Congenital Diaphragmatic Hernia with Delayed Diagnosis: Report of Two Cases  [PDF]

Introduction: Advances in prenatal imaging studies have allowed early diagnosis of Congenital Diaphragmatic Hernia (CDH), although neonatal mortality remains at high levels (60% survival). Despite advances, this study found delayed diagnosis, demonstrating subdiagnosis, possibly caused by lack of resources and/or prenatal diagnostic failure. Case Report 1: Infant, 4 months and 22 days old, dyspneic, cyanotic, with fever and vomiting for 3 days, treated as pneumonia. After the second chest radiography, a right CDH was revealed. Surgical correction was performed and removal of Meckel’s diverticulum was surgical findings. The patient was discharged on the 10th days after admission. Case Report 2: Infant, 11 months and 3 days old, distended abdomen, with fever and dyspnea for 2 days, treated as pneumonia. After the second chest radiography, a left CDH was revealed. The patient was submitted to surgery, intestinal loop incarceration without necrosis was observed, and the correction of CDH was performed. The patient was discharged on the 7th day after admission. Discussion: Three cases, between July 2012 and July 2013, were diagnosed at Dom Malam Hospital; 2 Cases of CDH, with delayed diagnosis, showed incidence of 66.66%. Delayed diagnosis of these patients with initial hypotheses of pneumonia demonstrated high risk, emphasizing the importance of diagnosis research through observation of the clinical course, the correct imaging interpretation, in order to establish early diagnosis and treatment.
Abnormalities of digestive tract innervation in rat fetus treated with ethylenethiourea

Abstract: purpose: the pathophysiology of abnormalities associated with myenteric plexus lesions remains imperfectly understood. such abnormalities have been correlated with subocclusive intestinal conditions in children with hirschsprung's disease, cases of chronic constipation and, postoperatively, in cases of anorectal anomalies. this study evaluated abnormalities of the myenteric plexus in fetus from female rats that received ethylenethiourea. methods: female rats were exposed to ethylenethiourea on the 11th day of pregnancy (experimental group) or to 0.9% physiological solution (control group). abnormalities were only found in the experimental group. the digestive tract muscle layer was analyzed morphometrically and changes to the frequencies of nerve plexus cells and interstitial cells of cajal were evaluated, using hematoxylin-eosin, s-100 protein, neuron-specific enolase and c-kit, respectively. results: muscle and skeletal abnormalities were observed in 100%, anorectal anomalies in 86%, absent tail in 71%, short tail in 29%, duodenal atresia in 5%, esophageal atresia in 5% and persistent omphalomesenteric duct in 5%. histopathological analysis showed a thinner muscle layer associated with lower frequencies of ganglion cells and interstitial cells of cajal, in all gastrointestinal tract. conclusion: severe nerve plexus abnormalities associated with muscle layer atrophy were observed throughout the gastrointestinal tract in newborn rats exposed to ethylenethiourea.

Abstract: purpose: to evaluate associations between parental exposure to pesticides and births with congenital defects in s?o francisco valley, as well as the demographic profile and the defects found. methods: in this case-control study, each case (newborns with congenital defects) had two controls (healthy newborns). the subjects were born in the city of petrolina, in s?o francisco valley, in 2009. the sample consisted of 42 cases and 84 controls. data were gathered by a structured questionnaire adapted from latin-american collaborative study of congenital malformations (eclamc), with the addition of questions related to exposure to pesticides, analysis of the medical records and contact with the hospital's pediatrician. the χ2 test was performed with a significance level of 5% to identify the variables with the greatest differences between case and control groups. odds ratio (or) for the sample was calculated, as well as the or obtained by logistic regression analysis, and finally, multivariate logistic regression analysis was performed. results: there was a greater exposure to pesticides during pregnancy in infants with congenital defects compared to healthy subjects. increased risk was observed when at least one parent was exposed to pesticides (adjusted or = 1.3; 95%ci = 0.4 - 3.9). the sociodemographic variables associated with congenital defects were: low school level, low weight, prematurity, young parents, chronic diseases, and physical factors. multiple malformations and defects of the musculoskeletal and nervous systems were more frequently found. conclusions: the present study suggests an association between exposure to pesticides and the occurrence of congenital defects, although the data were not significant.

