Hydrocephalies and Vestibular Schwannoma of Great Size: Therapeutic Strategies: About a Series of 32 Cases  [PDF]

Abstract: Introduction: The association hydrocephalus and vestibular schwannoma (VS) has been known for a long time. However, there is no therapeutic consensus, especially the place of drainage of cerebrospinal fluid (CSF). We report the result of our experience on the management of this pathology from a group of patients with a high volume VS (Koos IV) and operated consecutively. After reflections based on current literature data, we propose a therapeutic decisional algorithm. Materials and Methods: This is an analytical, retrospective study of 171 patients operated on KOOS IV vestibular schwannoma from January 2003 to December 2016 at the Marseille University Hospital Center. Of these, 32 patients with hydrocephalus and stage IV vestibular schwannoma were included. Radio-diagnostic criteria for hydrocephalus were based on Evans’ index, cortical furrow status, and the presence of trans-ependymal resorption. Our sample was divided into 2 groups. The first consisted of patients first operated on their hydrocephalus and secondarily treated with schwannoma surgery (group I); patients who underwent surgical resection of their first-line tumor were group II. Epidemiological, clinical, radiological, therapeutic and monitoring data were analyzed. The comparison of the quantitative variables was made by Fisher’s test. Results: During our study period (13 years), 171 cases of stage IV SV had been operated. The association between hydrocephalus and SV stage IV of Koos accounted for 18.7%. The average age of our patients was 53 years with a sex ratio of 0.7. The clinical picture was primarily composed of otological signs (90.6%), headache (56.3%) and cerebellar involvement (43.8%). The average diameter of VS inponto-cerebellar angle (PCA) was 31.5 mm. The treatment consisted of placing a first shunt of the CSF in 34.4% (group I). The ventriculoperitoneal shunt (VPS) was performed in 90.9% of cases. The first surgical removal of the tumor (group II) involved 65.6% of the patients. The postoperative tumor residue averaged 0.76 cc. The Evans index was evaluated on average at 0.33 in each of the 2 groups postoperatively. The average follow-up time for patients was 51 months. Eight cases of complications were recorded during the study. Secondly, in group II, VPS was performed in 9.5% (2 cases). Conclusion: Hydrocephalus is a condition commonly associated with stage IV vestibular schwannoma. The role of static disorder in negative thermal expansion in ReO3

Abstract: Time-of-flight neutron powder diffraction and specific heat measurements were used to study the nature of thermal expansion in rhenium trioxide, an electrically conducting oxide with cubic symmetry. The temperature evolution of the lattice parameters show that ReO3 can exhibit negative thermal expansion at low temperatures and that the transition from negative to positive thermal expansion depends on sample preparation; the single crystal sample demonstrated the highest transition temperature, 300 K, and largest negative value for the coefficient of thermal expansion, alpha = -1.1(1)x 10^-6 K^-1. For the oxygen atoms, the atomic displacement parameters are strongly anisotropic even at 15 K, indicative of a large contribution of static disorder to the displacement parameters. Further inspection of the temperature evolution of the oxygen displacement parameters for different samples reveals that the static disorder contribution is greater for the samples with diminished NTE behavior. In addition, specific heat measurements show that ReO3 lacks the low energy Einstein-type modes seen in other negative thermal expansion oxides such as ZrW2O8.

Abstract:

Introduction: Fractures of the lumbar spine and thoracolumbar junction are common in spinal trauma. The aim of this work is to analyze the nature of the indications, the morbidity as well as the results of these treatment regimens. Patients and Methods: A retrospective, single-center study, based on a review of 64 patients with lumbar spine and thoracolumbar junction fractures (T10-L2) without neurological disorders, was collected in the neurosurgery department of the North Hospital and University Hospital (CHU), Marseille over a period of 2 years from January 2015 to December 2016. Posterior percutaneous osteosynthesis were more or less associated with kyphoplasty preceded anterior arthrodesis. Clinical and radiological endpoints were collected at least 6 months later. Results: The mean follow-up was 9.5 months (6 - 24). The clinical evaluation found a mean VAS at last follow-up at 14/100 (0 - 30) and an average Oswestry score at the last follow-up at 88%. The initial average vertebral kyphosis went from 13° to 4° at the last follow-up with a correction loss of 1°, an absolute gain of 8°. No postoperative neurological complications were noted in our series. Conclusion: The implementation of a two-step therapeutic strategy with anterior reconstruction in Magerl’s lumbar spine or A3.3 thoracolumbar junction fractures allows effective and long-lasting correction of lumbar lordosis and thoracic kyphosis, and obtaining a balanced spine in the sagittal plane. Our functional results are close to normal, with low morbidity and a low complication rate.

