Abstract: objectives ? to determinate prevalence of helicobacter pylori infection in a group of symptomatic patients between 2 and 18 years old who were submitted to gastric biopsy during upper endoscopy done in the period of 1990-97. to correlate the histological findings with clinical and endoscopical data. methods ? a cross-sectional study done after review of clinical and histopathological data. histopathological sections were reviewed by a pathologist, who did not know the clinical information and the previous histopathological reports. results ? among 181 patients evaluated, prevalence of helicobacter pylori infection was 24.86% (45 positive cases). in pathological analysis, gastritis was found in 38/45 of the positive helicobacter pylori patients and in 45/136 negative helicobacter pylori. gastric ulceration was found in 6/45 positive helicobacter pylori and in 3/136 negative helicobacter pylori. conclusion ? this study stated a significative association between helicobacter pylori infection and pathological abnormalities in children evaluated in the "hospital de clínicas de porto alegre", rs, brazil.

Abstract: Objetivos - Determinar a prevalência de infec o pelo Helicobacter pylori em pacientes de 2 a 18 anos submetidos a biopsias gástricas por endoscopia es fago-gástrica-duodenal no Hospital de Clínicas de Porto Alegre, RS, durante o período de 1990 a 1997, correlacionando-a com os achados clínicos, endoscópicos e anatomopatológicos. Métodos - Estudo transversal embasado na revis o de arquivos médicos e de laudos anatomopatológicos, sendo estes últimos revistos por médico patologista que desconhecia as informa es clínicas e laudos anatomopatológicos prévios. Resultados - Foram avaliados 181 pacientes e obteve-se uma prevalência de infec o pelo Helicobacter pylori de 24,86% (45 casos). Na histologia, gastrite foi encontrada em 38 pacientes dos 45 com Helicobacter pylori positivo e em 45 do 136 Helicobacter pylori negativo. úlcera péptica foi encontrada em 6 dos 45 Helicobacter pylori positivo e em 3 dos 136 Helicobacter pylori negativo. Conclus o - O estudo demonstrou rela o significativa da infec o do Helicobacter pylori com altera es principalmente histológicas nas crian as avaliadas.
Hard Decision-Based PWM for MIMO-OFDM Radar  [PDF]

Abstract: For the purpose of target localization, Multiple Input Multiple Output-Orthogonal Frequency Division Multiplexing (MIMO-OFDM) radar has been proposed. OFDM technique has been adopted in order to a simultaneous transmission and reception of a set of multiple narrowband orthogonal signals at orthogonal frequencies. Although multi-carrier systems such as OFDM support high data rate applications, they do not only require linear amplification but also they complicate the power amplifiers design and increase power consumption. This is because of high peak-to-average power ratio (PAPR). In this work, a new proposition has been made based on the Pulse Width Modulation (PWM) to enhance the MIMO-OFDM radar systems’ performance. In order to check the proposed systems performance and its validity, a numerical analysis and a MATLAB simulation have been conducted. Nevertheless of the system characteristics and under same bandwidth occupancy and system’s specifications, the simulation results show that this work can reduce the PAPR values clearly and show capable results over the ones in the literature.

Abstract: Phase and group propagation in metamaterial-based microwave components has always been intellectually challenging for students and engineers new to the area of periodic structures and metamaterials. This paper aims in tackling this important topic by studying the wave propagation in a metamaterial-based microwave device. Hence, the contribution of this paper is twofold. First, design of a novel metamaterial ring hybrid (or rate-race) is presented which has a large rejection band so that the second and third harmonics are effectively suppressed. Second, the electromagnetic phase and the group propagation in the ring hybrid are investigated by numerically exciting the input ports with band-limited Gaussian pulses and then finding their responses at various locations in the device.

Abstract: the tumor-node-metastasis (tnm) staging system for lung cancer has been modified since its first edition in the late 1960s. its seventh edition has been recently published and, for the first time, a truly worldwide database was analyzed in order to propose modifications in the staging. significant changes have been made in the tumor and metastasis descriptors. although the recommendations for the node descriptor have remained unchanged, the analysis of the factors related to this descriptor suggests that modifications will be made in the future. the forthcoming revisions of the tnm staging system might take the molecular aspects of lung cancer into consideration, aiming at a more refined staging system.

