Abstract: background and objectives: cardiac output and preload as absolute data do not offer helpful information about the hemodynamic of critically ill patients. however, monitoring the response of these variables to volume challenge or inotropic drugs is a very useful tool in the critical care setting, particularly for patients with signs of tissue hypoperfusion. although pac remains the " gold standard" to measure cardiac output and preload, new and alternative technologies were developed to evaluate these hemodynamic variables. methods: modified delphi methodology was used to create and quantify the consensus between the participants. amib indicated a coordinator who invited more six experts in the area of monitoring and hemodynamic support to constitute the consensus advisory board. twenty three physician and two nurses selected from different regions of the country completed the expert panel, which reviewed the pertinent bibliography listed at the medline in the period from 1996 to 2004. results: recommendations regarding the use of arterial pulse pressure variation during mechanical ventilation, continuous arterial pulse contour and lithium dilution cardiac output measurements, esophageal doppler waveform, thoracic electrical bioimpedance, echocardiography and partial co2 rebreathing for monitoring cardiac output and preload were created. conclusions: the new and less invasive techniques for the measurement of cardiac output, preload or fluid responsiveness are accurate and may be an alternative to pac in critically ill patients.

Abstract: background and objectives: monitoring of vital functions is one of the most important tools in the management of critically ill patients. nowadays is possible to detect and analyze a great deal of physiologic data using a lot of invasive and non-invasive methods. the intensivist must be able to select and carry out the most appropriate monitoring technique according to the patient requirements and taking into account the benefit/risk ratio. despite the fast development of non invasive monitoring techniques, invasive hemodynamic monitoring using pulmonary artery catheter still is one of the basic procedures in critical care. the aim was to define recommendations about clinical utility of basic hemodynamic monitoring methods and the use of pulmonary artery catheter. methods: modified delphi methodology was used to create and quantify the consensus between the participants. amib indicated a coordinator who invited more six experts in the area of monitoring and hemodynamic support to constitute the consensus advisory board. twenty-five physicians and nurses selected from different regions of the country completed the expert panel, which reviewed the pertinent bibliography listed at the medline in the period from 1996 to 2004. results: recommendations were made based on 55 questions about the use of central venous pressure, invasive arterial pressure, pulmonary artery catheter and its indications in different settings. conclusions: evaluation of central venous pressure and invasive arterial pressure, besides variables obtained by the pac allow the understanding of cardiovascular physiology that is of great value to the care of critically ill patients. however, the correct use of these tools is fundamental to achieve the benefits due to its use.

Abstract: objective: to assess differentials in official birth and death data for estimating infant mortality. methods: an ecological study was conducted based on data obtained from birth and death information systems in the state of pernambuco, northeastern brazil, between 2006 and 2008. the following indicators were used: age-standardized mortality rate, relative mean deviation of mortality rate, ratio of reported to estimated live births, relative mean deviation of birth rate and proportion of deaths of unknown cause. these indicators were grouped into three dimensions: mortality, fertility and ill-defined causes. based on predetermined criteria, municipalities were classified as follows: consolidated vital data; vital data in the consolidation phase; and non-consolidated data. the data were analyzed using epiinfo and terraview for map preparation. results: of the 185 municipalities in the state of pernambuco, 141 (76.2%) were classified as having consolidated vital data, accounting for about 85% of the state population, and 17 (9.2%) were classified as having non-consolidated data, accounting for only 4.9% of the population. larger municipalities (50,000 inhabitants or more) showed better data quality. conclusion: the approach studied proved itself valuable to assess the quality of vital information and identify inequalities in pernambuco. reduction of inequalities is a challenge in this state in the sense of enabling vital information to be analyzed directly from data systems at the local level. it will also allow assessing the effectiveness of initiatives to reduce infant mortality in pernambuco.
The Tanaka-Johnston orthodontic analysis for Brazilian individuals

Abstract: purpose: to assess the applicability of the tanaka-johnston analysis in brazilian subjects. methods: a total of 650 plaster casts of dental arches were examined, and 95 were selected to comprise the study sample. the largest distance between the contact points of mandibular and maxillary incisors as well as mandibular canines and premolars on the right side were measured with a calliper held parallel to occlusal plane and perpendicular to the long axis of the tooth. a prediction of the length of canines, and first and second premolars in both arches was made by using the tanaka-johnston analysis. predicted and actual dental sizes were compared in relation to the gender and ethnic group of the sample subjects. results: the tanaka-johnston analysis overestimated the sum of mesiodistal widths of maxillary and mandibular canines and premolars only in the white female group. the difference between the tanaka-johnston prediction and the actual measurement was significant. for the other groups studied no statistical difference was observed. conclusion: the tanaka-johnston analysis provided an acceptable prediction of the sum of mesiodistal widths of maxillary and mandibular canines and premolars in black and white brazilian men, but not in white brazilian women.

Abstract: Objetivo: Avaliar a aplicabilidade da análise de Tanaka e Johnston em indivíduos brasileiros de cor de pele branca e de cor de pele negra. Metodologia: Um total de 650 modelos de gesso de arcadas dentárias foram examinados e 95 foram selecionados para compor a amostra. Mediu-se a maior distancia entre os pontos de contato dos incisivos inferiores e caninos e pré-molares, superiores e inferiores do lado direito, com o paquímetro paralelo ao plano oclusal e perpendicular ao longo eixo do dente. A predi o do comprimento dos caninos, primeiros e segundos pré-molares em ambos os arcos foi feita através da análise de Tanaka e Johnston. Comparou-se a predi o e o valor real do tamanho dos dentes em rela o ao gênero e à cor da pele dos sujeitos da amostra. Resultados: A análise de Tanaka e Johnston superestimou o somatório dos diametros mésiodistais dos caninos e pré-molares apenas no grupo de indivíduos de cor de pele branca do gênero feminino. Para os outros grupos n o foram observadas diferen as estatisticamente significantes. Conclus o: A análise de Tanaka e Johnston permite uma predi o aceitável do somatório dos diametros mésio-distais dos caninos e pré-molares dos indivíduos brasileiros de cor de pele negra e de cor de pele branca do gênero masculino. O pensamento de Vygotsky nas reuni?es da ANPEd (1998-2003)

Abstract: this article analyzes the works based on vygotsky's thought presented at the annual anped (national association for graduate studies and research in education) meetings from 1998 to 2003, aiming at an understanding of how that theoretical framework is being incorporated by the authors that participate in those events. the text is organized into three parts. firstly it describes the research process followed here. it then introduces and discusses the quantitative data extracted from the material analyzed, which is presented in the attached tables. the last part is a qualitative analysis that shows which bibliographical references are the most frequently used sources, which themes and concepts from the vygotskyan approach predominate in the works investigated, and the incorporation of the theory revealed by the texts. in this last part some of the works are highlighted, which seem to offer a more valuable contribution to the construction of the knowledge in this field, advancing the theory and expanding on vygotsky's original concepts. the article is concluded with some thoughts on the current incorporation of vygotsky's thought, as revealed in the works analyzed here, contrasting it with the author's doctoral thesis written twelve years ago, where she dealt with the same issue and focused on the arrival in brazil of authors of the historical-cultural approach, the beginning of the diffusion of their ideas and the initial efforts to incorporate their concepts. finally, some implications are raised of the present work to brazilian educators and to readers of vygotsky.
