Comparison of Rehabilitation Outcomes with Acoustic Radiation Force (ARFI) Elastosonography in Hemiplegic Patients Treated with Neuromuscular Electrical Stimulation  [PDF]

Abstract: To evaluate the gastrocnemius/soleus and biceps brachii muscle stiffness by Acoustic Radiation Force Impulse (ARFI) elastography in the hemiplegia patients, sixty patients with hemiplegia after stroke were recruited. Baseline data were collected including age, gender, body mass index, education level, dominant side, affected side, time since stroke, stroke etiology. All patients were evaluated with before treatment and posttreatment with Broonstroom staging, Modified Ashworth spasticity scale, and Functional Independence Measures scale. The patient was divided into 3 groups: 1) Neuromuscular electrical stimulation group, 2) Rehabilitation group, 3) Neurumusculer electrical stimulation + Rehabilitation group. Deep Neck Infections in Children (Peritonsillar, Retropharyngeal, and Parapharyngeal Abscesses)

Abstract: Deep neck infections are those that start as cellulitis and phlegmon and progress to abscess formation in the spaces surrounded by deep cervical fascia. Deep neck abscesses include peritonsillar, retropharyngeal and parapharyngeal abscesses. They usually occur as a result of upper airway infections in children. Deep neck abscesses are polymicrobial infections caused by aerobic and anaerobic bacteria together. Infections in that region extend rapidly and easily to other regions of neck and can cause life threatening complications by causing injury to the vital structures of neck such as larynx, great vessels and cranial nerves. The main pathology is the acute lymphadenitis followed by suppuration of the regional lymph nodes. The main symptoms in deep neck infections are pain in the neck, limited neck motion, fever, mass or swelling at neck, dysphagia and anorexia. The choice of imaging in diagnosis of deep neck infections is contrast enhanced computed tomography (CT). However, the limitations of CT in differentiation of abscess from cellulitis and phlegmon cause problem. Thus, it is recommended to evaluate the CT findings in association with clinical findings in decision of treatment. It is usually recommended to start with IV antibiotic therapy initially in uncomplicated deep neck abscess cases and to perform surgical drainage if there is no improvement in 24-48 hours. It is important to know the anatomy, causes, clinical findings, diagnosis and treatment of these infections with high morbidity and mortality in children.
Exudative Retinal Detachment Associated with Complicated Retrobulbar Anesthesia  [PDF]

Abstract: Retrobulbar anesthesia (block) is used for many ocular surgeries, whereas it is well known that this procedure has complications such as for retrobulbar hemorrhage, globe perforation, optic nerve injury and brain stem anesthesia. In this report, we present a unique case in the literature of isolated exudative retinal detachment (RD) secondary to iatrogenic retrobulbar hemorrhage. A 73-year-old woman underwent retrobulbar block for combined phaco-vitrectomy. Immediately after the injection, progressive proptosis was recognized. The globe was decompressed and she underwent combined phaco-vitrectomy after stabilization of the eye on the same day. At the be-ginning of the vitrectomy, a dome shaped serous RD was observed in the infero-temporal quadrant. Peripheral exploration was performed, whereas there was no retinal tear or hole. On the first day postoperatively, serous RD was disappeared. In conclusion, this report suggests that increased intraorbital pressure secondary to iatrogenic retrobulbar hemorrhage might lead exudative RD.

Abstract: This research on the immigrants of Turkish origin focused on the immigrants “of Turkish Origin” who immigrated to Turkey from Bulgaria after 1980. The research was launched in 2010 and completed in 2012 with respect to the immigrants of Turkish origin living in the regions which were densely settled or in which the immigrants concerned were placed mostly at the periphery of Izmir (Gorece, Menderes; Sarnic; Yesilova; and Atatürk Neighborhood). The subject of this study is about the social acceptances and rejections between the immigrants of Turkish origin and the Turkish citizens. Although the manuscript does not include the asylum seekers and refugees who have come to Turkey from the eastern and south-eastern borders, who do or do not have a kinship relationship with the peoples living in the country, and who are illegal and within the scope of forced migration, the social perceptions of this matter have also been noted when appropriate.
Polystyrene Attached Pt(IV)–Azomethine, Synthesis and Immobilization of Glucose Oxidase Enzyme

Abstract: Modified polystyrene with Pt(IV)–azomethine (APS–Sch–Pt) was synthesized by means of condensation and demonstrated to be a promising enzyme support by studying the enzymatic properties of glucose oxidase enzyme (GOx) immobilized on it. The characteristics of the immobilized glucose oxidase (APS–Sch–Pt–GOx) enzyme showed two optimum pH values that were pH = 4.0 and pH = 7. The insertion of stable Pt(IV)–azomethine spacers between the polystyrene backbone and the immobilized GOx, (APS–Sch–Pt–GOx), increases the enzymes’ activity and improves their affinity towards the substrate even at pH = 4. The influence of temperature, reusability and storage capacity on the free and immobilized glucose oxidase enzyme was investigated. The storage stability of the immobilized glucose oxidase was shown to be eleven months in dry conditions at +4 °C.
Empiric antibiotic therapy in acute uncomplicated urinary tract infections and fluoroquinolone resistance: a prospective observational study

Abstract: Among the patients which were admitted to outpatient clinics of Ankara University Medical Faculty, Ibni-Sina Hospital during 2005-2006, a total of 429 women between the age of 18 and 65 years old who were clinically diagnosed with uUTI and to whom prescribed empirical antibiotics were enrolled in this prospective observational study. Patients' demographical data, urine culture results, resistance rates to antimicrobial agents and prescribed empiric antimicrobial therapy were analyzed.Totally 390 (90.9%) patients among all study population were requested for urine culture by their physicians. 150 (38.5%) of these urine cultures were positive. The most common isolated uropathogen was Escherichia coli (E. coli) (71.3%). The variations of uropathogens according to age and menopause status were not significantly different.The resistance rates of E. coli isolates for ampicillin, ampicillin-sulbactam, amoxicillin-clavulonate, cefuroxime, ceftriaxone, fluoroquinolones (FQ), co-trimoxazole (TMP-SMX) and gentamicin were 55.1%, 32.7%, 32.7%, 23.4%, 15.9%, 25.2%, 41.1%, 6.1% respectively. FQ were the most common prescribed antibiotics (77.9%) (P < 0.001), followed by TMP-SMX (10.7%), fosfomycin (9.2%), nitrofurantoin (2.1%). Treatment durations were statistically longer than the recommended 3-day course (P < 0.001).Empirical use of FQ in uUTI should be discouraged because of increased antimicrobial resistance rates.Community acquired urinary tract infection (UTI) in women is a prevalent problem in primary care, accounting for approximately eight million ambulatory visits annually in the United States [1]. Abstract: One of the seal or stamp of the oldest traditions of the Turks hit the tradition is seen as a distinctive feature of the Turkish tribes. Abstract: International cooperation in Distance Education which is a very popular phenomenon today can be explained by the rise of social constructivism in social sciences, namely Education and International Relations, for the purpose of this paper.
