Randomized Clinical Trials of Constitutional Acupuncture: A Systematic Review

Abstract: The aim of this systematic review is to compile and critically evaluate the evidence from randomized clinical trials (RCTs) for the effectiveness of acupuncture using constitutional medicine compared to standard acupuncture. Ten databases were searched through to December 2008 without language restrictions. We also hand-searched nine Korean journals of oriental medicine. We included prospective RCTs of any form of acupuncture with or without electrical stimulation. The included trials had to investigate constitutional medicine. There were no restrictions on population characteristics. Forty-one relevant studies were identified, and three RCTs were included. The methodological quality of the trials was variable. One RCT found Sasang constitutional acupuncture to be superior to standard acupuncture in terms of the Unified PD Rating Scale and freezing gate in Parkinson's disease (PD). Another two RCTs reported favorable effects of eight constitutional acupuncture on pain reduction in patients with herniated nucleus pulposi and knee osteoarthritis. Meta-analysis demonstrated positive results for eight constitutional acupuncture compared to standard acupuncture on pain reduction (weighted mean difference: 10 cm VAS, 1.69, 95% CI 0.85–2.54, P < 0.0001; heterogeneity: τ2 = 0.00, Χ2 = 0.00, P = 0.96, I2 = 0%). Our results provide suggestive evidence for the effectiveness of constitutional acupuncture in treating pain conditions compared to standard acupuncture. However, the total number of RCTs and the total sample size included in our analysis were too small to draw definite conclusions. Future RCTs should assess larger patient samples with longer treatment periods and appropriate controls.
A Comparative Study of Concretes Containing Crushed Limestone Sand and Natural Sand  [PDF]

This paper describes the effects of high temperatures on the strength characteristic of crushed limestone sand concrete (CLSC). To compare, natural (river) sand concrete (NSC) and CLSC specimens were exposed to the three different high temperatures. Visual color-change and weight loss were also carefully examined through the tests. The test results indicated that the decreasing rate of compressive strength of CLSC after exposure to high temperature is slightly lower than that of NSC while the splitting tensile strength of CLSC indicated a very similar rate compared to NSC. Therefore, the strength variations of crushed limestone sand concrete after exposal to high temperature can be similarly treated as that of the natural sand concrete. Also it can be seen that the CLSC can use 0.5 power law equation to represent the relationship between compressive and splitting tensile strength before and after exposal to high temperature.

Bacterial Inactivation of Wound Infection in a Human Skin Model by Liquid-Phase Discharge Plasma

Abstract: Background We investigate disinfection of a reconstructed human skin model contaminated with biofilm-formative Staphylococcus aureus employing plasma discharge in liquid. Principal Findings We observed statistically significant 3.83-log10 (p<0.001) and 1.59-log10 (p<0.05) decreases in colony forming units of adherent S. aureus bacteria and 24 h S. aureus biofilm culture with plasma treatment. Plasma treatment was associated with minimal changes in histological morphology and tissue viability determined by means of MTT assay. Spectral analysis of the plasma discharge indicated the presence of highly reactive atomic oxygen radicals (777 nm and 844 nm) and OH bands in the UV region. The contribution of these and other plasma-generated agents and physical conditions to the reduction in bacterial load are discussed. Conclusions These findings demonstrate the potential of liquid plasma treatment as a potential adjunct therapy for chronic wounds.
Influence of Curing-Form Material on the Chloride Penetration of Off-Shore Concrete

Abstract: This paper presents an experimental study on the pore and penetration of chloride in seashore concrete depending on types of curing forms. Three types of concretes (Plain concrete, MSF concrete and FA concrete) with four different form types (wood, coating wood, steel and polypropylene film) were examined. The test results show that the air volume in concrete was relatively higher with steel and polypropylene forms than others, and wood form shows the least air volume. The penetration of chloride depending on type of form is showed a wide variability, that is, the values on plain concrete, MSF concrete and FA concrete are 115.2, 125.5 and 121.6 %, respectively. Based on the present study, concrete should be considered the conditions of curing form-type for durable concrete.
A Secure Routing Method for Detecting False Reports and Wormhole Attacks in Wireless Sensor Networks  [PDF]

Wireless sensor networks (WSNs) consist of a large number of sensor nodes that monitor the environment and a few base stations that collect the sensor readings. Individual sensor nodes are subject to compromised security because they may be deployed in hostile environments and each sensor node communicates wirelessly. An adversary can inject false reports into the networks via compromised nodes. Furthermore, an adversary can create a wormhole by directly linking two compromised nodes or using out-of-band channels. If these two kinds of attacks occur simultaneously in a network, existing methods cannot defend against them adequately. We thus propose a secure routing method for detecting false report injections and wormhole attacks in wireless sensor networks. The proposed method uses ACK messages for detecting wormholes and is based on a statistical en-route filtering (SEF) scheme for detecting false reports. Simulation results show that the proposed method reduces energy consumption by up to 20% and provide greater network security.

