Translating exercise biology into the Venezuelan medical education and health care system

Abstract: in the absence of pharmacological agents, physical exercise was widely used by physicians in the late 19th century to treat a number of maladies. in the 1950’s, epidemiological evidence suggested an association between physical activity and health, and increased interest in clinical exercise biology. by the 1990’s, sufficient research data was accumulated on the benefits of exercise, such that north american medical associations, government agencies, and the world health organization have published guidelines on exercise for public and clinical populations. despite this, leaders in medical education have remained reluctant to incorporate exercise biology into the core medical curriculum, or to systematically implement it in graduate medical education. this work reviews venezuelan exercise biology literature, and its medical applications. venezuelan scientists and clinicians have invested efforts in cardiopulmonary exercise testing, skeletal muscle adaptations to training and exercise cardiovascular pharmacology in patients, sedentary subjects and athletes. it is suggested here, that there is a need to develop education and research programs in basic and clinical exercise biology in the formal training of medical students, physicians in residency programs, and allied health care professionals. tentative steps to initiate this process are proposed.

En la ausencia de agentes farmacológicos, el ejercicio físico fue ampliamente usado por médicos a finales del siglo 19 para tratar numerosas enfermedades. Durante el siglo 20, en la década de los 50′s, la evidencia epidemiológica ya sugería una asociación entre la actividad física y la salud, y comenzaba un incremento en el interés en la biología del ejercicio con aplicación clínica. Ya en la década de los 90′s, suficiente investigación fue acumulada indicando el beneficio del ejercicio, en Norteamérica ya asociaciones de medicina, entes gubernamentales y la Organización Mundial de la Salud publicaron guías sobre ejercicio para poblaciones clínicas y público en general. No obstante de esto, los dirigentes de educación médica han permanecido renuentes a incorporar la biología del ejercicio al pénsum médico curricular, o a implementarlo sistématicamente en la educación médica de postgrado. Este manuscrito revisa la trayectoria de la biología del ejercicio en Venezuela y sus aplicaciones médicas. Characterization of the Metabolic Syndrome in a Multi-Ethnic Sample of Children: Is it Useful?

Abstract: The pathophysiological relevance of the metabolic syndrome is not currently understood, particularly when attempting to apply the diagnosis to children with varying degrees of adiposity or from ethnically diverse populations. The aim of this study was to evaluate the applicability of the metabolic syndrome characterization when the associations are explored by sex, race/ethnicity, and weight status. Participants were 247 multi-ethnic (African American (AA); n=90; European American (EA); n=102; Hispanic American (HA); n=55) children aged 7-12y. Anthropometric measurements, body composition, blood pressure and fasting blood samples were obtained. Approximately 9% of the children met the criteria for a clinical diagnosis of the metabolic syndrome. There were no differences in prevalence by sex, nor were there differences between girls and boys for mean values of each component. However, there were ethnic differences in characterization of having the metabolic syndrome, with HA more likely to meet the criteria. HA had greater waist circumference, higher triglyceride and glucose concentration and lower HDL-C concentration; whereas AA had higher blood pressure. Weight status was also positively associated with mean values for each component. Due to inherent variations in body composition, physiology and genetics, the usefulness of a characterization of the metabolic syndrome may be limited in the pediatric population.
Utility of a thematic network in primary health care: a controlled interventional study in a rural area

Abstract: In order to study the effects of a thematic network such as UniNet on primary care medicine in a rural area, we designed an interventional study that included a control group. The measurements included the number of patient displacements due to disease, number of patient hospital stays and the number of prescriptions of drugs of low therapeutic utility and generic drug prescriptions by doctors. These data were analysed and compared with those of the control center.Our study showed positive changes in medical practice, reflected in the improvement of the evaluated parameters in the rural health area where the interventional study was carried out, compared with the control area. We discuss the strengths and weaknesses of UniNet as a potential medium to improve the quality of medical care in rural areas.The rural doctors had an effective, useful, user-friendly and cheap source of medical information that may have contributed to the improvement observed in the medical quality indices.The Internet has tremendous potential for the delivery of instructional materials, allowing collaboration among a virtual community of users (VCU). The future success of the Internet in medicine, among other areas, will depend on their ability to enable physicians to participate in continuous medical education at a time and location convenient for them, along with useful communication. Functions such as electronic mail, chat rooms, mailing lists and computer conferencing enable physicians to establish and maintain communication and collaboration with colleagues investigating the same subjects. Abstract: Sabemos que de tiempo atrás en la sociedad rural colombiana reinan el empleo precario, los bajos ingresos, la violencia y la inseguridad. Por ello la estabilidad de la nación requiere que la actividad agropecuaria sea analizada y entendida más allá de su simple dimensión productiva. Así, para la solución de los problemas de los productores agropecuarios y de los habitantes rurales en general no basta con aumentar la producción o con mejorar la eficiencia.

Abstract: molecular biology methods offer an alternative for improving tuberculosis control strategies through m. tuberculosis strain typing techniques. the international literature shows that rflp of the insertion element is6110 is widely standardized internationally and has proved to be a useful tool to guide local tuberculosis control strategies. by means of a thorough literature review, this study aimed to determine if this molecular based method could be useful for the design and refinement of tuberculosis control strategies in colombia. results from epidemiologic studies published between 1995 and 2008 which used this technique in developing countries, including colombia, were analyzed. our results suggest that in the colombian context this molecular technique can provide useful information for tuberculosis program control directors and therefore it should continue to be performed. however, to establish periodicity, target populations and other aspects related to optimal conditions for accurate rflp performance, must await further evidence from operational research studies including cost effectiveness and cost utility analyses.

Abstract: Una alternativa de mejoramiento de las estrategias para el control de la tuberculosis, la ofrecen los métodos para la diferenciación de cepas de M. tuberculosis. La literatura evidencia que la técnica RFLP del segmento de inserción IS6110 está ampliamente estandarizada a nivel internacional y ha demostrado ser un buen instrumento para orientar estrategias locales de control. Mediante una revisión exhaustiva de la literatura, el presente estudio pretende establecer si esta técnica molecular puede contribuir al dise o y refinamiento de estrategias para el control de la tuberculosis en Colombia. En esta revisión se analizaron los resultados de los estudios publicados entre 1993 y 2008, realizados con la técnica en países en desarrollo, incluida Colombia. Los resultados sugieren que en el contexto colombiano esta técnica puede ofrecer información útil para los directores del programa de control de tuberculosis y, por tanto, debe seguir siendo realizada. Para establecer la periodicidad, las poblaciones blanco y otras condiciones óptimas para la realización de la técnica, se necesitan estudios de investigación operativa que incluyan análisis de costo-efectividad y costo-utilidad. Abstract: Los resultados que se presentan constituyen un aporte como resultado de la investigación a la discusión acerca de la manera en la que los seres humanos construimos conocimiento sobre el mundo social. Se analiza la comprensión del concepto de nacionalidad como un aspecto de la identidad personal y su consideración como una característica fija ó susceptible de cambio. Se explora, por un lado, los cambios que se presentan en la comprensión del concepto de nacionalidad en los distintos grupos de edad y el papel del contexto en la construcción de conocimientos. Se trabajó con una muestra de 98 sujetos de uno y otro género, de 7, 10, 13, 16 y 19 a os de edad, en una muestra de Barranquilla (Colombia) y Madrid (Espa a).