Abstract: pregnancy influences the respiratory system promoting anatomical and physiologic changes, which can have repercussions on the rates of the muscular respiratory strength. the objectives of this work were to compare the value of the respiratory pressure between nuligestas and primigestas during the third trimester of pregnancy, and to associate those to the predicted proposed by neder et al., 1999. a transversal corte study including eighty women was studied (40 nulliparous and 40 primiparous in the third term of pregnancy) between 20 and 29 years, eutrophic and without cardiorespiratory diseases history. the average of the values obtained for maximum inspiratory pressure (pimáx) was: -93.95 cmh2o in the nulliparous and -87.78 cmh2o in the primiparous, which shows statistic difference (p=0.0182). for maximum expiratory pressure (pemáx), it was reached 98.28 cmh2o and 96.73 cmh2o, respectively, not being statistically significant (p=0.710). differences between the anatomical and physiologic modifications during the gestation were not observed, but it was verified that pimáx was just decreased when compared within the nulliparous group, while pemáx didn't present significant alteration between groups. it was no correlation between actual and values predicted by neder et al.,1999.

Abstract: this paper summarizes the main results of a survey on literacy conditions of brazilian youths and adults. the data were collected in 2001 from a nationally representative sample of the population, aged 15 to 64. respondents were submitted to a reading test before answering a questionnaire aimed at providing information about their educational background, their uses of written language in different contexts as well as their judgments about their own literacy capacities and predisposition. after discussing the relevance of such a survey and some of the theoretical and methodological issues it involves, the literacy levels emerging from the test are described as well as some of the literacy practices to which they correspond. finally, the correlation between literacy levels and some intervening variables, among which formal schooling stands out, is analyzed.

Abstract: Este artigo sintetiza os principais resultados de uma pesquisa realizada em 2001 sobre as condi es de letramento dos jovens e adultos brasileiros. Os dados foram recolhidos em uma amostra representativa da popula o entre 15 e 64 anos, à qual foram aplicados um teste de leitura e um questionário visando a levantar informa es sobre background educacional, usos da linguagem escrita em diferentes contextos, além do julgamento das pessoas sobre suas capacidades e disposi es quanto à leitura e à escrita. Depois de discutida a relevancia de uma pesquisa dessa natureza, assim como os problemas teórico-metodológicos nela envolvidos, descrevem-se os níveis de alfabetismo que foram verificados por meio do teste e algumas práticas de letramento a que correspondem. Em seguida, analisam-se correla es entre os níveis de alfabetismo e algumas variáveis intervenientes, entre as quais se destaca o grau de escolaridade.

This study evaluated the nutritional characteristics of Brachiaria brizantha cultivars subjected to three cutting intensities in the course of one year. The experiment was conducted at the University of Rio Verde. The experiment was performed as a randomized 3 × 3 factorial in complete block design with three replications and repeated measures in the time. Three Brachiaria brizantha cultivars (Marandu palisadegrass, Xaraes palisadegrass, and Piata palisadegrass), were tested at three cutting intensities (10, 20, and 30 cm sward height). The evaluations were conducted on the same plots throughout one year and during all four seasons (autumn, winter, spring, and summer). The results showed that the Piata palisadegrass had the best chemical composition compared to the Marandu palisadegrass and Xaraes palisadegrass. The management of Brachiaria brizantha cultivars at the lowest residual height (10 cm) affected the nutritional value of these feed grasses. Seasonality also influenced the nutritional value of these forages.

Abstract: the literature indicates the relation between nursing students profile and its correlation to the educational process, which supports and applies the teaching project, concerning the subject of this process. the study aimed to evaluate the social demographic profile of nursing students in belo horizonte/mg. it's a describing study developed in five private colleges. the data collection was made by the application of an interview. data analysis was made by simple statistic as variable frequencies, and the results were clustered in tables. data demonstrated that the young group, from age 20 to 24, women, and single from public school is the major group at the study. a large number of students in this group receives some kind of salary to practice their activities. half of them work on nursing activities.

Abstract: the ocular ischemic syndrome is a manifestation of chronic hypoperfusion of the ocular arterial supply. takayasu's arteritis is characterized by inflammatory arteritis of big vessels, with variable grades of obstruction, generally aortic branchs. the purpose of this paper is to report a case of ocular ischemic syndrome secondary to takayasu's arteritis. digital fluorescein angiography showed signs of chorioretinal hypoperfusion and arteriovenous shunts, and arteriography showed an important reduction of more than 90% of the blood flow in both right and left carotid arteries.

Abstract: A síndrome ocular isquêmica representa manifesta o da hipoperfus o cr nica do suprimento arterial ocular. A arterite de Takayasu caracteriza-se pelo acometimento de grandes vasos, com graus variáveis de obstru o, geralmente ramos da aorta. O objetivo deste trabalho é relatar um caso de síndrome ocular isquêmica secundário à arterite de Takayasu. O exame da angiografia fluoresceínica digital evidenciou presen a de sinais de hipoperfus o coriorretiniana e shunts arteriovenosos, e a arteriografia mostrou sinais de diminui o acentuada do fluxo sangüíneo de ambas as carótidas, direita e esquerda, acima de 90%.
