Citología endometrial en pacientes posmenopáusicas con terapia hormonal

Abstract: objective: to demonstrate the efficacy of endometrial cytology with the uterobrush? sampler and compare it with endometrial biopsy, in postmenopausal women under hormonal replacement therapy. method: prospective study with 30 patients during a lapse of six months, fifteen of them refered abnormal uterine bleeding and the other fifteen did not. endometrial cytology sampling was done with uterobrush? and biopsy with novak′s currette to all patients. difficulty, pain, bleeding, cervical characteristics, celularity and adequacy of the sampling where assessed, cytology was compared with biopsy diagnosis. setting: gynecology service of the university hospital of los andes, results: 73.3 % of the patients did not refer pain. no bleeding was present after the introduction of uterobrush ? in 53.3 %. the samples were adequate in 70 % of cases. principal cause of limitation was poor fixation of cells. when comparing cytologic diagnosis with biopsy we found a sensitivity of 71 % and 75 % specificity. conclusions: the endometrial cytology with uterobrush? is a procedure easy to use, economic, and well tolerated by the patients; being useful to evaluate the endometrium in post menopausal women, but not pretending to overtake endometrial biopsy.

Abstract: Objetivo: Demostrar la utilidad de la toma de muestra con Uterobrush y compararlo con la biopsia endometrial, en pacientes posmenopáusicas con terapia hormonal. Método: Estudio prospectivo durante un lapso de seis meses, en el cual se incluyeron 30 pacientes, de las cuales 15 referían sangrado genital anormal y 15 no referían sintomatología. A cada paciente se le realizó citología endometrial con Uterobrush y posteriormente se le realizó biopsia de endometrio con cureta de Novak. El diagnóstico citológico se comparó con el diagnóstico biópsico, para obtener la sensibilidad y especificidad de la técnica con Uterobrush . Ambiente: Consulta externa del Servicio de Ginecología del Instituto Autónomo Hospital Universitario de Los Andes (IAHULA) Mérida. Resultados: El procedimiento fue fácil en un 73,3 % de las pacientes, no refiriendo dolor. No se presentó sangrado posterior a la introducción del Uterobrush en un 53,3 %. Las muestras fueron adecuadas en el 70 % de los casos. La principal causa de limitación fue la mala preservación de las células. Al comparar los diagnósticos citológicos con la biopsia encontramos una sensibilidad del 71 % y una especificidad del 75 %. Conclusión: La citología endometrial con Uterobrush es un procedimiento aceptable, fácil de usar, económico, y bien tolerado por las pacientes; siendo útil para la valoración endometrial en mujeres posmenopáusicas y en aquellas que así lo ameriten, pero no se pretende desplazar a la biopsia endometrial sino sugerir estudios más extensos. Abstract: objective: to evaluate the efficacy in decreasing the frequency of post operative associated shoulder pain using low intrabdominal pressure during the setting of pneumoperitoneum. methods: randomized clinical trial with thirty patients undergoing laparoscopy were randomized in two groups. group a (experimental group) and group b (control). interventions: gynecology laparoscopy for benigns conditions. group a surgery performed at low intrabdominal pressure (8 mmhg of pneumoperitoneum) and group b underwent surgery with the usual preset intrabdominal pressure for laparoscopy (15 mmhg of pneumoperitoneum). settings: private urban obstetric and gynecology clinic in merida, venezuela. results: shoulder tip pain was assesed 24, 48 and 78 hours after surgery. we found in the first 24 hours an occurrence of shoulder pain in 13% of the patients of group a and 53% of group b. difference between groups were statistically significative p<0,05 c.i.(0,012-0,973) and reduction of relative risk was 75%. no diffference were found at 48 and 72 hours after surgery. there was no increase in time of operation, blood loss or complication in the experimental group. conclusion: performing gynecology laparoscopy with low intrabdominal pressure (8mmhg of pneumoperitoneum) is safe and effective in reducing de frequency of post operative associated shoulder pain.

