Abstract: Objectives. To introduce the combined South African cohorts of the International epidemiologic Databases to Evaluate AIDS Southern Africa (IeDEA-SA) collaboration as reflecting the South African national antiretroviral treatment (ART) programme; to characterise patients accessing these services; and to describe changes in services and patients from 2003 to 2007. Design and setting. Multi-cohort study of 11 ART programmes in Gauteng, Western Cape, Free State and KwaZulu-Natal. Subjects. Adults and children (<16 years old) who initiated ART with .3 antiretroviral drugs before 2008. Results. Most sites were offering free treatment to adults and children in the public sector, ranging from 264 to 17 835 patients per site. Among 45 383 adults and 6 198 children combined, median age (interquartile range) was 35.0 years (29.8 - 41.4) and 42.5 months (14.7 - 82.5), respectively. Of adults, 68% were female. The median CD4 cell count was 102 cells/ êl (44 - 164) and was lower among males than females (86, 34 - 150 v. 110, 50 - 169, p<0.001). Median CD4% among children was 12% (7 - 17.7). Between 2003 and 2007, enrolment increased 11-fold in adults and 3-fold in children. Median CD4 count at enrolment increased for all adults (67 - 111 cells/ êl, p<0.001) and for those in stage IV (39 - 89 cells/ êl, p<0.001). Among children <5 years, baseline CD4% increased over time (11.5 - 16.0%, p<0.001). Conclusions. IeDEA-SA provides a unique opportunity to report on the national ART programme. The study describes dramatically increased enrolment over time. Late diagnosis and ART initiation, especially of men and children, need attention. Investment in sentinel sites will ensure good individual-level data while freeing most sites to continue with simplified reporting.

Abstract: A model is constructed to study the statistical properties of irregular trajectories of a log-gas whose positions are those of the complex eigenvalues of the unitary Ginibre ensemble. It is shown that statistically the trajectories form a structure that reveals the eigenvalue departure positions. It is also shown that the curvatures of the ensemble of trajectories are Cauchy distributed.
Gousiekte in African buffalo (Syncerus caffer) : clinical communication

Abstract: Three African buffalo (Syncerus caffer) that died after capture and translocation from Mutirikwe Recreational Park in southern Zimbabwe showed macroscopic and microscopic lesions of cardiomyopathy compatible with a diagnosis of gousiekte. The buffalo had had access to Pavetta schumanniana, a plant that is known to cause gousiekte. Death was attributed to cardiac failure as a result of previous consumption of the plant, exacerbated by the stress of translocation.
Performance Evaluation of Waste Stabilisation Ponds  [PDF]

Abstract: Waste stabilisation pond system has been used more especially in developing countries for sewage treatment. The objective of this study was to investigate the hydraulic and performance efficiency of Palapye waste stabilisation ponds. The hydraulic efficiency was evaluated through drogue, pond geometry and sludge accumulation assessment. The performance efficiency was evaluated through periodic sampling and analysis of physiochemical and bacteriological parameters of individual units and of the system as a whole. Except for the maturation ponds, the depth of the anaerobic pond had reduced from 4 m to 0.45 m, for facultative ponds from 2.0 m to a range of 0.52 m - 0.91 m. The design hydraulic retention time of the system had reduced from 20 days to 7.1 days. The concentration of some physiochemical parameters in the effluent was 305 mg·L-1, 277 Nephelometric Turbidity Units (NTU), 204 mg·L-1, 156 mg·L-1, 110 mg·L-1, and 15 mg·L-1 being total suspended solids, turbidity, nitrates, chemical oxygen demand, biochemical oxygen demand and phosphate respectively. These values were more than the standard limits of the country. Effluent total coliforms concentration was 3.6 log units and within the threshold of 4.3 log units, faecal coliforms concentration was 3.5 log units, slightly higher than the threshold of 3 log units. Though Escherichia coli have no limits for discharge into other environments, the concentration in the effluent was reasonable at 2.5 log units and also within irrigation limit of 3 log counts. Palapye wastewater treatment system hydraulic efficiency is lower than the design criterion. The system was overall poor in physiochemical parameters removal but better in bacteriological removal.

Abstract: Advocacy is a crucial component of VISION 2020: The Right To Sight. It has in fact been an important part of global health promotion initiatives for the past twenty years, since the nations of the world came together in Canada for the first International Conference on Health Promotion in 1986. The resulting ‘Ottawa Charter for Health Promotion’ specified three strategies to promote the health of all people of the globe: enabling people to take control of the things which affect their health; mediating between groups who may have different interests, in the pursuit of health for society as a whole; and advocating actions that will have a positive impact on the political, economic, social, cultural, environmental, behavioural, and biological factors which affect health. Advocacy has been emphasised at international conferences on health promotion ever since.
