Abstract:

Background: Non-Hodgkin’s lymphoma is an aggressive malignant disease in children and adolescents. Although it is the fourth most common malignancy in Saudi children as reported in Saudi cancer registry, less information is available about pediatric Non-Hodgkin lymphoma and its outcome in Saudi Arabia. Study Objectives: To provide demographic data, disease characteristics, treatment protocol, toxicity and outcome of treatment in children & adolescents with Non-Hodgkin’s lymphoma treated at KFMC. This study will form base line for future studies about pediatric Non-Hodgkin’s lymphoma in KFMC, which may help to improve outcome for children with cancer in Saudi Arabia. Study Patients and Method: We retrospectively analyzed 28 children and adolescents diagnosed to have Non-Hodgkin’s lymphoma at KFMC between December 2006 and December 2013, followed-up through June 2014. Results: Of the 28 patients, 10 (35.7%) girls and 18 (64.3%) boys, the male-to-female ratio was 1.8; 1. The median age at time of diagnosis was 6.4 years old (range 2.0 to 13.0 years old). The majority of patients (64.3%) were aged between 5 and 12 years old. Burkitt’s lymphoma BL/BLL was the most common pathological subtype (60.7%), and DLBCL was the second most common subtype (21.4%). Abdominal and Retroperitoneal involvement was the most common primary site (78.6%) including the ileocaecal region. Most of the children presented with advanced Stage III and IV (75%), Cytogenetic study which screens specifically for the t (8; 14) (q24; q32) a characteristic genetic feature of Burkitt’s Lymphoma was obtained from 21 patients, variant rearrangement was observed in 3/21 samples and complex chromosomes karyotype in addition to IGH/MYC rearrangement was observed in 2/21 samples. Those patients presented with very aggressive lymphoma and combined BM and CNS involvement. We use the French-American-British Mature B-Cell Lymphoma 96 Protocol (FAB LMB 96) for treatment fornewly diagnosed Mature B-Cell type NHL and high risk ALL CCG 1961 Protocol for lymphoblastic lymphoma and international Anaplastic Large Cell Lymphoma 99 Study Protocol for ALCL. The median follow-up in patients not experiencing an adverse event was 53.1 months. Abstract:

Aspirin and clopidogrel are the commonest dual antiplatelet agents being used in the secondary prevention of cardiovascular disease. In high risk patients with coronary heart disease, the use of aspirin was associated with a significant risk reduction of myocardial infarction, stroke and vascular death. The use of clopidogrel alone was slightly superior to aspirin, and associated with reduced risk of vascular death, ischemic stroke and myocardial infarction. Dual antiplatelet therapy has been well studied in patients with acute coronary syndrome and those undergoing percutaneous coronary intervention (PCI). In patients with stable coronary heart disease or multiple risk factors the combination of clopidogrel plus aspirin was not significantly more effective than aspirin alone in reducing MI, stroke or death from cardiovascular causes.

Abstract: Thiomorpholine (tetrahydro-1,4-thiazine), in the presence of Os(VIII) as catalyst, is oxidized by alkaline hexacyanoferrate(III) to 2-hydroxythiomorpholine. There is a first-order dependence in [Fe(CN)63-] and [Os(VIII)] and zero-order dependence in [thiomorpholine] and [OH-]. The observed rate constant was dependent on [Fe(CN)64-] the type of electrolyte and the permitivitty of the reaction medium (10-40% ethanol). The proposed mechanism includes the formation of a transient [OsO4(OH)2 ]2--thiomorpholine complex prior to the rate-determining regeneration of OsVIII by [Fe(CN)63-]. The values of the enthalpy and entropy of activation are calculated from rate constant values measured at different temperatures (20-40 ° C).

Abstract: Objective: The association between type 2 diabetes mellitus and certain endocrine disorders has the paucity of information and limited data in the province of Meisan (southern of Iraq). Hence, in the present study, an attempt was made to investigate the levels of plasma HbA1c, serum insulin resistance and levels of serum sex hormones in men and women of control and type 2 diabetic patients. Also, the present study aimed to explored the correlation coefficient (r) between insulin resistance and level of HbA1c, FSH, LH, Tt, progesterone and estradiol in men and women patients with NIDDM. Methods: One hundred Iraqi volunteers with abdominal obesity were selected in the present study. Fifty diabetic patients with NIDDM (26 men, and 24 women) aged between (37) and (65) years and 50 healthy individuals (25 men, and 25 women) aged between (36) and (65) years considered as control group. Complete medical history was taken from each subject and complete clinical examinations were performed for all volunteers. Then, the two groups were matched for their fasting blood sugar, BMI, insulin hormone; c-peptide, HbA1c, sex hormones and insulin resistance parameters (HOMA2-IR, HOMA%B and HOMA%S) were calculated using HOMA2 calculator software. Results: We found that insulin resistance demonstrated negative correlations (p < 0.05) with levels of serum follicle stimulating hormone (FSH), luteinizing hormone (LH), total testosterone, progesterone and estradiol. While, there was a positive and significant correlation (p < 0.01) with value of HbA1c in men and women NIDDM patients. Conclusion: We conclude that an increase in the level of insulin resistance is associated with increased level of blood HbA1c and decreased in level of serum sex hormones (FSH, LH, Tt, E2 and progesterone) in men and women of diabetic group. Abstract: The paper investigates the temporal variations of GPS irregularities at quasi-conjugate points in the polar region during the October-Novemebrr 2003 Halloween storm. The pseudorange and carrier phase observables obtained from dual frequency GPS receivers of Trimble 5700 at Scott Base Station in Antarctica and Ashtech Z-X113 at Resolute Bay in Nunavut, Arctic are employed in determination the ionospheric total electron content (TEC) and scintillation parameters. The comparison of TEC and scintillation parameters at both stations show: dominant occurrence of positive storm phases over the nightside hemisphere, dominant occurrence of negative storm phases over the dayside hemisphere ,the periods of pronounced scintillation activity at both hemispheres were coincident with the TEC enhancement periods, the weak scintillation periods are coincident with the TEC depletion periods. The strong TEC enhancement during the sudden storm commencement of the geomagnetic storm was only seen over the nightside station, and finally obvious asymmetry in the ionospheric TEC and scintillation activity between summer and winter hemispheres was observed. The results over the polar were in good agreement with previous measurements made by other researchers over the subauraral and midlatitude regions.
Potentiality of Secondary Aquifers in Saudi Arabia: Evaluation of Groundwater Quality in Jubaila Limestone  [PDF]

