Abstract: Objectives: To identify and assess the risk factor for transmission of HBV and HCV in pregnantladies and perinatal outcome, presenting to obstetric OPD at CMH Lahore. Design: Case control study. Setting:Combined Military Hospital Lahore. Period: From May 2003 to April 2004. Patients and Methods: All the patientspresenting to obstetrical OPD during one year were randomly screened for both hepatitis B and C using ELISA.Perinatal outcome was compared with control group. The risk factors for HBV and HCV were studied in 30 patients,who were positive for either HBV or HCV. A detailed history was taken followed by a questionnaire and screening forHBV and HCV. The results were compared with control group who were negative for hepatitis B and C but had samesocioeconomic back ground and similar living conditions (wives and soldiers). Results: In the study group 20 patientsgave history of surgical operation or dental procedure. These surgical operations included major or minor operations.In control group positive history of surgical or dental procedure was obtained in 12 patients. The odds ratio was foundto be 3.00 with a P value of 0.038. History of blood transfusion was found in 12 patients in study group and 4 patientsin control group with 0.019. Positive history of jaundice was found in 3 patients of study group and 1 patient in controlgroup with p value of P>0.05. History of drug abuse or multiple sexual partners was negative in both groups.Conclusion: The study concludes that HCV infection is three times more common than HBV infection. Surgicalprocedures are the leading risk factors for acquisition of these infections.

Abstract:

Pilomatrixoma, a rare benign tumour of sebaceous gland is commonly known as calcifying epithelioma of Malherbe or Tricholemmal cyst. We came across even the rarest form of this benign tumour of skin. Our case report was interestingly detected at a higher age group, unusual site of the body, multiple in nature following postoperative incision drainage of abscess in left axilla.

Abstract:

Background: Stapled hemorrhoidectomy, popularly known as Longo technique is in use for the treatment of hemorrhoids since its first description to surgical fraternity in the world congress of endoscopic surgeons in 1998. Objectives: To evaluate the feasibility, patient acceptance, recurrence and results of stapled haemorrhoidectomy in our early experience. Methods: Between Jan 2012 and Dec 2013, 42 patients with symptomatic GRADE III and IV hemorrhoids were operated by stapled hemorrhoidectomy by a single surgeon at our surgery department. The evaluation of this technique was done by assessing the feasibility of the surgery; and recording operative time, postoperative pain, complications, hospital stay, return to work and recurrence. Results: All the procedures were completed successfully. The mean (range) operative time was 30 (20 - 45) min. The blood loss was minimal. Mean (range) length of hospitalization for the entire group was 1 (1 - 3) days. Only 3 patients required more than 1 injection of diclofenac (75 mg) while as rest of the patients were quite happy switching over to oral diclofenac (50 mg) just after a single parenteral dose. All the patients returned to their routine work in less than a week’s time. The mean (range) follow-up was 6 (4 - 12) months. There were no major intraoperative or postoperative complications except for retention of urine in 8 patients. The patients are still on regular follow-up and have not had recurrence as yet. Conclusion: Stapled hemorrhoidectomy technique is a safe alternative to the traditional Milligan-Morgan technique. It can be performed as an office procedure, is well tolerated by patients and is cost effective than conventional surgical therapy.

Abstract:

Remarkable progress has been made during recent years on the development of gravitational wave detectors. The review describes the present status and future prospectus of the gravitational wave astronomy. The main theme is to review the prominent long baseline detectors in operation around the world and proposed baseline and space-borne interferometers. Looking to the future, the major upgrades to the ground based detectors and new planned detectors will be completed over the coming years, which will create a network of detectors with the significantly improved sensitivity required to detect gravitational waves. Beyond this, the concept and design of possible future “third generation” gravitational-wave detectors will be discussed. Sources such as coalescing compact binary systems, neutron stars in low-mass X-ray binaries, stellar collapses and pulsars are all possible candidates for detection.

Abstract: Thrombocytopenia is a common hematological disorder characterized by abnormally low number of platelets in circulating blood from multiple causes. The fundamental mechanisms involved are: ineffective production of platelets by the bone marrow, accerlated destruction of platelets or platelet splenic sequestration. Hair dye, containing Paraphenyllinediamaine has been linked to allergic reaction, nephrotoxicity, mutagenicity and hematological derangements as anemia. A female showed a clinical spectrum of decreasing platelet count along with anemia due to oral ingestion of hair dye. It is important to consider thrombocytopenia in management plan as it may impact the clinical outcome.

Abstract: This paper analyzes telecom consumer relationship by exploring customer switching behavior and classifying communication categories to support companies in planning applicable communication to thwart the consumer switching and improving consumer loyalty. Quantitative research is carried out by surveying 383 telecom consumers who have experienced the switching process. Findings uncover that telecom consumers selected distinct information methods before making switching decision. Active consumers prefer newsletter based on quality of service while passive consumers select newsletter largely based on competitive prices.

Dry Matter Partitioning and Ion Accumulation in Spring Wheat (Triticum aestivum L.) as Affected by Soil Salinity and Growth Regulator (IAA) Spray

Abstract: In a pot experiment with spring wheat cv. Kohistan-97 and Parwaz-94, the effect of foliar application of IAA and different salinity levels 8, 12 and 16 dS m -1 on dry matter partitioning and ion accumulation was studied. With the increase in salt concentration, there was decrease in roots, culms and leaves dry weights. The reduction in dry weights was more in Kohistan-97 than Parwaz-94. The Na+ and Cl+ contents in roots, culms and leaves increased with increasing salinity while K+ content decreased. Spraying with IAA proved effective in ameliorating the adverse effect of salinity on dry matter and ion accumulation, whether applied at the time of salinization or 15-days after salt treatment.
Improving Environment Through Strategic Environmental Assessment

Abstract: This study explains both strategic environmental assessment (SEA) and environmental impact assessment (EIA) as decision-making tools and argues that EIA at the project level does not suffice with maintaining and improving environmental assessment. The emphasis of this study was on regional planning as an example of SEA. The objective was to define SEA based on its strategic characteristics and to distinguish SEA form EIA. The research has attempted to present the argument through examples as to how SEA is a better option for improving environmental quality than EIA when it comes to integrating environmental assessment with regional planning and management. In this study it has suggested that SEA is a better tool in assessing the consequences of policies and plans at an early stage of decision-making as it considers a much "broader picture" of the environment. The study initially provides an overview of EIA and its flaws, discusses SEA and how it gives a better understanding of the environmental issues and finally some examples of integrating regional planning with SEA.
