A mediastinoscopia no diagnóstico de doen?as intratorácicas

Abstract: the cervical mediastinoscopy, developed by carlens in 1959 is used primarily to evaluate, before thoracotomy, the status of mediastinal limph nodes in patients with lung cancer. however, exploration of the anterior mediastinum by these techniques can also identify other diseases processes such as sarcoidosis, tuberculosis, mycotic granulomatous infections and neoplastic diseases involving the mediastinum such as lymphoma. ln this study we analyze the medical reports of 125 patients in which mediastinoscopy was performed for the diagnosis of intrathoracic diseases. the procedure was performed with general anesthesia and orotracheal intubation in all patients out in two, submitted to mediastinoscopy with local anesthesia. the surgical approaches used were: cervical (n=103). anterior (n=7) and cervical + anterior (n=15). there were 80 male and 45 female patients. the age ranged from 13 to 75 years. carcinoma was the most prevalent diagnosis obtained (36.8%), followed by lymphoma (16%) and sarcoidosis (14.4%). in nine patients the exam was inconclusive, being responsible for a 7.2% failure index of the method. in eleven patients presenting superior vena cava syndrome, mediastinoscopy was performed without aditional complications, except in one case in which symptoms worsenned. we conclude that mediastinoscopy is a safe procedure and it is a valuable tool for the diagnosis of paratracheal mediastinal masses and lymphadenomegalies.

Abstract: background: chronic obstructive pulmonary disease (cpod) is an important problem in brazil and other countries. the clinical treatment of ambulatory patients is now reasonably standardized. lung volume reduction surgery (lvrs) is an alternative of surgical treatment of emphysema (aside from lung transplantation). objectives: the aim of this study was to analyse patient selection criteria for lvrs. methods: this study was carried out using a sample of 31 patients referred to and given assistance to at the smoking and chronic obstructive pulmonary disease outpatient ward antonio pedro hospital, who were evaluated to check whether or not they could be eligible for lvrs. aiming at evaluating the major criteria for eligibility for lvrs, the patients underwent a protocol including standardized anamnesis and physical examination, spirometry with maximum inspiratory and expiratory flow volume curves associated with post-bronchodilatator test, room air arterial blood gases, a six-minute walking test, plain chest roentgenograms and left lateral chest roentgenograms, high-resolution chest computed tomographic scans and lung perfusion scans. statistical analysis was conducted with description of data proportions. results: contraindications to surgery included chronic bronchitis (29% or nine patients), forced expiratory volume in one second (fev 1) more than 80 percent predicted (13% or four patients), negative consent for surgery (10% or three patients), fibrosis pattern, severe depression and bronchiectasis (two patients each one), solitary pulmonary nodule, severe obesity, severe bullous emphysema, pulmonary hipertension, pulmonary resection history, current cigarette smoking, diffuse emphysema with alpha-1-antitrypsin deficiency and severe cardiac disease (one patient each). indication. one selected patient with severe symptoms of emphysema met criteria for lvrs (bilateral superior lobe heterogeneous pattern of emphysema on chest computed tomography and lung perfusion scan).

