Pelvic floor exercises with biofeedback for stress urinary incontinence

Abstract: objective: prospective study to objectively evaluate the benefits of pelvic floor strengthening exercises associated to biofeedback for the treatment of stress urinary incontinence. materials and methods: fourteen patients diagnosed with stress urinary incontinence (sui) were selected for this study. all patients underwent a pelvic floor training associated to biofeedback for 12 consecutive weeks. urodynamic tests, pad test and bladder diary were analyzed at the beginning of the study, at the end and after 3 months. the king's health questionnaire (khq) was applied before and after treatment to assess the impact in the quality of life. results: there was a significant reduction in the pad weight (from 14.21 g to 1 g), number of urinary leakage episodes (from 8.14 per day to 2.57 per day) and daytime frequency (from 7.93 per day to 5.85 per day). at urodynamics the authors observed a significant increase in valsalva leak-point pressure (from 103.93 cm h2o to 139.14 cm h2o), cistometric capacity (from 249.29 ml to 336.43 ml, p = 0.0015) and bladder volume at first desire to void (from 145 ml to 215.71 ml). those differences were kept during the first 3 months of follow up. the khq revealed significant differences except in the case of "general health perception", which covers health in general and not exclusively urinary incontinence. conclusion: treatment of sui with pelvic floor exercises associated to biofeedback caused significant changes in the parameters analyzed, with maintenance of good results 3 months after treatment.

Abstract: OBJETIVO: Traduzir e validar para a língua portuguesa o questionário de qualidade de vida condi o-específico denominado International Consultation on Incontinence Questionnaire - Short Form (ICIQ-SF) em pacientes com incontinência urinária. MéTODOS: Duas tradu es independentes do ICIQ-SF foram feitas por brasileiros, fluentes na língua inglesa. Após harmoniza o das mesmas, a tradu o resultante foi retrotraduzida independentemente por dois nativos de países de língua inglesa. As diferen as foram harmonizadas e pré-testadas em um estudo piloto. A vers o final do ICIQ-SF para o português, bem como a vers o em português do King's Health Questionnaire (KHQ) foram aplicadas simultaneamente em 123 pacientes consecutivos com queixa de incontinência urinária (29 homens e 94 mulheres) que procuraram o laboratório de uroginecologia e o servi o de urodinamica de um hospital universitário, localizado em Campinas. Foram testadas as propriedades psicométricas do questionário, como confiabilidade e validade de constructo. RESULTADOS: A idade mediana foi de 53 anos (intervalo de 16 a 86 anos). O período médio de reteste para o ICIQ-SF foi de 14,37 dias (intervalo de seis a 41 dias). Nenhuma altera o do formato original do ICIQ-SF foi observada no final do processo de tradu o e adapta o cultural. A consistência interna foi alta, como demonstrado pelo coeficiente alfa de Cronbach (0,88). O resultado do teste-reteste foi considerado de moderado a forte, como indicado pelo índice Kappa ponderado, cujos valores variaram de 0,72 a 0,75, e o coeficiente de correla o de Pearson que foi de 0,89. A correla o entre o ICIQ-SF e o KHQ foi considerada de moderada a boa para a maioria dos itens, variando de 0,44 a 0,77. A avalia o das validades de constructo e concorrente foi também satisfatória e estatisticamente significante. CONCLUS ES: A vers o para o português do ICIQ-SF foi traduzida e validada com sucesso para aplica o em pacientes brasileiros de ambos os sexos, com queixa de incontinência urinária, apresentando satisfatória confiabilidade e validade de constructo.
Responsiveness to the Portuguese version of the international consultation on incontinence questionnaire - short form (ICIQ-SF) after stress urinary incontinence surgery

Abstract: objective: to evaluate the reliability and responsiveness (internal and external) of the portuguese version of the iciq-sf. we assessed the responsiveness of the iciq-sf after surgical procedures for the treatment of stress urinary incontinence. materials and methods: prospective open label study in 2 tertiary referral centers. sixty-one patients of both genders (54 female and 7 male) were enrolled. patients were treated using surgical procedures, mostly with synthetic sling (82%). patients were assessed before surgery and at least 1 month postoperatively using the iciq-sf in its translated and validated portuguese version. patients also underwent pre-operative urodynamic tests, stamey incontinence grading and pad usage assessments. after surgery, patients underwent stress tests, stamey incontinence grading and pad usage assessments. results: the mean age was 57.2 (± 11.6) years and the mean duration of follow-up was 7.2 months (± 4.5). objective parameters such as urodynamic tests (by means of vlpp) and pad usage had significant correlation with changes in post-treatment scores on the iciq-sf (p = 0.0062 and p < 0.0001 respectively). the responsiveness expressed in terms of standardized effect sizes (ses) and standardized response means (srm) was large for both questionnaires (p < 0.0001). conclusion: the results showed high responsiveness (large effect sizes i and ii) for the portuguese version of the iciq-sf, indicating that this instrument is suitable for measuring outcomes in clinical trials for brazilian patients with stress urinary incontinence.

