Congenital Diaphragmatic Hernia with Delayed Diagnosis: Report of Two Cases  [PDF]

Introduction: Advances in prenatal imaging studies have allowed early diagnosis of Congenital Diaphragmatic Hernia (CDH), although neonatal mortality remains at high levels (60% survival). Despite advances, this study found delayed diagnosis, demonstrating subdiagnosis, possibly caused by lack of resources and/or prenatal diagnostic failure. Case Report 1: Infant, 4 months and 22 days old, dyspneic, cyanotic, with fever and vomiting for 3 days, treated as pneumonia. After the second chest radiography, a right CDH was revealed. Surgical correction was performed and removal of Meckel’s diverticulum was surgical findings. The patient was discharged on the 10th days after admission. Case Report 2: Infant, 11 months and 3 days old, distended abdomen, with fever and dyspnea for 2 days, treated as pneumonia. After the second chest radiography, a left CDH was revealed. The patient was submitted to surgery, intestinal loop incarceration without necrosis was observed, and the correction of CDH was performed. The patient was discharged on the 7th day after admission. Discussion: Three cases, between July 2012 and July 2013, were diagnosed at Dom Malam Hospital; 2 Cases of CDH, with delayed diagnosis, showed incidence of 66.66%. Delayed diagnosis of these patients with initial hypotheses of pneumonia demonstrated high risk, emphasizing the importance of diagnosis research through observation of the clinical course, the correct imaging interpretation, in order to establish early diagnosis and treatment.

Abstract: introduction: brazilian studies on physical activity practice are scarce, especially in postpartum women. objective: to investigate factors associated with the alterations in the physical activity practice in leisure (lpa) in postpartum women. methods: four hundred and seventy-eight women, age range of 15-45 were recruited and followed for nine months postpartum in a prospective study with four follow ups (15 days; 2; 6 and 9 months). the lpa was defined as a dependent variable and measured through a score. the designing of scores was based on the application of a validated questionnaire, which includes five questions concerning physical activity at work and six concerning lpa. the main co-variables investigated were skin color; age; total family income; parity and type. the data were analyzed using models of longitudinal linear regression with mixed effects. results: it was observed that 82.4% of the women practiced some degree of physical activity in postpartum leisure. the highest means of score for lpa at the nine months postpartum were observed among mixed color (2.41), older than 30 years (2.44) and with three children or more (2.44) women. the factors which remained associated with the most practice of lpa in the multivariate model were black and mixed skin color [(white/black b= 0.0925; white/mixed b = 0.1114)}; the oldest age (b = 0.0157); the lowest total family income (b = 0.0001); the highest parity (b = 0.1708) and the kind of birth c-section (b = -0.1058). conclusions: black and mixed, older and with higher parity women present the highest score of lpa in the postpartum period in the studied sample.

Abstract: the purpose of this systematic literature review was to examine publications that had investigated the effect of anxiety on prematurity and/or low birth weight. the pubmed, bvs, cinahl, and healthstar databases, published from 1966 to 2006, were tracked using the following key words: "anxiety", "pregnancy", "low birth weight", and "prematurity". thirteen studies were found: 11 cohorts, 1 cross-sectional, and 1 case-control. most studies (7/13) were conducted in the united states. the most reliable results came from four studies, whose strengths were: exclusion of adolescents and/or women older than 34 years, studies that analyzed anxiety during the second and/or third trimester of pregnancy, used validated scales to measure anxiety, kept loss-to-follow-up rates below 30%, and applied adequate control of confounders. anxiety during pregnancy was associated with prematurity and/or low birth weight in eight studies. odds ratios and relative risks varied from 1.08 to 2.31. carefully designed and well-conducted studies are still needed to clarify the relationship between anxiety during pregnancy, prematurity, and low birth weight considering that the accumulated evidence remains controversial.