Abstract: O tratamento clínico da doen a pulmonar obstrutiva cr nica (DPOC) está bem padronizado por consensos nacionais e internacionais. Junto com transplante pulmonar, a cirurgia redutora de volume pulmonar (CRVP) é alternativa de tratamento cirúrgico para o enfisema. OBJETIVOS: Avaliar critérios de inclus o e exclus o para realiza o de CRVP em pacientes com DPOC. MéTODOS: Trinta e um pacientes encaminhados e acompanhados no Ambulatório Multidisciplicar de Pesquisa em DPOC e Tabagismo dos Cursos de Pós-Gradua o em Pneumologia da Universidade Federal Fluminense, submetidos a um protocolo de avalia o para sele o de candidatos à CRVP que incluiu anamnese e exame físico padronizados, espirometria com curva fluxo volume e teste com broncodilatador, gasometria arterial com paciente respirando ar ambiente, teste de caminhada por seis minutos (TC6M), radiografia convencional de tórax em póstero anterior e perfil esquerdo, tomografia computadorizada de tórax de alta resolu o (TCAR) e cintilografia pulmonar de ventila o e perfus o. Análise estatística: descritiva. RESULTADOS: Contra-indica es para CRVP: bronquite cr nica (29% ou nove pacientes), VEF 1> 80,0% do previsto (13% ou quatro pacientes), recusa em realizar a cirurgia (10% ou três pacientes), áreas de fibrose, depress o e bronquiectasias (dois pacientes cada), nódulo pulmonar solitário neoplásico, obesidade mórbida, enfisema bolhoso grave, hipertens o pulmonar, passado de ressec o pulmonar, tabagismo corrente, enfisema difuso com deficiencia de alfa-1-antitripsina e doen a cardiaca grave (um paciente cada). Um dos pacientes selecionados concordou e foi submetido à CRVP. Sua indica o foi enfisema pulmonar heterogênio com predomínio nos lobos superiores, diagnosticado pela TCAR e cintilografia pulmonar. CONCLUS ES: Bronquite cr nica representou a principal contra-indica o em 29% dos casos. Pacientes com indica o de CRVP, freqüentemente, apresentam mais de um fator mórbido a contra-indicar o seu procedimento. A maioria dos pacientes com critérios para CRVP, a recusa, com receio do procedimento ou após programas de reabilita o pulmonar. O critério mais significativo que indicou CRVP em um paciente operado foi a presen a de enfisema heterogênio bilateral nos lobos superiores.

Abstract: Objetivou-se analisar as diferentes escalas de tempo para determinar qual o intervalo mais adequado no estudo do comportamento ingestivo de vacas leiteiras confinadas. O experimento foi conduzido na fazenda água Azul, Município de Macarani-Bahia, no período de outubro a novembro de 2005. Foram utilizadas 12 vacas mesti as Holandês, apresentando, em média, 478,5 15,89kg de peso corporal. Os tratamentos do presente estudo foram os intervalos (cinco, 10; 15; 20 e 30 minutos), observados durante o comportamento ingestivo. A observa o do comportamento ocorreu no penúltimo dia de cada período de 12 dias durante 24 horas. Procedeu-se a análise de variancia e a aplica o do teste de Dunnett, adotando-se 0,05 como nível crítico de probabilidade. N o houve efeito significativo das escalas de registro do tempo de alimenta o, rumina o, ócio e para as eficiências de alimenta o e rumina o. Para as variáveis dos números de períodos de alimenta o, rumina o e ócio e os tempos por período de alimenta o, rumina o e ócio, as escalas de 10, 15, 20 e 30 minutos foram diferentes quando comparadas com o intervalo de observa o de cinco minutos. As coletas totais dos tempos de alimenta o rumina o e ócio, e as eficiências de alimenta o e rumina o nos diferentes nutrientes podem ser realizadas com intervalo de observa o de até 30 minutos. Para a discretiza o das séries temporais do comportamento ingestivo recomenda-se a escala de cinco minutos entre as observa es. This study aimed to analyze different time scales to determine the most appropriate interval in the study of ingestive behavior of confined dairy cows. The experiment was conducted at the farm Blue Water, city of Macarani-Bahia, in the period from October to November 2005. 