Surgical Management of Lumbar and Thoracolumbar Spinal Fractures: Indications, Surgical Technique and Evaluation on a Series of 64 Patients Treated with Percutaneous Posterior Osteosynthesis Combined with Kyphoplasty or Anterior Arthrodesis  [PDF]

Abstract:

Introduction: Fractures of the lumbar spine and thoracolumbar junction are common in spinal trauma. The aim of this work is to analyze the nature of the indications, the morbidity as well as the results of these treatment regimens. Patients and Methods: A retrospective, single-center study, based on a review of 64 patients with lumbar spine and thoracolumbar junction fractures (T10-L2) without neurological disorders, was collected in the neurosurgery department of the North Hospital and University Hospital (CHU), Marseille over a period of 2 years from January 2015 to December 2016. Posterior percutaneous osteosynthesis were more or less associated with kyphoplasty preceded anterior arthrodesis. Clinical and radiological endpoints were collected at least 6 months later. Results: The mean follow-up was 9.5 months (6 - 24). The clinical evaluation found a mean VAS at last follow-up at 14/100 (0 - 30) and an average Oswestry score at the last follow-up at 88%. The initial average vertebral kyphosis went from 13° to 4° at the last follow-up with a correction loss of 1°, an absolute gain of 8°. No postoperative neurological complications were noted in our series. Conclusion: The implementation of a two-step therapeutic strategy with anterior reconstruction in Magerl’s lumbar spine or A3.3 thoracolumbar junction fractures allows effective and long-lasting correction of lumbar lordosis and thoracic kyphosis, and obtaining a balanced spine in the sagittal plane. Our functional results are close to normal, with low morbidity and a low complication rate.

Abstract: There are several tensions present in George Lindbeck’s postliberal theology. One of these is between realist intuitions and a non-epistemic account of truth, on the one hand, and a social-constructivist non-realism with regard to theological statements. Theology is relegated to the status of second-order discourse, while first order language comprises the practices, rituals, vocabulary of a religion. I am challenging this intermediary status of religion with the help of Donald Davidson’s critique of the dualism between scheme and content. Since there isn’t much to be made of the notion of epistemic intermediaries (either as ‘content’ or as ‘scheme’) it does not make sense to continue to speak either in terms of realism or of non-realism. This clears the way for treating theology as cognitive.

Abstract: Programmed cell death is a conserved, gene-directed mechanism for the elimination of unnecessary or dangerous cells from an organism. The core cell death pathway was first defined through genetic analysis in the nematode Caenorhabditis elegans and has since been found to be conserved in species as diverse as Drosophila melanogaster and Homo sapiens. The rapid progress of the various genome sequencing projects has greatly accelerated the discovery of homologs for key components of the cell death pathway as well as for its regulators. The C. elegans genome sequence was completed two years ago [1], and both the Drosophila [2] and human genomes are essentially completely sequenced at this point. Although many components of the programmed cell death pathway are conserved between species, there are some important differences as well.Programmed cell death has long been known to be a part of normal development, but it was not until the discovery of the first genes essential for the phenomenon that our understanding of the events leading up to the deliberate elimination of a cell began to take shape. Before this time, programmed cell death was defined by a set of specific morphological characteristics, including chromatin condensation, nuclear shrinkage and blebbing of the plasma membrane that could be observed in dying cells; the term apoptosis was coined to distinguish this type of cell death from necrotic deaths resulting from injury [3,4]. Four genes make up the core programmed cell death pathway in C. elegans, three of which, egl-1 (egg laying abnormal), ced-3 (cell death abnormal), and ced-4, are pro-apoptotic and one, ced-9, is anti-apoptotic (reviewed in [5]). Abstract: uterus is an organ of the gynecological tract, site of many of them benign and malignant affections. the first ones, more frequent, and including the myoma or uterine fibroma. between the third and the fifth decades of life is the time in which this affection is more frequent in the woman. symptoms are variables, as a result of the myoma location (submucous, intramural or subserous). the uterus growth is variable, may go from small tumors until large more evident intra-abdominal masses and to be enough palpable in the mesogastrium. its complications are the reason for the surgical treatment. a significant growth of uterus caused by a myoma may become it in a giant myoma. the case of this patient is the objective to alert to physician facing the suspicion of a intra-abdominal giant tumor.

Entre la tercera y quinta décadas de la vida es el tiempo en el que aparece mayormente esta afección en la mujer. Los síntomas son variables, en consecuencia de la localización del mioma (submucoso, intramural o subseroso). El crecimiento del útero es variable, puede ir desde peque as tumoraciones hasta grandes masas intraabdominales, que se hacen visibles y llegan a palparse en el mesogastrio. Sus complicaciones son las que la hacen acreedoras de tratamiento quirúrgico. Un gran crecimiento del útero a consecuencia de un mioma puede llegar a convertirlo en un mioma gigante. Se presenta esta paciente con el objetivo de alertar al médico que se encuentre ante la sospecha de un tumor gigante intraabdominal.
