Polypharmacy and suicide attempts in bipolar disorder

Abstract: objective: the aim of this study was to assess the association between suicide attempts and the use of multiple drugs in patients with bipolar disorder. method: one hundred sixty-nine bipolar disorder outpatients diagnosed using the dsm-iv structured clinical interview were included. demographic and socioeconomic data, number of medications currently in use, history of suicide attempts, number of years undiagnosed, age of onset and current psychiatric co-morbidities were assessed using a structured questionnaire and dsm-iv criteria. the main outcome measure was the number of psychotropic drugs currently in use. results: approximately half of all patients (48.5%) presented a history of suicide attempt; 84% were using more than one medication, and 19% were using more than three drugs. the most frequent combinations of drugs used by these patients were: lithium + valproate (17%); lithium + antipsychotics (10%); lithium + valproate + antipsychotics (9%); and antidepressants + any drug (6%). the number of suicide attempts was associated with the use of multiple drugs. conclusions: our findings support the notion that the use of combination therapy in bipolar disorder may be related to severity of the bd, such as number of suicide attempts.

Abstract: the aim of the present essay were: a) to discuss the achievements of the epidemiology of physical activity field over the last decades in brazil; b) highlight the need of qualitative studies in the field, by using some national and international examples; c) suggest some topics that could be explored in future studies using combined quantitative and qualitative methodologies. the field of physical activity epidemiology has grown substantially in brazil over the last years, particularly after 2000, which coincides with the dates in which physical education teachers started to join postgraduate programs in public health and epidemiology. however, these improvements were achieved based primarily on quantitative studies, given the fact that most qualitative studies in the field in brazil have failed to go beyond commenting on the methods used in quantitative studies and its limitations.

Abstract: objective: to estimate the prevalence of home care among older adults and to identify associated factors. methods: population-based cross-sectional study including 598 individuals aged > 60 years. subjects were selected through a two-stage cluster sampling strategy in the city of pelotas, southern brazil, between 2007 and 2008. home care was defined as a positive answer to the following question: "do you have someone here in your home to take care of you?" data on potential associated factors for home care were collected using a standardized questionnaire. poisson regression models with robust variance were used in the crude and in the adjusted analyses. the analysis took into account the clustering of the sample. results: the prevalence of home care was 49.5% (95%ci: 44.5;54.5). among those who have a caretaker, 39.5% reported to be cared for by their spouse, while 4.7% of subjects reported having a professional caretaker. in the adjusted analysis, home care was positively associated with male sex, having a partner, increased age and disability for instrumental activities of daily living. home care was inversely associated with schooling and physical activity levels. conclusions: the high prevalence of home care observed may overburden family members responsible for the most of the care provided. these findings are important for the planning of health interventions aimed the assistance of the elderly and their families. particular attention should be paid to individuals with advanced age, low educational level and with disability for activities instrumental to daily living.
Validade e fidedignidade de uma escala de avalia??o do apoio social para a atividade física

Abstract: objective: to assess the validity and reliability of the brazilian version of the physical activity social support scale for adults. methods: the scale was applied in conjunction with a face-to-face multi-dimensional questionnaire in 1,461 individuals aged between 18 and 79 years, living in curitiba, southeastern brazil, between april and july 2009. the questionnaire was submitted to validity (factorial and construct) and reliability testing (internal consistency and reproducibility). reliability was observed with the use of internal consistency, cronbach's alpha, test-retest procedure with a one-week interval (n = 74) and intraclass correlation coefficient. results: factorial analysis resulted in four factors with eigenvalues between 4.29 and 1.39, which explained 82.8% of the total variance. factors 1 and 2 included questions about social support from friends for moderate to vigorous physical activities and walking, respectively. factors 3 and 4 were comprised of groups of social support from the family for moderate to vigorous physical activities and walking, respectively. reproducibility showed an intraclass correlation coefficient between 0.63 and 0.80 and alpha internal consistency between 0.87 and 0.91. construct validity was observed from the significant association (p < 0.005) between scores of social support for walking and moderate to vigorous physical activities. conclusions: the instrument tested showed psychometric characteristics that were adequate for use in adult brazilians.
Epidemiology of physiotherapy utilization among adults and elderly

Abstract: objective: to analyze the prevalence of physiotherapy utilization and to explore the variables associated to its utilization. methods: a population-based cross-sectional study, including 3,100 subjects aged 20 years or more living in the urban area of pelotas, southern brazil, was carried out. the sample was selected following a multiple-stage protocol; the census tracts delimited by the instituto brasileiro de geografia e estatística (brazilian institute of geography and statistics) were the primary sample units. following descriptive and crude analyses, poisson regression models taking the clustering of the sample into account were carried out. data were collected through face-to-face interviews using a standardized and pre-tested questionnaire. results: the lifetime utilization of physiotherapy was 30.2%; and physiotherapy utilization in the 12 months prior to the interview was reported by 4.9%. women, elderly subjects, and those from higher socioeconomic levels were more likely to use physiotherapy. restricting analysis to subjects who attended physiotherapy, 66% used public health services, 25% used insurance health services and 9% had private sessions. conclusions: this is the first population-based study on physiotherapy utilization carried out in brazil. utilization of physio therapy was lower than reported in both developed and developing countries. the study findings might help public health authorities to organize healthcare service in terms of this important demand.