12 crossbred Holstein cows were used, with an average body weight of 478.5 15.89kg. The treatments of this study were the intervals (five; 10; 15; 20 and 30 minutes), observed during ingestive behavior. Behavior observations occurred on the second to last day of each period of 15 days data collection for 24 hours. There has been analysis of variance and Dunnett test application, adopting 0.05 as the critical level of probability. There was no significant effect of record scales of time spent eating, ruminating, idling and efficiencies of feeding and rumination. For the variables of the numbers of periods of eating, ruminating and idle and times per period of feeding, ruminating and resting, the ranges of 10, 15, 20 and 30 minutes were different when compared with the interval of five minutes of observation. Abstract: ResumoO sistema de cria o de bovinos a pasto é caracterizado por uma série de fatores e suas intera es podem afetar o comportamento ingestivo dos animais, comprometendo o seu desempenho e, conseqüentemente, aviabilidade da propriedade. Existe uma alta correla o entre o consumo de forragem e o desempenho animal, uma vez que esta é a principal fonte de nutrientes para ruminantes, principalmente nos trópicos, onde a pecuária se sustenta à base das pastagens. Estudando o comportamento ingestivo dos ruminantes, poderemos adequar práticas de manejo que venham a aumentar a produtividade, garantindo também um melhor estado sanitário e maior longevidade aos animais. A altura do dossel, a densidade, a disponibilidade, a morfologia, o valor nutritivo, e a preferência da forragem, a categoria, estado fisiológico, sanitário e seletividade do animal, topografia e temperatura do ambiente, entre outros, s o fatores que afetam a ingest o e digest o de plantas forrageiras, interferindo diretamente no comportamento ingestivo de bovinos a pasto. Os animaistendem a ser mais seletivos em pastagens que apresentam uma menorrela o lamina: colmo, bem como uma menor disponibilidade de forragem.O animal em pastejo está sob o efeito de muitos fatores, que podeminfluenciar a ingest o de forragem; entre eles, sobressai a oportunidade de selecionar a dieta, pois o pastejo seletivo permite compensar a baixa qualidade da forragem, permitindo a ingest o de partes mais nutritivas das plantas. SummaryThe system of breeding cattle to pasture is characterized by a number of factors and their interactions can affect the ingestive behavior of animals, affecting their performance and, consequently, the viability of the property. There is a high correlation between the consumption of forage and animal performance, since this is the main source of nutrients for ruminants, especially in the tropics, where the livestock is sustained on the basis of the pastures. Studying the ingestive behavior of ruminants, we can tailor management practices that will increase productivity, ensuring also a better health status and greater longevity of animals. The height of the canopy, density, availability, morphology, nutritional value and palatabilityof forage, category, physiological status, health and selectivity of the animal, topography and temperature of the environment, among others, are factors that affect the intake and digestion of forage, interfering directly in the ingestive behavior of cattle grazing. Abstract: Objective: to understand how mothers experienced the process of admission of a child in an Intensive Care Unit Neonatal. Methodology: this study was a descriptive and exploratory in the qualitative approach with approach to discourse analysis of subjects, from the question: How do you mother experiences the process of admission of the child in an intensive care unit? Data collection was conducted through interviews recorded with the signing of the terms of consent, held from December 2008 to February 2009, with 5 mothers of children admitted to an intensive care unit of a Hospital Neonatal philanthropic the interior of Minas Gerais, as approved by the Ethics in Research Involving Human Subjects, with the opinion 203/2008. Results: based on the analysis of the interviews yielded the 03 categories: A Unit of Neonatal Intensive Care as a strange environment, new, complex and full of machines; Unit Neonatal Intensive Care as a breach of the mother-son relationship the between staff and families in the Intensive Care Unit Neonatal. Conclusion: we observed that this experience is rather difficult for these mothers, and that despite the pain and anguish they go through, can understand the importance of the treatment team and the unit as a whole. Impacted foreign bodies in orbital region: review of nine cases

