, in randomized blocks in a 4 x 5 factorial design (four sowing dates and five sunflower cultivars) with three replications. each plot consisted of four rows 5.0m in length, spaced 0.80 m apart with 0.20 m between plants. for all sowing dates, the characteristics evaluated were: flowering, plant height, stem diameter, head diameter, normal achenes, grain weight per thousand and grain yield. there was significant correlation of sowing dates with the evaluated cultivars for the traits of flowering, plant height and grain yield. in relation to periods of sowing, there was generally a negative influence on all evaluated traits for the later dates, due to an indian summer causing moisture stress during the critical period of plant development and the occurrence of disease, especially alternaria leaf-spot. the first sowing period (november) showed higher productivity with an average of more than 3,000 kg ha-1. irrespective of sowing dates, the more productive cultivars were the hybrids helio 360 and helio 884, with an average of 1,709 and 1,697 kg ha-1 respectively.

Abstract: the genetic diversity is one of the most important parameters evaluated by plant breeders in the early stages of a genetic improvement program. the objective of this paper was to evaluate the genetic divergence by means of multivariate techniques, among 48 soybean genotypes grown in irrigated lowland in the state of tocantins, in order to select parents of hybrids for the production of oil and meal, as well as varieties of the panel, intended for human consumption. the experiment was conducted in the county formoso do araguaia - tocantins, brazil, in the cultivation of irrigated lowland, in the inter-cropping 2010. the experimental design was a randomized block with four replications. there was observed variability among the genotype tested. the tocher's method, upgma and canonic variables agreed among themselves, and found four distinct groups. the following hybrids are promising for the production of soybean oil and meal for the m-soy 8766, m-soy 9144, a-7002 and m-soy 9056 with amaralina rr crosses between and m-soy 8766, m-soy 9144 and amaralina rr with brsmg 790a, brs 257, brs 216 and brs 213, are listed in order especially soybeans for human consumption.

Abstract: sunflower (helianthus annuus l.)is highly tolerant to drought, cold and heat, thus, it becomes favorable to off-season crop. the objective of this study was to evaluate the performance of sunflower cultivars in four no-till planting times into soybean straw during the off-season 2009, in southern tocantins state, brazil. the experiment was arranged in a randomized block design, with 20 treatments and three repetitions.treatments were arranged in a 4 x 5 factorial, with four sowing times: st1 (03/15/2009), st 2 (28/3/2009), st 3 (04/09/2009) and st 4 (01/05/2009); with five cultivars: h250, h251, h358, h360 and h884, into soybean straw. there was significant interaction between times and cultivars for all the characteristics evaluated. sowing delay reduced the means of all characteristics evaluated, except for hectolitre weight. the highest yield was obtained with the treatment st 1. cultivar h884 had the best performance for most of the variables analyzed.

Abstract: objective: to obtain regional reference values for calcium, uric acid and citrate urinary excretion and establish a correlation between those excretions in 24-hour urine sample and single urine sample for their use in clinical practice. methods: a hundred and twenty-five healthy children and adolescents were randomly chosen and submitted to the following protocol: clinical examination, biochemical analysis of blood, blood cell count, parathormone, 24-hour urine, fasting urine sample and stool test.results: the maximum value of calcium excretion in 24-hour urine was 3.75mg/kg; in mg/dl of the glomerular filtration rate, it was 0.10; and for the calcium/creatinine (mg/dl) ratio in the fasting urine sample was 0.25. positive correlation was observed between calcium excretion in the 24-hour urine and the fasting sample (mg/dl and mg/dl of glomerular filtration rate). the maximum values of uric acid excretion in 24-hour urine were 600, 450, and 320mg and 13, 15 and 18mg/kg for adolescents, school and preschool children, respectively; in mg/dl of glomerular filtration rate, in the fasting urine sample, it was 0.47. positive correlation was observed for the uric acid excretion in 24-hour urine and fasting urine samples. the mean values for the citrate excretion in 24-hour urine were 1.6, 1.1 and 0.5mmol for adolescents, school and preschool children, respectively; for citrate/creatinine ratio, in the fasting urine sample the mean value was 0.3.conclusions: the calcium and uric acid excretion in 24-hour urine showed correlation with those in the fasting urine sample, which allows their use for metabolic diagnosis, population studies and follow-up of patients with hypercalciuria and hyperuricosuria without voiding control; the citrate/creatinine ratio in the fasting urine sample can be used for controlling patients with hypocitraturia.

