Condi es bucais e de reabilita o insatisfatórias dissociadas da percep o de qualidade de vida em idosos institucionalizados e n o-institucionalizados

Abstract: Objetivo: Muitos idosos apresentam edentulismo e dependem do uso de próteses dentárias. Os objetivos desse estudo foram avaliar a qualidade de vida, através do índice GOHAI, e sua associa o com necessidade e utiliza o de próteses removíveis parciais e totais e ocorrência de les es bucais em idosos institucionalizados e n o-institucionalizados. Metodologia: O estudo teve caráter transversal e descritivo. A amostra foi composta por idosos atendidos nas clínicas de Odontologia da Unimontes (n = 45) e idosos institucionalizados (n = 45) em asilos no município de Montes Claros, MG. Os dados foram coletados através de exame clínico e do teste GOHAI. Resultados: Houve alto percentual de uso de próteses totais em ambos os grupos e de edentulismo n o-reabilitado no grupo institucionalizado, condi es insatisfatórias das próteses e les es associadas. Quanto ao índice GOHAI, ambos os grupos demonstraram deficiência de autopercep o de saúde bucal. Conclus o: As condi es de saúde bucal e de reabilita o oral dos idosos institucionalizados foram piores do que as dos n o-institucionalizados. Todavia, em ambos os grupos, a condi o das próteses foi insatisfatória na maioria dos casos, necessitando interven o. Quanto à avalia o da qualidade de vida, a presen a ou a ausência de próteses ou sua condi o técnica n o foram fatores associados.

Abstract: Objetivo: Os objetivos deste trabalho foram conhecer o perfil dos egressos do Curso Técnico de Higiene Dental (THD) da Universidade Estadual de Montes Claros (Unimontes), bem como verificar a eficácia e a adequabilidade dos bin mios Técnico em Higiene Dental/Educa o Permanente e Técnico em Higiene Dental/Servi o Público. Metodologia: O delineamento do estudo foi transversal e descritivo. Para coleta dos dados utilizou-se um instrumento semi-estruturado e auto-aplicável, que foi preenchido por 128 egressos do Curso Técnico de Higiene Dental da Unimontes, matriculados no período de 1994 a 2006. Resultados: Houve predominancia de egressos do gênero feminino (94,5%), faixa salarial baixa (de um a dois salários mínimos: 57,0%), egressos trabalhando em empregos n o relacionados com a forma o de THD (22,7%), desvio de fun o (75,8%), índices intermediários de capacita o após a conclus o do curso técnico (47,7%) e reconhecimento de que o Programa de Saúde da Família é uma importante op o de trabalho para 93% dos participantes. Conclus o: Os resultados indicaram feminiliza o da profiss o, predominancia de inser o desse profissional no servi o público, baixa faixa salarial, desvio de fun o e constata o de que o THD conhece as suas atribui es no Programa de Saúde da Família, apesar de possuir baixa inser o nesse programa. Além disso, os egressos demonstraram interesse pela educa o continuada.
Choroid plexus carcinoma: case report = Carcinoma de plexo coroide: relato de caso

Abstract: Objetivos: Descrever um caso de carcinoma do plexo coroide no qual os sinais e sintomas iniciais foram relacionadas ao trato respiratório superior, ao contrário da sintomatologia clássica da doen a reportada na literatura. Descri o do caso: Um menino de dois anos e oito meses de idade foi internado por insuficiência respiratória aguda. O paciente foi tratado com antibióticos para suspeita de crupe, sem sucesso, e após oito dias mostrou sintomatologia neurológica, que levou a uma investiga o e ao diagnóstico de carcinoma do plexo coroide. A ressec o foi contra-indicada devido à extens o do tumor e envolvimento de áreas vitais. Conclus es: A evolu o rápida do carcinoma do plexo coroide neste caso mostra a necessidade da suspeita precoce de distúrbios do sistema nervoso central. Este caso pode alertar os pediatras para a necessidade de considerar a presen a de um tumor cerebral levando a les o do nervo vago em casos de dificuldade respiratória resistente ao tratamento. Aims: To describe a case of choroid plexus carcinoma which initial signs and symptoms were related to the upper respiratory tract, contrary to the classic symptomatology of this disease reported in the literature. Case description: A two years and eight months old boy was admitted due to acute respiratory failure. He was treated with antibiotics for suspected croup without success. After two weeks he presented neurological manifestations that led to investigation and diagnosis of choroid plexus carcinoma. Resection was contraindicated due to extension of the tumor and involvement of vital areas. Conclusions: The rapid evolution of choroid plexus carcinoma in this case points to the need for early suspicion of central nervous system disorders. This case may alert pediatricians to the need to consider the presence of a brain tumor causing injury to the vagus nerve in cases of respiratory distress resistant to treatment.