Abstract: Fish diet studies contribute to understanding resource partitioning and community trophic structure. The present paper characterizes the diet of gpeixe-cachorro h species Acestrorhynchus britskii and A. lacustris in Sobradinho reservoir, Bahia State, Brazil. Bimonthly samples (November 2006 to July 2008) were taken in the lotic, transition and lentic stretches of the reservoir, using gillnets overnight. A total of 899 A. britskii and 476 A. lacustris stomachs were analyzed, 70% of which were empty. Their content indicated the species are predominantly piscivorous, with 18 prey fish species identified. However, each prey occurred in a single month, except for Anchoviella vaillanti, recorded in 80% of months for A. bristski, andTetragonopterus chalceus in 25% for A. lacustris. The species presented high food overlap (C é = 0.992), owing to the occurrence of Actinopterygii (bits) and Actinopterygii in all months. Regarding prey fish alone, a differentiated intake and lower food overlap was detected, in the flood (C é = 0.427) and drought (C é = 0.172) seasons, in the lotic (C é = 0.434), transition (C é = 0.089) and lentic (C é = 0) stretches, and in overall overlap (C é = 0.236). The spatial and seasonal differences in the intake of different prey fish by both species suggest an opportunistic feeding habit and a competition reduction mechanism for food resources. Estudos sobre a dieta de peixes contribuem para o entendimento dapartilha de recursos e da estrutura trofica da comunidade. O presente trabalho caracteriza a dieta dos peixes-cachorro Acestrorhynchus britskii e A. lacustris no reservatorio de Sobradinho, Estado da Bahia. Coletas bimestrais noturnas (Novembro/2006 a Julho/2008) foram efetuadas nos trechos lotico, transicao e lentico do reservatorio, empregando redes de espera. Foram analisados 899 estomagos de A. britskii e 476 de A. lacustris, aproximadamente 70% dos quais se encontravam vazios. Seu conteudo evidenciou tratar-se de especies predominantemente piscivoras, com 18 peixes-presa identificados. Entretanto, cada presa ocorreu num unico mes, exceto Anchoviella vaillanti, registrada em 80% dos meses para A. bristski e Tetragonopterus chalceus em 25% deles para A. lacustris. As especies apresentaram elevada sobreposicao alimentar (C é = 0,992), pela ocorrencia de Actinopterygii (partes) e Actinopterygii nao-identificado em todos os meses. Entretanto, considerando apenas os peixespresa, foi observado consumo diferenciado e menor sobreposicao alimentar, nos periodos de cheia (C é = 0,427) e seca (C é = 0,172), nos trechos lotico (C é = 0,434
The factors that influence adherence of pregnant women with HIV/AIDS to anti-retroviral therapy

Abstract: Objective: To learn the experiences of pregnant women with HIV/AIDS in relation to adherence to antiretroviral therapy in two public hospitals of reference for HIV/AIDS in Fortaleza-CE, Brazil. Methods: A descriptive study conducted with 24 pregnant women who were in prenatal care and use of antiretroviral therapy. Sociodemographic and obstetric data and information regarding the experience with antiretroviral therapy adherence were collected from July to September 2009, through a semi-structured interview. Results: Womenhad a mean age of 29, low income, low education and a stable partner. It was found that some factors affect pregnant women adherence to antiretroviral therapy. Among these, stand out not accepting the diagnosis and the absence of signs and symptoms of AIDS. However,the fear of transmitting the virus to the baby acted as a stimulus for pregnant women adhere to treatment. Conclusion: The non-acceptance of diagnosis and the absence of signs and symptoms of AIDS negatively affect pregnant women adherence to antiretroviral treatment. On the other hand, the fear that the child be born with the virus and the desire to continue to live are stimuli to adherence.

