Carcinoma de ovário seroso e n?o seroso: tipo histológico em rela??o ao grau de diferencia??o e prognóstico

Abstract: purpose: to compare the clinical-pathological features of women with serous and non-serous ovarian tumors and to identify the factors associated with survival. methods: in this reconstructed cohort study, 152 women with ovarian carcinoma, who attended medical consultations between 1993 and 2008 and who were followed-up until 2010 were included. the histological type was clearly established for all women: 81 serous carcinomas and 71 non-serous tumors (17 endometrioid, 44 mucinous and 10 clear cell carcinomas). the crude and adjusted odds ratios (or), with the respective 95% confidence intervals (95%ci), were calculated for the clinical and pathological features, comparing serous and non-serous histological types. the hazard ratios (hr) with 95%ci was calculated for overall survival, considering the clinical and pathological features. results: comparison of serous to non-serous tumor types by univariate analysis revealed that serous tumors were more frequently found in postmenopausal women, and were predominantly high histological grade (g2 and g3), advanced stage, with ca125>250 u/ml, and with positive peritoneal cytology. after multivariate regression, the only association remaining was that of high histological grade with serous tumors (adjusted or 15.1; 95%ci 2.9-77.9). we observed 58 deaths from the disease. there was no difference in overall survival between women with serous carcinoma and women with non-serous carcinoma (hr 0.4; 95%ci 0.1 - 1.1). it was observed that women aged 50 years or less (hr 0.4; 95%ci 0.1-0.9) and those who were in menacne (hr 0.3; 95%ci 0.1-0.9) had a longer survival compared respectively to those above 50 years of age and menopaused. high histological grade (g2 and g3) (p<0.01), stages ii-iv (p<0.008) and positive cytology (p<0.001) were significantly associated with worse prognosis. ca125 and the presence of ascites did not correlate with survival. survival was poor when the disease was diagnosed in stages ii to iv and compared to st

Abstract: one of the most challenging topics in tinnitus clinical studies is the measuring method used. visual analogue scales (vas) and tinnitus handicap inventory (thi) are frequently used in tinnitus. aim: to verify the relationship between vas and thi scores in tinnitus patients in a prospective study. materials and methods: 43 patients classified their tinnitus according to vas and thi, and both scores were compared through the spearman's correlation coefficient test. results: there was a correlation between the vas and thi scores. conclusion: there is correlation between vas and thi scores in patients with sensorineural tinnitus.

Abstract: one of the most criticized points in tinnitus clinical studies arise from the lack of consensus about measurement methods. aim: to evaluate the correlation between audiometric thresholds, pitch matching (pm), minimum masking level (mml), tinnitus handicap inventory (thi) and the beck depression inventory (bdi) in tinnitus patients. study design: prospective, cross-sectional. materials and methods: subjects were submitted to tonal audiometry, pm and mml for tinnitus. they also filled out the thi and bdi. data was statistically compared for correlation purposes between audiometric thresholds, psycho-acoustic measures and questionnaires. results: there was no statistically significant correlation between thi and mml, both in patients with bdi scores under and over 14 points. there was no statistically significant correlation between the worst hearing frequency and pm, as well as between the cut-off frequency and the pm in patients with descending hearing curves in their audiograms. conclusions: there is no statistically significant correlation between psycho-acoustic measures (pm and mml), audiometric thresholds, thi and bdi. tinnitus is a very complex symptom and isolated measures by psycho-acoustic methods; tinnitus and depression questionnaires are not satisfactory.
Active teaching-learning methodologies: medical students' views of problem-based learning

Abstract: the prevailing undergraduate medical training process still favors disconnection and professional distancing from social needs. the brazilian ministries of education and health, through the national curriculum guidelines, the incentives program for changes in the medical curriculum (promed), and the national program for reorientation of professional training in health (pro-saúde), promoted the stimulus for an effective connection between medical institutions and the unified national health system (sus). in accordance to the new paradigm for medical training, the centro universitário serra dos órg?os (unifeso) established a teaching plan in 2005 using active methodologies, specifically problem-based learning (pbl). research was conducted through semi-structured interviews with third-year undergraduate students at the unifeso medical school. the results were categorized as proposed by bardin's thematic analysis, with the purpose of verifying the students' impressions of the new curriculum. active methodologies proved to be well-accepted by students, who defined them as exciting and inclusive of theory and practice in medical education.

Abstract: although tetanus is a preventable disease by vaccination, it continues to claim lives around the world. whereas cases of accidental origin reflect insufficient population immunization, tetanus neonatorum reveals a double-nature fault-poor vaccination coverage of adults coupled with difficulties accessing appropriate prenatal care; this situation is aggravated by the extreme severity of tetanus in this age group in which the mortality rate can reach up to 80%. the early detection of tetanus in neonates is essential for immediately initiating the proper therapy. therefore, although reaching an early diagnosis of tetanus is important, the most relevant aspect is related to the appropriate management and prophylaxis of this disease. consequently, the aim of this article is to review neonatorum tetanus with an emphasis on its therapy and prevention.

Abstract: the changes in medical training have long been discussed in medical schools. these attempted changes have primarily been of a methodological or pedagogical nature, with curriculum restructuring based on the introduction of problem based learning (pbl) as an underlying axis. the traditional curricular format is still predominant, with pbl emerging as the most common alternative. the aim of this article is to present this method and assessments of its results, in order to create a snapshot of how effective it is in preparing critically-minded, thoughtful and socially responsible doctors. a literature review was conducted based on searches of the scielo (scientific eletronic library online), pubmed (www.nlm.nih.gov) and bvs (virtual library in health) databases, with the selection of articles about the subject published in the last 20 years. the results reveal isolated pedagogical change to be insufficient as a solution to provide medical training capable of combining technical and ethical and humanistic skills.

Abstract: bullying is an english expression which was adopted in several countries to refer to intimidation. fights, offenses, mean commentaries, physical and psychological aggressions, and repression are types of violence generally associated to childhood. studies indicate that bad manners childhood play could generate serious consequences such as low self-esteem, new sources of violence, and, inclusively, suicide matters. this study is developed since july 2007 in four public schools in the city of porto alegre, rs, with 192 student?s participants. through questionnaire and focal groups, it was observed that more than 70% of the students perceive the existence of prejudice in the school environment and almost 40% consider it regular/bad or horrible the relations in schools, reflecting the discomfort of students concerning the school. this dynamic is leading to more exclusion and isolation among teenagers.