Abstract: the aim of this study was to investigate the self-perception of oral health in a population of community-dwelling elderly in montes claros city, mg. the research aimed further to investigate the existing association between self-perception and sociodemographic characteristics, dental needs, systemic conditions and medication use in those individuals. this is a cross-sectional study with a simple random sample (n=204). the self-perception was assessed using the geriatric oral health assessment index (gohai). according to the results, 53.9% of the elderly were female, 53.9% were not married, 57.8% were aged between 60 and 69 years old, 84.9% were not white, 69.1% were illiterate, 56.4% had an monthly household income > ? minimum wage, 59.8% went to the dentist for 3 years of more, 52.9% had a last consultation in the public service, 63.7% were edentulous, 67.2% used some type of dental prosthesis, 77% needed dental prosthetic. of them, 81.4% had no alteration in soft tissue, 31.9% had periodontal disease, the cpo-d medium was 29,39, 91.2% had systemic disease, 78.4% used medication and most (36.3%) self-perceived oral health as a regular. in the multivariate analysis, it was found that the older people that needed some kind of dental prosthesis and had no spouse were more likely to have a low self-perception of oral conditions. overall, it was observed that most elderly people had a satisfactory socioeconomic status, had dental needs, had any systemic disease, used medication and self-perceived oral health as fair or poor, consistent with the professional assessment.
Neuroblastoma in an adult: Case report Neuroblastoma en un adulto: Informe de un caso

Abstract: Neuroblastoma is the most common extracranial solid malignancy in children but rarely described in adults, being 10% of all cases diagnosed after the first decade of life. We report a 23 year-old black woman with a mass at paravertebral region of T3-T5, multiple lesions in vertebral bodies and expanding skull-brain lesion at the right parietal region. Immunohistochemical analysis (negative for CD99, CD20, CD3 and desmin; and positive chromogranin, synaptophysin and NB84) confi rmed the diagnosis of neuroblastoma. The patient was submitted to 12 cycles of chemotherapy receiving VAC (vincristine/doxorubicin/cyclophosphamide) interspersed with ICE (ifosfamide/mesna/etoposide) and doxorubicin was replaced by actinomycin in the 7th cycle. She had good tolerance to this therapy, and has been clinically stable. El neuroblastoma es el tumor maligno sólido extracraneal más común en ni os. Sólo el 10% de los casos se diagnostican después de la primera década de vida. Presentamos una paciente afroamericana de 23 a os, con una masa paravertebral en T3-T5, múltiples lesiones en los cuerpos vertebrales y una lesión expansiva en la región parietal derecha. El estudio inmmunohistoquímico (negativo para CD99, CD20, CD3 y desmina y positivo para cromogranina, sinaptofisina y NB84), confirmó el diagnóstico de neuroblastoma. La paciente fue sometida a 12 ciclos de quimioterapia recibiendo VAC (vincristina / doxorubicina/ cyclofosfamida) intercalada con ICE (ifosfamida/ mesna/ etoposido). La doxorubicina fue reemplazada por actinomicina en el séptimo ciclo. La paciente toleró bien la quimioterapia y está clínicamente estable.
Coloniza??o e espécies de Candida em pacientes submetidos à radioterapia cervicofacial

