Impact of supervised physiotherapeutic pelvic floor exercises for treating female stress urinary incontinence

Abstract: context and objective: urinary incontinence is a public health problem that affects more than 200 million people worldwide. stress incontinence is the most prevalent type. pelvic floor muscle exercises have been used for treating it, although there is no consensus regarding their application. the aim of this study was to compare the results from treating female stress urinary incontinence with pelvic floor muscle exercises with or without physiotherapist supervision. design and setting: this was a randomized, prospective, controlled trial in the urogynecology and vaginal surgery sector, universidade federal de s?o paulo. methods: forty-four women were randomized to be treated for stress urinary incontinence with pelvic floor exercises for three consecutive months, into two groups: one with and the other without physiotherapist supervision. they were evaluated before and after treatment using a quality-of-life questionnaire, pad test, micturition diary and subjective evaluation. descriptive analysis was used to evaluate the population. the homogeneity of the two groups was evaluated using the kruskal-wallis and chi-squared tests. the success of the two groups after treatment was evaluated using the wilcoxon test. results: the supervised group showed statistically greater improvement in the pad test, micturition diary and quality of life than did the control group. in the subjective evaluation, only 23.8% of the control group patients were satised with their treatment. in the supervised group, 66.8% of patients did not want any other treatment. conclusion: supervised pelvic floor muscle exercises presented better results in objective and subjective evaluations than did unsupervised exercises.

Abstract: purpose: to compare the effects of functional electrostimulation of the pelvic floor and therapy with cones in women with stress urinary incontinence (sui). methods: randomized clinical study for which 45 patients with sui were selected. the effects of functional electrostimulation of the pelvic floor were evaluated in the sui treatment of 24 women, with the use of clinical data (micturition diary, pad test and a questionnaire about quality of life - i-qol). the patients were submitted to two 20' weekly sessions for four consecutive months, under the supervision of a physiotherapist. the electrode used had 10 cm length and 3.5 cm width with a double metallic ring and a cylindrical shape, positioned in the medium third of the vagina. the electric parameters used were: intensity varying from 10 to 100 ma and 50 hz of fixed frequency, with pulse duration of 1 ms. also, we evaluated 21 patients who were submitted to vaginal cone treatment. the cone therapy was done with two 45 minute sessions per week. the cones' weight varied from 20 to 100 gr. results: there was no difference between the outcomes of electrostimulation of the pelvic floor and the vaginal cones for the treatment of sui (p>0.05). after four months, there was a significant improvement in the i-qol index of the patients treated both with electrostimulation (40.3 versus 82.9) or with the cones (47.7 versus 84.1). there was a significant decrease in pad weight in both groups, measured before and after the treatment (28.5 and 32 g versus 2.0 and 3.0 g for the electrostimulation and cone group, respectively). finally, there was a significant decrease in the number of urinary leakage evaluated by the micturition diary in both groups (p<0.0001). conclusions: both electrostimulation and vaginal cones were effective in the treatment of women with sui.
Evaluation of apical deviation in root canals instrumented with K3 and ProTaper systems

Abstract: objectives: this study evaluated the apical deviation of curved root canals instrumented with k3 and protaper systems. material and methods: twenty root canals of human maxillary and mandibular first molars were employed, which were divided into 2 groups: group a (10 teeth) was instrumented with the k3 system, and group b (10 teeth) with the protaper system. evaluation of deviation was performed by double radiographic exposure. radiographs were achieved before and after instrumentation, with 0.3-second, thus allowing superimposition of images. three-dimensional computerized tomograph was performed in 3 specimens in each group, as an additional means to evaluate the apical deviation. results: were evaluated by the parametric test student-newman-keuls at 5%, which did not reveal significance between groups concerning the apical deviation. the results of computerized tomograph images demonstrated that the larger deviation of the root canal occurred at the distolingual area for both systems. conclusions: both techniques produced a mild apical deviation. computerized microtomography was shown to be accurate for experimental endodontics studies.
Wear of K3 and ProTaper files after simulated clinical use in curved canals

Abstract: Objective: To assess wear, after simulated clinical use of K3 (SybronEndo, Orange, CA, EUA) and ProTaper files (Dentsply-Maillefer, Tulsa, Oklahoma, EUA) in 46 mesiovestibular roots of human maxillary first molars, recently extracted and with mean angulation of 40°.Methods: The teeth were divided as follows: a) K3 (SybronEndo, Orange, CA, EUA) (with radial bands) and b) ProTaper (Dentsply-Maillefer, Tulsa, Oklahoma, EUA) (without radial bands). The files were assessed by scanning electronic microscopy before first use to visualize fabrication defects, and after 7 and 13 cycles of debridement in the canals. Debridement with files K3 (SybronEndo, Orange, CA, EUA) and ProTaper (Dentsply-Maillefer, Tulsa, Oklahoma, EUA) was performed in accordance with the manufacturers’ recommendations. The criteria for wear assessment were: defects (stretching, shortening/spiral inversion, presence of craters), oblique cracks and fracture. After assessment by SEM and before they were used, metal barbs on the crest of the spirals and oblique scratches due to the machining process were found in all the files.Results: In the K3 system (SybronEndo, Orange, CA, USA), after being used seven times, all the instruments showed cutting angle defects,and 25/08 and 25/10 had oblique cracks in the body of the active part leading to ductile fracture after being used for the eighth time. In the ProTaper system (Dentsply-Maillefer, Tulsa, Oklahoma, USA), after being used seven times, in addition to cutting angle defects, the files also presented cracks, and ruptile fracture occurred in F3 during use for the thirteenth time. At the end of preparation, craters were seen along the active part of the files.Conclusion: In the final analysis, it was seen that the system are equivalent with regard to the defects found, irrespective of the active part design,being proportional to the number of cycles of use, and these defects could act as points of greater fragility, favoring fracture.