Abstract: La alta prevalencia de los macroadenomas hipofisarios en el mundo motivó la realización de esta investigación, cuyo objetivo fue demostrar el valor diagnóstico de la tomografía computadorizada y su utilidad para definir la técnica quirúrgica en estos casos. Para ello se estudiaron 124 pacientes operados con el diagnóstico clínico y tomográfico de macroadenomas hipofisarios y otros tumores de la región selar menos frecuentes. El 100 % de los tumores con crecimiento C-E y C-D se abordaron por vía transcraneal y el 99 restante con crecimiento A, B ó C se utilizó la vía transesfenoidal. Las complicaciones tomográficas y clínicas fueron superiores en la primera vía y se destacaron los restos tumorales (40 %), las recidivas (28 %) y da o de los nervios craneales (28 %). La mejoría clínica posoperatoria alcanzada fue ostensible al igual que el grado de control de los adenomas, lo cual demostró el alto valor diagnóstico de la tomografía computadorizada y su utilidad para trazar la estrategia quirúrgica correcta, esto muestra una alta sensibilidad en la detección de algunas complicaciones posoperatorias que se pueden valorar con este método. The high prevalence of hypophyseal macroadenomas in the world motivated us to conduct this research aimed at demonstrating the value of computerized tomography and its usefulness to define the surgical technique in these cases. To this end, 124 patients operated on with the clinical tomographic diagnosis of hypophyseal macroadenomas and other tumors of the sellar region that were less common were studied. 100 % of the tumors with C-E and C-D growth were approached by transcranial route, whereas in the other 99 with A, B or C growth, it was used the transsphenoidal route. The tomographic and clinical complications were higher in the first route, and the tumor debris (40 %), the relapses (28 %) and the damage of the cranial nerves (28 %) stood out. The postoperative clinical improvement attained was significant the same as the degree of control of the adenomas, which showed the high diagnostic value of computerized tomography and its usefulness to establish the suitable surgical strategy. This shows a high sensitivity in the detection of some postoperative complications that may be assessed by using this method.

Abstract: La alta prevalencia de los macroadenomas hipofisarios en el mundo motivó la realización de esta investigación, cuyo objetivo fundamental fue demostrar el valor de la tomografía computadorizada en el diagnóstico presuntivo de la variedad histológica de los macroadenomas hipofisarios así como de otros tumores menos frecuentes de la región selar, teniendo en cuenta el cuadro clínico. Para ello se estudiaron 124 pacientes operados con el diagnóstico clínico y tomográfico de macroadenomas hipofisarios y otros tumores de la región selar. Se registró la edad, sexo, clínica, signos tomográficos, resultados anatomopatológicos posquirúrgicos. Se observó mayor incidencia de los adenomas hipofisarios (110), con predominio de los no secretores (41,1 %). El grueso de los pacientes se ubicó en las edades entre 30 y 50 a os. El sexo femenino prevaleció en los adenomas adrenocorticotrópicos y los meningiomas, y el masculino en los productores de gonadotropina y prolactina. Dentro de los signos tomográficos, la erosión de las clinoides y el dorso selar, así como la hidrocefalia predominaron en los adenomas no secretores y los tumores no adenohipofisarios. Los adenomas productores de gonadotropina se destacaron en la erosión del piso y en el balonamiento selar al igual que los adrenocorticotrópicos. Las calcificaciones fueron frecuentes en los teratomas y craneofaringiomas, al igual que la captación no homogénea del contraste, la cual fue característica en estos casos como en los quistes de la bolsa de Rathke. La TC demostró ser de gran valor diagnóstico en los macroadenomas hipofisarios y otros tumores de la región selar teniendo en cuenta el cuadro clínico del paciente. The high prevalence of hypophyseal macroadenomas worlwide motivated us to carry out this research, whose fundamental aim was to demonstrate the value of CT in the presumptive diagnosis of the histological variety of hypophyseal macroadenomas, as well as of other less frequent tumours of the sellar region, considering the clinical picture. To this end, 124 patients operated on with a clinical and tomographic diagnosis of hypophyseal macroadenomas and other tumours were studied. Age, sex, hospital stay, tomographic signs, and postsurgical anatomopathological results were registered. Abstract: Objective: To assess a training approach in Emergency Obstetric and Neonatal Care (EmONC) to strengthen skills of healthcare providers and reduce maternal mortality. Materials and methods: The approach was based on the skills training using the so-called “humanist” method and “life saving skills”. Simulated practice took place in the classroom through thirteen clinical stations summarizing the clinical skills on EmONC. The evaluation was done in all phases and the results were recorded in a database to document the progress of each learner. Results: We trained 432 providers in 10 months. The increase in technical achievements of each participant was documented through a database. The combination of training based on the model “learning by doing” has ensured learning and mastering all EmONC skills particularly postpartum haemorrhage management and reduced missed learning opportunities. Conclusion: The impact of training on postpartum haemorrhage management and maternal mortality is a major challenge in terms of prospects.