Abstract: lymphangiomas are a malformation caused by the abnormal migration of lymphatic tissue, leading to failures in the communication and drainage of the lymphatic system. they usually present as groups of translucent papules and vesicles in the skin or mucous membranes. presentation as a verrucous plaque limited to a single toe is unusual and emphasizes the relevance of this case report. although considered a benign lesion, depending on the site affected by the lymphangioma and its size, it may lead to localized pain or recurrent infections, substantially affecting the patient's quality of life. the present case report describes a patient with lymphangioma of late onset developing in adulthood in the form of a verrucous plaque confined to a single toe. diagnosis of this unusual presentation could only be confirmed following histopathology. surgical excision resulted in a satisfactory functional and cosmetic outcome.

Abstract: the objective of this study was to determine carbohydrates and nitrogen compounds fractions of genotypes of elephant grass in a single cut at 56 days of regrowth, in the spring season. twelve elephant grass genotypes were evaluated in a in randomized block design with three replications, in which the experimental plot consisted of four lines with 3 m length, spaced 1 m and fertilized with 100 kg of p2o5, 100 kg of n, 60 kg k2o and 25 kg of micronutrients/ha. carbohydrates fractions a + b1, b2 and c differed among genotypes. the genotypes cnpgl 92-94-01, 92-70-02 cnpgl, cnpgl 91-06-02, cnpgl 91-25-01, 93-32-02 cnpgl and cameroon showed the lowest values of carbohydrates c fraction. among the nitrogen compounds fractions, only b3 and c fractions showed differences. the lowest values of nitrogen compounds c fraction were observed in genotypes cnpgl 92-70-02, napier, cnpgl cnpgl 94-07-02 and 91-25-01.
Evaluation of surface roughness of a nanofill resin composite after simulated brushing and immersion in mouthrinses, alcohol and water

Abstract: this study investigated the alteration of surface roughness of the nanofill composite filtek z350 3m/espe?, caused by simulated brushing associated with the use of mouthrinses with or without alcohol. sixty specimens were prepared and distributed into six groups: distilled water, ethylic alcohol, listerine? vanilla mint, plax? without alcohol, oral b? without alcohol and a control group. each group was submitted to two intercalary 5,000 simulated brushing cycles. at the end of each cycle, the specimens were washed in tap water and immersed for two cycles of six hours equivalent to one year of daily use of the solution for 2 minutes. it was possible to verify significant alteration in surface roughness of the composite influenced by ethylic alcohol. it was not significant for distilled water and the mouthrinses.

Abstract: this work was carried out with the purpose of determining the prevalence of intestinal parasitoses in the school children of bambuí, through parasitological examinations (direct and kato-katz methods) and reevaluating the snails' breeding places described in the county. of the 2,091 school children examined, 20.1% had at least one parasitic infection. giardia lamblia, entamoeba coli, ascaris lumbricoides and hookworm are the most frequent parasites, with a prevalence of 6.2%, 6.2%, 4.8% and 1.4%, respectively. the hookworms were significantly more frequent among students from the rural area and in those aged over 14 years, while the prevalence of e. coli was greater in the urban area and the g. lamblia was more frequent in children under 6 years old. only three children shed eggs of schistosoma mansoni. the only intermediate host found was biomphalaria glabrata and none of them was shedding cercariae of s. mansoni. when these data were compared with data from other surveys previously effected in the county, a decline was observed in the prevalence of all parasites. some hypotheses which may explain this decline are discussed, such as: intense urbanization process and improvement of social-sanitary conditions of the county.

Abstract: lithium is widely used in the therapy of bipolar disorder. its toxicity includes urinary concentration deficit and natriuresis, renal tubular acidosis, tubulointerstitial nephritis which complicates with chronic kidney disease and hypercalcemia. the most common adverse effect is diabetes insipidus, which occurs in 20-40% of patients some weeks after initiation of treatment. such chronic nephropathy correlates with duration of lithium use. early detection of renal dysfunction should be achieved by rigorous monitoring of patients and collaboration between the psychiatrist and nephrologist. recent experimental and clinical studies are now clarifying the mechanisms by which lithium induces renal abnormalities. the aim of this work is to review the pathogenesis, clinical presentation, histopathologic aspects and treatment of lithium nephrotoxicity.