Abstract: objective: to translate into and validate for portuguese the "international consultation on incontinence questionnaire - short form" (iciq-sf), a condition-specific quality-of-life questionnaire for patients with urinary incontinence. methods: two brazilians independently translated the original iciq-sf into portuguese. these two translations were harmonized, and then checked by independent back-translation by two native english speakers. the harmonized translation was pre-tested in a pilot study on 20 patients. the final version of the iciq-sf in portuguese was applied to 123 consecutive patients aged 16 or over (29 males and 94 females) with a complaint of urinary incontinence, who had sought the department of urogynecology and uroneurology of the school of medical sciences of unicamp. the portuguese version of the king's health questionnaire (khq) was also applied to the same group. the psychometric properties of the questionnaire, such as reliability and construct validity were assessed. results: the median age was 53 years (range: 16 to 86). the mean retest interval for the iciq-sf was 14.37 days (range: 6 to 41). no changes from the original format of the iciq-sf were observed at the end of the process of translation and cultural adaptation. the internal consistency was high (0.88), as measured by the cronbach alpha coefficient. the test-retest value was considered moderate to strong, as measured by the weighted kappa index (range: 0.72 to 0.75) and pearson correlation coefficient (0.89). the correlation between the iciq-sf and khq was considered to be moderate to good for most items (range: 0.44 to 0.77). the evaluation of the construct and concurrent validity was also satisfactory and statistically significant. conclusions: the "international consultation on incontinence questionnaire" (iciq-sf) was successfully translated into portuguese and validated for application to brazilian female and male patients complaining of urinary incontinence, with satisfactor

Abstract: this article concerns some of the multiple ethical-bioethical and gender issues in the field of new reproductive and contraceptive technologies. the literature presented points to the plurality of possible situations and approaches in a multidimensional and controversial field. it presents some ethical-bioethical principals of biomedical action found in the study of heterosexual couples who use assisted reproduction. and of medical specialists in human reproduction in southern brazil. it presents the ethical-bioethical presumptions that sanction medical behavior and the continuity of the so-called impregnation treatments, and analyzes the mechanisms used to raise the expectations of couples who lack confidence or success.

Variational approach to gravitational theories with two independent connections

Abstract: A new variational approach for general relativity and modified theories of gravity is presented. In addition to the metric tensor, two independent affine connections enter the action as dynamical variables. In the matter action the dependence upon one of the connections is left completely unspecified. When the variation is applied to the Einstein-Hilbert action the Einstein field equations are recovered. However when applied to $f(R)$ and Scalar-Tensor theories, it yields gravitational field equations which differ from their equivalents obtained with a metric or Palatini variation and reduce to the former ones only when no connections appear in the matter action.
Dynamics of cosmological scalar fields

Abstract: The background dynamical evolution of a universe filled with matter and a cosmological scalar field is analyzed employing dynamical system techniques. After the phenomenology of a canonical scalar field with exponential potential is revised, square and square root kinetic corrections to the scalar field canonical Lagrangian are considered and the resulting dynamics at cosmological distances is obtained and studied. These noncanonical cosmological models imply new interesting phenomenology including early time matter dominated solutions, cosmological scaling solutions and late time phantom dominated solutions with dynamical crossing of the phantom barrier. Stability and viability issues for these scalar fields are presented and discussed.

Abstract: A recently proposed variational approach for general relativity where, in addition to the metric tensor, two independent affine connections enter the action as dynamical variables, is revised. Field equations always reduce to the Einstein field equations for any dependence of the matter action upon an independent connection.

Abstract: a conceptual arrangement which describes well-succeeded aging in the lifespan perspective is the model of strategies called selection, optimization and compensation. such strategies evoke ways of attaining goals in life and promote personal growing. compensating and optimizing attitudes through learning processes is the focus of this study, whose purpose is to analyze the athletes' learning processes in years of running practice through a qualitative research of male subjects from different clubs and sporting societies in brazil. data collection was made through half-structured interviews and through sociodemographic questionnaires, after a screening involving 83 athletes. the group of 11 athletes interviewed ( = 59.91; dp = ±10.58 age) has shown different kinds of learning processes which provided personal and sporting development, and helped them to cope with daily activities. moreover, in order to develop these learning processes they used many kinds of spaces and people, what showed their self-government in terms of learning and their search for solving problems.
