Youth friendly health services and role of outreach activities to improve access to services  [PDF]

An attempt has been made to assess the Youth Friendly Health Services (YFHS) from the clients’ perspectives and the role of outreach community-based approach (Youth Information Center, YIC) in improving access to services. A cross- sectional study was undertaken in Arajiline and Hoskote blocks in Varanasi and Bangalore districts respectively using primarily a quantitative technique. A consecutive sample of 120 clients from 4 selected clinics was interviewed after seeking services from YFHS. Additionally, facility assessment of clinics and 8 FGDs were conducted among community members and the young people in the community. Majority of the clients (90.8%) are aware that YFHS provide services to young males and females separately on specific day and time. Nearly 66% clients visited YFHS to sought treatment for three key health problems i.e. menstruation problems (25.4%) followed by general illness (22.8%) and swelling/itching of private parts (21.1%). In Hos- akote privacy was maintained during the con- sultation with the doctor, however, situation was relatively not better in Arajiline. Out of those clients who had visited YIC atleast once, 49 out of 53 in Arajiline and 51 out of 60 in Hasokote reported that the YIC staff/activities had motivated them to sought services at YFHS. Study concludes that while majority of the clients were satisfied with the services at YFHS, there is a need for strengthening the existing “package” of the services. Abstract: Background and aims: Acute coronary syndrome (ACS), which comprises acute ST-segment elevation myocardial infarction, non-ST segment elevation myocardial infarction and unstable angina is a major health problem and represents a large number of hospitalizations annually worldwide. We aim to describe pattern of the ACS admission and in-hospital mortality at tertiary national heart centre of the country. Methods: A hospital database was used to analyze all 7424 patients admitted in coronary care unit of the centre for ACS from September 2001 till December 2012. We evaluated trend of ACS admission and in-hospital mortality. Results: Five thousand three hundred ninety one (72.6%) were male and two thousand thirty three (27.4%) were female. Patient of 21years to 98 years were admitted for ACS .Four thousand five hundred and ninety nine(61.9%) patient were admitted due to ST elevation myocardial infarction, whereas one thousand nine hundred and thirteen (25.8%) were admitted for Unstable angina and nine hundred twelve (12.3%) were admitted for Non ST elevation myocardial infarction. In-hospital mortality was 5.74% for acute coronary syndrome. There was significant difference in in-hospital mortality between ST elevation myocardial infarction (7.76%), Non ST segment elevation acute coronary syndrome (3.61%) and Unstable Angina (1.88%).There is a gradual increase in Primary Percutaneous Coronary intervention as a mode of reperfusion therapy whereas there is a decrease in the rate of thrombolysis. Conclusion: Our study provides us some important information about the trend and in-hospital mortality rate in national heart centre. Abstract: This research is about the study of the Saving and Credit Co-operative Societies (SACCOS) employees’ personality traits, self-efficacy, and organizational commitment of Kathmandu district. The researchers have used Mini-IPIP five-factor model personality. The scale consists of 20-items short form of the 50-items IPIP-FFM. The Big Five personality traits have measured employee’s agreeableness, conscientiousness, emotional stability, extraversion and intellect. The Organizational commitment scale is used to measure the employee’s affective, normative and continuance commitment. The employees are human capital and without their contribution, no one can imagine of this present digital world. The Self-efficacy scale is used to find the optimistic beliefs of employees’ competence to deal efficiently with a variety of stressful situations. The total sample size consists of 260 employees (Male = 127 & Female = 133). The samples are drawn from Sixty Three SACCOS of Kathmandu district only. This study finds the relationships among the SACCOS employees’ personality traits, commitment and self-efficacy to handle the employees’ behavior efficiently for the overall productivity of an organization.

Considering the representations configured in the idea of a Thucydidean Clio in the end of the sixteenth century, the article proposes to think about the association between the ancient concept of the hero and the modern concept of the genius as a epistemological criterium that catalizes the appreciation of the excellence of historiographical praxis. Bayesian Estimation of Shrubs Diversity in Rangelands under Two Management Systems in Northern Syria  [PDF]

The diversity of shrubs in rangelands of northern Syria is affected by the grazing management systems restricted by the increase in human and livestock populations. To describe and estimate diversity and compare the rangeland grazing management treatments, two popular indices for diversity, the Shannon index and the Simpson index, were studied for the four combinations of two sites, Hammam and Obeisan, and two grazing methods, Closed and Open, using frequentist and Bayesian approaches. We simulated the a priori and a-posteriori distributions of the Shannon and Simpson diversity indices, where from a range of values for a constant in the a priori distribution the best value normalizing the distribution of the diversity indices was chosen. The Bayesian diversity estimates were higher than their frequentist counterparts and had lower standard errors. The grazing methods at each site and sites under each grazing method delivered significant diversity of shrub species. The Bayesian estimates resulted in lower p-values than the frequentist approach for two cases reflecting in Bayesian methods higher power. Bayesian approach is recommended as it has a wider framework for inference on diversity studies.

Technical efficiency in milk production in underdeveloped production environment of India*

Abstract: The study was undertaken in Kumaon division of Uttarakhand state of India with the objective of estimating technical efficiency in milk production across different herd-size category households and factors influencing it. Total of 60 farm households having representation from different herd-size categories drawn from six randomly selected villages of plain and hilly regions of the division constituted the ultimate sampling units of the study. Stochastic frontier production function analysis was used to estimate the technical efficiency in milk production. Multivariate regression equations were fitted taking technical efficiency index as the regressand to identify the factors significantly influencing technical efficiency in milk production. The study revealed that variation in output across farms in the study area was due to difference in their technical efficiency levels. However, it was interesting to note that smallholder producers were more technically efficient in milk production than their larger counterparts, especially in the plains. Apart from herd size, intensity of market participation had significant and positive impact on technical efficiency in the plains. This provides definite indication that increasing the level of commercialization of dairy farms would have beneficial impact on their production efficiency.