Abstract: the study of 38 children with paracoccidioidomycosis, aged up to 14, treated for 24 to 30 months with either a sulfonamide derivative or ketoconazole either alone or, after the use of amphotericin b. laboratory data at admission were analyzed and compared with those of sequential tests after up to 30 months follow-up. anemia, eosinophilia, increased bilirubin and aminotransferases normalized, in most patients, after three months treatment and hypoalbuminemia normalized after six months, suggesting that these laboratory findings are useful for monitoring early therapeutic response. peripheral leucocytes, erythrocyte sedimentation rate, igg, and serological titers for paracoccidioides brasiliensis were increased and frequently normalized after nine to 12 months of treatment. they may be useful for monitoring the entire treatment and emphasize the need for long term treatment of paracoccidioidomycosis in children.

Abstract: El objetivo de este estudio fue describir el funcionamiento de los servicios y las acciones del Plan de Control de la Tuberculosis ofrecidos a los usuarios en la red básica de la salud en la ciudad de Porto Velho, Brasil. Se optó por la investigación de carácter descriptivo con enfoque exploratorio. Se han estudiado 10 Unidades Básicas de Salud (UBS) de la zona urbana de la ciudad. Han sido parte del estudio los profesionales de la salud que llevan a través del servicio de atención al cliente de las unidades de salud, después de llegar a un acuerdo para participar de la investigación. En total han sido 10 enfermeros, 10 médicos, seis biomédica/bioquímicos y tres agentes comunitarios de salud. Los resultados del estudio sugieren que los profesionales, aunque capacitados, tienen dificultades para implementar las acciones de control de la tuberculosis, debido a la inadecuación de la estructura de las unidades a los parámetros considerados por el Departamento de Salud. También se alan la necesidad de la aplicación de la estrategia DOTS y de aumento de la cobertura de la Estrategia de Salud de la Familia.

Abstract: Resumo Este estudo analisa, por meio das cartas produzidas pelos prelados do século XVIII, o emprego dos ritos litúrgicos e festas oficiais em prol da afirma o da autoridade da Igreja e do Estado na capitania de Minas Gerais. àquela altura, a engrenagem do governo colonial, que envolveu atividades administrativas, econ micas e militares; n o prescindiu de um aparato de representa o pública dos poderes. Tratados sobre a doutrina o dos gentios circulavam pela Europa; os ritos e festas oficiais eram regulados pelas Leis do Reino e constitui es diocesanas. Deveriam teatralizar a ordem colonial de domina o, produzir admira o, temor e respeito nas pessoas. As autoridades coloniais eram obrigadas a tomar parte nos ritos, de modo a reafirmar a ordem colonial e defender a hierarquia e o domínio dos espa os conquistados. Abstract This study examines, through various correspondences in the eighteenth century, the use of liturgical rites and official parties in order to affirming the authority of church of state. Gear colonial involved several activities - administrative, economic and military; but also understood an apparatus of public representation of powers. Treaties about the indoctrination of the gentiles circulated throughoutEurope. The rites and official celebrations were regulated by laws ofkingdom and diocesan constitutions. They should dramatize the colonial order of domination, produce admiration, fear, and respect in people. The colonial authorities were obliged to take part in them; aimed at reaffirm the colonial order and defend to the hierarchy and the domination of the conquered areas. key-words: Catholic Church; episcopacy; colonization; pastoral visits; liturgy.

Abstract: Introduction: Tako-Tsubo cardiomyopathy describes a form of acute and reversible left ventricular dysfunction with a clinical presentation, ECG and cardiac biomarkers that makes it indistinguishable from acute coronary syndrome. Case Presentation: The authors present two cases of tako-tsubo cardiomyopathy in postmenopausal women, the first case following an emotional stressful event and a second case following a blood transfusion and probably associated with intravenous catecholamine perfusion. Both had unobstructed coronary arteries and regional wall-motion abnormalities typical of this syndrome. Conclusions: Tako-tsubo cardiomyopathy is a condition often misdiagnosed. A clinical presentation suggestive of acute coronary syndrome in a postmenopausal woman without history of coronary disease and in whom a precipitating stressful event can be found should lead the physician to suspect the diagnosis of tako-tsubo cardiomyopathy. This syndrome associated with blood transfusion has rarely been described.
Oviposition behavior and performance aspects of Ascia monuste (Godart, 1919) (Lepidoptera, Pieridae) on kale (Brassica oleracea var. acephala)

Abstract: host part selection by ovipositing females of ascia monuste (godart, 1919) (lepidoptera, pieridae) on kale (brassica oleracea var. acephala) was determined in greenhouse and field. performance of offspring (larval period, efficiency of food utilization, number of eggs/female and others) was investigated under laboratory conditions. in the field, the number of a. monuste egg clutches on the apical and medium parts of kale leaves was greater than on the basal part. in greenhouse, a. monuste exhibited a strong preference for the apical part of kale leaves for ovipositing. the best results on food utilization indices, pupal mass and female wing size were obtained with the leaf apical part. this part of kale leaves exhibited the highest nitrogen and protein concentration and the smallest water content, when compared to the other leaf parts. however, the apical part of the leaves seems not to provide ovipositing females with enough protection against birds, making them easy preys in the field. we suggest that good relationship between oviposition preference and performance of offspring was hindered by predation in field conditions.