Experience of limited pancreatic head resection for management of branch duct intraductal papillary mucinous neoplasm in a single center

Abstract: AIM: To share our surgical experience and the outcome of limited pancreatic head resection for the management of branch duct intraductal papillary mucinous neoplasm (IPMN).METHODS: Between May 2005 and February 2008, nine limited pancreatic head resections (LPHR) were performed for IPMN of the pancreatic head. We reviewed the nine patients, retrospectively.RESULTS: Tumor was located in the uncinate process of the pancreas in all nine patients. Three patients had stents inserted in the main pancreatic duct due to injury. The mean size of tumor was 28.4 mm. Postoperative complications were found in five patients: 3 pancreatic leakages, a pancreatitis, and a duodenal stricture. Pancreatic leakages were improved by external drainage. No perioperative mortality was observed and all patients are recorded alive during the mean follow-up period of 17.2 mo.CONCLUSION: In selected patients after careful evaluation, LPHR can be used for the treatment of branch duct type IPMN. In order to avoid pancreatic ductal injury, pre- and intra-operative definite localization and careful operative techniques are required.
Structures and lower bounds for binary covering arrays

Abstract: A $q$-ary $t$-covering array is an $m \times n$ matrix with entries from $\{0, 1, ..., q-1\}$ with the property that for any $t$ column positions, all $q^t$ possible vectors of length $t$ occur at least once. One wishes to minimize $m$ for given $t$ and $n$, or maximize $n$ for given $t$ and $m$. For $t = 2$ and $q = 2$, it is completely solved by R\'enyi, Katona, and Kleitman and Spencer. They also show that maximal binary 2-covering arrays are uniquely determined. Roux found the lower bound of $m$ for a general $t, n$, and $q$. In this article, we show that $m \times n$ binary 2-covering arrays under some constraints on $m$ and $n$ come from the maximal covering arrays. We also improve the lower bound of Roux for $t = 3$ and $q = 2$, and show that some binary 3 or 4-covering arrays are uniquely determined.
Using Guasha to treat musculoskeletal pain: A systematic review of controlled clinical trials

Abstract: We searched 11 databases (without language restrictions): MEDLINE, Allied and Complementary Medicine (AMED), EMBASE, Cumulative Index to Nursing and Allied Health Literature (CINAHL), Korean Studies Information (KSI), DBPIA, Korea Institute of Science and Technology Information (KISTI), KoreaMed, Research Information Service System (RISS), China National Knowledge Infrastructure (CNKI) and the Cochrane Library. The search strategy was Guasha (OR scraping) AND pain. Risk of bias was assessed with the Cochrane criteria (i.e. sequence generation, blinding, incomplete outcome measures and allocation concealment).Five randomized controlled trials (RCTs) and two controlled clinical trials (CCTs) were included in the present study. Two RCTs compared Guasha with acupuncture in terms of effectiveness, while the other trials compared Guasha with no treatment (1 trial), acupuncture (4 trials), herbal injection (1 trial) and massage or electric current therapy (1 trial). While two RCTs suggested favorable effects of Guasha on pain reduction and response rate, the quality of these RCTs was poor. One CCT reported beneficial effects of Guasha on musculoskeletal pain but had low methodological quality.Current evidence is insufficient to show that Guasha is effective in pain management. Further RCTs are warranted and methodological quality should be improved.Guasha was defined as a therapeutic modality that uses several tools to scrape or rub the surface of the body to relieve blood (Xue) stagnation. Guasha is used for pain relief in Chinese medicine. Tools for Guasha including a Chinese soup spoon, an edge-worn coin, a slice of water-buffalo horn, a cow rib, honed jade and a simple metal cap with a smooth round lip with oil or water are used in Guasha to scrape or rub the skin to relieve blood stagnation at the body surface [1]. Ginseng for Health Care: A Systematic Review of Randomized Controlled Trials in Korean Literature

Abstract: Objective This systematic review was performed to summarise randomised clinical trials (RCTs) assessing the efficacy and safety of ginseng in the Korean literature. Method The study involved systematic searches conducted in eight Korean Medical databases. The methodological quality of all of the included studies was assessed using the Cochrane Risk of Bias tool. We included all RCTs on any type of ginseng compared to placebo, active treatment or no treatment in healthy individuals or patients regardless of conditions. Results In total, 1415 potentially relevant studies were identified, and 30 randomised clinical trials were included. Nine RCTs assessed the effects of ginseng on exercise capacity, cognitive performance, somatic symptoms, quality of life, and sleeping in healthy persons. Six RCTs tested ginseng compared with placebo for erectile dysfunction, while another four studies evaluated the effects of ginseng against no treatment for gastric and colon cancer. Two RCTs compared the effect of red ginseng on diabetes mellitus with no treatment or placebo, and the other nine RCTs assessed the effects of ginseng compared with placebo or no treatment on various conditions. The methodological caveats of the included trials make their contribution to the current clinical evidence of ginseng somewhat limited. However, the 20 newly added trials (66.7% of the 30 trials) may provide useful information for future trials. Ginseng appears to be generally safe, and no serious adverse effects have been reported. Conclusions The clinical effects of ginseng have been tested in a wide range of conditions in Korea. Although the quality of RCTs published in the Korean literature was generally poor, this review is useful for researchers to access studies that were originally published in languages that they would otherwise be unable to read and due to the paucity of evidence on this subject.
Analysis of liver metastasis after resection for pancreatic ductal adenocarcinoma

Abstract: AIM: To investigate the risk factors affecting the liver metastasis (LM) of pancreatic ductal adenocarcinoma (PDAC) after resection. METHODS: We retrospectively analyzed 101 PDAC patients who underwent surgical resection at the Samsung Medical Center between January 2000 and December 2004. Forty one patients with LM were analyzed for the time of metastasis, prognostic factors affecting LM, and survival. RESULTS: LM was found in 40.6%. The median time of the LM (n = 41) was 6.0 ± 4.6 mo and most LM occurred within 1 year. In univariate analysis, tumor size, preoperative carbohydrate antigen 19-9, and perineural invasion were factors affecting LM after resection. In multivariate analysis, tumor size was the most important factor for LM. In univariate analysis, tumor cell differentiation was significant to LM in low-risk groups. CONCLUSION: LM after resection of PDAC occurs early and shows poor survival. Tumor size is the key indicator for LM after resection.
