Cost-effectiveness analysis on spinal anesthesia versus local anesthesia plus sedation for loop colostomy closure

Abstract: context: studies in the area of health economics are still poorly explored and it is known that the cost savings in this area is becoming more necessary, provided that strict criteria. objective: to perform a cost-effectiveness analysis of spinal anesthesia versus local anesthesia plus sedation for loop colostomy closure. methods: this was a randomized clinical trial with 50 patients undergoing loop colostomy closure either under spinal anesthesia (n = 25) or under local anesthesia plus sedation (n = 25). the duration of the operation, time spent in the post-anesthesia recovery room, pain, postoperative complications, length of hospital stay, laboratory and imaging examinations and need for rehospitalization and reoperation were analyzed. the direct medical costs were analyzed. a decision tree model was constructed. the outcome measures were mean cost and cost per local and systemic postoperative complications avoided. incremental cost-effectiveness ratios were presented. results: duration of operation: 146 ± 111.5 min. vs 105 ± 23.6 min. (p = 0.012); mean time spent in post-anesthesia recovery room: 145 ± 110.8 min. vs 36.8 ± 34.6 min. (p<0.001). immediate postoperative pain was lower with local anesthesia plus sedation (p<0.05). local and systemic complications were fewer with local anesthesia plus sedation (p = 0.209). hospitalization + rehospitalization: 4.5 ± 4.1 days vs 2.9 ± 2.2 days (p<0.0001); mean spending per patient: r$ 5,038.05 vs 2,665.57 (p<0.001). incremental cost-effectiveness ratio: r$ -474.78, indicating that the strategy with local anesthesia plus sedation is cost saving. conclusion: in the present investigation, loop colostomy closure under local anesthesia plus sedation was effective and appeared to be a dominant strategy, compared with the same surgical procedure under spinal anesthesia.

Abstract: this study aimed at evaluating different kinds of cutting with variable bud number on rooting, budding percentage and development of grapes scions stock cv. iac 572. a trial was carried out in a nursery at embrapa in petrolina, brazil, in a randomized design, with four replications and five treatments, with 25 cuttings each plot. the treatments are the following: 1) three bud ligneous cutting; 2) two bud ligneous cutting; 3) one bud ligneous cutting; 4)two bud herbaceous cutting and 5) one bud herbaceous cutting. at 60 days after planting, three bud ligneous cutting presented the greatest percentages of rooting (99%) and budding (99%) and best development of shoots and roots. however, these results are not significantly different (p>0.5) to the values obtained to rooting and budding using one bud ligneous cutting.

Abstract: the present work aimed at evaluating and selecting seeddless grape varieties adapted to the semi-arid tropical conditions of the s?o francisco river valley, northeastern - brazil. the experiment was carried out in an experimental vineyard which belongs to embrapa semi-árid, in petrolina, pernambuco state, brazil during five growing seasons in 1997 and 1998. the varieties tested were: vênus, arizul, beauty seedless, thompson seedless, marroo seedless, canner, cg 39915, pasiga, saturn, emperatriz, a1581, paulistinha and loose perlette, grafted on iac 572 ('campinas') rootstock. aspects of vegetative and productive characteristics and chemical composition of the berries were evaluated. all varieties presented small bunches, and 'vênus' and 'marroo seedless' berries showed mean diameters of 17.83 and 18.26 mm, respectively, without necessity to apply growth regulators. the total soluble solids (tss) was high in most varieties. however, the total tritable acidity (tta) was low, resulting in satisfactory ratio (tss/att). 'vênus' and 'marroo seedless' were the most productive varieties, with the annual average production of 24 tons/ha and 20 tons/ha, respectively.

