Abstract: the duodenal diverticular disease is considered within surgery of digestive tracts as unusual. the less frequent complication referred in medical literature is the acute perforation and when it occurs always will be carried out the emergency surgical treatment. it is polemic to arrive to a general consensus directing the diagnostic methodology due to its atypical way of presentation. the abdominal tomography has been very useful in diagnosis of this entity. authors report high mortality rates (25-30 %) in general originated by the diagnostic difficulty in face of this disease. a new case of perforated duodenal diverticulum of a female sex patient aged 60. the surgical technique used in this opportunity was the duodenal exclusion and the retroperitoneal drainage.
Randomised Study to Assess the Efficacy and Safety of Once-Daily Etravirine-Based Regimen as a Switching Strategy in HIV-Infected Patients Receiving a Protease Inhibitor–Containing Regimen. Etraswitch Study

Abstract: Background Etravirine (ETR) was approved for patients with virological failure and antiretroviral resistance mutations. It has also shown antiviral efficacy in antiretroviral-na?ve patients. However, data on the switching from protease inhibitors (PI) to ETR are lacking. Methods HIV-1-infected patients with suppressed viral load (VL) during a PI-containing regimen (>12 months) and no previous virological failure were randomized to switch from the PI to ETR (400 mg/day, dissolved in water) (ETR group, n = 22) or to continue with the same regimen (control group, n = 21). Percentage of patients with VL≤50 copies/mL were assessed at week 48, as well as changes in CD4 T-cell counts and metabolic profile. Results We included 43 patients [72.9% male, 46.3 (42.2; 50.6) years]. Two patients receiving ETR (grade-1 diarrhea and voluntary discontinuation) and another in the control group (simplification) discontinued therapy early. No patients presented virological failure (two consecutive VL>50 copies/mL); treatment was successful in 95.2% of the control group and 90.9% of the ETR group (intention-to-treat analysis, missing = failure) (p = 0.58). CD4+ T-cell counts did not significantly vary [+49 cells/μL in the ETR group (p = 0.25) and ?4 cells/μL in the control group (p = 0.71)]. The ETR group showed significant reductions in cholesterol (p<0.001), triglycerides (p = <0.001), and glycemia (p = 0.03) and higher satisfaction (0–10 scale) (p = 0.04). Trough plasma concentrations of ETR were similar to observed in studies using ETR twice daily. Conclusion Switch from a PI-based regimen to a once-daily combination based on ETR maintained undetectable VL during 48 weeks in virologically suppressed HIV-infected patients while lipid profile and patient satisfaction improved significantly. Acute hepatitis C in Spain: a retrospective study of 131 cases

Abstract: background and aims: the management of acute hepatitis c (ahc) is controversial. we have conducted a retrospective study to determine the epidemiological and biochemical aspects, the genotypes, the spontaneous clearance of hcv (svc), and the treatment responses in patients with ahc. methods: we have retrospectively collected data from 131 patients with ahc from 18 spanish hospitals. results: the mean age was 43 ± 16 years (17-83), 69% were symptomatic. the causes of infection were nosocomial in 40% and intravenous drug users in 20%. eighty two percent had genotype 1. the delay from symptoms-onset to hcv-rna confirmation was 50 ± 68 days (range, 11-350 days) and to treatment (in 59%) 14 ±13 weeks (range, 2-58 days). in the treated group, 80% achieved sustained virological response (svr) versus 57% svc in untreated patients (p = 0.004). up to 96% of those treated within the first 12 weeks had svr versus 86% of those treated later (p = 0.04). patients with hcv-rna(-) at week 4 resolved with or without treatment more frequently than those hcv-rna(+) (98 versus 69%, p = 0.005). the treatment was not beneficial if hcv-rna was undetectable at week 12. no differences in svr were found in genotype 1 patients treated for 24 or 48 weeks. patients with low baseline viral load achieved higher svc and svr. the svc in patients with bilirubin > 5 mg/dl was 78 versus 40% in those with lower values (p = 0.004). conclusions: the most common transmission route was nosocomial. svr was higher in patients treated than svc in non-treated. early treatment (before week 12) achieved the highest response rate. svc and svr were more common in patients with a low baseline viral load. undetectable hcv-rna at week 4 was associated with high svr and svc rates. jaundice was related with svc.

Abstract: the development of the qualitative studies in the information sphere, both in the world and in cuba, specifically in the national system of public libraries, and of those referred to the evaluation of libraries, is dealt with. diverse typologies of yielding indicators useful to evaluate the performance of the public libraries, as well as the statistical control carried out by the public librares in the country are analysed. finally, a proposal of indicators is made to assess the yielding of the cuban public libraries based on the set of statistical variables and controls these libraries have at present.

Abstract: Se analizan las nuevas proyecciones de la organización de la información, a partir de la descripción bibliográfica y sus elementos, en el entorno actual y a la luz del paradigma tecnológico. Se retoman los metadatos, algunas de sus definiciones e intentos de normalización y se comparan estos con los elementos tradicionales de la descripción bibliográfica y los términos de la indización. Se incursiona en la Web semántica, los lenguajes y los formatos, así como por un conjunto de software y servidores clientes disponibles para generar los metadatos. Abstract: Se reflexiona sobre el desarrollo de los estudios cuantitativos e n la esfera informativa, tanto en el mundo como en Cuba, específicamente en el Sistema Nacional de Bibliotecas Públicas y los referidos a la evaluación de bibliotecas. Se incursiona por diversas tipologías de indicadores de rendimiento útiles para evaluar el desempe o de las bibliotecas públicas y por el control estadístico que realizan las bibliotecas públicas en el país. Finalmente, se realiza una propuesta de indicadores para evaluar el rendimiento de las bibliotecas públicas cubanas sobre la base del conjunto de variables y controles estadísticos que dichas bibliotecas poseen en la actualidad.
