A Rare Case of Pulmonary Mucormycosis Successfully Treated with Dual Antifungal Agents and Surgery in a Patient with Uncontrolled Diabetes  [PDF]

Abstract: Pulmonary mucormycosis is an emergent and life-threatening invasive fungal infection underdiagnosed by clinicians due to unspecific clinical picture and lack of awareness. Although the acknowledgment of risk factors, clinical and radiological findings may increase early recognition, the definite diagnosis is challenging to establish and the ideal treatment has never been delineated. The authors describe a rare case of pulmonary mucormycosis that was successfully treated with dual antifungal treatment and surgery in a patient with uncontrolled diabetes. When last evaluated, he was asymptomatic with no evidence of relapse. To our knowledge this is a rare report of a successfully treated diabetic patient with pulmonary mucormycosis with L-AmB, triazole posaconazole and surgery with no evidence of recurrence. We highlight the importance of clinicians’ awareness to early diagnosis, combined antifungal treatment and adjuvant surgery as the greatest chance of cure of a rapidly progressive disease with high mortality and morbidity.
Estudo da influência de diferentes alimentos sobre características quantitativas e qualitativas da gordura em porcos de ra?a alentejana

Abstract: in this study, we tested three foodstuffs, complementary to the traditional breeding system (?montanheira?): a commercial foodstuff, a cereal (triticale) and a mixture of triticale, oats and soybean meal. each foodstuff was given to the alentejano pigs in order to supply the following digestible energy contents daily: 4250 kcal between 30 and 40 kg of live weight; increasing thereafter until reaching 6800 kcal at 60-70 kg of live weight; and then progressively decreasing until 4800 kcal, at 90-100 kg of live weight. after analysing the results, we found positive correlations between the percentages of palmitic acid (c16:0) and estearic acid (c18:0), both in the l. dorsi muscle and in the subcutaneous adipose tissue. we also found negative correlations between the percentages of oleic (c18:1) and linoleic (c18:2) acids in both tissues. we found significant differences between feedstuffs in what concerned total amount of fat, as well as relative values of c18:1 and c18:2 in subcutaneous fat. triticale was the feedstuff that led to a larger amount of fat in the carcasses (16,65 ± 1,06 kg). it also led to a greater amount of intramuscular lipids in the l. dorsi muscle (5,54%), even though this difference was statistically non significant. on the other hand, this was the feedstuff that led to a higher c18:1 value (48,76%) and to a lower c18:2 value (5,52%). these characteristics, combined with the ease of production of this feedstuff at the farm, can make triticale an interesting option for organic pig farmers, as long as the amounts fed are restricted, in order not to diminish the commercial worth of carcasses because of excess of fat.

Abstract: ferns are an important component of the tropical flora, representing about 10% of all vascular plants of tropical forests, constituting the dominant group among the herbaceous vegetation. the aim of this study was to determine whether the ferns communities in two areas of atlantic forest, restinga (frs) and lowland ombrophilous dense (ftb) forests, located on soils remarkably distinct and separated by a highway, differ in their floristic composition and diversity. these two faces are located in picinguaba locality, parque estadual da serra do mar, ubatuba, s?o paulo (23° 31' to 23° 34' s and 45° 02' to 45° 05' w). we delimited seven plots of 20 × 20 m in each area, collected, identified and listed the fern individuals. we performed three methods of multivariate analysis (twinspan, upgma and pco) to verify the existence of floristic groups; calculated the payandeh index to define the level of aggregation of each species in each of the two areas (frs and ftb); obtained indicator species for each area; and performed diversity analysis (richness, evenness and heterogeneity). the two areas sampled 3482 individuals, 1366 of which occur in frs and 2116 in ftb. we found 14 species in frs and 29 in ftb; these areas shared five species. we found two well-defined floristic blocks (frs × ftb) in cluster analysis. in the frs, relatively most species showed aggregated distribution and fewer species showed random distribution in relation to ftb. we found four indicator species for frs and eight for ftb. the curves resulting from the individual-based rarefaction indicated a strong difference in species richness between the two communities, in favor to ftb. these differences may be related to physical and chemical characteristics of soil (more fertile and less toxic in the ftb), water dynamics (plant species in the frs are generally subject to water stress) and the sharp break caused by highway that cuts through the park. these aspects could be related to different forms of occupati

Abstract: human actions take place at the confluence of circumstances that require us to discern the proper way to act. ethics falls within the terrain of practical knowledge, of knowledge about what is contingent. it belongs to the domain of moral judgments or value judgments. the counterpart of disenchantment with our contemporary world lies in an effort to re-establish an interest in ethics. there are basically three orders of relations between public health and human rights: the quest for balance between the collective good and individual rights; methods and techniques for identifying human rights violations and assessing their negative impact; and the tie between protecting individual rights and promoting health. the relationship between ethics and epidemiology goes beyond the ethical aspects involving research on human beings to encompass political commitments, practices within health services, and the production of knowledge.

