Abstract: hyperparathyroidism is a common complication of end-stage renal insufficiency, with a high morbi-mortality and difficult medical control. classical indications for parathyroidectomy are: persistent hypercalcemia, especially after kidney transplantation, untreatable pruritus, pathologic fractures, bone pain refractory to medical treatment, and metastatic calcifications. unfortunately, this latter complication does not respond to parathyroidectomy, and blood vessels calcifications are associated with increased mortality. severe osteitis fibrosa and bone deformities also never disappear. thus, new criteria for early parathyroidectomy are needed. serum intact pth higher than 10 times the upper limit of normality despite adequate calcium and calcitriol supplements, calcium-phosphorus product higher than 70(mg/dl)2, ''brown tumor'' whenever a rapid regression is needed, incapacitating arthritis and/or periarthritis, and rupture of tendons are among other indications that should be considered. care must be taken to exclude concomitant diseases such as amiloidosis and aluminum intoxication. this review intends to advise endocrinologists about the indications and timing of surgery in hyperparathyroidism due to chronic renal failure.

Abstract: background: pregnancy has a symbolic meaning for each woman. it varies according to personality structure and is related to women's previous life experiences. objectives: the aim was to characterize the women that suffered abortion, asking about anxiety and depression, looking for guilty feelings after abortion, and to compare results between women who suffered spontaneous abortion and those who had intentional abortion. methods: fifty women with spontaneous and fifty with induced abortion were interviewed 30 days after the procedure. a semistructured questionnaire with open and closed-end questions and hospital anxiety and depression scale were administered. results: woman who induced abortion revealed to be more anxious (mean 11) and depressed (mean 8.3) than woman with spontaneous abortion (means 8.7 and 6.1 respectively, p<0.05). conclusions: women who presented induced abortion were more anxious and depressed, as shown by later life events, full of problematic feelings and the need fort sychological support.

Abstract: O hiperparatireoidismo é uma manifesta o comum na insuficiência renal cr nica (IRC), com alta morbi-mortalidade e difícil manejo clínico. As indica es clássicas da paratireoidectomia s o: hipercalcemia persistente, principalmente após transplante renal, prurido intratável, fraturas patológicas, dor óssea refratária ao tratamento medicamentoso e calcifica o metastática. Infelizmente, esta última n o responde à paratireoidectomia e a calcifica o dos vasos está relacionada ao aumento da mortalidade. Assim, novos critérios para indica o mais precoce de paratireoidectomia s o necessários. Níveis séricos de PTH maiores que 10 vezes o limite da normalidade, apesar da adequada reposi o de cálcio e calcitriol, produto cálcio x fósforo maior que 70(mg/dl)2, tumor marrom quando é urgente a regress o da massa, artrite e/ou periartrite incapacitantes e ruptura de tend es est o entre outras indica es a serem consideradas. Alguns cuidados s o necessários para excluir doen as ósseas concomitantes, como amiloidose e intoxica o por alumínio. Esta revis o visa a orientar os endocrinologistas sobre as indica es e melhor momento de realizar paratireoidectomia no hiperparatireoidismo da IRC.
Prevalence of radiological findings among cases of severe secondary hyperparathyroidism

Abstract: context and objective: patients with end stage renal disease (esrd) and secondary hyperparathyroidism (hpt2) are prone to develop heterotopic calcifications and severe bone disease. determination of the sites most commonly affected would decrease costs and patients' exposure to x-ray radiation. the aim here was to determine which skeletal sites produce most radiographic findings, in order to evaluate hemodialysis patients with hpt2, and to describe the most prevalent radiographic findings. design and setting: this study was cross-sectional, conducted in one center, the hospital universitário clementino fraga filho (hucff), in rio de janeiro, brazil. methods: whole-body radiographs were obtained from 73 chronic hemodialysis patients with indications for parathyroidectomy due to severe hpt2. the regions studied were the skull, hands, wrists, clavicles, thoracic and lumbar column, long bones and pelvis. all the radiographs were analyzed by the same two radiologists, with great experience in bone disease interpretation. results: the most common abnormality was subperiosteal bone resorption, mostly at the phalanges and distal clavicles (94% of patients, each). "rugger jersey spine" sign was found in 27%. pathological fractures and deformities were seen in 27% and 33%, respectively. calcifications were presented in 80%, mostly at the forearm fistula (42%), abdominal aorta and lower limb arteries (35% each). brown tumors were present in 37% of the patients, mostly on the face and lower limbs (9% each). conclusion: the greatest prevalence of bone findings were found on radiographs of the hands, wrists, lateral view of the thoracic and lumbar columns and femurs. the most prevalent findings were bone resorption and ectopic calcifications.
Ectopic and extranumerary parathyroid glands location in patients with hyperparathyroidism secondary to end stage renal disease

