O sistema de saúde e as políticas de saúde na produ??o científica odontológica brasileira no período 1986-1993

Abstract: the brazilian scientific literature on social and preventive dentistry from 1986 to 1993 was identified and analyzed to verify whether themes of papers referred to health policy and the national health system. published scientific articles were used as the unit of analysis for the study. an analytical survey was conducted considering the following variables, amongst others: author's institutional affiliation; author's title; author's areas of interest; type of research; type of article; and research funding sources. articles reviewed were published between the first and second national oral health conferences. there were 386 articles published in 19 journals, by at least 866 authors. more than 75% of the studies came from public universities. rgo was the largest publisher in this field, followed by revista da apcd. original articles accounted for 56.7% of the studies, while 30.3% were reviews and essays. the majority of the authors were from the state of s?o paulo. male writers predominated. a "quantitative deficiency" was identified, since scientific production remained below 50% of its potential. health policy was considered a specific issue in only 3 articles (0.8%) while 7 (1.8%) discussed the national health system.

Abstract: A produ o científica brasileira na área de odontologia preventiva e social, no período 1986-1993, foi analisada para verificar a ocorrência de temas relacionados às políticas de saúde e ao sistema de saúde. Artigos em revistas científicas brasileiras serviram como indicadores dessa produ o. Realizou-se um survey para conhecer a origem institucional dos autores, suas titula es e preocupa es temáticas, os tipos de pesquisas realizadas, as modalidades de artigos delas decorrentes e as fontes de financiamento. Foram analisados 386 artigos em 19 periódicos. Mais de três quartas partes da produ o tiveram origem na universidade pública. RGO foi o periódico que mais publicou. Dos textos, 56,7% eram artigos originais. Revis es de literatura e ensaios somaram 30,3%. Mais da metade dos autores atuavam em S o Paulo. O sexo masculino predominou. Política de saúde foi tema específico em 3 artigos (0,8%) e sistema de saúde em 7 (1,8%). No crucial período histórico de reconquista e consolida o das liberdades democráticas, em pleno apogeu do processo da Reforma Sanitária, quest es dessa ordem n o despertaram significativamente o interesse dos pesquisadores científicos brasileiros da área odontológica.
Revised criteria for the assessment and interpretation of occlusal deviations in the deciduous dentition: a public health perspective

Abstract: secondary data from a randomized sample of brazilian preschool children were analyzed to evaluate the prevalence of occlusal deviations in the deciduous dentition that can adversely affect the permanent dentition, using revised criteria. overjet and overbite used cut-off points described in the literature to exclude mild malocclusion cases. overjet > 3mm and overbite > 3mm were present in 16% and 7% of the children, respectively. on the sagittal plane, only bilateral deviations were considered: molar relationship in distal step (9.7%) and mesial step (6%) and canine relationship class 2 (11%) and class 3 (2.9%). regarding other occlusal deviations, severity criteria were not reported in the literature. crude prevalence rates of openbite (27.9%), posterior crossbite (11.3%), and crowding in the maxillary arch (7%) and mandibular arch (11.3%) were recorded. the assessment of malocclusion in the deciduous dentition should focus on the severity of occlusal deviations for the identification of cases and non-cases that are relevant to public health. need for further consensus and improvement in the interpretation of epidemiological data on malocclusion related to this developmental stage is emphasized.

Abstract: in the early of twentieth century, dental caries were a big public health problem around world. infection, pain, suffering and mutilation reached all people. the discovering of preventive effect of fluorine became them, through the century, the main agent in fighting to disease worldwide. in various countries, including brazil, fluoridated products have been pointed as main causes for dental caries prevalence decline. also in brazil, at the 50s and 60s, the preventive effect of water fluoridation was ratified for some pioneer studies. for the period 1986-1996 epidemiological data shown a significant reduction of 53% in the dmf-t index value at 12-years-old schoolchildren. water and dentifrices are largely used as vehicle for fluoride in public health actions. in brazil 42% of population has access to fluoridated water. most dentifrices contain fluoride. this paper approach historical aspects of fluoride uses in public health and presents any epidemiological data on dental caries prevalence in brazil. conclude asking if fluoridated products will have same importance for oral health promotion in next decades.

Abstract: this essay focuses on the pioneer activities of public health dentistry in brazil and its evolution in the 20th century with emphasis on the emergency of a landmark, the sanitary dentistry. social and preventive dentistry and market dentistry, with reference to the main theoretical works representative of these trends, are presented. the essential characteristics of collective oral health (brazilian variant of public health dentistry) and buccality are presented. the relationship between collective oral health and collective health as well as the implications of the buccality concept for the development of public health dentistry actions and new guidance of clinical practice in public services of the brazilian health system (sus) are discussed. the key elements of an agenda for collective oral health based on proposals presented at the 3rd national conference on oral health are addressed.