Abstract: in examining the issue of causality whithin epidemiology, the text begins with a brief historical overview that reclaims the different meanings which the west's main philosophers have lent to this concept. it next delves into the historical roots of epidemiology as a scientific discipline and the transformations the concept of cause has undergone to within this realm. with these presuppositions in place, the text goes on to analyse the 20th century development of epidemiology and the crisis ít currently faces in terms of paradigm. three alternatives are explored as ways out of this crisis: social epidemiology, popperian criticism and the contributions of molecular biology. lastly, the tent discusses the need for a new epidemiological theory grounded on the theory of complexity.

Abstract: the aim of this paper was to analyze two brazilian scientific journals included in the scielo library of public health, using a group of bibliometric indicators and scrutinizing the articles most viewed. cadernos de saúde pública was accessed 3,743.59 times per month, with an average of 30.31 citations per article. the 50 articles most viewed (6.72 to 524.5 views) were mostly published in portuguese (92%). 42% were theoretical essays, 20% surveys, and 16% descriptive studies. 42% used argumentative techniques, 34% quantitative techniques, 18% qualitative techniques, and 6% mathematical modeling. the most common themes were: health and work (50%), epidemiology (22%), and environmental health (8%). revista de saúde pública was accessed 1,590.97 times per month, with an average of 26.27 citations per article. the 50 articles most viewed (7.33 and 56.50 views) were all published in portuguese: 46% were surveys, 14% databases analysis, and 12% systematic reviews. quantitative techniques were adopted in 66% of such articles, while mathematical modeling was the same as observed in cadernos de saúde pública, as were qualitative techniques. the most common themes were health services organization (22%), nutrition (22%), health and work (18%), epidemiology (12%), and environmental health (12%).
Epidemiologia clínica: nova ideologia médica?

Abstract: in this work we discuss the emergence of clinical epidemiology with its doubly ideological nature. we begin by presenting different connotations of ideology, two of which are chosen to be discussed here. the first is the positivistic concept of ideology as the set of ideas of a given historical time; the second is the marxist concept of ideology, as concealing relations of dominance. under the first meaning, clinical epidemiology is discussed vis-à-vis ideas in force in the 1980's, giving it a predominantly post-modern character along with the load of individualism born by this word. finally, we present the myths underlying clinical epidemiology, seen now according to the second meaning of ideology.

Abstract: this text focus, on a series of author's opinions, on the difficulties that the current system of regulation of ethics in research represents for the practice of the epidemiological research, in the country. it introduces a few understandings concerning the present subject in the international literature, pointing out some of the most relevant themes and problems. it also examines part of the difficulties faced by brazilian epidemiologists. the main topic developed in the article is the specificity of the science's reasoning that guides acting of the public health and epidemiology with repercussions for the practice of scientific research in this field, plenty different from the science's reasoning that preside medical practice and biomedical research. hence the inadequacy of the ethical recommendations in force, all of them based on biomedical research, particularly at that with experimental design. it concludes with the indication that procedures adopted by the system of the ethics in research should be reviewed, adapting such procedures to the characteristics of different kinds of research.

Abstract: this paper traces, in brief, a broad view in the main epidemics and endemics that attacked the population in the state of s?o paulo during the 20th century. the facts are presented contained in five defined periods starting from outstanding facts of the national life. it tries to find the occurrence of the problems of health in the social political and economical context of each period. the information are based on revision, not exhausting, of some scientific journals of the public health and medicine subjects. the considered periods are: from the beginning of the century to the end of the "república velha", the "getulista" period, the "golden years" from 1945 to 1964, the military dictatorship and the current period after the fall of the dictatorship. for each period it describes the epidemic situation and the main endemics for which there were available information. given the limited extension of an article, it was not possible to deepen the aspects related to the appearance of each one of the problems, nor the details of its control. we opted for the construction of a panel that makes possible to the readers a landscape vision of the 20th century. the list of bibliographical references can be useful to those who want to deep themelves in the different themes.