Du réalisme dans l’analyse économique des conflits d’usage : les enseignements de l’étude du contentieux dans trois départements fran ais (Isère, Loire-Atlantique, Seine-Maritime).

Abstract: Les conflits d’usage renvoient à la confrontation de préférences, individuelles ou collectives, sur l’allocation des espaces et des actifs naturels localisés, non mobiles, à des usages alternatifs voire à des non usages. Ils révèlent des externalités négatives induites par les changements dans l’allocation des espaces à différentes fonctions (agricole, industrielle, de transport, résidentielle, récréative, etc.) ; leur proximité géographique ou symbolique au sein d’un territoire renforce la montée des antagonismes. Les cas des conflits d’environnement, qui marquent l’entrée en scène des associations de protection de l’environnement dans la gestion des usages des territoires, et des conflits entre agriculteurs et néo-ruraux dans le cadre de l’extension des localisations résidentielles périurbaines, sont deux exemples significatifs de l’essor d’externalités négatives liées à la proximité. L’article s’intéresse à ce que la théorie économique laisse dans un angle mort : la dimension juridique des conflits d’usage. Il analyse les conflits d’usage exprimés dans le langage du droit devant les tribunaux judiciaires et, surtout, administratifs, dans trois départements fran ais : l’Isère, la Loire-Atlantique et la Seine-Maritime. La démarche suivie vise au réalisme dans la compréhension des conflits d’usage tels qu’ils sont exprimés et tranchés dans la pratique juridictionnelle et privilégie, à ce titre, une approche empirique, inductive et institutionnelle. L’ambition de l’article est de caractériser la conflictualité dans ces trois départements et à en dégager des enseignements en termes de gestion juridictionnelle des conflits en cherchant à déterminer si les conflits liés à la proximité font l'objet d'une approche "territorialisée" par les tribunaux. Land-use and environmental conflicts reveal the existence of antagonism between individual or collective preferences on space and natural resources allocation to alternative uses. Such conflicts are grounded on various negative external effects which are provoked by the combination of different uses (agricultural, industrial, residential, touristic, etc.) in a proximity pattern. Analysis of a fractal boundary: the graph of the Knopp function

Abstract: A usual classification tool to study a fractal interface is the computation of its fractal dimension. But a recent method developed by Y. Heurteaux and S. Jaffard proposes to compute either weak and strong accessibility exponents or local Lp regularity exponents (the so-called p-exponent). These exponents describe locally the behavior of the interface. We apply this method to the graph of the Knopp function. The Knopp function itself has everywhere the same p-exponent. Nevertheless, using the characterization of the maxima and minima done by B. Dubuc and S. Dubuc, we will compute the p-exponent of the characteristic function of domain under the graph of F at each point (x,F(x)) and show that p-exponents, weak and strong accessibility exponents change from point to point. Furthermore we will derive a characterization of the local extrema of the function according to the values of these exponents.
CRDB: a database of charged cosmic rays

Abstract: This paper gives a description of a new on-line database http://lpsc.in2p3.fr/crdb and associated on-line tools (data selection, data export, plots, etc.) for charged cosmic-ray measurements. The experimental setups (type, flight dates, techniques) from which the data originate are included in the database, along with the references to all relevant publications. The database relies on the MySQL5 engine. The web pages and queries are based on PHP, AJAX and the jquery, jquery.cluetip, jquery-ui, and table-sorter third-party libraries. In this first release, we restrict ourselves to Galactic cosmic rays with Z<=30 and a kinetic energy per nucleon up to a few tens of TeV/n. This corresponds to more than 200 different sub-experiments (i.e., different experiments, or data from the same experiment flying at different times) in as many publications. We set up a cosmic-ray database and provide tools to sort and visualise the data. New data can be submitted, providing the community with a collaborative tool to archive past and future cosmic-ray measurements.