Abstract: Com o objetivo de introduzir, avaliar e selecionar variedades de uva sem sementes, adaptadas às condi es tropicais semi-áridas e oferecer novas alternativas aos viticultores do Vale do S o Francisco, implantou-se, em 1994, uma cole o com dezenove variedades de uvas sem sementes no Campo Experimental de Bebedouro, da Embrapa Semi-árido, em Petrolina - PE. Foram avaliadas treze variedades ao longo dos anos de 1997 e 1998, correspondendo a cinco ciclos de produ o. As variedades utilizadas foram Vênus, Arizul, Beauty Seedless, Thompson Seedless, Marroo Seedless, Canner, CG 39915, Pasiga, Saturn, Emperatriz, A1581, Paulistinha e Loose Perlette, enxertadas sobre o porta-enxerto IAC 572 ('Campinas'). Foram avaliados aspectos relacionados ao desenvolvimento vegetativo e produtivo das plantas e características e composi o química dos frutos. Todas as variedades apresentaram cachos com tamanho pequeno. As variedades Vênus e Marroo Seedless destacaram-se em rela o ao diametro de bagas, apresentando, respectivamente, 17,83 e 18,26 mm, sem a necessidade de aplica o de reguladores de crescimento. O teor de sólidos solúveis totais foi elevado na maioria das variedades, enquanto a acidez total titulável foi reduzida, resultando em rela es SST/ATT satisfatórias. As variedades Vênus e Marroo Seedless foram as mais produtivas, com produtividades anuais de 24 t/ha e 20 t/ha, respectivamente.

Abstract: Este estudo teve como objetivo avaliar diferentes tipos de estacas sobre o enraizamento, brota o e desenvolvimento de mudas do porta-enxerto de videira cv. IAC 572. O experimento foi conduzido no viveiro do Servi o de Negócios Tecnológicos (SNT), da Embrapa, em Petrolina-PE, utilizando-se delineamento experimental inteiramente casualizado com cinco tratamentos e quatro repeti es, sendo vinte e cinco estacas por parcela. Os tratamentos foram: estacas lenhosas com três gemas, estacas lenhosas com duas gemas, estacas lenhosas com uma gema, estacas herbáceas com duas gemas e estacas herbáceas com uma gema. Aos 60 dias após o plantio, as estacas lenhosas com três gemas apresentaram os maiores percentuais de enraizamento (99%) e brota o (99%) e melhor desenvolvimento da parte aérea e do sistema radicular, entretanto, n o diferiu dos valores obtidos para enraizamento e brota o pelo emprego de estacas lenhosas com uma gema.
Rotulagem de suplementos vitamínicos e minerais: uma revis?o das normas federais

Abstract: in brazil, there is a steadily increasing offer and demand for products on the basis of vitamins and minerals. according to brazilian law, the differences in the dosages offered to the consumer in each product are the parameter for its classification as alimentary supplement or medicament. the limit between these two concepts, however, is confusing and lacks clearness. considering the risk posed by imprudent consumption of such products and seeking to facilitate the interpretation and consolidation of the norms dealing with vitamin and mineral products as well as to create the basis for a master's degree dissertation, a bibliographical survey and evaluation of the entire juridical basis regarding the labeling of these products was conducted. it was concluded that the normative regulations are extensive, complex and of difficult understanding, with a great number of norms dealing with the same subject issued by different authorities. these norms are not consolidated leading to difficult interpretation by retailers, health professionals and consumers and even to failures in the application of these norms by the control authorities. suggestions are made for helping to correct the failures identified in the study.
Avalia??o de seis variedades de uvas sem sementes no Submédio S?o Francisco

Abstract: this work aimed to study aspects related to vegetative and productive development of plants and characteristics and chemical components of bunches of six seedless grape varieties at environmental conditions of s?o francisco river's valley, northeastern brazil. the experiment was carried out during 1997 and 1998. the varieties tested were grafted on iac 572 (`jales') rootstock. the experimental model utilized was totally randomized, and with parcels subdivided into different production cycles. the results showed significative differences among varieties at different pruning times. the average weight of bunches varied from 164.8 g for `marroo seedless' and 203.5 g for `beauty seedless'. the average diameter of berries was superior to 15.7 mm for all of the varieties. the total soluble solids varied from 14.05obrix in `canner' to 19.6obrix in `venus'. the total titratable acidity (att) was lower than 0.91 g tartaric acid/100 ml of juice. the ratio sst/att varied from 19.05 in `beauty seedless' to 28.57 in `vênus'. `vênus' and `marroo seedless' were the most productive varieties, with the average annual production of 24 ton/ha and 20 ton/ha, respectively.