Skeletonized left internal thoracic artery is associated with lower rates of mediastinitis in diabetic patients

Abstract: background: mediastinitis is a serious complication of median sternotomy and is associated to significant morbidity and mortality. diabetes is a feared risk factor for mediastinitis and viewed with caution by cardiovascular surgeons. objective: to identify risk factors for mediastinitis in diabetics undergoing cabg surgery with use of unilateral ita in the division of cardiovascular surgery of pronto socorro cardiológico de pernambuco - procape. methods: retrospective study of 157 diabetics operated between may 2007 and april 2010. nine preoperative variables, five intraoperative variables and seven postoperative variables possibly involved in the development of postoperative mediastinitis were evaluated. univariate and multivariate logistic regression analyses were applied. results: the incidence of mediastinitis was 7% (n=11), with a lethality rate of 36.1% (n=4). variables associated with increased risk of mediastinitis were: use of pedicled ita (or 8.25, 95% ci 2.03 to 66.10, p=0.016), postoperative renal complications (or 5.10, 95% ci 1.03 to 25.62, p=0.049) and re-operation (or 7.45, 95% ci 1.24 to 42.17, p=0.023). in multivariate analysis using backward logistic regression, only one variable remained as independent risk factor: use of pedicled ita (or 7.64, 95% ci 1.95 to 61.6, p=0.048), in comparison to skeletonized ita. conclusions: we suggest that diabetics should be considered for strategies to minimize risk of infection. in diabetics that undergo unilateral ita, the problem seems to be related to how ita is harvested. diabetics should always be considered for use of skeletonized ita.

Abstract: The present article is characterized by the analysis of the insertion of engineer José de Oliveira Reis in a specific moment (not the only one) of the continued construction process of the municipal administrative instances in charge of urban planning and urban plans in Rio de Janeiro. From 1956 through 1960, José de Oliveira Reis reassumes, for the last time, the Department of Urban Planning of Distrito Federal City Hall, actively taking part in the ascribed institutional construction-creationimprovement process of municipal organs responsible for the planning, financing and execution of public urban works. In this case, for the creation of the Superintendency of Urban Planning and Sanitation (SURSAN) of the Special Fund for Public Works, and for the implementation of the Execution Plan issued in deliberations in Law no. 899, of November 28th 1957. The understanding of the national context – in the scope of the Brazilian Municipalist Movement – of this whole process of the creation of municipal instances in the urban planning sector, during the 1950s, is also fundamental for comprehending engineer José de Oliveira Reis’s positions as the director of the Department. Centered in the basic precepts of political and financial autonomy, Brazilian Municipalism preconizes an important set of deliberations resulting from National Congresses of Brazilian Municipalities – especially from the four first ones - , among which the need for the implementation of mechanisms and instruments for increasing municipal budgets in the execution of works, that is, “financing for urbanizing”. In the case of the Execution Plan, through the focus in the road system problematics undertaken by the Department of Urban Planning, explicating debate on the construction of the Perimetral Avenue.

Abstract: Pretendo fazer neste artigo um resgate da produ o acadêmica sobre o integralismo, desde a funda o do movimento, com a A o Integralista Brasileira (AIB – 1932-1937) e a rearticula o enquanto agremia o política, o Partido de Representa o Popular (PRP – 1945-1965). Abordarei desde as primeiras pesquisas, desenvolvidas ainda nos anos de 1930 até o ressurgimento dos estudos sobre essa temática nos anos 1970 chegando nas pesquisas atuais. I intent to do in this paper a ramsom of the academic producion over the integralism, since the foundation of the movement, with the Brazilian Integralist Action (AIB – 1932-1937) and his rearticulation as political association, the Popular Representation Party (PRP – 1945-1965). I will brouches since the first researches, which were developed still in the early 1930’s untill the resuscitation of the studies about this thematic in the 1970’s to the present.