Abstract: objective: to evaluate the intensity and duration of physical efforts in physical education classes in primary and secondary school. methods: school-based cross-sectional study carried out by means of the observation of 218 physical education classes, including a total of 272 students (each one of the students was evaluated three times). the study was carried out in the city of pelotas (southern brazil), between august and december 2009. in order to evaluate the intensity of the efforts, accelerometers were used and the following cut-off points were adopted (in counts per minute): sedentary activities (0-100), light activities (101-2,000), moderate (2,001-4,999), vigorous (5,000-7,999), and very vigorous activities (>8000). results: the mean duration of the classes was 35.6 minutes (sd 6.0). the mean proportion of time spent in moderate to vigorous physical activity was 32.7% (sd 25.2). boys (44.1%) were involved significantly more in moderate to vigorous physical activity as compared to girls (21.0%; p<0.01). students who practice physical activities outside the classes participated more in moderate to vigorous physical activity in the physical education classes. conclusions: besides the fact that physical education classes have a short duration, students practice moderate to vigorous physical activity during one third of the class, with a poor significant contribution to students' level of physical activity.
Quedas e fraturas entre residentes de institui es de longa permanência para idosos Falls and fractures among older adults living in long-term care

Abstract: OBJETIVO: Investigar a ocorrência de quedas e fraturas no último ano e fatores associados entre residentes de institui es de longa permanência para idosos (ILPI). MéTODOS: Estudo transversal exploratório, a partir de um censo realizado em ILPI localizadas em Pelotas, Rio Grande do Sul, em 2008. As quedas e fraturas decorrentes dessas foram investigadas a partir de autorrelato referente ao último ano. Sexo, idade, escolaridade, incapacidade funcional para atividades básicas da vida diária, tipo de financiamento da institui o e hospitaliza o no último ano foram coletados como potencias fatores associados à ocorrência de queda no último ano. Empregou-se o teste qui-quadrado para heterogeneidade e tendência linear e, na análise ajustada, a regress o de Poisson com variancia robusta. RESULTADOS: Nas 24 ILPI incluídas no estudo, coletaram-se dados de 466 indivíduos. A prevalência de quedas no último ano foi de 38,9% (IC95%: 34,5 - 43,4). Dentre aqueles que caíram, as fraturas acometeram 19,2%. As fraturas mais frequentes foram: fêmur/quadril (43,3%) e punho (10%). Na análise ajustada, o avan o da idade, apresentar incapacidade funcional em uma a cinco atividades da vida diária, residir em institui es públicas/filantrópicas e ter sido hospitalizado no último ano associaram-se à queda no último ano. CONCLUS O: A alta ocorrência de quedas e fraturas entre residentes de institui es de longa permanência para idosos revela a fragilidade da popula o avaliada. Aten o específica deve ser destinada a indivíduos mais velhos e hospitalizados no último ano. OBJECTIVE: To investigate the prevalence of falls and fractures over the past 12 months and associated factors among older adults living in long-term care. METHODS: Census of all long-term care located in the city of Pelotas, Brazil, in 2008. Falls over the past 12 months were assessed using the following question: "Over the last 12 months, have you fallen?" For those who replied positively, another question was asked: "In any of these falls, have you fractured a bone?" Sex, age, schooling, disability relating to basic activities of daily living, type of financing of the long-term care and hospital admissions were the independent variables. We used chi-square tests for heterogeneity and linear trend in the unadjusted analysis, and Poisson regression with robust variance in the adjusted one. RESULTS: Within the 24 long-term care studied, we collected data for 466 individuals. The prevalence of falls in the past year was 38.9% (95%CI 34.5; 43.4). Abstract: juvenile bipolar disorder (jbd) has been recognized more frequently in the last years. the disorder might have an atypical presentation in this age range. thus, irritability with "affective storms" are more frequent than euphoria, the evolution of the disorder is more chronic than episodic and mixed symptoms are more frequent. high prevalence of comorbid conditions, specially attention-deficit/hyperactivity disorder, seems to be the rule. despite the devastating effect of jbd in the child development, few pharmacological investigations were conducted in these patients. this review aims to present a critical discussion of the findings from this emerging new area of research, the psychopharmacology of the jbd. to accomplish this task, a systematic computerized search of the literature was conducted through the pubmed. findings are presented in three sections: 1) the strength of the scientific evidence in the field; 2) critical description of the main investigations; 3) proposition of an algorithm to guide treatment options. only one randomized, double-blind, controlled trial was found in the literature. almost all studies are open prospective trials, case series, or retrospective analyses of medical records. the most investigated drugs are lithium and valproate sodium. this review suggest that a scarce availability of high quality evidence to guide clinicians in the decision on which pharmacological treatment should be used to address bipolar disorder in children and adolescents.

Abstract: this article describes teachers' deeds and contents in physical education classes in the city of pelotas, brazil. a school-based cross-sectional study was conducted including two hundred and forty observations in sixty-eight classes. the system for observing fitness instruction time (sofit) was used to assess physical education classes. the "free-play" (45.3%) was the methodological strategy more used in the classes. collective sports were the most frequent contents and teachers' deeds were described as "other tasks" or "monitoring activities" according to the observation. the participation of teachers in the class context was low, expressed by exaggerated use of "free play". such situation needs to be changed urgently in order to avoid questions on the role of physical education at the school system.