Abstract: orbital injuries with a foreign body may result in severe structural and functional damage to the eye or orbital contents. management and prognosis depend on the composition and location of the foreign body and whether there is secondary infection. metallic objects and glass are the most frequently encountered and well-tolerated, whereas organic foreign bodies can elicit an inflammatory reaction and lead to serious complications. despite the modern imaging methods, it is often difficult to identify and locate organic intraorbital foreign bodies. this paper presents a review of nine cases of impacted foreign bodies in the orbital region and discusses the diagnosis and treatment of this kind of injury. the following data were collected: age, gender, etiology of injury, occurrence of fracture, anatomical location of fracture, type of object, signs and symptoms, type of imaging exam used, approach, transoperative complication and occurrence of death. foreign body injuries in the orbital region can be treated with a combination of clinical suspicion, basic knowledge and diagnostic tests and depend on the skill and experience of the surgeon, thereby decreasing the surgical risk of iatrogenic injury in relation to the inherent risk of retaining an organic intraorbital foreign body.

Abstract: Objetivou-se verificar o efeito dos intervalos e números de observa es no comportamento ingestivo de novilhas leiteiras confinadas. Foram utilizadas 20 novilhas mesti as Holandês x Zebu, com peso corporal médio inicial de 165,5kg e idade média de 13 meses. As novilhas foram distribuídas em quatro grupos, compostos por níveis de inclus o de farelo de cacau com base na matéria seca das dietas (0; 7; 14 e 21%). Os tratamentos do presente estudo foram os intervalos (cinco; 10; 15; 20 e 30 minutos), números de repeti es (quatro, três, duas e uma), e turnos (quatro, três, dois e um) de observa o durante o comportamento ingestivo. A avalia o ocorreu no fim de cada período de 21 dias de coleta de dados, durante 24 horas, sendo as atividades registradas em cinco escalas diferentes: cinco; 10; 15; 20 e 30 minutos de intervalo. Os aspectos do comportamento ingestivo foram observados em quatro, três, duas e uma repeti es e turnos. N o houve efeito significativo das escalas de registro sobre o tempo de alimenta o, rumina o e ócio, nem sobre as eficiências de alimenta o e rumina o. Para as variáveis relacionadas com a atividade de rumina o, n o houve diferen a entre os diferentes números de observa es, nem para os diferentes turnos de observa o. A determina o dos tempos totais das atividades de alimenta o, rumina o e ócio, e as eficiências de alimenta o e rumina o dos diferentes nutrientes podem ser realizadas com intervalo de observa o de até 30 minutos. The objective of this study was to assess the effect of the intervals and numbers of observations on the ingestive behavior of confined dairy heifers. We used 20 heifers Holstein x Zebu, with initial body weight of 165.5kg and mean age of 13 months. The heifers were divided into four feeding groups, composed of inclusions of cocoa meal based diets (0; 7; 14 and 21%). The treatments were the intervals (five; 10; 15; 20 and 30 minutes), number of repetitions (four, three, two and one), and turns (four, three, two and one) the observation of ingestive behavior. The evaluation came at the end of each period of 21 days of data collection for 24 hours, and activities recorded in five different intervals: 0; 10; 15; 20 and 30 minutes. Aspects of ingestive behavior were observed in four, three, two and one replicates and turns. There was no significant effect of scales of record on the time of feeding, ruminating and idle, either on the efficiencies of feeding and rumination. Body composition and net and dietary macrominerals requirements of Nellore steers under grazing

Abstract: this experiment was carried out with the objective of determining the macrominerals (ca, p, mg, k and na) requirements of nellore steers under grazing. twenty four nellore steers (371 ± 14 kg of bw and 26 mo old) were used. four steers were slaughtered at the beginning of the experiment (reference group), serving as a reference in subsequent study. the remaining 20 animals were weighed and distributed into a completely randomized design with four supplementation levels offer: 0.0 (mineral mixture - control), 0.3, 0.6 and 0.9% of bw, with five replications. the supplements, based on ground corn, soybean meal and/or urea, were previously balanced to achieve an average daily gain of 350, 650 and 850g, respectively, for the different supplementation levels offer. the contents of macrominerals retained in the animal body were determined by regression equations of the macrominerals body content logarithm in function of the empty body weight logarithm (ebw). net macrominerals requirements for a gain of 1kg of ebw were obtained using the equation y'= b.10a.xb-1, with a and b, respectively, the intercept and the regression coefficient of the prediction equations of macrominerals in the animal body contents for each macromineral considered. the concentrations of all macrominerals, in the empty body weight and gain of the empty body weight, decreased with the increase in the body weight. total calcium and phosphorus dietary requirements are higher than those recommended in the literature.
Prehospital care: nursing professors’ perception ahead caring to multiple victims

Abstract: a qualitative study, from phenomenological approach. The sample was composed of five nurses from University professorJosé do Rosário Vellano Unifenas, which has experience and who work or have worked in prehospital care from August2006 to September 2008, directed by the following question: what is your perception before the assistance pre-hospitalto multiple victims? Results: from the analysis of the data, obtained through testimony, it was possible determine fivecategories: 1) events with multiple victims: living with the stress all the time; 2) the criterion for assessing the scene andvictims: putting into practice the training; 3) training and professional recognition: important aspects in the quality ofcare; 4) work in teams: fundamental aspect in response to multiple victims; 5) experiencing traumatic scenes: how to dealwith death. Conclusion: the results show that serve multiple victims is a difficult, stressful, delicate, requiring knowledgea lot and practice skill on the part of professional, and an emotional balance and great human sensitivity.