Abstract: O ensaio aborda o surgimento das primeiras atividades odontológicas sob responsabilidade do poder público no Brasil e sua evolu o no século XX, enfatizando a emergência do marco referencial denominado odontologia sanitária. Caracteriza, sumariamente, a odontologia social e preventiva e a odontologia de mercado, indicando as principais publica es que se ocuparam dessas correntes. As características essenciais da saúde bucal coletiva e da bucalidade s o apresentadas, tecendo-se considera es sobre a articula o da saúde bucal coletiva com a saúde coletiva. Discutem-se as implica es do conceito de bucalidade para a organiza o da prática odontológica e a necessidade de reorienta o do trabalho clínico nos servi os de saúde bucal, no ambito do Sistema único de Saúde brasileiro. Indicam-se os elementos essenciais de uma agenda para a saúde bucal coletiva, com base nas proposi es apresentadas na 3a Conferência Nacional de Saúde Bucal.
Lei n.o 11.889/2008: avan?o ou retrocesso nas competências do técnico em saúde bucal?

Abstract: this study aimed to analyze the skills that have been attributed to oral health technicians (tsb) in brazil since the enactment of act 11,889/08, including the terms set forth for their supervision. documentary analysis was performed comparing the skills set out in mentioned instrument with those called for in legal opinion no. 460/75, issued by the federal council of education, and under federal council of dentistry resolution no. 63/2005. thematic analysis techniques were employed considering the skills in terms of direct and indirect actions distributed in four ability areas: health planning and administration, health promotion, disease prevention, and individual care. although the powers approved under the act have been distributed to a smaller amount of items, compared to the two other documents, from the qualitative point of view, the results of the analysis showed that considerable progress was made with the regulation of the profession, under the approved terms, in all areas of competence. there was a positive impact on the health work process, both regarding inter-professional cooperation and insofar as the supervision of technical activities is concerned. this represents an important victory for those who work in the area and a significant contribution to driving the enhancement of access to dental services
Aspectos éticos do uso de produtos fluorados no Brasil: uma vis?o dos formuladores de políticas públicas de saúde

Abstract: fluoride has been used to prevent dental caries. it is used in supply water and toothpastes. one undesirable effect has been found: mild dental fluorosis. such stains have aroused concern among specialists. this paper examines, in the literature, ethical aspects resulting from the use of fluoride in public health associated with the "caries prevention versus mild fluorosis occurrence" contradiction and investigates the occurrence of any ethical dilemma among the professionals involved in decision on the use of such products. databases were assessed to identify papers through terms such as ética, bioética, ethics and fluor. the material contributed to the drawing up of a questionnaire answered by researchers and health authorities. the data provided by the interviews underwent content analysis. the ethics dilemma may be stated as: "the use of fluoride products to prevent dental caries has in public health the inconvenience of producing mild dental fluorosis in the population, however, the non-use of fluoride has the inconvenience of not preventing the appearance of a disease (caries) that can be prevented if such product is used." the dilemma has shown to be solved, by admitting that the use of such products would be fair and that its benefit would exceed its damages. nevertheless, some of the respondents admitted a violation of the principle of autonomy.
Suplementos fluorados pós-natais: recomenda??es de pediatras, entidades profissionais e institui??es públicas de saúde

Abstract: objectives: to identify the opinion of pediatricians and the recommendations of professional organizations and public health institutions regarding the prescription of fluoride supplements. methods: a questionnaire was sent to pediatric doctors. additional information on fluoride supplements was obtained from seven professional organizations and four public health institutions. results: approximately one in every ten pediatricians reported having prescribed some fluorine supplement. the risk of dental fluorosis, when water is fluoridated, was the main reason given for believing that this should not be done. with regard to professional organizations only two gave appropriate responses. the others did not have a satisfactory opinion regarding reasonable use of fluorine. the response of the public institutions was appropriate, even though the ministry of health has not stated a position on the issue. conclusions: one in every 10 pediatricians reported having prescribed supplements containing fluorine to children living in s?o paulo, a city in which the water is already fluoridated. there is, therefore, a need for the public authorities and dental associations to take on their responsibilities with regard to the use of fluoride supplements.
Políticas de saúde bucal no Brasil e seu impacto sobre as desigualdades em saúde

Abstract: this text systematizes available knowledge about the main dental health policies in brazil in regards to their current degree of implementation and their impact on health inequalities. although the fluoridation of publicly distributed water is legally mandated in brazil, its implementation has been subject to marked regional inequalities. data are presented about the extent of implementation for the intervention, and studies are reviewed that evaluate the intervention's impact upon increasing inequality in the experience of dental caries. the provision of public dental services, which expanded considerably after the implementation of the national unified health care system, is also discussed in relation to service provision and its impact on reducing inequality in access to dental treatment. the discussion of the differential effect of these interventions allowed for the proposal of targeted strategies (directing fluoridation to areas of greater need), aiming to reduce inequalities in the experience of dental caries in brazil.