Abstract: Este trabalho teve como objetivo avaliar aspectos relacionados ao desenvolvimento vegetativo e produtivo das plantas, às características físicas e à composi o química dos frutos de seis variedades de uvas sem sementes nas condi es do Submédio S o Francisco. Os dados foram registrados durante os anos de 1997 e 1998. As variedades utilizadas foram enxertadas sobre o porta-enxerto IAC 572 (`Jales'). O delineamento experimental foi inteiramente casualizado, com parcelas subdivididas nos ciclos de produ o. Houve diferen as significativas na resposta da maioria das variedades entre as épocas de poda. O peso médio dos cachos variou entre 164,8 g na variedade Marroo Seedless, e 203,5 g na `Beauty Seedless'. O diametro médio das bagas foi superior a 15,7 mm em todas as variedades. Foram obtidos teores de sólidos solúveis compreendidos entre 14,05oBrix na variedade Canner e 19,6oBrix na `Vênus', enquanto a acidez total titulável foi inferior a 0,91 g de ácido tartárico/100 mL de suco, resultando em adequadas rela es SST/ATT, que variaram entre 19,05 na variedade Beauty Seedless a 28,57 na `Vênus'. As variedades Vênus e Marroo Seedless foram as mais produtivas, com produtividades anuais de 24 t/ha e 20 t/ha, respectivamente.

Abstract:

The fruit production for export is an economically significant activity in the Valley of S?o Francisco River, especially in the irrigated lands of Petrolina-PE/Juazeiro-BA, Brazil. The development of new genetic material most suitable to the tropical climate and the demands of the consumer market have led to the selection of new seedless grapes cultivars. In this case, the use of the embryo rescue technique has produced satisfactory results for obtaining such materials, especially in the semiarid region. This study aimed to evaluate the in vitro development of intraspecific hybrids of grapevine (Vitis vinifera L.), derived from the rescue of immature embryos resultant from the crossing of “Superior Seedless” and “Thompson Seedless” Brazilian clones. To establish and develop the cultivation, the culture media was supplemented with 30 g/L sucrose, 0.1 g/L myo-inositol, 0.002 g/L glycine, 0.1 mg/L indoleacetic acid (IAA), 6.5 g/L of agar, adjusted pH to 5.7. The experiment was evaluated after 90 days. The variables measured were: number of nodes, number of leaves, plant height (cm), number of roots and length (cm) of the root system and internodes. The period of 60 days of in vitro culture of ovules resulted in the highest values of embryos (about 50%), as well as better characterized developmental stages with higher germination (47.3%). The three types of hybrid grapes evaluated in micropropagation showed very similar values of the measured parameters, even having originated from embryos of different developmental stages.

Abstract:
Introduction: Advances in prenatal imaging studies have allowed early diagnosis of Congenital Diaphragmatic Hernia (CDH), although neonatal mortality remains at high levels (60% survival). Despite advances, this study found delayed diagnosis, demonstrating subdiagnosis, possibly caused by lack of resources and/or prenatal diagnostic failure. Case Report 1: Infant, 4 months and 22 days old, dyspneic, cyanotic, with fever and vomiting for 3 days, treated as pneumonia. After the second chest radiography, a right CDH was revealed. Surgical correction was performed and removal of Meckel’s diverticulum was surgical findings. The patient was discharged on the 10th days after admission. Case Report 2: Infant, 11 months and 3 days old, distended abdomen, with fever and dyspnea for 2 days, treated as pneumonia. After the second chest radiography, a left CDH was revealed. The patient was submitted to surgery, intestinal loop incarceration without necrosis was observed, and the correction of CDH was performed. The patient was discharged on the 7th day after admission. Discussion: Three cases, between July 2012 and July 2013, were diagnosed at Dom Malam Hospital; 2 Cases of CDH, with delayed diagnosis, showed incidence of 66.66%. Delayed diagnosis of these patients with initial hypotheses of pneumonia demonstrated high risk, emphasizing the importance of diagnosis research through observation of the clinical course, the correct imaging interpretation, in order to establish early diagnosis and treatment.