Abstract: Objetivo: Reducir el dolor de hombro asociado al posoperatorio de la cirugía ginecológica por vía laparoscópica, mediante la utilización durante el transoperatorio de una presión intraabdominal baja. Método: Ensayo clínico aleatorio. Se seleccionaron al azar 30 pacientes y se dividieron en dos grupos. Las pacientes seleccionadas para el grupo experimental (grupo A) se les realizó un pneumoperitoneo inicial de 15 mmHg, para colocar los puertos de entrada con seguridad e inmediatamente se descendió la presión a 8 mmHg. Las pacientes seleccionadas para el grupo de control (grupo B), se les realizó pneumoperitoneo y el acto quirúrgico con presión intraabdominal de CO2 a 15 mmHg. Posteriormente se determinó el nivel de dolor a las 24, 48 y 72 horas del posoperatorio. Ambiente: Maternidad Santa María en Mérida, Estado Mérida. Resultados: La técnica demostró una reducción significativa en el riesgo a presentar dolor de hombro en el posoperatorio. Hubo solamente dolor en 13% de los casos cuando se realizaba la intervención a baja presión, a diferencia del grupo B con 53%, hubo una reducción del riesgo relativo de 75%, Intervalos de Confianza (0,012- 0,973). No se presentaron complicaciones en ninguno de los grupos. Conclusiones: El realizar cirugía laparoscópica a una presión de 8 mmHg durante todo el transoperatorio, disminuye la aparición del dolor de hombro, además de no aumentar los riesgos ni complicaciones propias de la intervención. Abstract: this article presents the results of a study, started in 2006, on information competency in university students. the study was based on the idea that we should use a semiotic focus of discourse to rethink the traditional understanding of information competency. we argue that information competency is represented in already-existing conditions (cognitive, affective, and pragmatic modalities of understanding). these conditions make it possible to interact in a relative manner with the information. we also developed an in situ method to analyze understanding. this method was tested with a student studying education at the universidad industrial de santander (colombia) at a distance. the results show that the dimensions of information competency can be understood based on the subject's experience. the student showed examples of the clear relationship between the modalities (the semiotic perspective) and the experiences accrued over his or her life (the socio-cultural perspective). the strong relationships between the modalities that required careful analysis of the results were also evident.

Abstract: rotaviruses have been identified as one of the main etiological agents of diarrhea and enteritis in mammals, including humans, and in avian species. few studies have been published about enteric viruses in brazilian poultry, including those related to rotavirus infection. such studies demonstrate significant occurrence and the importance of enteric viruses in poultry presenting intestinal problems. enteric viruses are the primary cause of injuries to the gut, allowing other agents, especially bacteria, to attach, to penetrate, and to replicate in the enteric tissue, leading to further damage. the aim of the present study was to detect rotavirus in the intestinal contents of layers and broilers by polyacrylamide gel electrophoresis (page) and virus isolation in ma-104 cell culture. a total of 45.3% of all samples were positive to rotavirus; rotavirus frequencies were 48.7% in samples from flocks with diarrhea, 46.4% in flocks with delayed growth, and 30% in asymptomatic flocks. it was possible to isolate rotavirus in ma-104 cells from the nine rotavirus-positive randomly chosen samples. these results indicate that rotavirus may have an important role in pathogenesis of enteric disease.

Abstract: this article reports a survey on turkey astrovirus (tastv) and turkey coronavirus (tcov) infections with rt-pcr in 17 turkey flocks affected by acute enteritis and two apparently normal turkey flocks located in the southeastern region of brazil by pcr (tastv and tcov). seven out of the 17 affected flocks were positive for tastv and 14 for tcov, with seven co-infections. in one of the two apparently normal flocks, a tastv-tcov co-infection was found. although a definitive association of these agents and the signs can not be made, the implications of these findings are discussed.

Abstract: ventilator associated pneumonia is a life threatening disease, in spite of advances in its treatment, consequently the development of prevention strategies is a key factor in improving the morbidity and mortality in intensive care units (icu). the new developments in this field in the last years led to the need to update the recommendations done in 2001. then, a new search and analysis of scientific literature was performed. the obtained data support different strategies highlighting: semi-recumbent position at 45° of patients; incorporation of routinely oral hygiene procedures with chlorhexidine; preference of orotracheal intubation; performing subglotic aspiration; use of standard hand hygiene techniques; not routine change of ventilator circuits; and defining bundles to organize the work at icu. some strategies were recommended for being of similar efficacy to others but cost-effective as: use of heat and moisture humidificators; and changing humidificators every 5 to 7 days. the use of open or closed endotracheal suctioning system does not affect the incidence of pneumonia. some recommendations were not incorporated because of lack of evidence supporting effectiveness, controversial data or doubtful application to our country as selective digestive decontamination.