Abstract: the aims of this study were to investigate candida carriage associated with reduced salivary flow and to identify candida species in 20 patients with squamous cell carcinoma before, during and after head and neck radiotherapy, comparing with 2 control groups, one compounded by 24 healthful patients and the other, by nine patients submitted to head and neck radiotherapy one year at least. whole non stimulated saliva were collected from all patients. candida unit form colonies were quantified and identified using biochemistry tests, chromagar and api 20 aux. the results showed progressive reduction of salivary flow during radiotherapy (p = 0.001) and increased candida carriage, mainly c. tropicalis and c. albicans, with species diversification. the correlation between reduced salivary flow and increased candida carriage was statistically demonstrated (p = 0.009). after 12 months of the radiotherapy, the patients showed higher incidence of c. albicans and c. tropicalis. in conclusion, head and neck irradiated patients showed reduced salivary flow during radiotherapy, increasing of candida colonization and higher variety of candida species on saliva than non irradiated patients during and after radiotherapy.
Dental management of low socioeconomic level patients before radiotherapy of the head and neck with special emphasis on the prevention of osteoradionecrosis

Abstract: head and neck cancer patients need to receive dental care previously to radiotherapy. even patients who regularly visit dental offices need special attention including profilatic and curative treatments. the purposes of this study were to evaluate the dental status of brazilian head and neck squamous cell carcinoma patients with low socioeconomic level as well as to discuss the dental treatment performed and the oral side effects of radiotherapy. forty patients with head and neck squamous cell carcinoma received dental care and dental extractions prior to radiotherapy and were were followed up for a mean period of 28.7 months after the cancer treatment. before radiotherapy, 28 patients were dentulous and 12 edentulous, and all of them had poor oral health and hygiene. the most common treatment performed were dental extraction and 23 patients had 8.6 teeth extracted on average. one out of 9 (11.1%) patients developed radiation caries and 5 out of 23 cases (21.3% - group i) developed osteoradionecrosis, being only 1 case associated with previous dental extraction. brazilian low-socioeconomic level patients with head and neck cancer were submitted to multiple dental extractions due to poor dental condictions and inadequate oral care. the dental treatment did not prevent osteoradionecrosis, which presumably presented a multifactorial etiology in most cases.
Evaluation of salivary flow in patients during head and neck radiotherapy

Abstract: radiotherapy is frequently employed for the treatment of head and neck squamous cell carcinoma. among the side effects, xerostomia is one of the most important. with the objective of evaluating the role of radiotherapy in salivary flow, we performed three salivary sample collections: at the beginning of, during, and immediately after radiotherapy. the results showed that the salivary flow values of the first collection were very similar to those of the control group. however, during treatment, there was a significant decrease of the salivary flow (p = 0.0008), which continued low immediately after radiotherapy (p = 0.0009). our study showed that radiotherapy leads to an important reduction of salivary flow during and after radiotherapy.
Ethical and legal questions as regards filling out dental clinical charts

Abstract: Objective: Evaluate imperfections in filling out dental clinical history charts of patients attended at the “Universidade Estadual de Montes Claros – Unimontes”, in 2005, from the ethical and legal aspects. Method: Descriptive statistical analysis, Pearson’s correlation, Chi-Square test (p<0.05) with Bonferroni correction in a contingency table (p<0.003) tests were performed, and Anova – Tukey (p<0.05) were calculate using SPSS software. This study was conducted using 881 clinical history charts of 19 subjects. Results: The highest percentage of charts concerned Stomatology (12%) and 8 th period of the course (25%). The majority (63.3%) of chartshad fields left blank and in 68% the handwriting was illegible. Unjustifiable erasures were found in 74.7% of charts. The majority of charts (98%) were filled out in ink. The treatment plan was signed by course tutor in 83% of the cases. The term of consent was signed in the 94.9 % of the charts. As regards mistakes, 5.1% of documents had one error; 42% two errors; 23.5% three or more errors (average 1.89(± 0.9); percentile 25%=1; 50%=2 and 75%=2). The difference in the proportion of errors as regards filling out all fields differed statistically among the periods (p<0.05). Conclusion: It was concluded that an alarming number of documents were filled out incorrectly. The worst filling out performance was shown in the 5th, 6th and 7th periods (p<0.05).