Abstract: supranumerary or ectopic parathyroid glands are the main cause of persistent hyperparathyroidism (hpt) in patients with end stage renal disease (esrd) submitted to parathyroidectomy (ptx). purpose: to evaluate the prevalence and location of parathyroid glands in these patients. methods: thirty-five patients with esrd and severe secondary hyperparathyroidism (hpt2) had been submitted to total ptx at hucff from december 2001 to july 2005. surgery was always performed by the same surgeon, who described in details the location of the glands. results: sixteen patients (45.7%) had ectopic glands, which were also extranumerary in five of them (14.3%). the most common locations were the thyroid parenchyma (33.3%), thyroid-thymus conduit (18.5%), and thymus (14.8%). before ptx, the sensibility of ultrasonography and scintigraphy with technetium-99m sestamibi was low (48.3% and 35.3%, respectively). moreover, 51.4% of the nodules found at us were thyroid nodules. however, 99mtc-sestamibi was useful to identify ectopic glands in those two patients with persistent hpt after ptx. conclusion: the presence of extranumerary and ectopic parathyroid glands in hpt2 is sufficiently important to justify their exhaustive search. as the preoperative image exams present low sensibility to locate them, it is necessary to develop an exploratory routine embracing the most common sites of location.

Abstract: objective: to report the experience with the vacuum-assisted venous drainage (vavd) technique in cardiopulmonary bypass (cpbp) and blood transfusion need. methods: a retrospective study was made about data from 111 patients who were operated, using vavd between october 2006 and february 2008, at the esperan?a hospital, recife, pernambuco. the necessity of blood transfusion was verified on the single group of patients who underwent vavd, comparing with sex, age and weigh, before the beginning of the cpbp and during the surgery, using chi-square test and t-student test. results: before the beginning of the cpbp only 10% of patients had need for blood transfusion and 12% during the surgery. it was observed that 17% of women received blood transfusion versus 4.7% of men before the beginning of cpbp (p=0.51), and 38% of women versus 9% of men during the surgery (p<0.001). the weight of patients who received blood transfusion were lower both before the beginning of the cpbp as during the surgery (p=0.049 e p=0.001, respectively). conclusion: the vavd technique has been used safely and satisfactorily, optimizing venous drainage during cpbp, in the hospital that conducted the study. however, prospective and comparative investigations between conventional drainage and vavd are needed to better clarify this relation with blood transfusion.

Abstract: the aim of this text is to present two important queer theoreticians, beatriz preciado and marie-hélène bourcier. after outlining their work and highlighting their definitions of sex and gender, i discuss the centrality of the body in the general economy of their works. i conclude by posing some questions, in which i emphasize the urgency of inquiring into the various vectors of differences that result from inequalities and exclusions.

Abstract: the article proposes a classification of two basic views on the strategy of capitalist development after the neoliberalism crisis in brazil: the private sector's export-oriented developmentalism ("newdevelopmentalism"), and state-led distributive developmentalism. both views bring back the state's role in guiding the development model, but with different ways of relating to the market. after a brief introduction, the two views are discussed, and the article concludes with a discussion of their political prospects.

Abstract: the paper studies the formation of the development strategy in getúlio vargas' second term, since the estado novo's regime. the argument is that this strategy was embedded in the national-developmentalist ideology which vargas embraced at least since the 1930', but it was anticipated in the coherent set of policies executed in the 1950's during the second world war, and advocated in the postwar period of neo-liberal predominance and during the electoral campaign of 1950, before undertaken in vargas' second term.