Abstract: Groundwater scarcity in arid regions may hinder development plans and cause many inconveniences for the population and authorities. Saudi Arabia has limited groundwater resources stored in the sedimentary sequence of the Arabian Shelf. Some of these resources were classified as major aquifers, secondary and minor aquifers, and some were considered as aquicludes. The Jubaila Limestone is one of the secondary aquifers of Saudi Arabia. The main purpose of this paper is to evaluate the groundwater resources of the Jubaila Limestone in Riyadh area, with emphasis on groundwater quality. Groundwater was found to occur in fractures and within solution openings of the Jubaila Limestone at depths which range between 19 and 210 m. The transmissivity value was 1.7 × 10–3 to 7.2 × 10–3 m2/s; the storage coefficient was of 1.3 × 10–4. The electrical conductivity for collected water samples ranged between 831 and 7670 μS/cm. The major ionic relationships were Na > Ca > Mg and SO4Cl > HCO3. The groundwater evolves from NaCl dominated at the southern end of the study area, into Ca, MgSO4 water in the north. The main chemical process responsible of this variation was found to be dissolution of anhydrite and gypsum. The groundwater was not found suitable for drinking purposes but can be used by livestock and for some agricultural purposes.

Abstract: The temperature variability was studied using linear regression models, Man-Kendall (M-K) rank statistics, mean monthly and annual temperature anomalies, number of hot and cold days and nights per annum. The temperature time series (mean, minimum and maximum) has shown a warming trend of the local air during last two decades. The maximum and minimum of the monthly mean temperatures of 23.35°C and 13.25°C were observed in June and January, respectively. Similarly, the maximum and minimum temperature ranges of 13.4°C and 8.2°C were found in December and June. The ratios between the hottest and coldest month’s monthly mean maximum and monthly mean minimum temperatures were 1.039 and 1.033, respectively. The best fit linear line showed an increase of 0.048°C per annum in the annual mean temperature with a coefficient of determination of 52%. An overall increase of 1.01°C was found between 1985. The Mann-Kendall rank statistics test confirmed the warming trends of the local atmospheric environment of Abha city.

Abstract: Malate dehydrogenase, α-esterase and β-esterase isoenzymes have been analyzed by native polyacryla-mide gel electrophoresis in different tissues (liver, kidney, muscle and hump) of the Arabian Camel, Camelus dromedaries in order to study the tissue specificity of these isoenzymes. Malate dehy-drogenase recorded three fractions while both este-rases recorded two fractions in different studied tissues. Malate dehydrogenase was considered as a key isoenzyme in discriminating among the different studied tissues. The percentage amount of both Mdh-1 and Mdh-2 showed significant difference among the different tissues while Mdh-3 recorded significant variation between muscle and hump tissues. The significant variation in the amount of Mdh isoenzyme (on the level of either total or fractions) among the different studied tissues may reflect the role of this enzyme for energy production in this desert animal. With respect to esterases, the percentage amount for both fractions and the total enzyme did not show significant difference among different tissues except β-Est-2 which was significantly higher in hump than in muscle. This enzyme showed higher thickness and intensity in both liver and kidney than in the muscle and hump indicating its greater activity in both liver and kidney.

Abstract: Native polyacrylamide gel electrophoresis have been applied for the analyses of alkaline phosphatase (ALP), malate dehydrogenase (Mdh) and malic (ME) isoenzymes in Arabian camel for racing and production. Two fractions for each of these isoenzymes have been recorded in the studied breeds. ALP showed very weak patterns without remarkable difference between the two breeds and this is an indication to that the samples used were healthy and being from the same age. The cytosolic Mdh-1 and ME-1 have been recorded in both camel breeds with high intensity. The mitochondrial Mdh-2 and ME-2 have been recorded with small intensity in production breeds commonly. The present data indicate the necessity of the mitochondrialMdh-2 for energy production in racing breed and the responsibility of the cytosolic Mdh-1 for lipogenesis and energy production in both breeds. We therefore may assume that the appearance of both Mdh forms is necessary for both energy and lipid production in the production breeds while Mdh-1 was useful as bioenergetic enzyme necessary for racing. The different expressions are indications of the difference in the physiological adaptations of both camel breeds and are not for a systematic value.
